RATIONALE: Traumatic bronchial rupture in infants usually necessitates surgical intervention, with few reports documenting instances of multiple cardiac arrests occurring during surgery under conditions of severe hypoxemia. PATIENT CONCERNS: A 3-year-old boy after trauma presented with severe hypoxemia for 2 days and was urgently transferred to the operating room for surgery, 6 episodes of cardiac arrest happend during surgery. DIAGNOSES: The baby was diagnosed with bronchial rupture based on the history of trauma, clinica manifestations, and intraoperative findings. INTERVENTIONS: Intrathoracic cardiac compression and intravenous adrenaline were administrated. OUTCOMES: The normal sinus rhythm of the heart was successfully restored within 1 minute on each occasion, facilitating the smooth completion of the surgical procedure. By the end of surgery, SpO2 levels had rebounded to 95% and remained stable. LESSONS: Inadequate management of bronchial ruptures in infants frequently coincides with severe hypoxemia, necessitating immediate surgical intervention. Prompt identification and management of cardiac arrest by anesthetists during surgery is imperative to reduce mortality.

PURPOSE: The purpose of this study was to retrospectively analyse the pattern of injury to the medial knee structures in anterior cruciate ligament (ACL) injured patients. It was hypothesised that anteromedial injuries would be more common than posteromedial lesions. METHODS: One hundred and twenty subjects aged 18-25 years with a primary ACL injury were included. Patients were excluded if the time between injury and magnetic resonance imaging (MRI) was more than 28 days or if a knee dislocation or fracture was present. The MRIs were analysed with particular emphasis on injuries to the medial knee structures, menisci and bone bruise patterns. Injuries to the ligaments and anteromedial retinaculum (AMR) were graded according to severity, ranging from periligamentous oedema (grade I), partial fibre disruption of less or more than 50% (grade IIa or IIb) to complete tears (grade III). RESULTS: AMR injury was seen in 87 subjects (72.5%) on the coronal plane and in 88 (73.3%) on the axial plane, with grade III lesions observed in 27 (22.5%) and 29 knees (24.2%). Injuries to the superficial medial collateral ligament (sMCL), deep MCL (dMCL) and posterior oblique ligament (POL) were detected in 60 patients (50%), 93 patients (77.5%) and 38 patients (31.6%). However, grade III injuries to the POL were observed in only seven knees (5.8%). Medial meniscus injuries were associated with lesions of the sMCL and AMR (p < 0.05), while lateral meniscus injuries were significantly more common in patients with dMCL rupture (p < 0.05). CONCLUSION: Data from this study suggest that injuries to the AMR are much more common than posteromedial lesions in subjects with ACL injuries. LEVEL OF EVIDENCE: Level IV.

BACKGROUND: Spontaneous or non-traumatic bladder rupture is rare but can be life-threatening. Bladder rupture caused by a diverticulum is extremely rare, with only a few case reports in medical literature. CASE PRESENTATION: We report the case of a 32-year-old woman admitted to hospital complaints of abdominal pain, oliguria and ascites with no history of trauma. Laboratory tests revealed an elevated serum urea nitrogen(UN) level of 33.5 mmol/l and an elevated creatinine levels of 528 umol/l. X-ray cystography confirmed the rupture of a bladder diverticulum. Subsequent transurethral catheterization led to a prompt increase in urinary output, and serum creatinine level returned to 40 umol/l within 48 h. The patient was successfully treated with laparoscopic diverticulectomy. CONCLUSION: Clinicians should maintain a high level of suspicion for urinary bladder rupture in cases presenting with acute lower abdominal pain, urinary difficulties, and oliguria. When acute renal failure, complicated ascites, and an elevated peritoneal fluid creatinine or potassium level exceeding serum levels are observed, intraperitoneal urine leakage should be suspected without delay. This case emphasizes the importance of early diagnosis and intervention in managing this rare but serious condition.

