RESUMO

A young woman was admitted to Akranes Regional Hospital because of dyspnea, fatigue and fever. She was found to have bilateral pneumonia but etiology was not found. She was treated with antibiotics with good resolution and was discharged after eight days from the hospital. Four weeks later she noticed rapidly progressive dyspnea and was found to be hypoxemic, and to have restrictive spirometry and diffuse interstitial changes on chest radiography. Computerized tomography of the lungs showed diffuse ground glass changes. Transbronchial biopsies from the lungs showed numerous small granulomas. She was treated with prednisolone for a short time with excellent recovery. She stall-fed horses and underneath them was straw containing organic dust. This is important to keep in mind as a differential diagnosis to farmers lung disease that is caused by hay.

The aim of this study was to compare high-resolution computed tomography (HRCT) findings of long-term farmer's lung (FL) patients and control farmers. We studied 88 FL patients and 83 matched control farmers with a mean follow-up time of 14 years. Emphysematous, fibrotic, and miliary changes were recorded by HRCT. The pattern of emphysema and location and distribution of other findings were evaluated in detail. Emphysema was found in 20 (23%) FL patients and in 6 (7%) controls (p=0.005). Recurrences of FL attacks increased (p=0.021) the risk of emphysema. Prevalence of fibrosis (17 vs 10%, p=0.16) and miliary changes (11 vs 4%, p=0.06) did not differ significantly in patients and controls. Among FL patients, emphysematous, fibrous, and miliary changes were more pronounced at the base than in the upper parts of the lung (p<0.02). In slice analysis, the pattern of emphysema was more polymorphous (p=0.001) and the distribution of fibrotic and miliary changes was more variable among FL patients than controls. Emphysema in HRCT is more common in FL patients than matched control farmers, and the occurrence is increased by recurrences of FL. Emphysematous, fibrous, and miliary changes in FL patients HRCT are multiform and predominate in the lower parts of the lung.

PURPOSE: We evaluated the reproducibility of three visual scoring methods of emphysema and compared these methods with pulmonary function tests (VC, DLCO, FEV1 and FEV%) among farmer's lung patients and farmers. MATERIAL AND METHODS: Three radiologists examined high-resolution CT images of farmer's lung patients and their matched controls (n=70) for chronic interstitial lung diseases. Intraobserver reproducibility and interobserver variability were assessed for three methods: severity, Sanders' (extent) and Sakai. Pulmonary function tests as spirometry and diffusing capacity were measured. RESULTS: Intraobserver kappa-values for all three methods were good (0.51-0.74). Interobserver kappa varied from 0.35 to 0.72. The Sanders' and the severity methods correlated strongly with pulmonary function tests, especially DLCO and FEV1. CONCLUSION: The Sanders' method proved to be reliable in evaluating emphysema, in terms of good consistency of interpretation and good correlation with pulmonary function tests.

High resolution computed tomography (HRCT) scans are increasingly used in hypersensitivity pneumonitis (HP). This study looked at HRCT findings at different phases of farmer's lung (FL), a common form of HP. A cross sectional analysis of 95 HRCT scans of FL cases (20 acute, 75 with a history of FL, 48 still in contact (Ex +) (dairy farm), and 27 who had ceased contact (Ex-)) was made. All scans were read independently by two, and if needed by three, radiologists blinded to the category. The lungs were divided into six regions (fives lobes + lingula), and read for attenuation/mosaic, ground-glass, micronodules, fibrosis, and emphysema. A score of 0-3 was given for each region and each variable: 0 = absence, 1 =<25% of the surface, 2 = 25-50%, 3 =>50%. Mediastinal lymphadenopathy was also noted. Ground glass, predominating in the lower lobes, was the most frequent feature in the acute and Ex+ cases. Other abnormalities had no preferential distribution. Ex+ had more ground-glass than the Ex- (p=0.0025). Emphysema was more frequently seen than interstitial fibrosis (p=0.004). Mediastinal lymphadenopathy was present in 26 cases (9 acute, 10 Ex+ and 7 Ex-). In conclusion, in farmer's lung: 1) ground-glass predominates in the lower lobes while the other abnormalities have no anatomic predilection; 2) contact avoidance allows a better resolution of computed tomography abnormalities than continued exposure; 3) emphysema is a more frequent finding than interstitial fibrosis; and 4) the presence of mediastinal lymphadenopathy has no negative diagnostic value.

