BACKGROUND: This study constitutes a secondary analysis of a prospective cohort aiming to evaluate the potential correlation between nutritional risk and status at admission with the occurrence of post-discharge complications and hospital readmissions in children receiving care at high resource Centres. METHODS: Data was collected from 5 Canadian tertiary pediatric Centers between 2012 and 2016. Nutritional risk and status were evaluated at hospital admission with validated tools (STRONGkids and Subjective Global Nutrition Assessment [SGNA]) and anthropometric measurements. Thirty days after discharge, occurrence of post-discharge complications and hospital readmission were documented. RESULTS: A total of 360 participants were included in the study (median age, 6.1 years; median length of stay, 5 days). Following discharge, 24.1% experienced complications and 19.5% were readmitted to the hospital. The odds of experiencing complications were nearly tripled for participants with a high nutritional risk compared to a low risk (OR = 2.85; 95% CI [1.08-7.54]; P = 0.035) and those whose caregivers reported having a poor compared to a good appetite (OR = 2.96; 95% CI [1.59-5.50]; P < 0.001). According to SGNA, patients identified as malnourished had significantly higher odds of complications (OR, 1.92; 95% CI, 1.15-3.20; P = 0.013) and hospital readmission (OR, 1.95; 95% CI, 1.12-3.39; P = 0.017) than to those well-nourished. CONCLUSIONS: This study showed that complications and readmission post-discharge are common, and these are more likely to occur in malnourished children compared to their well-nourished counterparts. Enhancing nutritional care during admission, at discharge and in the community may be an area for future outcome optimization.

BACKGROUND: The COVID-19 pandemic has necessitated rapid changes in healthcare delivery in the United States, including changes in the care of hospitalized children. The objectives of this study were to identify major changes in healthcare delivery for hospitalized children during the COVID-19 pandemic, identify lessons learned from these changes, and compare and contrast the experiences of children's and community hospitals. METHODS: We purposefully sampled participants from both community and children's hospitals serving pediatric patients in the six U.S. states with the highest COVID-19 hospitalization rates at the onset of the pandemic. We recruited 2-3 participants from each hospital (mix of administrators, front-line physicians, nurses, and parents/caregivers) for semi-structured interviews. We analyzed interview data using constant comparative methods to identify major themes. RESULTS: We interviewed 30 participants from 12 hospitals. Participants described how leaders rapidly developed new hospital policies (e.g., directing use of personal protective equipment) and how this was facilitated by reviewing internal and external data frequently and engaging all relevant stakeholders. Hospital leaders optimized communication through regular, transparent, multi-modal, and bi-directional communication. Clinicians increased use of videoconference and telehealth to facilitate physical distancing, but these technologies may have disadvantaged non-English speakers. Due to declining volumes of hospitalized children and surges of adult patients, clinicians newly provided care for hospitalized adults. This was facilitated by developing care teams supported by adult hospitalists, multidisciplinary support via videoconference, and educational resources. Participants described how the pandemic negatively impacted clinicians' mental health, and they stressed the importance of mental health resources and wellness activities/spaces. CONCLUSIONS: We identified several major changes in inpatient pediatric care delivery during the COVID-19 pandemic, including the adoption of new hospital policies, video communication, staffing models, education strategies, and staff mental health supports. We outline important lessons learned, including strategies for successfully developing new policies, effectively communicating with staff, and supporting clinicians' expanding scope of practice. Potentially important focus areas in pandemic recovery include assessing and supporting clinicians' mental health and well-being, re-evaluating trainees' skills/competencies, and adapting educational strategies as needed. These findings can help guide hospital leaders in supporting pandemic recovery and addressing future crises.

PURPOSE: The optimal pain management concept in children after tonsil surgery is controversial. Ibuprofen on an "around-the-clock" basis has been suggested to control postoperative pain sufficiently. Therefore, we established a standard scheme with weight-adapted recommended maximum ibuprofen dose. A reliable assessment of pain intensity can be performed with the Children's and Infants' Postoperative Pain Scale (CHIPPS) in children < 5 years, or with the Faces Pain Scale-Revised (FPS-R) in children aged ≥ 5 years. The Parents' Postoperative Pain Measure (PPPM-D) may be a useful tool for both age groups. We hypothesized that not more than 30% of the children would need an opioid rescue medication during their in-hospital stay and analyzed the consistency of the PPPM-D with other pain scales. METHODS: We included 158 in-patients aged 2-12 years. Ibuprofen was orally administered every 8 h. Three times daily, pain scores were assessed by CHIPPS or FPS-R, respectively. The PPPM-D was used in all children. Exceeding the cut-off value in one of the tools was regarded as relevant pain. RESULTS: A rescue medication was needed in 82.1% of children after tonsillectomy and 51.3% of children after tonsillotomy (P < 0.001). The cut-off value for relevant pain was mostly exceeded in the PPPM-D, but its overall concordance to the reference scales was low. CONCLUSION: High-dose ibuprofen "around-the-clock" is insufficient to control pain in children after tonsil surgery. Research is needed to find an optimal schema for management and assessment of postoperative pain.