PURPOSE: This study reports the mechanisms, complications and graft survival following sight-threatening traumatic globe rupture in patients having previously undergone corneal transplantation in the same eye. METHODS: A retrospective, observational, single-center consecutive cohort study at the Royal Victoria Infirmary, Newcastle upon Tyne, UK over a 20-year period. Medical records and Newcastle Corneal Transplantation Service electronic database (eNCTS) review was undertaken of all consecutive patients who underwent corneal transplantation with a history of traumatic globe rupture. Main outcome measures include mechanism of injury, final best-corrected distance visual acuity (BCDVA), graft survival and complications. RESULTS: A total of 921 keratoplasties were undertaken between 1997 and 2017 with 24 (3.0%) patients identified with a history of traumatic globe rupture. A bimodal relationship of age and mechanism of trauma was observed. The mean age (SD) of individuals reporting cause as a fall was 71.5 (14.8) years, and 45.3 (20.8) years (P < 0.05) amongst individuals reporting accidental trauma or assault. The pre- and post-trauma mean (SD) LogMAR BCDVA was 0.6 (0.9) and 1.7 (1.0), respectively (P = 0.001). The overall graft-failure rate was 60.9% (11 grafts) during a mean (SD) follow-up period of 3.5 (4.1) years. Globe rupture with lens damage was associated with poorer final BCDVA (P < 0.05). CONCLUSIONS: This study represents the first published series from England for this type of patient cohort. Overall visual outcomes were poor with a bimodal relationship of age and mechanism of trauma. Worse prognostic factors included lens and posterior segment complications. Re-grafting in these select group of patients may prove valuable.

INTRODUCTION: Delayed intraperitoneal bladder rupture is a rare clinical occurrence, frequently overlooked and misdiagnosed due to its nonspecific clinical manifestations. However, literature provides only a limited number of cases reporting delayed intraperitoneal bladder rupture resulting from blunt abdominal injury. PATIENT CONCERNS: A 72-year-old female pedestrian was struck by a vehicle and experienced sudden, severe abdominal pain on the 8th day following the injury. Abdominal B-ultrasound revealed a significant accumulation of peritoneal effusion. The abdominal puncture retrieved serosanguinous ascites. Then the patient was promptly transferred to our hospital. Upon transfer, the physical examination revealed the patient vital signs to be stable, accompanied by mild abdominal distension, slight tenderness, tension, and an absence of rebound tenderness. Urinalysis detected microscopic hematuria, while contrast-enhanced computed tomography (CT) revealed considerable fluid accumulation in the abdominal cavity, without evidence of solid organ damage, and the bladder was adequately filled. DIAGNOSIS: The diagnosis of delayed intraperitoneal bladder rupture primarily relied on intraoperative observations. INTERVENTIONS: An emergency exploratory laparotomy was performed, revealing a linear rupture at the dome of the bladder. Subsequently, the bladder rupture was repaired. OUTCOMES: Postoperative cystography demonstrated full recovery and the patient was discharged 28 days post-surgery. The postoperative recovery was uneventful without any complications. CONCLUSIONS: A well-distended bladder observed in CT does not definitively rule out the potential for bladder injury. False negatives may occur due to incomplete bladder filling during CT cystography. Retrograde cystography can identify cases missed by CT cystography. In cases of substantial intra-abdominal free fluid, surgical intervention should be actively considered for patients with blunt abdominal trauma without concurrent solid organ damage.

Blunt cardiac injury, including a rupture of the atria or ventricle, is most commonly caused by motor vehicle collisions and falls from great heights. A rupture of a cardiac chamber is an extremely rare diagnosis with a high mortality rate. The best chance at survival can only be accomplished with timely intervention.To raise awareness of this potentially life-threatening injury, we describe the case of a male adolescent with cardiac rupture after blunt thoracic trauma. While the focused assessment with sonography in trauma (FAST) examination was negative, an additional CT showed pericardial effusion. During the operation a rupture of the right ventricle was observed.Even though the physical recovery of our patient is remarkable, the traumatic event still affects his mental well-being and activities in daily life. This case emphasises the need of a multidisciplinary approach to achieve the best possible physical and psychological recovery in multitrauma patients.