We evaluated the long-term outcome of farmer's lung (FL) patients and matched control farmers using high-resolution computed tomography (HRCT). The study population consisted of 88 FL patients and 83 control farmers, matched by age, sex, and smoking habits. The mean time after the first diagnosed episode of FL was 14 yr. The great majority, 82%, of the studied subjects were nonsmokers. Clinical studies included HRCT, spirometry, and pulmonary diffusing capacity. Emphysema was found significantly more often (23%) in FL patients than in control farmers (7%) (p = 0.006). The presence of emphysema was 18% in nonsmoking and 44% in smoking FL patients, the respective values being 4% and 20% in control farmers. Patients with recurrent attacks of FL tended to have emphysema more often (p = 0. 08) than patients who had experienced only a single attack. Fibrosis was observed in 17% of the FL patients and in 10% of the control farmers (p = 0.2). Miliary changes were found in 12% of the FL patients compared with 4% of the control farmers (p = 0.07). Both emphysematous and fibrotic but not miliary changes correlated significantly with impaired pulmonary function. In conclusion, farmer's lung disease seems to be associated with an increased risk of developing emphysema.

BACKGROUND: Surfactant protein A (SP-A) acts as an immune system modulator in the lungs and may therefore be involved in the pathogenesis of hypersensitivity pneumonitis. METHODS: The levels of SP-A in bronchoalveolar lavage (BAL) fluid were measured in 20 subjects with acute farmer's lung, 16 asymptomatic dairy farmers, and 14 normal controls. Eight patients had a second evaluation after one month of treatment by either contact avoidance (n = 3) or oral prednisolone (20 or 25 mg/day, n = 5). Chest radiographs and lung function measurements were also obtained in all farmers, twice in those re-evaluated after treatment. RESULTS: Patients with acute farmer's lung had significantly higher levels of SP-A than asymptomatic farmers and normal controls (p = 0.005) with mean (SE) values of 1.43 (0.29) micrograms/ml, 0.62 (0.09) microgram/ml, and 0.68 (0.11) microgram/ml, respectively. In eight subjects tested after one month of treatment the level of SP-A was unchanged although all were clinically improved. No correlations were seen between levels of SP-A in BAL fluid and numbers of BAL cells, lung function measurements, or chest radiographic scores. CONCLUSION: Although the level of SP-A is increased in the BAL fluid of patients with acute farmer's lung, it is not correlated with clinical abnormalities of this disease.

RESUMO

Lung is the most involved site by infective manifestations in immunodepressed patients. In these patients the variability of pathogenic and opportunistic agents is expressed in the lungs with interstitial and alveolar changes, which are not pathognomonic. CT is the appropriate method to study lung parenchyma. CT by means of fast scans and high resolution algorithms (HRCT) allows an accurate and detailed representation of the lung parenchyma, detecting the secondary lobe, the anatomic and functional unit of the lung and its pathological changes. Our study aim to show the clinical-radiological correlation among the various types pulmonary infections in immunocompromised host.

A 35-year-old dairy farmer presented with recurrent febrile episodes over more than three years. Fever up to 39 degrees C occurred only if he was in contact with the dairy barn and typically appeared at 4 to 8 h after an otherwise usual barn exposure. He had no dyspnea and throughout this period repeated physical examinations, chest roentgenograms, and lung functions remained normal. He was bronchoscoped and lavaged on three occasions, a lymphocytic alveolitis was always present and once, immediately after an acute episode, an important increase in lavage neutrophils was also found (47 percent). Transbronchial lung biopsy specimens showed a moderate cellular infiltrate but no granuloma or fibrosis. Three other persons who worked at the same farm had no similar manifestations or respiratory illnesses. The patient therefore had typical systemic manifestations of acute farmer's lung, but without the lung involvement required to confirm that diagnosis. Furthermore, his disease does not fit the entities known as grain fever and organic dust toxic syndrome. We believe that this patient presented a different entity that we coined "farmer's fever."

