RESUMO

Pulmonary alveolar proteinosis (PAP) is a chronic disorder of surfactant clearance from the alveoli. Its prevalence is rare, especially in the pediatric population. Although there is no cure for this condition, symptoms of PAP are managed most effectively through whole-lung lavage (WLL). Perioperative RNs caring for children with PAP undergoing WLL in the OR should implement patient interventions to maintain vital signs and normothermia and preserve skin integrity. Additionally, perioperative RNs often are responsible for assembling closed-drainage systems for WLL. Detailed procedural preference cards, targeted education sessions, and multidisciplinary collaboration are crucial for establishing a comprehensive plan of care for the pediatric patient with PAP undergoing WLL in the OR.

Bronchoalveolar Lavage (BAL) was first described more than fifteen years ago; it is still the treatment of choice in Alveolar Proteinosis. Despite several clinical therapeutic trials, for new therapeutics none of them has achieved the results of Bronchoalveolar Lavage. It has been proved that, not only are they ineffective, but they can also have dangerous consequences. Alveolar proteinosis is a rare lung illness that implies the patient's hypoxia and progressive incapacity, being clinical dyspnea its most frequent characteristic. The BAL. with sodium chloride by filling and emptying the lungs allows to wash the fosfolipid material. That its on the alveoli giving the patient a great improve in short space of time. The aim of our work is to describe the technique used in Bronchoalveolar Lavage as well as the changes that have taken place since the beginning of its practice in our Critical Care Unit for the last fifteen years and so the nursing work that it is on this technic. This procedure needs to be made on a UCI for the need of mechanical ventilation. The time of instance on UCI is 24 h. In our experience we are able to make the BAL. in both lungs in the same session, there are no important complications and there is no need of readmission. We conclude that there is evidence that these treatment is effective.

Pulmonary alveolar proteinosis (PAP) is a rare, debilitating, sometimes fatal disease of uncertain etiology and pathophysiology. The medical literature defines the illness and describes current theories related to its pathophysiology. Little nursing literature addresses PAP. This case study describes and discusses nursing interventions utilized in the home management of a young, female adolescent with this illness. A retrospective analysis of the chart reveals investigative treatment involving daily subcutaneous injections of bacterially synthesized, granulocyte-macrophage colony-stimulating factor. Communication and collaboration among health care providers and identification of diverse issues influencing the health of the client resulted in the development of effective nursing interventions. Leininger's Theory of Transcultural Care Diversity and Universality provides a model for interpretation and generalization of nursing interventions. PAP can be managed successfully in the home, but more information on the illness and ethnic and age-specific responses to treatment is needed.
