RESUMO

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a benign proliferative disease of synovial joint, synovial sac and tendon sheath. PVNS is usually treated by surgery, but postoperative joint dysfunction and pain will be accompanied, which seriously affects the quality of life. The purpose of this review is to evaluate the effectiveness and safety of this intervention in patients with pain and dysfunction caused by postoperative symptoms of PVNS. METHODS: We will search the EMBASE, the Cochrane Library, Ovid MEDLINE, PubMed, Web of Science, Chinese Biomedical Literature Database (CBM), Chinese National Knowledge Infrastructure (CNKI), Wanfang Database, the Chongqing VIP (VIP), the US National Institute of Health, the NIH clinical registry Clinical Trials, the ICTRP, and the Australian New Zealand Clinical Trials Registry and the Chinese clinical registry, from their inception to 1st July 2020. Randomized controlled trials that include patients with postoperative symptoms of pigmented villonodular synovitis receiving acupuncture therapy versus a control group will be included. The selection of studies, data extraction and risk of bias assessment will be conducted by 2 independent researchers. A third review author resolved disagreements. The dichotomous data will be presented as risk ratios with 95% confidence intervals (CIs) and the continuous data will be presented as weighted mean differences or standardized mean differences with 95% CIs. Evidence quality will be evaluated using the GRADE system. RESULTS: The results will be disseminated through a peer-reviewed journal publication. CONCLUSIONS: This systematic review will provide updated evidence of various types of acupuncture specifically focuses on its effectiveness and safety for patients' pain and dysfunction caused by post-operation of pigmented villonodular synovitis. ETHICS AND DISSEMINATION: Ethical approval is not necessary as this review will not require data from individual patients. STUDY DESIGN: Case report. BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare, benign disorder characterized by idiopathic proliferation affecting the synovium of joints, tendon sheaths, and bursae. Diagnosing PVNS in the knee is difficult because the clinical presentation and symptoms mimic those of more common disorders at the joint, such as internal derangements or arthritis. Operative treatment of PVNS typically consists of arthroscopic or open synovectomy, but no reports of postoperative rehabilitation exist. CASE DESCRIPTION: This case describes the postoperative rehabilitation of a 46-year-old female who had left knee surgery secondary to PVNS. Rehabilitation consisted of combined manual therapy, exercise, and gait training to improve function and gait, and cognitive-behavioral techniques to improve self-efficacy. OUTCOMES: All impairments improved in 2.5 months of physical therapy to normal, and the patient estimated 80% to 90% return to function. DISCUSSION: This patient obtained excellent outcomes in 2.5 months of physical therapy following surgery for PVNS. Although no firm conclusions can be drawn from a case report, this patient responded well to a biopsychosocial approach that combined physical therapy with cognitive-behavioral techniques. Os autores apresentam sete pacientes portadores de Sinovite Vilonodular Pigmentada Localizada (SVNPL) tratados através de ressecção artroscópica, entre o período de junho de 1994 e Outubro de 2001. A avaliação inicial, os sintomas variaram desde dor difusa, até um aumento do volume localizado, com ou sem bloqueio mecânico do movimento articular. A ressonância magnética auxiliou na localização das lesões e no controle pós-operatório. O diagnóstico foi confirmado pelo exame anatomo-patológico. Na última avaliação nenhum dos pacientes apresentou recidivas dos sintomas do pré-operatório, nem no exame por imagem com ressonância magnética. Apesar da casuística pequena, própria dessa afecção, os autores acreditam que a excisão artroscópica da SVNPL é um método eficaz, de baixa morbidade, proporciona remissão dos sintomas e com pouca possibilidade de recidivas.

We have reviewed the results of arthroscopic treatment of pigmented villonodular synovitis (PVNS) with reference to both recurrence and to function. Between 1985 and 1995, a single surgeon treated eight patients. At an average 5-year follow-up, all patients were interviewed and had assessment of Hospital for Special Surgery (HSS) knee score for both the affected and unaffected knees. Also recorded were age, sex, and whether disease was recurrent, localized, or diffuse. Disease recurred in 4 patients, all with diffuse PVNS, and 3 of them required a further arthroscopic synovectomy at a mean of 16 months after the index procedure. All patients had good or excellent functional results. There was no significant difference between HSS knee scores for affected and unaffected knees. Arthroscopic synovectomy is a successful treatment in patients with localized PVNS of the knee and results in a knee that is functionally not different from its unaffected partner.