Patients with farmer's lung were classified into five groups according to their chest X-ray pattern, and the background of each group was studied with respect to stage, severity, and prognosis. Type I group, which has fine nodular shadows on chest X-ray, showed a significantly high percentage of lymphocytes in BAL cells (p less than 0.01). Type V group, which has irregular patchy shadows on chest X-ray, showed increased peripheral WBC counts, increased CRP score, and decreased PaO2 compared to the other groups. Type II, III, and IV groups, which are characterized by combined X-ray patterns such as nodular shadows, ring-like shadows, and irregular pulmonary markings, had no specific background features according to our study. Our results suggest that type I and type V groups are likely to represent acute phase and severe phase, respectively. We could not identify any significant factors indicative of chronic disease in terms of restrictive and diffusion impairment of pulmonary function tests and pathologic findings obtained by TBLB. The chest X-ray classification we proposed is useful for the diagnosis of the acute and severe stages of farmer's lung.

Agricultural workers are at risk for developing many respiratory tract disorders. Crop production disorders include chronic bronchitis, asthma, organic dust toxicity syndrome, chemical poisoning, farmer lung, and silo-filler disease. Livestock production disorders include toxic manure poisoning (dung lung) and several zoonoses. Radiographic manifestations can be classified into three patterns: (a) normal findings, characteristic of chronic bronchitis, asthma, or organic dust toxicity syndrome; (b) acute diffuse interstitial or alveolar pattern, characteristic of farmer lung, silo-filler disease, dung lung, and chemical poisoning; and (c) chronic interstitial pattern in the upper lobes (farmer lung) or lower lobes (silo-filler disease, paraquat poisoning). Differential diagnosis is aided by knowledge that most of these disorders have a seasonal presentation. Zoonoses generally exhibit focal areas of consolidation in the lung: The type of splenic calcification, presence of mediastinal or hilar adenopathy, and development of bronchiectasis are additional findings useful in differentiating these rare infections.

Extrinsic Allergic Alveolitis (E.A.A.) is an immunological disorder caused by inhalation of various etiological agents (vegetable and animal dusts, fungi, organic simple chemicals) mostly present in work environment. Intensive and prolonged occupational exposure to antigens dimensionally able to reach peripheral airways, where they bring about their immunological effects, allows to consider hypersensitivity pneumonitis as a typical work-related disease. The wide range of pathological findings and evolution of E.A.A. often imply difficulty in diagnosis inspite of availability of diagnostic consolidated criteria. The integrated assessment of history and physical findings, radiological features, pulmonary functions tests, and specific and aspecific immunological data, often allows to make diagnostical conclusion without use of invasive methods (BAL and lung biopsy). In this note we refer two cases of E.A.A. occurred respectively in agricultural and industrial environment, whom etiological and evolutive features suggest an occupational pathogenesis. The first one is a typical "farmer's lung", the second one a hypersensitivity pneumonitis in a cheese washer. The diagnosis was performed by means of historical, clinical, radiological and pulmonary functional outcomes. In the first case specific serum precipitins against M. Faeni were present, in the second one the immunological pathogenesis of the pneumonitis was demonstrated by means of bronchial lavage cytological data and serum IgG dosage. In both cases the removal from specific antigen exposure and the prolonged steroid treatment induced symptomatological regression and complete recovery of functional tests.

Following a study of the diagnosis of farmer's lung (PDF) carried out in 1763 active farm workers in Doubs, 294 (16.7%) were suspected of PDF and they were invited to a secondary consultation in hospital for a further work up which consisted of a clinical examination for crepitations (RC) on pulmonary auscultation (AP) and for evidence of an interstitial lung disorder (SI) on pulmonary radiograph (RP) and for evidence of a restrictive syndrome (SR) on pulmonary function tests (EFR). 238 patients (80%) accepted the additional assessment and were divided into two groups on the basis of the history and serology (precipitin tests), group 1: probable PDF (n = 65): symptoms suggestive of PDF in the presence of serum precipitins; group 2: possible PDF (n = 173): non specific symptoms with or without serum precipitins. RC were present in 21.9% of patients in group 1 and 6.4% of patients in group 2 (p less than 0.001). There was an SI in 9.2% of patients in group 1 and 4% of patients in group 2 (non significant). For the elevated radiology scores for SI there was a significant difference: 7.7% for SI in group 1 and 1.73% in group 2 (p less than 0.05). There was 38.1% of abnormal EFR in group 1 and 14.7% in group 2 (p less than 0.001) but the difference of the frequency of SR in the 2 groups was not statistically significant (9.5% in group 1 and 4.7% in group 2). In this study, an additional assessment (AP, RP, EFR) carried out at a difficult moment during winter does not appear to confirm great benefit.(ABSTRACT TRUNCATED AT 250 WORDS)

