RESUMO

Chilaiditi's sign is the presence of pseudopneumoperitoneum caused by colonic distension and interposition with the liver on radiographic films. Most patients with Chilaiditi's sign are asymptomatic. Chilaiditi's syndrome is defined as the development of abdominal pain or symptoms of bowel obstruction along with the presence of Chilaiditi's sign. It is a rare entity and it poses significant diagnostic challenges due to its similar radiographic appearance to pneumoperitoneum. Most patients with Chilaiditi syndrome can be managed conservatively. However, surgery is indicated for those who do not respond to conservative management or for suspicion of severe complications such as bowel ischaemia or perforation. In this case report, we described the surgical management of a patient who presented with bowel obstruction and significant hepatic displacement from Chilaiditi syndrome.

We read the interesting case study of interposition of the colon between the liver and the right diaphragm (Chilaiditi's sign) that was recently reported by Ruiz Pardo et al. in a 73-year-old male with epigastric pain, ectopic cholecystitis and segmental hepatic hypoplasia. The condition was managed by laparoscopic cholecystectomy with an uneventful outcome. This sign was first described in 1910 by the Greek radiologist Demetrius Chilaiditi. The incidence of Chilaiditi's sign in plain imaging studies is from 0.025 % to 0.28 % and may be reported by abdominal computed tomography in between 1.18 % and 2.40 % of cases.

BACKGROUND: Chilaiditi syndrome is a rare condition characterized by impaired fixation of the colon resulting in symptoms secondary to colonic interposition. It commonly presents with nonspecific abdominal pain and constipation, making clinical diagnosis difficult, especially in pregnancy. CASE REPORT: A 29-year-old afebrile woman in the week 28 of pregnancy presented with right-sided abdominal pain and constipation. With an Alvarado score of 6, the working diagnoses were acute appendicitis and intestinal obstruction. After a normal transabdominal ultrasound, emergent abdominal magnetic resonance imaging showed abnormal fixation of hepatic flexure wedged between the falciform ligament and liver. Without clinical, laboratory, and radiologic signs of complete obstruction or colonic perforation, conservative therapy was introduced and was successful. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Symptoms associated with Chilaiditi syndrome in pregnancy include nonspecific abdominal pain, and the correct preoperative diagnosis is difficult. The most common differential diagnoses are intestinal obstruction and acute appendicitis. It is important to diagnose Chilaiditi syndrome early because it can lead to severe complications, including intestinal obstruction, perforation, and ischemia. Therefore, in patients with right-sided abdominal pain with inconclusive transabdominal ultrasound, emergent magnetic resonance imaging leads to early diagnosis and treatment, minimizes maternal and fetal complications, avoids unnecessary explorations, and shortens the hospitalization.

RESUMO

Chilaiditi sign is a rare radiologic finding usually diagnosed incidentally. If it presents with symptoms such as nausea, vomiting, abdominal discomfort, and abdominal pain, it is called Chilaiditi syndrome. Here, we present an 81-year-old male patient who demonstrated Chilaiditi syndrome and signs after open-heart surgery.

El signo de Chilaiditi es la interposición de intestino delgado o colon entre el hígado y el diafragma. Este hallazgo incidental se ve en el 0,025-0,28% de las radiografías de tórax y abdomen. Entre los factores predisponentes destacan la ausencia, hiperlaxitud o elongación de los ligamentos suspensorios del colon transverso, el colon redundante o la elevación del hemidiafragma derecho. La atrofia o hipoplasia hepática es una causa infrecuente de este signo radiológico. Por otro lado, la vesícula biliar suprahepática es la localización más infrecuente de las ectopias vesiculares (0,026-0,7%). Se asocia con un desarrollo anormal del lóbulo hepático derecho como la agenesia, hipoplasia o atrofia. Presentamos el caso de un varón de 73 años con signo de Chilaiditi, hipoplasia hepática y colecistitis aguda litiásica en una vesícula biliar ectópica suprahepática

El signo de Chilaiditi consiste en la interposición permanente o temporal del colon, el intestino delgado o el estómago en el espacio hepatodiafragmático. Este hallazgo es raro, con una incidencia radiológica del 0,025-0,028% de la población. Cuando está asociado a una sintomatología gastrointestinal, se designa como síndrome de Chilaiditi. Las dos entidades son benignas y no requieren tratamiento quirúrgico


Chilaiditi sign means that a permanent or temporary interposition of the colon, small intestine or stomach is visible on the X-ray at the level of hepatodiaphragmatic space. This finding is rare, with a radiological incidence of 0.025 to 0.028% of the population. The Chilaiditi syndrome occurs with the onset of digestive symptoms such as abdominal pain or acute bowel obstruction. These two findings are benign and do not require surgical treatment
