RESUMO

PATIENTS: Singleton-Merten syndrome is an extremely rare autosomal dominant condition with less than 10 reported cases in the literature. It is characterized by abnormal aortic calcifications and dental abnormalities. The goal of this case report is to discuss the abnormal oral clinical features and the modified treatment protocol that was used in order to achieve osseointegration of dental implants in a patient having abnormal bone density and bone turnover associated with Singleton-Merten Syndrome. DISCUSSION: Following extraction of the remaining teeth, titanium implants (Friadent GmbH, Mannheim, Germany and Straumann(®), Basel, Switzerland) were placed in the upper and lower jaw of the patient. The upper jaw which was treated with dental implants, received a bar supported implant retained prosthesis and the lower jaw an implant retained telescopic prosthesis. The patient was regularly followed up for the past 13 years during which, clinical and radiological evaluation of osseointegration was undertaken. All the loaded implants showed clinical and radiographic evidence of osseointegration. With a follow up of 13 years after insertion of the first implant, the patient reported functioning well with no complications. CONCLUSION: The treatment with dental implants in the extremely rare Singleton-Merten syndrome patients is a reasonable treatment option to rehabilitate maxillofacial aesthetics and establish normal function of the jaws.

Osseous dysplasia (OD) is one of the fibro-osseous jaw lesions characterized by cellular fibrous tissue with hard tissue formation. We report a rare fine-needle aspiration cytology (FNAC) for periapical OD. A 34-year-old woman had well-defined, round, mixed radiolucent lesions in the periapical regions of the bilateral mandibular canines. FNAC showed hypocellular with some multinucleated giant cells and a few clusters of fibroblastlike cells. These cells had bland nuclei, with no nuclear atypia or mitotic figures. No calcified materials or bony spicules were evident. Microscopically, the lesions consisted of cellular fibrous tissue and hard tissue. The final diagnosis was bilateral periapical OD. FNAC alone may not be an accurate procedure for definitive diagnosis of a fibro-osseous lesion, but it can be a useful and reliable diagnostic method when used in conjunction with clinical and imaging findings.

AIM: This report presents an unusual case of regional odontodysplasia (RO) in the mandible which crosses the midline along with its clinical management. BACKGROUND: RO is an uncommon, nonhereditary unilateral developmental anomaly involving the dental hard tissues, most frequently affecting the maxillary teeth. REPORT: RO was diagnosed in a 12-year-old. The dysplastic teeth were the mandibular canines, lateral incisors, central incisors, and the right premolars. The treatment was extraction of the affected teeth followed by provisory prosthetic rehabilitation. SUMMARY: The etiology of RO remains obscure. In this case a possible association with pre-natal trauma is suspected. The literature is not unanimous as to the management of RO. Nevertheless, the presence of a dental abscess is an indication for extraction. Since general bone quality is not affected, a provisory prosthesis was fabricated as the patient waits for future implant rehabilitation. The executed treatment met the expectations of the patient and her mother as it improved the patient's masticatory function, esthetics, and quality of life. CLINICAL SIGNIFICANCE: The general practitioner must be prepared to deal with different situations in the dental office. The diagnosis of RO relies mainly on clinical and radiographic findings, and its treatment depends on the affected teeth, the severity of the dysplasia, the presence of infection, and the patient's age.

Benign "mixed"odontogenic tumors consist of an epithelial and ectomesenchymal tumor component, distinguishing them from pure epithelial and pure ectomesenchymal odontogenic tumors. In addition, they may have the ability to produce dentin, enamel or cementum. Therefore, they can sometimes already be differentiated radiologically from epithelial odontogenic tumors. Some of the mixed odontogenic lesions are regarded as true tumors (ameloblastic fibroma, odontoameloblastoma, dentinogenic ghost cell tumor), while others are assumed to represent hamartomatous lesions (complex and compound odontoma, probably also ameloblastic fibrodentinoma and ameloblastic fibroodontoma). Preceded by keratocystic odontogenic tumor, complex and compound odontomas are the second most common odontogenic tumors, while other members of the "mixed" odontogenic tumor group are far less frequently diagnosed. Odontoameloblastoma and dentinogenic ghost cell tumors are locally aggressive lesions that require total resection. All other lesions of this group are treated by local excision. Since ameloblastic fibrosarcoma may evolve from ameloblastic fibroma, patients with ameloblastic fibroma should remain in long-term follow-up.