PURPOSE: To thematically describe parent-clinician communication during a child's first burn dressing change following emergency department presentation. DESIGN AND METHODS: An observational study of parent-clinician communication during the first burn dressing change at a tertiary children's hospital. Verbal communication between those present at the dressing change for 87 families, was audio recorded. The recordings were transcribed verbatim and transcripts were analysed within NVivo11 qualitative data analysis software using qualitative content analysis. FINDINGS: Three themes, underpinned by parent-clinician rapport-building, were identified. Firstly, knowledge sharing was demonstrated: Clinicians frequently informed the parent about the state of the child's wound, what the procedure will involve, and need for future treatment. Comparatively, parents informed the clinician about their child's temperament and coping since the accident. Secondly, child procedural distress management was discussed: Clinicians and parents had expectations about the likelihood of procedural distress, which was also related to communication about how to prevent and interpret procedural distress (i.e., pain/fear). Finally, parents communicated to clinicians about their own distress, worry and uncertainty, from the accident and wound care. Parents also communicated guilt and blame in relation to injury responsibility. CONCLUSIONS: This study provides a description of parent-clinician communication during paediatric burn wound care. PRACTICAL IMPLICATIONS: The results can assist healthcare professionals to be prepared for a range of conversations with parents during potentially distressing paediatric medical procedures.

PURPOSE: With the recent increase in interest in patient safety, prevention of falls in hospitalized children has become important. This study aimed to identify the incidence rate of falls among hospitalized children and explore fall-related characteristics. METHODS: This retrospective descriptive study analyzed the medical information of 18,119 patients aged <18 years admitted to a general hospital in South Korea from electronic medical records and fall event reports between January 1, 2018, and September 30, 2023. The study variables included the general and clinical characteristics of the fall group and fall-related characteristics of the fall events. This study employed descriptive statistics and a chi-square test using IBM SPSS version 26.0. RESULTS: Among the patients, 82 fall events were identified. Therefore, the fall incidence rate was 4.5 falls per 1,000 patients. Furthermore, a statistically significant difference was found concerning the type of injury sustained between children >1 year old and those <1 year old, and most cases had no damage or bruises. More cases were found in which falls occurred two days after hospitalization in winter and summer than on the day or the day after hospitalization, which was a statistically significant difference. In addition, fall prevalence was higher between 8 am and 4 pm, and when no caregiver was present. CONCLUSION: Based on the results of this study, education and interventions to prevent fall events in infants and toddlers should persist throughout hospitalization, and it is necessary to guide continuous management and observation of the caregivers.

Objective: To evaluate children and adolescents with polytrauma and fractures of the pelvis and proximal and diaphyseal femur and correlate the impact of these conditions and clinical outcomes. Methods: Retrospective study carried out in a public hospital in Taboão da Serra (SP), with pediatric patients with polytrauma from January 2012 to December 2021. In total, 44 patients were evaluated, 70.44% boys and 29.55% girls, aged from 12 to 17 years. Results: Diaphyseal fracture of the femur affected 70.44% of the patients, mainly caused by a fall from a height (56.81%). Linear external fixation was the most used treatment (45.45%). All patients were discharged from hospital. Conclusion: We found essential sociodemographic information: 84.11% of patients did not have associated injuries; 88.63% were hospitalized from 3 to 11 days; 90.91% did not need to be admitted to an ICU, 77.27% did not need reoperation, and 22.73% underwent another surgery; 45.45% used the external fixator to stabilize injuries; 11.36% converted the external fixator to the intramedullary nail; 9.09% needed an intramedullary nail remover; 2.27% converted to a plate (bilateral) and 2.27% to a rigid nail; 2.27% had loss of reduction and revision with rod; 2.27% underwent corrective osteotomy; 2.27% had clinical hospitalization; 2.27% had osteonecrosis of the femoral head and screws removed; 2.27% removed the plate. No deaths were recorded. Level of Evidence II, Retrospective Study.


Objetivo: Avaliar crianças e adolescentes politraumatizados com fraturas da pelve, proximal e diafisária do fêmur e correlacionar o impacto dessas condições e desfechos clínicos. Métodos: Estudo retrospectivo realizado em hospital público de Taboão da Serra (SP), com pacientes pediátricos politraumatizados entre janeiro de 2012 e dezembro de 2021. Avaliaram-se 44 pacientes, 70,44% meninos e 29,55% meninas, de 12 a 17 anos. Resultados: A fratura diafisária fechada do fêmur acometeu 70,44%, sendo causada principalmente por queda de altura (56,81%). A fixação externa linear foi o tratamento mais utilizado (45,45%). Todos os pacientes receberam alta hospitalar. Conclusão: Identificaram-se importantes informações sociodemográficas: 84,11% dos pacientes não apresentaram lesões associadas; 88,63% ficaram internados de 3 a 11 dias; 90,91% não necessitaram de internação em unidade de terapia intensiva (UTI), 77,27% não precisaram ser reoperados; 22,73% realizaram nova cirurgia; 45,45% utilizaram fixador externo para estabilização de lesões;11,36% converteram o fixador externo para a haste intramedular; 9,09% precisaram remover as hastes intramedulares; 2,27% converteram para placa (bilateral) e 2,27% para haste rígida; 2,27% tiveram perda de redução e revisão com haste; 2,27% realizaram osteotomia corretiva; 2,27% tiveram internação clínica; 2,27% tiveram osteonecrose da cabeça femoral e parafusos removidos; e 2,27% retiraram a placa. (1) Background: Hospitalists are healthcare providers who focus on hospitalized patients, but research on the roles of pediatric hospitalists is lacking. This study investigates the role of a supervisor-type hospitalist in a pediatric hematology/oncology ward at a tertiary children's hospital, assessing the impact on satisfaction levels among patient caregivers, resident physicians, and nurses. (2) Methods: A retrospective analysis and online surveys were conducted to assess satisfaction levels before and after the introduction of hospitalists in the Department of Pediatric Hematology/Oncology at Seoul National University Children's Hospital in the Republic of Korea. (3) Results: The introduction of hospitalists led to a 19.3% reduction in prescription error interventions over six months. Unexpected transfers to the intensive care unit decreased from 1.4% to 0.7% (p = 0.229). Patient caregivers reported elevated satisfaction levels with physicians (rated 8.47/10), and there was a significant enhancement in overall satisfaction among nurses (increasing from 3.23 to 4.23/5, p < 0.001). The majority of resident physicians (83.3%) expressed contentment with the hospitalist system, with 77% indicating an interest in transitioning to a hospitalist role. However, these resident physicians also expressed concerns regarding job stability. (4) Conclusions: Supervisor-type pediatric hospitalists have the potential to elevate satisfaction levels not only among patient caregivers but also among nurses and resident physicians, showing promise in improving medical care quality. Nonetheless, ensuring favorable perception and securing job stability within the hospitalist system are pivotal for achieving successful implementation.

