RESUMO

The Research Institute for Fragrance Materials (RIFM) and Creme Global Cremeglobal.com partnered to develop an aggregate exposure model for fragrance ingredients. The model provides a realistic estimate of the total exposure of fragrance ingredients to individuals across a population. The Threshold of Toxicological Concern (TTC) and Dermal Sensitization Threshold (DST) were used to demonstrate the magnitude of low exposure to fragrance materials. The total chronic systemic, inhalation, and dermal 95th percentile exposures on approximately 3000 fragrance ingredients in RIFM's inventory were compared to their respective TTC or DST. Additionally, representative fragrance ingredients were randomly selected and analyzed for exposure distribution by product type (i.e., cosmetic/personal care, household care, oral care, and air care) and route of exposure. It was found that 76 % of fragrance ingredients fall below their respective TTC limits when compared to 95th percentile systemic exposure, while 99 % are below inhalation TTC limits. The lowest 95th percentile aggregate exposure by product type was from household care products, then air care, and oral care products. The highest exposure was from personal care/cosmetic products. The volume of use for most fragrance ingredients (63 %) was <1 metric ton, estimating that environmental exposure to fragrance ingredients is likely low.

Household products intoxication is a common and preventable problem. Household product hazard awareness is lacking among consumers in Lebanon, posing a public health hazard. A cross-sectional observational study was conducted at the American University of Beirut Medical Center by administering surveys to 176 adult participants. The surveys focused on demographics, awareness of product toxicity, practices used to avoid intoxication, and sources of information. Each participant was given a score for behavior and awareness. Informative brochures were handed to increase awareness among consumers. We surveyed 176 patients, of whom (84.7%) were females with a mean age of 42.2 ± 13.5 years. Most were married (77.3%), had a college education (76.7%), were employed (62.5%), and were in charge of household cleaning (76.7%). Toilet cleaners were the substances most perceived to be toxic (94.0%). Most people (86.4%) had low to medium scores on behavior, while most (77.3%) had high scores on awareness. Male gender and using product labels as sources of information were associated with higher behavior scores, while referring to a friend or a relative as a source of information on product intoxication was associated with a lower behavior score. Greater awareness scores were strongly associated with being married, having a higher monthly income, and referring to warning signs presented on the labels as a source of knowledge. Unsafe handling and storage of household products are common among consumers in our population. Therefore, it may be necessary to launch education campaigns to improve consumer handling of household products.

The following paper presents the method of determination of the percolation threshold in cement composites with expanded graphite by impedance spectroscopy. Most of the applications of cement composites with conductive additives require exceeding the percolation threshold. The ionic conductivity of cement matrix below the percolation threshold has a major impact on the conductivity of the composite, as a result, it significantly hinders the exploitation of these composites. The electric properties of cement composites with expanded graphite were evaluated by DC measurements and impedance spectroscopy (IS). Based on Nyquist plots, two equivalent circuits were adopted for the composites. Next, the values of capacitance and inductance of cement composites with expanded graphite were calculated from the fitted equivalent circuits. The analysis of the results shows that the percolation threshold occurs when the reactance of the composite changes from captative to inductive. Comparison between the values of percolation threshold obtained from DC measurements and IS shows that the method is effective for cement composites with conductive additives.

Accidents with chemical household products are prevalent worldwide and often involve young children. Previous research has shown that consumers use both analytical factors (e.g., warning symbols) and potentially misleading heuristics to evaluate the riskiness. However, it remains unclear whether consumers intuitively think of the specific risks of these products when handling them. We conducted an online survey with Swiss consumers (N = 1109) aiming to investigate consumer's spontaneous and prompted risk perception of chemical household products. First, we asked them about their free associations to laundry detergent, descaler, mold remover and essential oils. While participants were aware of the risks when prompted about them, they did not name them intuitively, but mainly thought of the possible uses and attributes of the product. Next, we examined prerequisites to safe handling of chemical household products. This included applied knowledge about the products, but also factors like perceived barriers to safe use. Finally, we assessed the predictors of perceived severity of accidents in order to estimate the potential behavioral changes that these predictors might elicit. For interventions, particular attention should be accorded to personal risk awareness and the perception of potential barriers, which were significantly related to the perceived severity of accidents.

