RESUMO

OBJECTIVE: The Unified Medical Language System (UMLS) is 1 of the most successful, collaborative efforts of terminology resource development in biomedicine. The present study aims to 1) survey historical footprints, emerging technologies, and the existing challenges in the use of UMLS resources and tools, and 2) present potential future directions. MATERIALS AND METHODS: We collected 10 469 bibliographic records published between 1986 and 2019, using a Web of Science database. graph analysis, data visualization, and text mining to analyze domain-level citations, subject categories, keyword co-occurrence and bursts, document co-citation networks, and landmark papers. RESULTS: The findings show that the development of UMLS resources and tools have been led by interdisciplinary collaboration among medicine, biology, and computer science. Efforts encompassing multiple disciplines, such as medical informatics, biochemical sciences, and genetics, were the driving forces behind the domain's growth. The following topics were found to be the dominant research themes from the early phases to mid-phases: 1) development and extension of ontologies and 2) enhancing the integrity and accessibility of these resources. Knowledge discovery using machine learning and natural language processing and applications in broader contexts such as drug safety surveillance have recently been receiving increasing attention. DISCUSSION: Our analysis confirms that while reaching its scientific maturity, UMLS research aims to boundary-span to more variety in the biomedical context. We also made some recommendations for editorship and authorship in the domain. CONCLUSION: The present study provides a systematic approach to map the intellectual growth of science, as well as a self-explanatory bibliometric profile of the published UMLS literature. It also suggests potential future directions. Using the findings of this study, the scientific community can better align the studies within the emerging agenda and current challenges.

BACKGROUND: As Director of the US National Library of Medicine (NLM) for 30 years, Dr. Donald A. B. Lindberg was instrumental in bringing biomedical research and healthcare worldwide into the age of genomic and translational medicine through the informatics systems developed by the NLM. Lindberg opened free access and worldwide public dissemination of all the NLM's biomedical literature and databases, thus helping transform not only biomedical research like the Human Genome Project and its successors, but also the practices of medicine and healthcare internationally. Guiding, leading, and teaching-by-example at national, regional, and global levels of biomedical and healthcare informatics, Lindberg helped coalesce a dynamic discipline that provides a foundation for the human understanding which promotes the future health of our world. OBJECTIVES: To provide historical insight into the scientific, technological, and practical clinical accomplishments of Donald Lindberg, and to describe how this led to contributions in the worldwide interdisciplinary evolution of informatics, and its impact on the biosciences and practices of medicine, nursing, and other healthcare-related disciplines. METHODS: Review and comment on the publications, scientific contributions, and leadership of Donald Lindberg in the evolution of biomedical and health informatics which anticipate the vision, scholarship, research in the field, and represent the deeply ethical humanism he exhibited throughout his life. These were essential in producing the informatics systems, such as the Unified Medical Language System (UMLS), MEDLINE, PubMed, PubMed Central, and ClinicalTrials.gov, which, together with NLM training programs and conferences, made possible the interactions among researchers and practitioners leading to the past quarter-century of rapid and dramatic advances in biomedical scientific inquiry and clinical discoveries, openly shared across the globe. CONCLUSION: Dr. Lindberg was a uniquely talented physician and pioneering researcher in biomedical and health informatics. As the main leader in developing and funding innovative informatics research for more than 30 years as Director of the National Library of Medicine, he helped bring together the most creative interdisciplinary researchers to bridge the worlds of biomedical research, education, and clinical practice. Lindberg's emphasis on open-access to the biomedical literature through publicly shared computer-mediated methods of search and inquiry are seen as an example of ethical scientific openness.

RESUMO

The American College of Medical Informatics is an honorary society established to recognize those who have made sustained contributions to the field. Its highest award, for lifetime achievement and contributions to the discipline of medical informatics, is the Morris F Collen Award. Dr Collen's own efforts as a pioneer in the field stand out as the embodiment of creativity, intellectual rigor, perseverance, and personal integrity. The Collen Award, given once a year, honors an individual whose attainments have, throughout a whole career, substantially advanced the science and art of biomedical informatics. In 2009, the college was proud to present the Collen Award to Betsy Humphreys, MLS, deputy director of the National Library of Medicine. Ms Humphreys has dedicated her career to enabling more effective integration and exchange of electronic information. Her work has involved new knowledge sources and innovative strategies for advancing health data standards to accomplish these goals. Ms Humphreys becomes the first librarian to receive the Collen Award. Dr Collen, on the occasion of his 96th birthday, personally presented the award to Ms Humphreys.

RESUMO

MetaMap is a widely available program providing access to the concepts in the unified medical language system (UMLS) Metathesaurus from biomedical text. This study reports on MetaMap's evolution over more than a decade, concentrating on those features arising out of the research needs of the biomedical informatics community both within and outside of the National Library of Medicine. Such features include the detection of author-defined acronyms/abbreviations, the ability to browse the Metathesaurus for concepts even tenuously related to input text, the detection of negation in situations in which the polarity of predications is important, word sense disambiguation (WSD), and various technical and algorithmic features. Near-term plans for MetaMap development include the incorporation of chemical name recognition and enhanced WSD.

Nursing's contributions to the development of quality of care measures, guidelines, and standardized languages are described and current and future opportunities for nursing are discussed.

RESUMO

In 1986, the National Library of Medicine (NLM) assembled a large multidisciplinary, multisite team to work on the Unified Medical Language System (UMLS), a collaborative research project aimed at reducing fundamental barriers to the application of computers to medicine. Beyond its tangible products, the UMLS Knowledge Sources, and its influence on the field of informatics, the UMLS project is an interesting case study in collaborative research and development. It illustrates the strengths and challenges of substantive collaboration among widely distributed research groups. Over the past decade, advances in computing and communications have minimized the technical difficulties associated with UMLS collaboration and also facilitated the development, dissemination, and use of the UMLS Knowledge Sources. The spread of the World Wide Web has increased the visibility of the information access problems caused by multiple vocabularies and many information sources which are the focus of UMLS work. The time is propitious for building on UMLS accomplishments and making more progress on the informatics research issues first highlighted by the UMLS project more than 10 years ago.

Conceptual connections between users and information sources depend on an accurate representation of the content of available information sources, an accurate representation of specific user information needs, and the ability to match the two. Establishing such connections is a principal function of medical librarians. The goal of the National Library of Medicine's Unified Medical Language System (UMLS) project is to facilitate the development of conceptual connections between users and relevant machine-readable information. The UMLS model involves a combination of three centrally developed Knowledge Sources (a Metathesaurus, a Semantic Network, and an Information Sources Map) and a variety of smart interface programs that make use of these Knowledge Sources to help users in different environments find machine-readable information relevant to their particular practice or research problems. The third experimental edition of the UMLS Knowledge Sources was issued in the fall of 1992. Current priorities for the UMLS project include developing applications that make use of the Knowledge Sources and using feedback from these applications to guide ongoing enhancement and expansion of the Knowledge Sources. Medical librarians are involved heavily in the direction of the UMLS project, in the development of the Knowledge Sources, and in their experimental application. The involvement of librarians in reviewing, testing, and providing feedback on UMLS products will increase the likelihood that the UMLS project will achieve its goal of improving access to machine-readable biomedical information.