OBJECTIVE: We sought to determine if corticosteroid administration is associated with a SARS-CoV-2 nucleic acid test-positive result and to describe therapies administered to SARS-CoV-2 infected children. METHODS: We collected cross-sectional data from participants recruited in 41 pediatric emergency departments (ED) in 10 countries between March 2020 and June 2021. Participants were <18 years old, had signs or symptoms of, or risk factors for acute SARS-CoV-2 infection, and had nucleic acid testing performed. To determine if SARS-CoV-2 test status was independently associated with corticosteroid administration, we used a multivariable conditional logistic regression model matched by study site to compare treatments administered based on SARS-CoV-2 test and disposition status. This analysis was repeated for the subgroup of study participants who were hospitalized. RESULTS: 30.3% (3,121/10,315) of participants were SARS-CoV-2-positive. Although remdesivir was more commonly administered to SARS-CoV-2-positive children, use was infrequent (25/3120 [0.8%] vs 1/7188 [0.01%]; P = .001). Corticosteroid use was less common among SARS-CoV-2-positive children (219/3120 [7.0%] vs 759/7190 [10.6%]; P < .001). Among hospitalized children, there were no differences in provision of inotropes, respiratory support, chest drainage or extracorporeal membrane oxygenation between groups. Corticosteroid administration was associated with age, history of asthma, wheezing, study month, hospitalization and intensive care unit admission; it was not associated with a positive SARS-CoV-2 test result overall (aOR: 0.91; 95%CI: 0.74, 1.12) or among the subgroup of those hospitalized (aOR: 1.04; 95%CI: 0.75, 1.44). CONCLUSIONS: Few disease-specific treatments are provided to SARS-CoV-2-positive children; clinical trials evaluating therapies in children are urgently needed.

BACKGROUND: Hospital infections in pediatric units increase the length of hospital stay and the use of antibiotics, and this causes exposure to more procedures. This study was aimed to determine the microorganisms represented in the hand flora of pediatric patients at the beginning of hospitalization and before discharge. METHODS: The study was designed as a prospective cross-sectional study. This prospective study was performed with 124 pediatric patients. After completion of the admission procedures, an initial sample was taken from the hands of the hospitalized patients. Another sample was taken from the patients just before discharging. RESULTS: Growth of coagulase-negative staphylococcus (CNS) was observed in the culture samples of 28 patients. Cultures from 23 patients showed different microorganisms such as Staphylococcus aureus, Escherichia coli, and S. epidermis. Examination of final discharge cultures showed CNS in 43 patients, S. aureus in 5 patients, E. coli in 8 patients, Acinetobacter baumannii in 11 patients, and Kocuria rhizophila, K. kristinae, Candida spp., Pseudomonas spp., and Enterococcus in 1 patient. CONCLUSION: The cultures from samples obtained at discharge showed the presence of antibiotic-resistant pathogenic microorganisms causing healthcare associated infection.

Acute gastroenteritis (AG) is responsible for 525,000 deaths worldwide in children under-5-years and is caused by the Human Cosavirus (HCoSV; family Picornaviridae, Genus Cosavirus). Although its health importance, a significant percentage of diarrhea cases (≈ 40 %) still of unknown etiology. In Brazil, few studies have reported HCoSV-A sequences analyzing partial 5' UTR. This study characterized the first near-complete genome of a Cosavirus A (strain AM326) from a child hospitalized with AG in Amazonas state, Northern Brazil. High throughput sequencing (HTS) was performed using the HiSeq™ 2500 platform (Illumina) in one fecal specimen collected from the Surveillance of Rotavirus Network of the Evandro Chagas Institute collected in 2017. Sequence reads were assembled by the De Novo approach using three distinct algorithmic (IDBA-UD, Spades, and MegaHit). The final contig was recovered from the HCoSV-AM326 sample revealing 7,735 nt in length (SRA number SRR12535029; GenBank MT023104) and the genetic characterization, as well as phylogenetic analysis demonstrated a new variant strain from Brazil, highlighting the association of HCoSV-A as a possible causative agent of AG. This finding demonstrates the importance of the metagenomic approach to elucidate cases of diarrhea without a defined etiology, as well as providing a better understanding about the virus genetics, evolution and epidemiology.

