RESUMO

Skin squamous cell carcinoma is the second most common non-melanoma skin tumor worldwide. Most skin squamous cell carcinoma patients have underlying diseases. Here, we report a 56 year-old patient diagnosed with skin squamous cell carcinoma and with a 30 year course of neglected lupus vulgaris, which was very rare. In this case, we adopted a treatment strategy involving a small wound: superficial resection combined with photodynamic therapy with a satisfied result.

RESUMO

Cutaneous tuberculosis is rare today and is often confused with other granulomatous lesions. Its diagnosis remains bothersome, because detecting mycobacteria in skin lesions using a conventional laboratory examination remains difficult. A 59-year-old woman presented with lupus vulgaris of the earlobe. Surgical treatment was employed, with an initial diagnosis of hemangioma. After the operation the condition was diagnosed correctly and preventive antituberculous drugs were prescribed. The incidence of tuberculosis has increased since the mid-1980s chiefly due to the expansion of the HIV-infected, immunocompromised population. Thus the diagnosis and treatment of cutaneous tuberculosis has become once again important.

In the developed industrial countries, the frequency of tuberculotic diseases of the skin including lupus has decreased to a very low level because of the significant decline of morbidity and mortality, the elimination of Bovine infections, the introduction of BCG vaccination, as well as effective anti-tuberculotic drugs employed for about 30 years. In spite of the very favorable therapeutic results, we have to reckon with the continuance of the present lupus incidence as long as the number of tuberculotic affections in the world is about 20 million, and even in developed industrial countries, the yearly rate of tuberculotic patients amounts to 43 in 100,000 inhabitants. We know that the disease still occurs very frequently in the developing countries of the Far East.

RESUMO

In the paper, the author's experience and literature data on studies of the role of biological processes developing in the host wound and grafted tissues in formation of the pedicled graft are reported. Considering these processes in the modern aspect in due account of general state of the organism, the status of local tissues, causes conditioning the defects formation and other factors, the author suggests new grounds for his concepts concerning the rational of active biological preparation of tissues in creating the pedicled graft. The technic of the preparation in question and results of its successful utilization in 25 patients are described, the conditions for wound and tissue healing being unfavourable due to different reasons, and in case of need of large quantities of plastic material the former also may be used.