An adolescent female presented with an acute abdomen and elevated beta-human chorionic gonadotropin levels and underwent a laparoscopy for a suspected ruptured ectopic pregnancy. Intraoperatively, a ruptured haemorrhagic corpus luteal cyst and tissues suggestive of products of conception were noted in the same ovary. Histology confirmed an ovarian ectopic pregnancy. Haemorrhagic ovarian cysts, and ectopic pregnancies, can cause acute pelvic pain in women of childbearing age. Their similar clinical signs and symptoms pose a diagnostic dilemma for any gynaecologist. Ruptured corpus luteal cysts, as well as ruptured ovarian ectopic pregnancies, should be considered rare but differential diagnoses in women presenting with acute abdominal pain, an adnexal mass and ultrasound features of haemoperitoneum. The mainstay of treatment is a diagnostic laparoscopy, which is a safe and feasible management strategy without compromising patient safety or ovarian function in the long run.

BACKGROUND: Although chest trauma happens very often, accompanying tricuspid valve injuries occur rarely and may be manifested by scarce symptoms and signs. Pericardial rupture with cardiac herniation is even a bigger rarity. Transthoracic echocardiography plays a key role in the diagnosis of valve injuries but is of limited value in cardiac herniation. CASE PRESENTATION: We present the case of 58-year-old man who experienced severe chest trauma in a car accident. Symptoms of right heart failure occurred 10 years after the injury, due to the loss of tricuspid leaflet support caused by the rupture of tendinous chords with significant tricuspid regurgitation. Intraoperatively, old posttraumatic pericardial rupture into left pleura was also found, with partial cardiac herniation and pressure of the edge of pericardium on all left-sided coronary arteries simultaneously. The patient was successfully operated and is free of symptoms 4 years later. CONCLUSIONS: This case emphasizes the importance of timely diagnosis and underlines a mechanism that leads to delayed rupture of the tricuspid valve apparatus. Repeated echocardiography in all patients who experienced chest trauma could be of great importance. Also, given the limited value of echocardiography in posttraumatic pericardial rupture and cardiac herniation, cardiac computed tomography should be performed.

Anterior cruciate ligament (ACL) rupture is a very important epidemiological pathology in our environment. It has a peak incidence between 16 to 39 years of age. It is estimated that between 70-84% of ACL injuries are non-contact. The "no return" position describes the knee in valgus, femoral external rotation, tibial internal rotation and semiflexion, promoting injury to this ligament. Geometric measurements of the knee have been associated with an increased probability of non-contact ACL injury. The management of ACL tears is divided into two: conservative treatment and surgical management. Early OA (osteoarthritis) is the most common consequence of an ACL tear. We present the case of a 35-year-old patient with an inveterate ACL rupture of 10 years of evolution. With conservative management initially that progresses to knee instability and pain in the medial and lateral joint line as well as increased volume and functional limitation. After diagnostic studies, it was decided to perform diagnostic-therapeutic arthroscopy and continued close follow-up for associated pathology.


La ruptura de ligamento cruzado anterior (LCA) es una patología epidemiológicamente muy importante en nuestro medio. Tiene un pico de incidencia entre los 16 a 39 años de edad. Se calcula que entre 70-84% de las lesiones de LCA son sin contacto. La posición de "no retorno" describe a la rodilla en valgo, rotación externa femoral, rotación interna tibial y semiflexión, promoviendo la lesión de este ligamento. Las medidas geométricas de la rodilla se han asociado con un aumento en la probabilidad de lesión del LCA sin contacto. La ruptura crónica del LCA conlleva al desarrollo de artrosis en pacientes jóvenes. El manejo de la ruptura del LCA se divide en dos: tratamiento conservador y manejo quirúrgico. La osteoartrosis temprana es la consecuencia más común de la ruptura del LCA. Presentamos el caso de un paciente de 35 años con ruptura inveterada del LCA de 10 años de evolución. Con manejo conservador inicialmente que progresa a inestabilidad de rodilla y dolor en línea articular medial y lateral, así como aumento de volumen y limitación funcional. Tras estudios diagnósticos, se decide realizar artroscopía diagnóstica-terapéutica y se continúa con seguimiento estrecho por patología asociada.