PURPOSE: The purpose of this study was to develop content for safety education to prevent hospital safety accidents among hospitalized children and to investigate the status of safety education performed by nurses. METHODS: First, the Delphi method was used, with 18 experts, to develop educational contents for preventing safety accidents. Second, an exploratory survey was performed of the actual status of safety education for preventing safety accidents among hospitalized children using a questionnaire developed based on the Delphi method. The participants of this study were 159 nurses with at least 6 months ofwork experience. RESULTS: The educational content developed through the Delphi method for preventing safety accidents among hospitalized children contained seven domains (falls, injury, electric shocks/burns, suffocation/aspiration, poisoning/abuse, kidnapping, medical devices) with 44 topics. The item mean of nurses'perceptions of the importance of child safety education was 4.18, and the actual performance score was 3.72, which was a statistically significant difference (t=11.58, p<.001). CONCLUSION: These seven comprehensive domains of accident prevention education for hospitalized children are expected to be useful for interventions to support the safety of hospitalized children.

PURPOSE: This study was conducted to design services for improving the quality of care of hospitalized children with acute diseases. METHODS: The service design process had four phases: discovery, definition, development, and delivery. The participants were 23 mothers of hospitalized children with an acute disease, and seven nurses and three doctors working at a pediatric hospital. Data were collected through self-report questionnaires, in-depth interviews, and observations. The data were analyzed using content analysis and descriptive statistics. RESULTS: The participants reported needs for explanations about the treatment, skillful nursing, and environmental improvements. The concept of the services was familiarity and enjoyment, aimed at solving the problems of unfamiliarity and boredom. A six-guideline was presented for improving the quality of care of hospitalized children with acute diseases: improvement of awareness, development of educational materials, improvement of skills, environmental improvements, play activities, and evaluations of user satisfaction. CONCLUSION: These findings indicate that nursing services should deliver familiarity and enjoyment to hospitalized children and their families. The findings of this study emphasize that the service design methodology can be used to improve the quality of care of hospitalized children with acute diseases.

BACKGROUND AND OBJECTIVES: There is a limited appreciation of the epidemiology of dialysis-receiving AKI in children. The primary objective of the study was to evaluate changes in the incidence of dialysis-receiving AKI among children over a period of 20 years in Ontario, Canada. The secondary objectives were to assess temporal trends in the utilization of various dialysis modalities and 30-day mortality among children with dialysis-receiving AKI. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: All children (29 days to 18 years) who received their first dialysis for AKI between 1996 and 2015 were identified from healthcare administrative databases. Those who received dialysis for ESKD, inborn errors of metabolism, and poisonings were excluded. The incidence rates of dialysis-receiving AKI were reported annually. The Cochran-Armitage test was used to assess trends in the incidence and short-term mortality after dialysis-receiving AKI. RESULTS: We identified 1394 children treated with dialysis for AKI during a hospital stay. There was a significant increase in the incidence of dialysis-receiving AKI among hospitalized children from 1996 (0.58 per 1000 person-years) to 2015 (0.65 per 1000 person-years) (P=0.01). The use of continuous kidney replacement therapy and intermittent hemodialysis increased whereas the relative use of peritoneal dialysis declined over time. Thirty-day mortality rates after dialysis-receiving AKI increased from 14% to 25% between 1996 and 2009 and reduced to 19% in the more recent years (P=0.03). CONCLUSIONS: In Ontario, the incidence of dialysis-receiving AKI among children has increased between 1996 and 2015. The use of peritoneal dialysis for AKI has declined and the short-term mortality after dialysis-receiving AKI has increased.

Objetivo: analisar os fatores intervenientes na gerência do cuidado de enfermagem à criança hospitalizada com cardiopatia reumática. Método: estudo descritivo-exploratório com abordagem qualitativa, que utilizou a Teoria Fundamentada em Dados e o Interacionismo Simbólico, respectivamente, como referencial metodológico e teórico. A coleta de dados foi realizada em uma instituição especializada em atendimento cardiológico, no munícipio do Rio de Janeiro. Foram entrevistados 19 profissionais de enfermagem através de um roteiro semiestruturado. Resultado: emergiram os seguintes fatores intervenientes na prática da gerência do cuidado: condição socioeconômica da família, comportamento da criança, condições de trabalho, comunicação ineficaz, educação permanente, trabalho em equipe e experiência profissional. Conclusão: os resultados apontam para a necessidade de proposição de estratégias de ação e interação que facilitem a prática gerencial de cuidado à criança com cardiopatia reumática e sua família face aos fatores intervenientes identificados.