INTRODUCTION: Automatic dishwashing rinse aids are drying aids which contain non-ionic surfactants, usually ethoxylated alcohols, typically at concentrations of ≤30%. OBJECTIVE: To assess the reported toxicity of rinse aids. METHODS: Telephone enquiries to the UK National Poisons Information Service were analysed from January 2008 to June 2019. RESULTS: Ingestion: Ingestion was involved in 976 cases and produced gastrointestinal features, coughing and central nervous system depression, particularly in young children. In those in whom the amount ingested was known, the majority (56%) of children <18 years and of adults (57%) ingested <50 mL of rinse aid. Although moderate or severe exposures (Poisoning Severity Score (PSS) ≥ 2) were uncommon, they occurred significantly (p < 0.0008) more often in adults (9.0%) than in children (1.8%); however, three of the four adults with PSS ≥ 2 co-ingested other substances. Eye exposure: Ocular exposure was reported in 35 cases, of whom 29 developed features. Eye irritation (n = 10, 28.6%) and eye pain (n = 10, 28.6%) were reported most commonly, and three patients (8.6%) developed corneal abrasions (PSS 2). Dermal exposure: Thirty-four patients were exposed dermally, and six (17.6%) reported features, including rash, numbness, pruritus and burns (PSS 1). CONCLUSIONS: Overall, clinical features developed in 47% of patients exposed to rinse aids, but more severe features (PSS ≥ 2) were rare (<3%) following exposure by any route.

INTRODUCTION: Our objective was to describe the characteristics of liquid laundry detergent packet (LDP) exposures and to develop referral and treatment recommendations. METHODS: This retrospective cohort study investigated LDP exposures reported to the National Poison Data System from January 1, 2013 through June 30, 2014. Three medical toxicologists reviewed the most significant exposures (n = 450). RESULTS: Of 17,857 reported LDP exposures, 13,307 involved only an LDP (no other substance) and were followed to a known medical outcome. The median age was 2 years (range 12 days to 100 years). Approximately 10% of exposures reported a major or moderate effect. The most common symptom was vomiting (51.7%; n = 6875), but stridor or aspiration pneumonia and respiratory depression secondary to central nervous system effects also occurred. Two pediatric and two adult deaths occurred, but no causal mechanism leading to death could be identified in any of the deaths. CONCLUSIONS: LDPs occasionally produce a toxidrome of vomiting, stridor, hypoxia, and sedation with metabolic acidosis and respiratory failure. These symptoms and the availability of LDPs highlight the need for referral and treatment recommendations and efforts to minimize unintentional exposures. Review of data from US poison centers may provide referral and treatment recommendations that improve patient outcomes.

OBJECTIVE: To describe the profile of children and adolescents admitted for exogenous unintentional poisoning in the emergency room and analyze factors associated with subsequent in-hospital admissions. METHODS: This is a cross-sectional study based on hospital records of all subjects up to 19 years-old admitted in 2013 at a specialized toxicology service on a major public emergency hospital due to unintentional intoxication (as reported). Accidents with poisonous animals and insects were excluded. Percentages and frequencies were calculated for the qualitative variables, and measures of central tendency and dispersion for the continuous quantitative variables. Multivariate analysis was performed using binary logistic regression to identify variables associated with subsequent in-hospital admissions. RESULTS: In 2013, 353 cases were reported. Poisonings were more frequent in children 0-4 years-old (72.5%) and in boys (55%). The vast majority was of dwellers of the Metropolitan Region of Belo Horizonte (83%), and 90% of the accidental poisonings occurred at home. 82.7% of the poisonings occurred by oral ingestion, especially of medicinal (36.5%) and cleaning products (29.4% of all poisonings). Only 12.2% of the cases resulted in hospitalization, and only one resulted in death. Residing outside Belo Horizonte (OR=5.20 [95%CI 2.37-11.44]) and poisoning by two or more products (OR=4.29 [95%CI 1.33-13.82]) were considered risk factors for hospitalization. CONCLUSIONS: Accidental poisonings occurred most frequently by ingestion of household medications and cleaning products, especially among children under 4 years-old. Preventive strategies should be primarily directed for this prevalent profile.

