Isquemia crítica de un pie causada por policitemia vera y tratado con Heberprot-P® / Critical foot ischemia caused by Polycythemia vera and treated with Heberprot-P®

RESUMEN

La policitemia vera es un incremento anormal de las células sanguíneas, principalmente de los glóbulos rojos, como resultado de un aumento en su producción en la médula ósea. Se plantea que la sangre es más espesa de lo normal, lo que puede lentificar la velocidad del flujo sanguíneo a través de las venas y arterias. El propósito de este trabajo es describir la evolución de un paciente amputado por isquemia critica, portador de policitemia vera y tratado con Heberprot-P®. Se trata de un individuo de 74 años de edad, blanco, con antecedentes de cardiopatía isquémica y de policitemia vera, que ingresa en el Instituto Nacional de Angiología y Cirugía Vascular, con el diagnóstico de isquemia critica del pie derecho que lo llevó a la necrosis del antepié. Se le realizó examen físico y se indicaron los exámenes complementarios correspondientes. Fue necesario realizarle una amputación transmetatarsiana cerrada. Evolutivamente presentó necrosis y dehiscencia de los bordes de la herida. Se le realizó desbridamiento quirúrgico y se comenzó la infiltración intralesional del Heberprot-P®;de 75 µg, de forma ambulatoria, tres veces por semana. Se aplicaron 23 dosis. Recibió el alta de la consulta externa con una granulación de la lesión de 100 por ciento y 15 días después, el cierre completo del muñón transmetatarsiano. Durante el tratamiento no se registró ninguna reacción adversa. El Heberprot-P®;es eficaz en el tratamiento del paciente con lesión compleja, de etiología diferente a la de las úlceras del pie diabético(AU)

ABSTRACT

Polycythemia vera is an abnormal increase in the blood cells, principally in the red blood cells, as a result of an increase of their production in the bone marrow. It is stated that blood is thicker than normal what can slow down the speed of the blood flow through veins and arteries. The objective of this work is to describe the evolution of a patient amputated by critical ischemia, carrier of Polycythemia vera and treated with Heberprot-P®. We are referring to a 74 years old white male patient with antecedents of ischemic cardiopathy and polycythemia vera that is admitted to the National Institute of Angiology and Vascular Surgery with the diagnosis of critical ischemia of the right foot that caused necrosis of the forefoot. Physical examination was done and the corresponding complementary tests were indicated. It was necessary to perform closed transmetatarsal amputation. During the evolution, he presented necrosis and dehiscence of the wound edges. Surgical debridement was performed and intralesion infiltration of Heberprot-P®;75 µg began in an ambulatory form 3 times a week. 23 doses were applied. The patient was discharged from outpatient consultation with a 100 percent granulation of the lesion and, 15 days later, a complete lock of the transmetatarsal amputation stump was achieved. No adverse reaction was registered during the treatment. Heberprot-P® is effective in the treatment of a patient with a complex lesion, of different etiology from the ones of ulcers of the diabetic foot(AU)
