RESUMO

Chronic urologic conditions, including benign prostatic hyperplasia, recurrent urinary tract infections, and urinary incontinence, are common in older adults. This article highlights the urologic and transitional care needs of an elderly, cognitively impaired male during and after an acute hospitalization. Collaboration between the patient, his family, the advanced practice nurse, primary care providers, and outpatient urology office are described. The importance of mutual goal setting and a focused plan for transitional care are discussed.

RESUMO

Radical prostatectomy is the most common cause of urinary incontinence in males. Urinary slings are a relatively new treatment option for men. This article describes two urinary sling procedures as possible surgical approaches for incontinence in men.

RESUMO

PURPOSE: We developed a nursing process decision support system (NPDSS) based on three clinical pathways, including benign prostatic hypertrophy, inguinal hernia, and urinary tract stone. NPDSS included six major nursing diagnoses - acute pain, impaired urinary elimination, impaired skin integrity, anxiety, infection risk, and risk of falling. This paper aims to describe the design, development and validation process of the NPDSS. METHODS: We deployed the Delphi method to reach consensus for decision support rules of NPDSS. A team of nine-member expert nurses from a medical center in Taiwan was involved in Delphi method. The Cronbach's α method was used for examining the reliability of the questionnaire used in the Delphi method. The Visual Basic 6.0 as front-end and Microsoft Access 2003 as back-end was used to develop the system. A team of six nursing experts was asked to evaluate the usability of the developed systems. A 5-point Likert scale questionnaire was used for the evaluation. The sensitivity and specificity of NPDSS were validated using 150 nursing chart. RESULTS: The study showed a consistency between the diagnoses of the developed system (NPDSS) and the nursing charts. The sensitivities of the nursing diagnoses including acute pain, impaired urinary elimination, risk of infection, and risk of falling were 96.9%, 98.1%, 94.9%, and 89.9% respectively; and the specificities were 88%, 49.5%, 62%, and 88% respectively. We did not calculate the sensitivity and specificity of impaired skin integrity and anxiety due to non-availability of enough sample size. CONCLUSIONS: NPDSS can help nurses in decision making of nursing diagnoses. Besides, it can help them to generate nursing diagnoses based on patient-specific data, individualized care plans, and implementation within their usual nursing workflow.

RESUMO

In this paper the authors discuss the use of Parse's humanbecoming theory in Japan. Elements of the theory are used in the nursing approach to an 88 year-old Japanese man who had complications following surgery. Process recordings of the dialogues between the patient, the patient's wife, and the nurse were made and considered in light of the three methodologies of Parse's theory; illuminating meaning, synchronizing rhythms, and mobilizing transcendence. The theory is seen as useful in Japan.

A chart audit of patients who had undergone a transurethral resection of the prostate established that 15.4% of 156 patients had postoperative acute urinary retention (AUR). Pre-operative prostate size, clot retention, and pre-operative urinary tract infection distinguished men at risk for post-operative AUR.

Hemos estudiado el cumplimiento de los indicadores de calidad del Proceso Asistencial Hipertrofia Benigna de Próstata (HBP) – Cáncerde Próstata (CP) en la Zona Básica de Salud de Vejer de la Frontera por parte de médicos y enfermero, y el papel de estos últimos enla implantación, desarrollo y control de calidad de dicho proceso.Los resultados obtenidos a los 7 meses de la evaluación indican un alto grado de implicación por parte del servicio de Enfermería deAtención Primaria tanto en la implicación como en el desarrollo de dicho proceso así como un alto índice de satisfacción por parte delos usuarios de la atención enfermera recibida (AU)


We have studied the fulfillment of the indicators of quality of the Process Assisted Hypertrophy Benign of Prostate (HBP) - Cancer ofProstate (CP) in the Basic Area of Health of Vejer de la Frontera which was carried out by doctors and nurses, and role of nurses inthe introduction, development and quality assurance of this process.The results obtained at the 7 months of the evaluation indicate a high degree of implication on the part of the service of nursing ofPrimary Care in the implication as well as in the development of this process as well as a high index of satisfaction of users of thenurse attention receive (AU)

RESUMO

Botox is a sterile, vacuum-dried, purified BoNT-A produced from fermentation of Clostridium botulinum. There is an abundance of literature to support the use of Botox in urology, and its off-label use continues to grow. While there are no FDA-approved clinical indications, there are ongoing phase II and III studies to approve Botox for use in some urological conditions. This article will explore the use of Botox in urology and provide a brief review of the literature supporting its use.

PURPOSE: Increasingly, men with lower urinary tract symptoms (LUTS) are seeking treatment in the primary care setting. This article reviews the use of alpha-blockers and 5alpha-reductase inhibitors (5ARIs) in the management of LUTS and enlarged prostate. DATA SOURCES: Information is based on a critical review of the published literature. Relevant studies were identified using MEDLINE and review of reference lists of published studies. CONCLUSIONS: Enlargement of the prostate is a common occurrence among aging men. Nurse practitioners (NPs) are in a unique, frontline position to evaluate symptoms and bother and to recommend appropriate treatment of patients with enlarged prostate. Both alpha-blockers and 5ARIs are effective at reducing symptoms in the short term. However, only the 5ARIs impact disease progression and maintain improvement in symptoms in the long term. IMPLICATIONS FOR PRACTICE: NPs play an important role in assessing and treating LUTS in men with enlarged prostate. When treating men with LUTS, assess the severity of symptoms and the extent of prostate enlargement. For symptomatic men with enlarged prostate, long-term therapy with 5ARIs should be considered to treat symptoms as well as address the disease progression.

A challenge for urologic nurses and nurse educators is how to present information to staff, students, and patients in a way that will capture their interest and engage them in the learning process. The use of adult-learning principles and innovative teaching strategies can make the learning experience dynamic, and encourage learners to take a more active role in their own learning. Games are a creative, fun, and interactive way to assist in the emphasis, review, reinforcement, and retention of information for urology nurses.

PHOTOSELECTIVE VAPORIZATION of the prostate is a significant advancement in the treatment of benign prostatic hypertrophy (BPH), one of the most common ailments experienced by men older than age 50. MANY MEN WITH BPH who are not candidates for transurethral resection of the prostate because they are undergoing anticoagulation therapy may be excellent candidates for the photoselective vaporization procedure. BENEFITS of the photoselective vaporization procedure over conventional procedures include reduced length of the procedure, decreased length of hospital stay, reduced postoperative recovery time, fewer complications, little or no pain, and faster return to normal activities.

RESUMO

OBJECTIVE: To study the perioperative nursing the benign prostate hyperplasia (BPH) patients with diabetes mellitus. METHODS: The nursing and treatment of 126 BPH patients with diabetes mellitus were investigated. RESULTS: Satisfactory nursing effect was seen in 122 of the patients, and postoperative improvement was significantly associated with post-void residual urine volume and urine flow in the 122 patients. CONCLUSIONS: Perioperative nursing lays an important base for successful operation and ensures curative effect of BPH patients with diabetes mellitus.