Esta afección supone un difícil diagnóstico, por lo que requiere una alta sospecha y un tratamiento temprano


We describe an unusual case of axillary vein thrombosis after extensive muscular and fibrillar rupture of the long tendon of the right biceps brachii in a 43-year-old patient after an effort. The difficult diagnosis of this condition requires high suspicion and early treatment

ABSTRACT Objectives: Evaluate the demographic data, etiology, operative findings and results of surgical treatment of penile fracture (PF) in men who have sex with men(MSM) with emphasis on sexual complications. Materials and Methods: We studied 216 patients underwent surgical correction of PF at our hospital. Patients self-identified as MSM were followed for at least 6 months. Demographic data, presentation, operative findings, International Index of Erection Function - 5 (IIEF-5) and the Premature Ejaculation Diagnostic Tool. Results: Of 216 PF cases, 4 (1.8%) were MSM. All cases resulted from sexual activity and all patients reported using the "doggy style" position during anal intercourse. Unilateral or bilateral injury of corpus cavernosum was found in 2 patients each. One (25%) patient had complete urethral injury associated with bilateral corpus cavernosum lesion. During the follow-up period, all patients developed some type of sexual complication. One patient reported penile pain during intercourse. Another patient experienced low sexual desire and premature ejaculation. This patient was also dissatisfied with the aesthetic result of the surgical scar and complained about decreased penis size after surgery. The third case developed delayed ejaculation. The fourth patient experienced mild to moderate erectile dysfunction. This same patient presented with penile curvature. Finally, palpable fibrotic nodules in the operative area were observed in all cases. Conclusions: Sexual activity in the "doggy style" position was the commonest cause of PF in MSM. Sexual dysfunction is always present in gay man after surgery for PF. However, additional studies with larger samples should be coinducted.

Introducción y objetivos: La sutura directa en los casos de rotura del extensor pollicis longus (EPL) puede llevar asociado el fallo de la misma. Por ello, la transferencia tendinosa del extensor indicis proprius (EPI) es una buena alternativa. Nuestro objetivo es describir nuestra experiencia con esta técnica. Material y métodos: Estudio observacional descriptivo y retrospectivo sobre 29 casos. Variables: Edad, sexo, mano dominante, actividad laboral, tiempo quirúrgico. Análisis de resultados obtenidos en test de Geldmacher, SEEM y EVA. Resultados: Edad media de 48.04 ± 9.4 años. 72.4% varones, 27.6% mujeres. Brazo dominante en el 55.2%. 3.4% diabéticos y 6.9% con tratamiento corticoideo. Traumatismo directo en el 58.6%. Diagnóstico ecográfico en el 89.7%. Tiempo quirúrgico: 51.8 ± 12.5 minutos. Escala Geldmacher: 15.79 ± 5.8 puntos. Escala SEEM: 70.36 ± 20.4 puntos. Rotura de plastia en 13.8%. Altas por mejoría en 96.6%. Conclusiones: Las roturas de este tendón se asocian a fracturas de radio distal, tratamiento con corticoides, artritis reumatoide o tras osteosíntesis de fracturas de radio con placas volares. En nuestra experiencia, en los casos de rotura del EPL, consideramos que la trasposición del EPI es una alternativa eficaz, con un reducido número de complicaciones


Introduction and objectives: Direct suture in cases of rupture of extensor pollicis longus (EPL) tendon has been associated to suture tear. For this reason, tendon transfer of extensor indicis proprius (EPI) tendon is a good alternative. Our objective is describe our experience with this technique. Matherial and methods: Observational descriptive and retrospective study about 29 cases. Variables: Age, sex, dominant hand, laboral activity, time of surgery. We analize results with Geldmacher’ scale, SEEM and VAS scores. Results: Average age was 48.04 ± 9.4 years. 72.4% were males and 27.6% were females. Dominant arm was affected in 55.2% of cases. 3.4% were diabetics and 6.9% received corticoid treatment. Direct trauma appeared in 58.6% of cases. Sonographic diagnosis was in 89.7% of cases. Surgical time was of 51.8 ± 12.5 minutes. Geldmacher’ scale: 15.79 ± 5.8 points. SEEM score: 70.36 ± 20.4 points. Plastia rupture happened in 13.8% of patients. 96.6% of patients returned to their work. Conclusions: Ruptures of this tendon have been associated with distal radius fractures, corticoid treatment, rheumathoid arthritis or after osteosynthesis with volar plates in radius fractures. In our experience, in cases of rupture of EPL, we consider that transposition of EIP it is an effective alternative, with minimal number of complications