RESUMO Objetivo: Descrever o perfil dos atendimentos de crianças e adolescentes vítimas de intoxicações exógenas acidentais e os fatores associados às internações hospitalares. Métodos: Foi realizado um estudo transversal com base na revisão dos registros de todas as intoxicações acidentais de indivíduos com até 19 anos de idade, atendidos no setor de toxicologia de um hospital público de referência em 2013, excluídos os acidentes com animais peçonhentos e insetos. A intencionalidade da intoxicação foi baseada nos relatos. Foram calculadas percentagens e frequências para as variáveis qualitativas, e medidas de tendência central e de dispersão das variáveis quantitativas contínuas. Foi realizada análise múltipla, utilizando regressão logística binária para identificar as variáveis associadas à internação hospitalar das vítimas atendidas. Resultados: Em 2013, foram identificados 353 atendimentos em crianças e adolescentes. A faixa etária mais prevalente foi a de zero a quatro anos (72,5%), e predominaram indivíduos do sexo masculino (55%). A maioria dos atendimentos foi de pacientes residentes na região metropolitana (83%). Noventa por cento das intoxicações ocorreram nos domicílios; 82,7% se deram pela via oral, especialmente por medicamentos (36,5%) e produtos de limpeza (29,4% de todas as intoxicações). Resultaram em internações 12,2% dos casos, ocorrendo um único óbito. As variáveis associadas à internação foram: residir fora do município sede (razão de chances [OR]=5,20; intervalo de confiança de 95% [IC95%] 2,37-11,44) e o envolvimento de mais do que uma substância na intoxicação (OR=4,29; IC95% 1,33-13,82). Conclusões: O ambiente doméstico é o principal local em que ocorrem as intoxicações em crianças e adolescentes, especialmente por ingestão de medicamentos e produtos de limpeza e abaixo de quatro anos de idade. Esses achados justificam a priorização de ações preventivas direcionadas para esse perfil de acidentes.

We report the case of a 59-year-old man with a history of type 2 diabetes, hypertension and chronic kidney disease who presented with symptomatic severe hypercalcaemia (calcium 15.8 mg/dL) and acute kidney injury. Evaluation revealed that the hypercalcaemia was not mediated by parathyroid hormone (PTH), PTH-related peptide or 1,25-hydroxyvitamin D. Adrenal insufficiency was subsequently diagnosed and was initially thought to be the aetiology of the hypercalcaemia. He was treated with intravenous fluid, pamidronate and started on hydrocortisone with resolution of his hypercalcaemia. Over the next several months, despite adherence to hydrocortisone therapy, the patient continued to have recurrent severe hypercalcaemia requiring hospitalisation. Additional laboratory evaluation showed similar results to the initial evaluation. On further questioning, the patient admitted to routinely ingesting the household cleaning product Comet, which contains a large amount of calcium. Psychiatric assessment confirmed the diagnosis of pica. The patient eventually discontinued ingestion of Comet with resolution of his hypercalcaemia.

Air fresheners contain various chemicals that may or may not be harmful to human health and the environment. These products are widely used in different settings such as homes, schools, offices, and hospitals with ignorance of their real ingredients and their relative health effects. Thus, this preliminary study was carried out to identify the presence of different compounds in spray air fresheners that were not disclosed on the product's label. Four different brands of spray air fresheners were selected randomly from a local store, in which two were of mid-to-high cost and the remaining two of low cost. The samples were analyzed using gas chromatography/mass spectrometry headspace, in which single components of the samples were identified by the mass spectrometry detector. The results were shown as a chromatogram of several peaks, each representing different compounds. The chemicals found in the samples include; lilial, galaxolide, benzenemethanol, musk ketone, butylated hydroxytoluene, and linalool. These chemicals may cause irritation and other health problems. However, none of them were revealed on the product's label. The study concludes that air fresheners need to be free of any toxic or harmful chemicals and include natural ingredients instead.

Freshwaters are increasingly exposed to complex mixtures of pharmaceutical and personal care products (PPCPs) from municipal wastewater, which are known to alter freshwater communities' structure and functioning. However, their interaction with other disturbances and whether their combined effects can impact ecological resilience (i.e., the ability of a system to tolerate disturbances without altering the system's original structure and processes) remain unexplored. Using in situ mesocosms in 2 lakes with different nutrient levels (mesotrophic and eutrophic), we assessed whether a pulse exposure to sublethal concentrations of 12 PPCPs affects the ecological resilience of natural phytoplankton communities that experienced an abrupt environmental change involving the destabilization of the water column through mixing. Such mixing events are predicted to increase as the effects of climate change unfold, leading to more frequent storms, which disrupt stratification in lakes and force communities to restructure. We assessed their combined effects on community metrics (biomass, species richness, and composition) and their relative resilience using 4 indicators (cross-scale, within-scale, aggregation length, and gap length), inferred from phytoplankton communities by discontinuity analysis. The mixing disturbance alone had negligible effects on the community metrics, but when combined with chemical contaminants significant changes were measured: reducing total biomass, species richness, and altered community composition of phytoplankton. Once these changes occurred, they persisted until the end of the experiment (day 20), when the communities' structures from the 2 highest exposure levels diverged from the controls. The resilience indicators were not affected by PPCPs but differed significantly between lakes, with lower resilience found in the eutrophic lake. Thus, PPCPs can significantly alter community structures and reinforce mechanisms that maintain ecosystems in a "degraded state." Environ Toxicol Chem 2019;38:2197-2208. © 2019 The Authors. Understanding how indoor-air contaminants affect human health is of critical importance in our developed society. We assessed the combined exposure by inhalation of preschool children and children to household products. A total of 1175 families with 72 infants, 158 toddlers, 230 children, and 239 youths were surveyed to determine the combined respiratory exposure concentrations and amounts associated with 21 substances in eight household product groups. We determined the mean concentrations of these substances in each product, and derived reference toxicity values based on the information gathered in order to identify respiratory health risks. On average, cleaners were used at a rate of 1.0 × 10³ g/month, while coating agents and other substances were used at 43 g/month. The combined inhalation exposure concentrations of methanol to infants and toddlers were 5.1 and 4.2 mg/m³ per month, respectively, with values of 2.1 and 1.7 mg/m³ for isopropanol, respectively. Risks to preschool children and children should be assessed on the basis of the toxicity values of combined exposed hazardous substances, as well as their combined concentrations and amounts. This exposure assessment approach can be used to establish improved guidelines for products that may pose inhalation hazards to preschool children and children.