La complicación más importante del Liquen Plano Oral (LPO), es el desarrollo de un carcinoma oral de células escamosas (COCE). Desde 1910 en que se comunicó el primer caso en una paciente con LP, se han publicado numerosos casos, y series de casos de malignización, que nos aportan cifras que varían entre el 0 y el 12,5%de LPO malignizados, con periodos de seguimiento también muy variables, que oscilan entre 0,5 y 20 años. Pero estos estudios han sido refutados por un grupo de autores, que se cuestionan los criterios diagnósticos utilizados. Por todo ello, el potencial maligno del liquen plano oral, es un tema actualmente muy controvertido en Medicina Oral. Se realiza una revisión crítica de la literatura más actual sobre los conceptos clave, implicados en esta controversia (AU)


The most important complication of oral lichen planus (OLP), is the development of oral squamous cellcarcinoma (OSCC).From 1910 when was reported the first case in a woman suffered OLP, many cases and cases series have been reported with values from 0 to 12,5%, and with follow-up ranking from 0,5 to 20 years. But many of this cases not had been accepted by some authors who disagree with the diagnostic criteria employed. Regional odontodysplasia (RO), also known as ghost teeth, is an unusual nonhereditary developmental anomaly of tooth formation that characteristically affects enamel and dentin formation of the primary and/or permanent dentition. In the present paper, we report a case of RO affecting a 7-year-old boy, with 9 years of follow-up. During this time, progressive development of dental tissue was observed, including complete root formation. However, delayed dental eruption was evident. In view of these findings, we discuss the clinical presentation, pathogenesis, differential diagnosis, and treatment of RO.

RESUMO

A case of regional odontodysplasia in an 8 year-old male whose chief complaint was the absence of eruption of permanent teeth is presented. The regional odontodysplasia is a rare development anomaly of the dental tissues that affects the primary and permanent dentitions involving several adjacent teeth without crossing the midline. The clinical, radiographic and histological aspects are described as well as the treatment accomplished through surgery and prosthetic rehabilitation.

Se trata de un estudio descriptivo de corte transversal que tienen como objetivo el profundizar sobre el manejo y tratamiento de las infecciones odontogénicas en los pacientes ingresados por el servicio de cirugía oral y maxilofacial del Hospital Roberto Calderón Gutiérrez. La recolección de la información fue obtenida del expediente clínico evaluándose a través de una ficha clínica las características generales del paciente, así como medios diagnosticos, tratamiento médico y quirúrgico. Se estudiaron 52 pacientes predominado el sexo masculino 65.4 porciento. Las edades mas afectadas fueron de 20 a27 años 32.69 porciento. El mayor procentaje de los pacientes fueron sanos 86.3 porciento. La región anatómica más afectada fue el espacio submandibular por caries dental en las primeras, segundas y terceras molares inferiores. Se obtuvo a través del resultado del cultivo en el 3.8 porciento estreptococo B hemolótico. El antibíotico mas usado intrahospitalario fue penicilina cristalina más cloranfenicol obteniendo una menor estadía con dicho esquema. Al realizar el drenaje y la exodoncia en las primeras 24 horas se logró una menor permanencia de los pacientes hospitalizados, de 1 a 8 días. Las complicaciones más frecuentes fue angina de Ludwig, falleciendo el 80 porcineto de dichos pacientes...

Regional odontodysplasia (RO) is an unusual, non-hereditary anomaly of the dental hard tissues with characteristic clinical, radiographic and histological findings. Etiology is unknown, and treatment, while varied, should be appropriate for each individual's clinical findings. In the case reported here, a 2-year-old female was diagnosed with RO. Extractions were performed under general anesthesia. Clinical, radiographic, and histological findings are described.

RESUMO

A case of a 14-year-old male with regional odontodysplasia is reported. In this presentation many atypical clinical and radiographical features of this condition are present. The chief complaint of the patient was the enlargement of the gingiva and, according to the literature, inflammatory processes are the main reason why patients look for care. Moreover, there was no radiographic evidence of unerupted teeth in this report. The functional and psychological benefits of the conservative approach are emphasised.

RESUMO

Regional odontodysplasia is an uncommon dental malformation with characteristic clinical and radiographic findings. Affected teeth appear discolored with irregularly shaped surfaces. Radiographically wide pulp chambers and thin poorly defined hard tissue outlines are typical features. This report describes a 9-year-old girl with recurrent and recalcitrant periapical infections as a result of unilateral mandibular odontodysplasia. With a 6-year follow-up, this is the longest documented case of regional odontodysplasia treated by tooth autotransplantation.

RESUMO

An unusual case of regional odontodysplasia in an 8-year-old boy is presented. He was referred to our clinic because of failure of tooth eruption on the left side of the maxilla and frequent fever attacks. Treatment consisted of extraction of the affected teeth, and removal of inflamed tissue.