Objective: to analyze the factors involved in the management of nursing care for children hospitalized with rheumatic heart disease. Method: this is a descriptive-exploratory study with a qualitative approach, which used Data-Based Theory and Symbolic Interactionism, respectively, as methodological, and theoretical references. Data was collected in an institution specializing in cardiac care in the city of Rio de Janeiro. Nineteen nursing professionals were interviewed using a semi-structured script. Result: the following intervening factors in the practice of care management emerged: the family's socioeconomic status, the child's behavior, working conditions, ineffective communication, continuing education, teamwork, and professional experience. Conclusion: the results point to the need to propose strategies for action and interaction that facilitate management practice in caring for children with rheumatic heart disease and their families, given the intervening factors identified.


Objetivo: analizar los factores que intervienen en la gestión del cuidado de enfermería al niño hospitalizado con cardiopatía reumática. Método: estudio descriptivo-exploratorio con enfoque cualitativo, cuyos marcos metodológico y teórico fueron la Teoría Fundamentada y el Interaccionismo Simbólico, respectivamente. La recolección de datos se realizó en una institución especializada en atención cardiológica, en la ciudad de Río de Janeiro. Fueron entrevistados 19 profesionales de enfermería mediante un cuestionario semiestructurado. Resultado: surgieron los siguientes factores intervinientes en la práctica de la gestión del cuidado: condición socioeconómica de la familia, comportamiento del niño, condiciones de trabajo, comunicación ineficaz, educación continua, trabajo en equipo y experiencia profesional. Conclusión: los resultados indican que es necesario proponer estrategias de acción e interacción que faciliten la práctica de la gestión del cuidado al niño con cardiopatía reumática y a sus familiares, con respecto a los factores intervinientes identificados.

Objetivo: descrever as percepções de acompanhantes sobre o impacto da contação de histórias durante a hospitalização de crianças. Métodos: trata-se de estudo qualitativo, apoiado no Interacionismo Simbólico, desenvolvido na unidade de internação pediátrica de um Hospital Universitário. Foram entrevistadas, entre junho e julho de 2023, nove mulheres, acompanhantes de crianças hospitalizadas, que participaram da atividade semanal de contação de histórias. A análise dos dados foi direcionada pela Análise Temática de Conteúdo. Resultados: a contação de histórias foi significada enquanto um contraponto ao cotidiano entediante e imerso em preocupações que marcam a hospitalização infantil. Emergiram duas categorias temáticas da percepção das mães sobre contação de histórias no contexto da hospitalização de crianças, "Estreitamento das relações entre crianças, acompanhantes e contadores de histórias", que contribuiu para o fortalecimento do vínculo entre acompanhante, criança e equipe multiprofissional e "Momento de acolhimento para as crianças e acompanhantes", que revela o efeito de proporcionar um momento de distração, alegria e conforto para as mães. Conclusão: a contação de histórias promove ambiência propícia para o enfrentamento da doença, proporciona bem-estar, alegria e conforto para as mães acompanhantes. Impacta positivamente no comportamento das crianças e acompanhantes durante a internação e estimula sua continuidade no ambiente domiciliar.


Objective: to describe companions' perceptions of the impact of storytelling during children's hospitalization. Methods: this is a qualitative study, supported by Symbolic Interactionism, developed in the pediatric inpatient unit of a university hospital. Between June and July 2023, nine women, companions of hospitalized children, who participated in the weekly storytelling activity, were interviewed. Data analysis was directed by thematic content analysis. Results: storytelling was meant as a counterpoint to the boring daily life and immersed in concerns that characterize children's hospitalization. Two thematic categories emerged from mothers' perception of storytelling in the context of children's hospitalization, "Strengthening relationships between children, companions and storytellers", which contributed to strengthening the bond between companion, child and multidisciplinary team, and "Moment of welcoming for children and companions", which reveals the effect of providing a moment of distraction, joy and comfort for mothers. Conclusion:storytelling promotes an environment conducive to coping with the disease, providing well-being, joy and comfort for accompanying mothers. It positively impacts the behavior of children and companions during hospitalization and encourages their continuity in home environments.


Objetivo: describir las percepciones de los acompañantes sobre el impacto de la narración de cuentos durante la hospitalización de los niños. Métodos: se trata de un estudio cualitativo, sustentado en el Interaccionismo Simbólico, desarrollado en la unidad de internación pediátrica de un hospital universitario. Entre junio y julio de 2023, fueron entrevistadas nueve mujeres, acompañantes de niños hospitalizados, que participaron de la actividad semanal de cuentacuentos. El análisis de los datos fue guiado por el análisis de contenido temático. Resultados: la narración de cuentos pretendía ser un contrapunto a la vida cotidiana aburrida y sumergida en las inquietudes que caracterizan la hospitalización de los niños. Dos categorías temáticas surgieron de la percepción de las madres sobre la narración de cuentos en el contexto de la hospitalización infantil, "Fortalecimiento de las relaciones entre niños, acompañantes y narradores", que contribuyó a fortalecer el vínculo entre acompañante, niño y equipo multidisciplinario, y "Momento de acogida para niños y acompañantes", que revela el efecto de brindar un momento de distracción, alegría y consuelo a las madres. Conclusión: la narración de cuentos promueve un ambiente propicio para el afrontamiento de la enfermedad, brindando bienestar, alegría y consuelo a las madres acompañantes. Impacta positivamente en el comportamiento de los niños y acompañantes durante la hospitalización y fomenta su continuidad en el ambiente hogareño.