Increasingly diverse chemicals are used in consumer products, while our understanding of their exposure pathways and associated human health risks still lags behind. This paper aims to identify the dominant patterns of exposure pathways and associated health risks of chemicals used in consumer products reported in the peer-reviewed literature. We analyzed 342 articles covering 202 unique chemicals, and distilled the information on the functional uses, product applications, exposure routes, exposure pathways, toxicity endpoints and their combinations. We found that the volume of the literature addressing human health risks of chemicals in consumer products is increasing. Among others, phthalates, bisphenol-A, and polybrominated diphenyl ethers were the most frequently discussed chemical groups in the literature reviewed. Emerged from our review were a number of frequently reported functional use/product application combinations, including plasticizers, polymers/monomers, and flame retardants used in food contact products, personal care products, cosmetics, furniture, flooring, and electronics. We also observed a strong tendency that the number of publications on a chemical surges following major regulatory changes or exposure incidents associated with the chemical. We highlight the need to develop the capacity and the mechanism through which human health risks of chemicals in consumer products can be identified prior to their releases.

Exposure assessment, a fundamental element of the risk assessment process, is the estimation of what consumers may be exposed to by using household products. Reliable exposure factors were investigated to understand the pattern of household products usage. Six household product categories were selected and divided by application types. Two surveys were carried out and 6397 participants completed the web-based questionnaires, and laboratory evaluations were also conducted to measure the actual level of exposure (g/use). We focused more on inhalation exposure from products (and/or their application types) that affect indoor air such as diffusible types. To estimate actual exposure amounts, the time spent to apply the products was investigated from the beginning to end of the product use. For spraying type products, the consumer could be exposed to the maximum amount of used product while using the triggering action and pressing the button on the aerosol spray product, and during cleaning, adhesion, and coating target, consumer could be exposed continuously. For diffuser type products, exposure duration was the time for which consumers were exposed to the diffused product. The exposure amount for trigger and aerosol spray type products were measured using the time taken to trigger or spray the product and the mass generated from the same. This study provides valuable information on the individual exposure factors that may be useful to conduct accurate exposure assessments and to manage household products.

This study examines the effects of household corrosive products on 105 restored (silver amalgam and porcelain-fused-to metal) and non-restored teeth. Five household products were utilized, including hydrochloric acid, sulfuric acid and detergent. Teeth were radiographed before and after exposure and were submerged for 120 or 264h. Documentation included weight, mesiodistal and buccolingual crown measurements, ordinal scores, and photography at specific hours of exposure. Results indicate that 81.9% of the teeth could be positively matched by radiographs. Hydrochloric acid had the most destructive effects mainly to non-restored and silver amalgam teeth followed by sulfuric acid. Porcelain samples were more resistant to the effects of acid and conferred protection to the underlying teeth. Acid type, acid concentration and the restoration type are statistically significant contributors to alterations and in radiographic matching. Household corrosive substances may affect the morphology of teeth, and in some cases completely destroy teeth, which could conceal identifications.