A 55-year-old female farmer had all the clinical, biochemical and radiological signs diagnostic of allergic bronchopulmonary aspergillosis. Remarkably she also had symptoms of farmer's lung and exogenous allergic bronchial asthma with specific types I and III sensitisation due to Aspergillus antigens inhaled with straw-dust. In addition to the usual asthma treatment she also received antimycotic agents and corticosteroids. Antigen exposure was strictly avoided. These measures brought about lasting regression of the disease manifestations and of the high concentrations of IgE and IgG antibodies against many Aspergillus antigens.

Reports of the radiological features in farmer's lung are inconsistent. We have reviewed clinical and radiological findings in eight patients with farmer's lung in the acute and sub-acute phases. In the acute phase the main feature is transient widespread diffuse shadowing of air space consolidation. In the sub-acute phase, the predominant feature is fine nodular shadowing tending to involve either the upper half or upper two-thirds of the lungs with relative sparing of the basal segments. This is in accordance with both the pathophysiological behaviour of the inhaled particulate antigen and the subsequent distribution of pulmonary changes in chronic farmer's lung.

A 33-year-old woman farmer developed an acute episode of fever, cough, and shaking chills with persistent shortness of breath. Her PaO2 was 51 with a restrictive pattern on pulmonary function. Her diffusion capacity was 36 percent of predicted. In spite of these abnormalities, she always had normal chest roentgenographic findings. Further studies, including a lung biopsy, led to the diagnosis of farmer's lung disease.

Data are presented on 9 subjects with Farmer's lung encountered in 1981-85. In all cases dyspnea arose within several hours after exposure to the allergen and in 4 cases only a few minutes after exposure. The radiological picture was altered in 7 cases (6 miliary pictures and 1 case of medial and basal nodulation). In functional terms 6 restrictive and 3 mixed forms were identified. The specific bronchial provocation test was positive wherever it was performed. Bronchoalveolar lavage revealed predominantly lymphocytic alveolitis in the most recent cases. Transbronchial biopsy also revealed the presence of pulmonary granulomatosis in various stages of development. Finally skin tests were positive to various antigens in several cases while specific precipitin were found in all cases. These data are compared with reports in the literature.

RESUMO

A 40-year-old woman who worked on a farm and was exposed to moldy hay presented with acute respiratory failure requiring mechanical ventilation and homogeneous radiologic consolidation. Treatment with steroids produced rapid improvement. These features of presentation of hypersensitivity pneumonitis with acute respiratory failure and homogeneous consolidation are rarely encountered.

Serum galactosylhydroxylysyl glucosyltransferase activity (S-GGT) and serum procollagen type III aminoterminal propeptide concentration (S-PRO(III)-N-P) were measured in 40 patients with farmer's lung at the time of an acute attack of the disease and 6 months later in order to study the usefulness of these serum markers for predicting the development of interstitial lung fibrosis. These 2 serum parameters have previously been found to reflect tissue collagen synthesis, especially in fibrotic hepatic diseases. The mean S-GGT was significantly increased when compared with the reference mean both at the time of the acute attack (p less than 0.001) and after 6 months (p less than 0.001). Approximately half of the patients had a S-GGT value higher than the upper normal limit at the acute stage, after which the levels decreased significantly (p less than 0.01). S-PRO(III)-N-P remained at the control level in most patients at the acute stage of the disease, followed by a small but significant increase (p less than 0.01), so that by the end of 6 months of follow-up one third of the patients had a S-PRO(III)-N-P value slightly above the upper normal reference value. Double blind tests on the administration of corticosteroids to half of the patients suggested that corticosteroids might have some favorable effect on the disease, although no significant response was found. The patients with definitely abnormal pulmonary function tests 1 yr after the acute stage had significantly higher initial S-GGT values than the rest of the patients (p less than 0.001). Elevated S-GGT at the acute stage was observed in 90% of the patients with definitely abnormal lung function after 1 yr but in only 30% of the other patients.(ABSTRACT TRUNCATED AT 250 WORDS)