RESUMO

INTRODUÇÃO: A inserção da psicologia e o método lúdico de intervenção na esfera hospitalar surge para promover bem-estar frente às vivências das crianças em condição de internação. O enfrentamento de uma hospitalização no período da infância pode provocar impactos significativos do ponto de vista orgânico, psíquico e ambiental, por ser um momento de estruturação do indivíduo enquanto sujeito psíquico. OBJETIVO: Posto isso, o presente artigo objetiva refletir sobre a relação da família e da equipe de saúde com crianças hospitalizadas e a relevância da utilização dos recursos lúdicos, a partir de um relato de experiência de estagiários de um curso de graduação em psicologia. MÉTODO: Logo, trata-se de um relato de experiência, de abordagem qualitativa e de natureza descritiva, realizado de março de 2022 a junho de 2023, em uma Unidade Pediátrica de um Hospital Público Brasileiro. RESULTADOS E DISCUSSÃO: A partir disso, observaram-se na prática reações de estranhamento dos pacientes ao se depararem com a permanência no hospital, o adoecimento e os procedimentos das equipes multiprofissionais de saúde; bem como evidenciou-se a importância da transmissão das informações e do preparo verbal ante intervenções, visando a participação ativa do paciente em seu tratamento. Também observou-se a significância do uso de ferramentas lúdicas nas intervenções com as crianças, que facilitam a adaptação ao contexto e proporcionam âncora para estas elaborarem suas vivências. CONCLUSÃO: Portanto, conclui-se que a utilização de recursos lúdicos favorece o atendimento psicológico na infância e período de hospitalização.


INTRODUCTION: The integration of psychology and the playing method of intervention in the hospital setting emerges to promote wellbeing in the face of children's experiences during hospitalization. Confronting hospitalization during childhood can lead to significant impacts from organic, psychological, and environmental standpoints, as it represents a pivotal moment in the individual's psychological development. OBJECTIVE: With this in mind, the present article aims to reflect on the relationship between families and healthcare teams with hospitalized children and the relevance of using playful resources, based on an experience report from interns of an undergraduate psychology course. METHOD: Therefore, it constitutes an experiential account, employing a qualitative and descriptive approach, conducted from March 2022 to June 2023, in a Pediatric Unit of a Brazilian Public Hospital. RESULTS AND DISCUSSION: From this perspective, practical observations revealed patients' feelings of unfamiliarity upon facing hospital stays, illness, and procedures performed by multidisciplinary healthcare teams; as well as the significance of conveying information and verbal preparation before interventions was highlighted, aiming to actively involve the patient in their treatment. It was also observed the significance of using playful tools in interventions with children, which facilitates adaptation to the context and provides an anchor for them to elaborate on their experiences. CONCLUSION: Thus, it can be concluded that the use of playful resources enhances psychological care during childhood and the hospitalization period.


INTRODUCCIÓN: La inserción de la psicología y el método lúdico de intervención en el ámbito hospitalario surge con el propósito de promover el bienestar frente a las experiencias de los niños en condiciones de hospitalización. Enfrentar la hospitalización durante la infancia puede generar impactos significativos desde un punto de vista orgánico, psicológico y ambiental, ya que representa un momento de estructuración del individuo como sujeto psíquico. OBJETIVO: En este sentido, el presente artículo tiene como objetivo reflexionar sobre la relación de la familia y el equipo de salud con niños hospitalizados y la relevancia de utilizar recursos lúdicos, a partir de un relato de experiencia de practicantes de un curso de grado en psicología. MÉTODO: Por lo tanto, se trata de un informe de experiencia con enfoque cualitativo y de naturaleza descriptiva, llevado a cabo desde marzo de 2022 hasta junio de 2023 en una Unidad Pediátrica de un Hospital Público Brasileño. RESULTADOS Y DISCUSIÓN: A partir de esto, se observaron reacciones de desconcierto por parte de los pacientes al enfrentar la permanencia en el hospital, la enfermedad y los procedimientos realizados por los equipos de salud multidisciplinarios; así como también se resaltó la importancia de la transmisión de información y la preparación verbal antes de las intervenciones, con el objetivo de involucrar activamente al paciente en su tratamiento. También se observó la importancia del uso de herramientas lúdicas en las intervenciones con niños, ya que facilitan la adaptación al contexto y brindan un punto de referencia para que estos niños elaboren sus experiencias. CONCLUSIÓN: Por lo tanto, se concluye que el uso de recursos lúdicos favorece la atención psicológica durante la infancia y el período de hospitalización.

ABSTRACT Objective: To evaluate children and adolescents with polytrauma and fractures of the pelvis and proximal and diaphyseal femur and correlate the impact of these conditions and clinical outcomes. Methods: Retrospective study carried out in a public hospital in Taboão da Serra (SP), with pediatric patients with polytrauma from January 2012 to December 2021. In total, 44 patients were evaluated, 70.44% boys and 29.55% girls, aged from 12 to 17 years. Results: Diaphyseal fracture of the femur affected 70.44% of the patients, mainly caused by a fall from a height (56.81%). Linear external fixation was the most used treatment (45.45%). All patients were discharged from hospital. Conclusion: We found essential sociodemographic information: 84.11% of patients did not have associated injuries; 88.63% were hospitalized from 3 to 11 days; 90.91% did not need to be admitted to an ICU, 77.27% did not need reoperation, and 22.73% underwent another surgery; 45.45% used the external fixator to stabilize injuries; 11.36% converted the external fixator to the intramedullary nail; 9.09% needed an intramedullary nail remover; 2.27% converted to a plate (bilateral) and 2.27% to a rigid nail; 2.27% had loss of reduction and revision with rod; 2.27% underwent corrective osteotomy; 2.27% had clinical hospitalization; 2.27% had osteonecrosis of the femoral head and screws removed; 2.27% removed the plate. No deaths were recorded. Level of Evidence II, Retrospective Study.