PURPOSE: Clustering methods may be useful in epidemiology to better characterize exposures and account for their multidimensional aspects. In this context, application of clustering models allowing for highly dependent variables is of particular interest. We aimed to characterize patterns of domestic exposure to cleaning products using a novel clustering model allowing for highly dependent variables. METHODS: To identify domestic cleaning patterns in a large population of French women, we used a mixture model of dependency blocks. This novel approach specifically models within-class dependencies, and is an alternative to the latent class model, which assumes conditional independence. Analyses were conducted in 19,398 participants of the E3N study (women aged 61-88 years) who completed a questionnaire regarding household cleaning habits. RESULTS: Seven classes were identified, which differed with the frequency of cleaning tasks (e.g., dusting/sweeping/hoovering) and use of specific products (e.g., bleach, sprays). The model also grouped the variables into conditionally independent blocks, providing a summary of the main dependencies among the variables. CONCLUSIONS: The mixture model of dependency blocks, a useful alternative to the latent class model, may have broader application in epidemiology, in particular, in the context of exposome research and growing need for data-reduction methods.

Sodium hydroxide is a corrosive, highly alkaline (PKa=14.8) household product. Ingestion of sodium hydroxide liquid is common, showing toxicity on the oesophagus and stomach. Nevertheless, cases of sodium hydroxide ingestions in pellet are rare and the management of them is unknown. We report the case of a 65-year-old man who accidentally swallowed a bleach tablet of 3.5 g. Six hours later, the patient developed an aphonia associated with dysponea stage IV, motivating a nasofibroscopy showing glottis and supraglottic necrosis and oedema for which the patient received intravenous steroids, was intubated and then underwent a tracheotomy. After 2 weeks under tracheotomy, local evolution was favourable allowing a removal of the cannula and a return back home.

Ingredient chemicals like fragrances may cause adverse health effects. Frequent health risk assessments and stringent management of consumer products are of paramount importance to reduce these serious occurrences. In this study, the respiratory and dermal health effects were assessed in relation to air fresheners. Twenty six fragrance ingredients, thirty four biocidal ingredients, and sixteen hazardous ingredients were analyzed and assessed according to their risk to human health on five groups by application type in eighty two air fresheners. For hazard characterization of ingredients, toxicological information on the intrinsic properties of the ingredients was collected, and reference values were determined as chronic NOAEL. Exposure assessment was performed in two steps. The 95th exposure factor values were used to estimate exposure to assume the worst-case scenario and the maximum concentration determined by the product purpose and application type was used type in tiered 1 assessment. The values input into the exposure algorithms were developed via the exposure route. In the tiered 2 assessment, the 75th exposure factor values were used to estimate the assumed reasonable exposure to ingredients. Six ingredients for the inhalation and twelve ingredients for the dermal route were conducted for tiered 2 assessment. This study showed that the assessed ingredients have no health risks at their maximum concentrations in air fresheners. The approach should be used to establish improved guidelines for specific ingredients that may pose inhalation and dermal hazard.

Uncertainty factors (UFs) are commonly used during hazard and risk assessments to address uncertainties, including extrapolations among mammals and experimental durations. In risk assessment, default values are routinely used for interspecies extrapolation and interindividual variability. Whether default UFs are sufficient for various chemical uses or specific chemical classes remains understudied, particularly for ingredients in cleaning products. Therefore, we examined publicly available acute median lethal dose (LD50), and reproductive and developmental no-observed-adverse-effect level (NOAEL) and lowest-observed-adverse-effect level (LOAEL) values for the rat model (oral). We employed probabilistic chemical toxicity distributions to identify likelihoods of encountering acute, subacute, subchronic and chronic toxicity thresholds for specific chemical categories and ingredients in cleaning products. We subsequently identified thresholds of toxicological concern (TTC) and then various UFs for: 1) acute (LD50s)-to-chronic (reproductive/developmental NOAELs) ratios (ACRs), 2) exposure duration extrapolations (e.g., subchronic-to-chronic; reproductive/developmental), and 3) LOAEL-to-NOAEL ratios considering subacute/acute developmental responses. These ratios (95% CIs) were calculated from pairwise threshold levels using Monte Carlo simulations to identify UFs for all ingredients in cleaning products. Based on data availability, chemical category-specific UFs were also identified for aliphatic acids and salts, aliphatic alcohols, inorganic acids and salts, and alkyl sulfates. In a number of cases, derived UFs were smaller than default values (e.g., 10) employed by regulatory agencies; however, larger UFs were occasionally identified. Such UFs could be used by assessors instead of relying on default values. These approaches for identifying mammalian TTCs and diverse UFs represent robust alternatives to application of default values for ingredients in cleaning products and other chemical classes. Findings can also support chemical substitutions during alternatives assessment, and data dossier development (e.g., read across), identification of TTCs, and screening-level hazard and risk assessment when toxicity data is unavailable for specific chemicals.
