OBJECTIVE: The study aimed to provide a guideline for continuous rehabilitation nursing for patients with severe finger replantation, and establish a satellite contact point for patients with severe finger replantation after discharge, so as to ensure scientific and effective rehabilitation training after discharge and explore the role of continuous rehabilitation nursing measurement in severe finger rehabilitation and neurovascular preservation. METHODS: A total of 380 patients accepting neurovascular preservation finger replantation in the hand surgery department were divided into an observation group and a control group according to the number of hospitalizations. All patients underwent reconstructive surgery of severed finger. X-ray filming was used to monitor the postoperative nursing effect of neurovascular preservation of severed finger. The discharged patient information questionnaire was filled 3 days before the discharge. Then, a patient information database was established, and rehabilitation training was performed. Finally, sexual rehabilitation nursing follow-up intervention, telephone follow-up, and cross-referring intervention were carried out. Postoperative x-ray images were taken regularly to observe the recovery of reconstructed finger. RESULTS: There was no difference in daily life ability scores and hand function scores between the 2 groups at discharge (P > 0.05). The daily life ability scores and hand function scores of the observation group were better than those of the control group at 1 and 6 months after discharge (P < 0.05), the difference is statistically significant. As the discharge time prolonged, the daily life ability score and hand function score of the 2 groups of patients gradually increased. X-ray images showed that the replanted finger body was well developed, phalanx was in good antithesis, and there was no epiphysis. CONCLUSIONS: Continuous rehabilitation nursing measures should be taken after the replantation of the severed finger after neurovascular preservation, which provides standardized rehabilitation training standards for patients with replantation of severed finger after discharge, improves patient training compliance, promotes hand function recovery as soon as possible, and enables patients to return to society as soon as possible, which is worthy of clinical promotion and application.

Digits amputated through trauma can often be replanted and regain function. Success depends on preoperative care of the digit and minimising the time the digit is without blood supply. Long-term patient compliance with postoperative care and therapy is necessary to maximise postoperative functioning.

OBJECTIVE: To report a case of successful total auricle replantation 10 h after complete amputation. MATERIAL AND METHODS: Replantation was achieved by means of microvascular anastomosis and other therapies, and assisted by careful nursing. The patient's auricle was completely cut off with a sharp blade by an assailant, who retained it for 5 h. Having been retrieved, it was then preserved in ice for 5 h. Using microscopy, an artery and vein were found at the confluences of the upper and middle and lower and middle parts of the amputated auricle and head wound, respectively An end-to-end anastomosis was performed on these vessels. After operation, the following treatments were used: drainage; reopening of the drainage channels with a needle: flushing or soaking with heparin sodium solution; controlling infection and coagulation; increasing blood volume: dilating vessels; and special nursing. RESULTS: The replanted auricle survived with a normal contour and a very favorable esthetic appearance. CONCLUSIONS: The neat, uncontaminated wound margins and the fact that the amputated auricle was preserved in ice, even though it had been amputated 10 h before the operation, were prerequisites for successful auricle replantation. The microvascular anastomosis technique played a very important role in the survival of the amputated auricle. The postoperative treatment, observation and nursing were difficult but vital aspects of the procedure. The establishment of an effective venous return was a sign of the survival of the amputated auricle.

RESUMO

Cardiac explantation and autotransplantation is a procedure by which the heart is removed from the chest and replaced. Explantation of the heart for tumor removal has been documented six times in the literature; however, with this aggressive approach, the median patient survival rate is only eight months. This article presents a case study in which this procedure was used to treat a patient with malignant fibrous histiocytoma.

RESUMO

This article presents a protocol for the perioperative care of patients undergoing digital replantation, which is the most common microsurgical procedure performed today. Venous congestion, a common complication of digital replantation, often has been treated through surgical exploration and creation of arteriovenous anastomosis. Leech therapy, however, is experiencing a resurgence among surgeons as an alternative method for treating venous congestion. This article discusses the anatomical, physiological, and clinical indications and methods of leech therapy in digital replantation.

RESUMO

A 30-year-old female patient lost her scalp in an industrial accident. Surgeons used a microscope to reconnect the arteries and veins in her scalp and to reattach the scalp to the patient's cranium. The patient tolerated the procedure without complications, although she developed venous congestion in her right ear while she was in the neuroscience intensive care unit. After several days of leech therapy, the patient experienced an uneventful postoperative course. One month after discharge from the hospital, the patient's hair was growing back in and her incision lines were fading.

Replantation surgery has come of age in the past 20 years. Sophisticated microsurgical techniques, improved preservation methods, and increased understanding of ischemia time have all contributed to the growth of digital replantation as a viable surgical process. The complexity of the surgeries warrants a review of nursing management, pharmacologic modalities, and monitoring alternatives. This article explores rehabilitative therapies in relation to successful patient outcomes.