RESUMO Objetivo: Avaliar crianças e adolescentes politraumatizados com fraturas da pelve, proximal e diafisária do fêmur e correlacionar o impacto dessas condições e desfechos clínicos. Métodos: Estudo retrospectivo realizado em hospital público de Taboão da Serra (SP), com pacientes pediátricos politraumatizados entre janeiro de 2012 e dezembro de 2021. Avaliaram-se 44 pacientes, 70,44% meninos e 29,55% meninas, de 12 a 17 anos. Resultados: A fratura diafisária fechada do fêmur acometeu 70,44%, sendo causada principalmente por queda de altura (56,81%). A fixação externa linear foi o tratamento mais utilizado (45,45%). Todos os pacientes receberam alta hospitalar. Conclusão: Identificaram-se importantes informações sociodemográficas: 84,11% dos pacientes não apresentaram lesões associadas; 88,63% ficaram internados de 3 a 11 dias; 90,91% não necessitaram de internação em unidade de terapia intensiva (UTI), 77,27% não precisaram ser reoperados; 22,73% realizaram nova cirurgia; 45,45% utilizaram fixador externo para estabilização de lesões;11,36% converteram o fixador externo para a haste intramedular; 9,09% precisaram remover as hastes intramedulares; 2,27% converteram para placa (bilateral) e 2,27% para haste rígida; 2,27% tiveram perda de redução e revisão com haste; 2,27% realizaram osteotomia corretiva; 2,27% tiveram internação clínica; 2,27% tiveram osteonecrose da cabeça femoral e parafusos removidos; e 2,27% retiraram a placa. Não foram observados óbitos. Nível de Evidência II, Estudo Retrospectivo.

Objetivo: analisar o conhecimento da equipe de enfermagem no preparo para alta hospitalar de crianças dependentes de tecnologias. Métodos: Trata-se de uma pesquisa de caráter descritivo e exploratório com abordagem qualitativa, realizado a partir do ponto de vista de profissionais de enfermagem atuam na clínica pediátrica de um Hos- pital Universitário, localizado no estado da Paraíba, Brasil. Resultados: As participantes eram todas do sexo feminino, sendo elas quatro enfermeiras e quatro técnicas de enfer- magem. Os dados foram organizados em categorias temáticas de acordo com Minayo. Elegeram-se duas categorias: conhecimento da equipe de enfermagem sobre a alta hospi- talar segura de crianças dependentes de tecnologias e ações de enfermagem na promoção à alta hospitalar segura de crianças dependentes de tecnologias. Conclusão: A inexistência de um fluxo ou protocolo assistencial dificulta o processo de orientações dos familiares no retorno ao domicílio, impossibilitando a capacitação adequada para a realização do cuidado.


Objective: to analyze the knowledge of the nursing team in preparation for hospital discharge of technology-dependent children. Methods: This is a descriptive and exploratory research with a qualitative approach, carried out from the point of view of nursing professionals working in the pediatric clinic of a University Hospital, located in the state of Paraíba, Brazil. Results: The participants were all female, four nurses and four nursing technicians. Data were organized into thematic categories according to Minayo. Two categories were chosen: knowledge of the nursing team about the safe hospital discharge of technology-dependent children and nursing actions to promote safe hospital discharge of technology-dependent children. Conclusion: The lack of a care flow or protocol makes it difficult for family members to guide them back home, making it impossible to provide adequate training to provide care.


Objetivo: analizar el conocimiento del equipo de enfermería en la preparación para el alta hospitalaria de niños dependientes de tecnología. Métodos: Se trata de una investigación descriptiva y exploratoria con enfoque cualitativo, realizada desde el punto de vista de los profesionales de enfermería que actúan en la clínica pediátrica de un Hospital Universitario, ubicado en el estado de Paraíba, Brasil. Resultados: Los participantes fueron todos del sexo femenino, cuatro enfermeros y cuatro técnicos de enfermería. Los datos fueron organizados en categorías temáticas según Minayo. Fueron elegidas dos categorías: conocimiento del equipo de enfermería sobre el alta hospitalaria segura de niños dependientes de tecnología y acciones de enfermería para promover el alta hospitalaria segura de niños dependientes de tecnología. Conclusión: La falta de un flujo o protocolo de atención dificulta que los familiares los guíen de regreso a casa, imposibilitando la capacitación adecuada para brindar el cuidado.

