RESUMO

The objective of this study was to test the performance of a prototype vision system in phenotypically diverse beef and lamb carcasses against visual grading of eye muscle area (EMA), marbling and chemical intramuscular fat (IMF%). Validation in beef demonstrated that the camera prototype in combination with analytical techniques enabled prediction of EMA (r2 = 0.83, RMSEP = 6.4 cm2), MSA marbling (r2 = 0.76, RMSEP = 66.1), AUS-MEAT marbling (r2 = 0.70, RMSEP = 0.74) and chemical IMF% (r2 = 0.78, RMSEP = 1.85%). Accuracy was also maintained on validation with all four traits displaying minimal bias of -3.6, 6.3, 0.07 and - 0.01, for EMA, MSA marbling, AUS-MEAT marbling and IMF% respectively. Preliminary analysis in lamb indicates potential of the system for the prediction of EMA (r2 = 0.41, RMSEP = 1.87) and IMF% (r2 = 0.28, RMSEP = 1.10), however further work to standardise image acquisition and environmental conditions is required.

Generic dry-contact ear-EEG allows for discreet, user-friendly, unobtrusive, cost-effective and convenient recordings of EEG in real-life settings. In this study we introduce a new generic earpiece design with larger internal ear electrode distances, resulting in an increased spatial coverage compared to previous generic earpiece designs. The signal quality of ear-Fpz, within-ear (the measuring and reference electrode located in the same ear) and cross-ear (the measuring electrodes located in one ear and the reference electrode in the opposite ear) electrode configurations of the developed generic earpiece was evaluated with auditory steady-state responses (ASSR) and compared to dry-contact cEEGrid. Ten subjects with different ear sizes were included. The recordings were performed in a sleep setup, where the subjects were lying on a bed and the effect of sleeping position (back vs. sides) was investigated. We found that the generic earpiece attained statistically significant ASSRs with ear-Fpz, within-ear and cross-ear electrode configurations. However, the dry-contact cEEGrid achieved significantly higher average ASSR signal-to-noise ratio (SNR) compared to the generic earpiece. Additionally, this study showed no significant difference between back and side positions for the ear-EEG.

OBJECTIVE: To evaluate the cumulative incidence of nephrotic-range albuminuria (NRA), the frequency of remission, and the impact on progression, we analyzed data from a prospective cohort study of type 1 diabetic patients with diabetic nephropathy. RESEARCH DESIGN AND METHODS: All of the albuminuric type 1 diabetic patients (n = 321, 121 women), who had at least yearly measurements of glomerular filtration rate (GFR) with a (51)Cr-EDTA plasma clearance technique and were followed for at least 3 years, were evaluated. NRA, defined as persistent albuminuria >2,500 mg/24 h, occurred in 126 patients (35 women) aged (mean +/- SD) 34 +/- 8 years, with duration of diabetes 22 +/- 8 years and follow-up time from onset of NRA (median [range]) 8.7 (3.0-20.9) years. Remission of NRA was defined as sustained albuminuria <600 mg/24 h for at least 1 year. RESULTS: The cumulative incidence of NRA was 39%. Remission was induced in 28 of 126 (22%) patients; 21 were predominantly treated with ACE inhibitors, 7 with non-ACE inhibitor medications. Remission lasted 3.6 (1.0-18.1) years. More women (37%) than men (16%) obtained remission (P = 0.01). In the remission group compared with the no-remission group, mean arterial blood pressure (mean +/- SEM) was reduced (102 +/- 1 vs. 106 +/- 1 mmHg, P < 0.01), the rate of decline in GFR was diminished (3.8 +/- 0.6 vs. 7.5 +/- 0.5 ml x min(-1) x year(-1), P < 0.001), and serum cholesterol was lower (5.3 +/- 0.2 vs. 6.1 +/- 0.1 mmol/l, P < 0.01) during the whole follow-up period. No difference in glycemic control was found between groups (HbA(1c) 9.2 vs. 9.4%, NS). CONCLUSIONS: In contrast to observations made before the use of antihypertensive treatment, our prospective observational study suggests that aggressive antihypertensive treatment with and without ACE inhibitors can induce long-lasting remission in a sizeable fraction of type 1 diabetic patients with NRA. The group of patients obtaining remission is characterized by slow progression of diabetic nephropathy and improved cardiovascular risk profile.

A hypothyroid patient with severe coronary disease is described. Coronary artery bypass was undertaken without preoperative treatment with thyroid hormone. Coronary artery bypass surgery may be undertaken in hypothyroid patients without significant risks. As a rule, preoperative administration of thyroid hormone should be omitted on account of the risk of aggravating the ischaemic cardiac condition. Treatment with L-thyroxin is instituted rapidly postoperatively and, in the great majority of cases, it is possible to obtain complete thyroid hormone substitution therapy after coronary revascularisation has been performed. It is emphasized that all middle-aged women with raised serum cholesterol should be screened for myxoedema even if they do not appear to be hypothyroid clinically.

RESUMO

Moderate hyperhomocysteinaemia is associated with hypothyroidism. Increased levels of homocysteine might contribute to a greater cardiovascular risk. This paper reports a case of transient myxoedema, owing to postpartum thyroiditis. The fluctuation of P-homocysteine was inversely correlated to the free thyroxine index. We recommend TSH screening of patients with unexplained hyperhomocysteinaemia.

RESUMO

Each of eight adipose but otherwise normal patients on a reducing diet received daily oral doses of frusemide for four days; the degree of hypercalciuria was found to be significant during the first three days, but on day four the calcium excretion in the urine had become normal though frusemide was still being given. The rates of excretion of sodium and calcium in the urine were closely correlated.

A series of mixed-metal complexes coupling ruthenium light absorbers to platinum reactive metal sites through polyazine bridging ligands have been prepared of the form [(tpy)RuCl(BL)PtCl(2)](PF(6)) (BL = 2,3-bis(2-pyridyl)pyrazine (dpp), 2,3-bis(2-pyridyl)quinoxaline (dpq), 2,3-bis(2-pyridyl)benzoquinoxaline (dpb); tpy = 2,2':6',2' '-terpyridine). These systems possess electron-rich Ru metal centers bound to five polyazine nitrogens and one chloride ligand. This leads to complexes with low-energy Ru --> BL charge-transfer bands that are tunable with BL variation occurring at 544, 632, and 682 nm for dpp, dpq, and dpb, respectively. This tuning of the charge-transfer energy results from a stabilization of the BL(pi) orbitals in this series as evidenced by the cathodic shift in the first reduction of these complexes occurring at -0.50, -0.32, and -0.20 V vs Ag/AgCl, for dpp, dpq, and dpb, respectively. The chlorides bound to the Pt(II) center are substitutionally labile giving these complexes the ability to covalently bind to DNA. All three title bimetallics, [(tpy)RuCl(BL)PtCl(2)](PF(6)), avidly bind double-stranded DNA with t(1/2) = 1-2 min, substantially reducing the migration of DNA through an agarose gel. Details of the synthetic methods, FAB MS data, spectroscopic and electrochemical properties, and DNA binding studies are presented.
