RESUMO

Chest wall defects are an unusual complication of burn injury, generally seen after high-voltage electrical burns. Here we report the case of a 57-year-old man who developed costal chondritis and osteomyelitis 23 months after flame injury, which covered 50% of the total body surface area. Management included the resection of two ribs and coverage with an omental flap, overlaid by a split-thickness skin graft during the same surgical procedure. Describir la incidencia y manejo del trauma torácico severo en los Servicios de Cirugía del Hospital "Dr. Domingo Luciani" durante el período 2008-2009. Estudio retrospectivo, descriptivo, transversal. Se seleccionaron pacientes diagnósticados como traumatismo torácico en el centro, que presentaron fracturas del primer, segundo o tercer arco costal, fractura escapular, fractura esternal, ó la combinación de cualquiera de estas, desde enero de 2008 hasta diciembre 2009. Se evaluo edad, sexo, evento traumático, radiología, días hospitalización, toracotomía mínima, valores de laboratorio e ingreso a UTI. El grupo mayormente afectado fue 41 a 50 años con 31,1% y el sexo masculino (68,8%) 44% Accidentes en vehículos 50,9% de los casos evidenciaron fracturas del 2do-3er arco costal, 86,6% de los casos ameritaron drenaje torácico y 11% requirieron manejo en UTI. El TTS es una entidad seria, comprende fracturas de los 3 primeros arcos costales y/o fracturas de esternón y/o fracturas de escapula. Englobamos todas estas como TTS pues hay relación probada entre estas fracturas y traumas de alto impacto, causando lesiones potencialmente fatales. Consideramos la necesidad de redefinir el término de TTS, crear un protocolo de atención y estandarización para optimizar la atención del paciente lesionado.


To describe the incidence and management of severe thoracic trauma (STT) in General Surgery Service of Hospital "Dr. Domingo Luciani" during 2008-2009 period. A retrospective study descriptive, trasnversal. Patients diagnosed with severe thoracic trauma, (first, second or third rib fracture, sternal or scapular fracture or a combination of any), during January 2008 to December 2009. We reviewed, age, sex, traumatic event, radiologic images, hospitalization time, chest tube drainage, laboratory values and ICU admission. Group most affected was 41 to 50 years (31,1%) and males (68,8%). Traffic accidents caused 44% of trauma. In 50,9%, 2nd and 3rd rib fractures was present. 86,6% Needed chest tube dranage and 11% required ICU admission. The STT is a serious entity. It is defined by fracture of any of the 3 first ribs and/or sternal or scapular fracture. We included all these injuries as STT because there is a relationship between these fractures and high energy trauma, leading to potentially fatal consequences. We consider the need of redefining the STT term, creating an approach protocol and setting it up for optimizing the injured patient attention.

RESUMO

We present a case of primary costochondritis by Escherichia coli, in a diabetic woman. The patient suffered a urinary infection by E. coli which may have been the source of haematogenous spread. She received medical and surgical therapy and 5 months later, she was asymptomatic.

RESUMO

Costosternal chondrodynia (Tietze's syndrome variant) should be considered in those rare patients who present with severe breast pain several months after reconstructive breast surgery. The authors treated 25 cases, 3 of which are discussed in detail, where the origin of the pain was traced to the costosternal cartilages. The differential diagnosis and management of this hitherto undescribed problem after breast surgery are discussed.

RESUMO

Candida costochondritis is a rare disease of complex aetiology. Pathogenetic factors range from postoperative and posttraumatic complications to haematogenous dissemination in intravenous drug addicts. In addition to clinical examination, possible diagnostic procedures include scintiscan and magnetic resonance imaging. The treatment of choice is extensive debridement and resection of the structures affected by the inflammatory process. The long-term prognosis is good.

RESUMO

The authors report 4 cases of Tietze's syndrome. In 3 of them the syndrome onset during a rheumatic disease (psoriatic arthritis, sternoclavicular hyperostosis). It is thus necessary to distinguish between the idiopathic of unknown ethiology and the secondary form due to, a well defined disease, in 3 cases observed.

Involvement of the sternocostal joints was investigated in a series of 46 males and 18 females following median sternotomy annually in a 5-year period and compared to 62 age- and sex matched control subjects after one year solely. Both groups had a mean age of 49.2 years. The degenerative chondroarthropathy of sternocostal joints was 1.69-times more frequent in heart operated upon patients as compared to control persons. Based on radiographic findings the degenerative chondroarthropathies were classed in 0 to 3 severity groups. Were seen articular space narrowing in 95.4%, osteophytes of the margin of the articular surface in 88%, subchondral bony eburnation in 79% and cystic radiolucencies in 48.9% of sternocostal joints on poststernotomy standard plain film tomograms. Not occurred intraarticular gas phenomenon and bony ankylosis. The development of arthropathies is traced back to mechanical stress-related predisposing factors and stressed the importance of oculoneutral dehiscences that simulated normal roentgenanatomic projections and caused a masked insufficiency in sternocostal junctions.

RESUMO

Relapsing polychondritis is a rare multisystemic connective tissue disorder found mainly in Caucasians. Oriental patients with relapsing polychondritis are uncommon. We report 2 Chinese patients with relapsing polychondritis who had severe laryngotracheal narrowing requiring tracheostomy. Unlike most patients, there was marked costal chondritis resulting in depression of the anterior chest wall with minimal involvement of other joints. One of the patients had treatment with prednisolone, dapsone, azathioprine, cyclophosphamide and cyclosporin A at various stages of the disease without significant remission of the disease.