Objetivo: Describir los resultados funcionales y las complicaciones a medio plazo de la reparación quirúrgica de la avulsión del tendón distal del bíceps braquial y establecer una relación con la vía de abordaje empleada. Métodos: 17 pacientes consecutivos fueron intervenidos. Se registró la vía de abordaje empleada. Se recogieron los valores postoperatorios de rango de movilidad articular, capacidad funcional medido como puntuación del cuestionario Quick - DASH score y se registraron las complicaciones. Resultados: En 14 pacientes se reparó el tendón distal del bíceps. En 3 se confirmó su integridad. En 3 casos se empleó una vía anterior y en 11 una doble vía. Flexión media: 131'43º; extensión: - 3'21º pronación: 70'71º; supinación: 74’29º; puntuación Quick - DASH score: 17'04. Seguimiento: 25'5 meses. Aparecieron complicaciones en el 57’14%. La más frecuente fueron las parestesias locales, con resolución espontánea en todos los casos. Los pacientes intervenidos por doble vía obtuvieron resultados mejores en comparación con los intervenidos por vía única. Conclusiones: La reparación quirúrgica de las roturas agudas del tendón distal del bíceps braquial presenta aceptables resultados postoperatorios con una alta tasa de complicaciones, si bien estas últimas son en su mayoría de escasa relevancia. Estos resultados mejoran cuando se emplea la técnica de doble vía descrita por Boyd Anderson y modificada por Morrey


Purpose: Describe mid-term functional results and complications rate after surgical reattachment of acute distal biceps tendon ruptures and relate them with the surgical approach employed. Methods: 17 consecutive patients were evaluated. Surgical approach employed was registered, as well as post-operative range of motion of the elbow, physical function measured as Quick - DASH Score and complications. Results: Complete rupture of biceps tendon was observed in 14 cases during surgery. The anterior approach was used in 3, the double incision technique was used in 11. Average flexion: 131'43º; extension: -3'21º; pronation: 70'71º; supination: 74'29º; Quick - DASH Score: 17'04. Average follow - up: 25'5 months. Complication rate was 57'14%. The more frequent complications were local paresthesias which were solved spontaneously in all cases. The double incision technique group of patients had overall better results compared to the anterior approach group. Conclusions: Reattachment of acute distal biceps tendon ruptures shows acceptable postoperative results and high complications rate with low clinical relevance. These results seem to be better when the Boyd - Anderson double incision technique modified by Morrey is employed

RESUMO

ABSTRACT Rupture of the left ventricular wall after mitral valve replacement is an infrequent but lethal complication. Reporting correction technique of ventricular rupture with bovine pericardium patch secured with glue and without suturing: a 51 years-old female patient, with double rheumatic mitral lesion, severe stenosis and discrete insufficiency, who had a mitral valve replacement. During surgery, the patient presented a ventricular rupture of the posterior wall (atrioventricular disruption), which was successfully repaired using bovine pericardium with sutureless biological glue over the epicardium of the damaged area. Sixty months after surgery the patient has no symptoms.

La rotura del catéter peritoneal es una emergencia de la técnica, infrecuente, pero que obliga a una actuación inmediata ante la fuga del dializado y el riesgo de infección. Una intervención temprana y adecuada puede salvar los catéteres rotos sin interrumpir la diálisis peritoneal. Presentamos nuestra experiencia reparando catéteres dañados utilizando el kit de reparación de Peri-Path de Quinton® (Quinton Instrument Co., Tyco Healthcare Group LP. Mansfield, MA., U.S.A.) (AU)


Breakage of peritoneal catheters is an emergency of the technique that is uncommon but which requires immediate action when there is leakage of the dialysate and risk of infection. Early and adequate intervention can save broken catheters without interrupting peritoneal dialysis.