Resumo Introdução Terapeutas ocupacionais têm consolidado sua assistência nos contextos hospitalares atuando em diversos setores dessas instituições com populações diversas. A enfermaria pediátrica é um dos cenários de atuação desses profissionais, que prestam seus serviços às crianças e adolescentes hospitalizados, assim como as suas acompanhantes, as mães. Objetivos Apresentar a intervenção de terapeutas ocupacionais com mães acompanhantes em enfermaria pediátrica e a percepção desses profissionais sobre sua contribuição nesse contexto. Método Trata-se de estudo descritivo, exploratório, de abordagem qualitativa. Participaram 14 terapeutas ocupacionais que atuavam com mães acompanhantes em enfermaria pediátrica. Os dados foram coletados por meio de questionário autoaplicável contendo 39 questões disponibilizado via Google Forms®, codificados com auxílio do software ATLAS.ti® (versão 8) e analisados por análise de conteúdo na modalidade temática. Resultados Os dados apontam que as terapeutas ocupacionais avaliam as dificuldades e necessidades das mães acompanhantes durante a internação dos seus filhos, traçam objetivos que englobam a mulher ou a díade e percebem as contribuições decorrentes da sua prática. Os dados apontam ainda modelos, técnicas, recursos e referenciais teóricos utilizados pelos profissionais nas intervenções com as mães acompanhantes, além dos locais disponibilizados pelas instituições para essa prática. Conclusão Terapeutas ocupacionais auxiliam de forma integral as mães acompanhantes durante a internação dos seus filhos. Os resultados ilustram o processo de intervenção da terapia ocupacional com esse público, perpassando pela avaliação, intervenção e resultados observados pelos profissionais.


Abstract Introduction Occupational therapists have consolidated their assistance in hospital settings through their work in various sectors of these institutions with diverse populations. The pediatric ward is one of the scenarios where these professionals act, providing services to hospitalized children and adolescents, as well as to their companions: their mothers. Objective To present the intervention of occupational therapists with accompanying mothers in pediatric wards and these professionals' perception of the mothers' contribution in this context. Method This is a descriptive, exploratory, qualitative study. 14 occupational therapists working with accompanying mothers in pediatric wards participated in this study. The data were collected through a self-administered questionnaire containing 39 questions available via Google Forms®, coded using the ATLAS.ti® 8.0 software, and analyzed by Content Analysis in its thematic modality. Results The data showed that occupational therapists assess the difficulties and needs of accompanying mothers during the hospitalization of their children, set goals that encompass the woman or the dyad, and perceive the contributions arising from their practice. The data also pointed out the models, techniques, resources, and theoretical frameworks used by these professionals in their interventions with the accompanying mothers, as well as the places made available by these institutions for this practice. Conclusion Occupational therapists fully help accompanying mothers during the hospitalization of their children. The results illustrate the occupational therapy intervention process with this population, from the evaluation, intervention, to the results observed by the professionals.

Resumo Enquadramento: A avaliação da perceção dos pais de crianças hospitalizadas sobre o apoio dos enfermeiros constitui um contributo para aferir a qualidade dos cuidados de enfermagem pediátricos. Objetivo: Concluir o processo de Adaptação Cultural e Validação da Nurse-Parent Support Tool (NPST) para a realidade portuguesa, avaliando a confiabilidade e validade. Metodologia: Estudo metodológico quantitativo, com uma amostra de 291 pais de crianças hospitalizadas. As propriedades psicométricas do instrumento foram aferidas, avaliando a confiabilidade, através do cálculo do coeficiente do alfa de Crohnbach, e a validade de construto, pela análise fatorial exploratória, com rotação Varimax. Resultados: A NPST apresenta características psicométricas apropriadas à população portuguesa, de pais de crianças hospitalizadas (alfa de Cronhbach 0,925), garantindo-lhe a confiabilidade e validade para medir o apoio dado pelos enfermeiros. Conclusão: Os resultados demonstraram que a Escala de Apoio dos Enfermeiros aos Pais (EAEP), versão portuguesa da NPST, é um instrumento fiável e válido para avaliar o apoio dos enfermeiros, aos pais da criança hospitalizada e apresenta muito boa consistência interna.


Abstract Background: Assessing the perceptions of parents of hospitalized children about the support provided by nurses can contribute to assessing the quality of pediatric nursing care. Objective: To complete the cultural adaptation and validation of the Nurse-Parent Support Tool (NPST) for the Portuguese population by assessing its reliability and validity. Methodology: Quantitative methodological study, with a sample of 291 parents of hospitalized children. The psychometric properties of the instrument were measured by assessing its reliability using Cronbach's alpha coefficient and its construct validity through exploratory factor analysis with Varimax rotation. Results: The psychometric characteristics of the NPST show that it is appropriate to the Portuguese population of parents of hospitalized children (Cronbach's alpha of 0.925), as well as a reliable and valid tool to measure the support provided by nurses to these parents. Conclusion: The European Portuguese version of the NPST - Escala de Apoio dos Enfermeiros aos Pais (EAEP) - is a reliable and valid instrument with very good internal consistency to assess the support provided by nurses to parents during a child's hospitalization.


Resumen Marco contextual: La evaluación de la percepción de los padres de niños hospitalizados sobre el apoyo de los enfermeros es una contribución para evaluar la calidad de la atención de enfermería pediátrica. Objetivo: Completar el proceso de Adaptación Cultural y Validación del Nurse-Parent Support Tool (NPST) para la realidad portuguesa, evaluando fiabilidad y validez.
