RESUMO

Listeria monocytogenes is the foodborne pathogen responsible for listeriosis in humans. Its ability to grow at refrigeration temperatures, particularly in products that support its growth and have a long-refrigerated shelf-life, poses a significant health risk, especially for vulnerable consumer groups such as pregnant woman and immunocompromised individuals. A comprehensive analysis of L. monocytogenes in aquatic food products (AFPs) was conducted, examining the prevalence of the bacterium, the associated outbreaks, and the resulting deaths. Data from 66 studies, comprising a total of 19,373 samples, were analysed from the scientific literature to determine prevalence of the pathogen. The mean pooled prevalence of L. monocytogenes was 11% (95% CI: 8-14%) among different AFPs categories. Additionally, an overview of worldwide listeriosis outbreaks associated with contaminated AFPs between 1980 and 2023 was provided, totalling 1,824 cases, including 41 deaths. Furthermore, a compilation of bio-based mitigation strategies was presented, including the use of lactic acid bacteria (LAB) and bacteriophages as bio-protective cultures to inhibit L. monocytogenes in AFPs. A variety of predictive microbiology models, based on growth prediction and interaction for Listeria, were reviewed to assess the effectiveness of control strategies in different types of AFPs, offering insights into pathogen behaviour throughout the production chain. The reported growth models described the impact of product storage temperature on pathogen growth parameters, while interaction models, based on the inhibitory effect of LAB presence against Listeria, were described using the Jameson-effect approach and Lotka-Volterra model (i.e., predator-prey model). Both models can be used to describe the simultaneous growth of two bacterial populations and their interactions. Several Quantitative Risk assessment studies have been conducted for AFP, identifying the food category as a relevant contributor in Listeriosis risk, and providing predictive insight into how storage temperature, food microbiota, product shelf-life and population aging influence the risk by L. monocytogenes. More importantly, this quantitative approach can serve as a key tool to assess the effectiveness of specific mitigation and intervention strategies to control the pathogen, such as sampling schemes or bio-preservation techniques.

In recent years, nature-induced urban disasters in high-density modern cities in China have raised great concerns. The delayed and imprecise understanding of the real-time post-disaster situation made it difficult for the decision-makers to find a suitable emergency rescue plan. To this end, this study aims to facilitate the real-time performance and accuracy of on-site victim risk identification. In this article, we propose a victim identification model based on the You Only Look Once v7-W6 (YOLOv7-W6) algorithm. This model defines the "fall-down" pose as a key feature in identifying urgent victims from the perspective of disaster medicine rescue. The results demonstrate that this model performs superior accuracy (mAP@0.5, 0.960) and inference speed (5.1 ms) on the established disaster victim database compared to other state-of-the-art object detection algorithms. Finally, a case study is illustrated to show the practical utilization of this model in a real disaster rescue scenario. This study proposes an intelligent on-site victim risk identification approach, contributing significantly to government emergency decision-making and response.

Consumers can be exposed to many foodborne biological hazards that cause diseases with varying outcomes and incidence and, therefore, represent different levels of public health burden. To help the French risk managers to rank these hazards and to prioritize food safety actions, we have developed a three-step approach. The first step was to develop a list of foodborne hazards of health concern in mainland France. From an initial list of 335 human pathogenic biological agents, the final list of "retained hazards" consists of 24 hazards, including 12 bacteria (including bacterial toxins and metabolites), 3 viruses and 9 parasites. The second step was to collect data to estimate the disease burden (incidence, Disability Adjusted Life Years) associated with these hazards through food during two time periods: 2008-2013 and 2014-2019. The ranks of the different hazards changed slightly according to the considered period. The third step was the ranking of hazards according to a multicriteria decision support model using the ELECTRE III method. Three ranking criteria were used, where two reflect the severity of the effects (Years of life lost and Years lost due to disability) and one reflects the likelihood (incidence) of the disease. The multicriteria decision analysis approach takes into account the preferences of the risk managers through different sets of weights and the uncertainties associated with the data. The method and the data collected allowed to estimate the health burden of foodborne biological hazards in mainland France and to define a prioritization list for the health authorities.

Extensive monitoring programs of chemical hazards in the animal feed chain are in place, both organized by public and private organizations. The objective of this review was to prioritize chemical hazards for monitoring in the European animal feed supply chain. A step-wise approach was designed for the prioritization, based on: historical occurrence of the chemicals in animal feed ingredients and animal feeds (in relation to European guidance values or maximum limits in feed); information on transfer of the chemical to edible animal products, and; the extent of human dietary intake of the products and possible adverse human health effects of the chemical. Possible prioritization outcomes were: high (H), medium (M), or low (L) priority for monitoring, or classification not possible (NC) because of limited available data on the transfer of the chemical to edible animal tissues. The selection of chemicals included (with results in parentheses): dioxins and polychlorinated biphenyls (H); brominated flame retardants (H); per- and polyfluorinated alkyl substances (H); the heavy metals arsenic (H) and cadmium (H) as well as lead (M) and mercury (M); aflatoxins (H), ochratoxin A (NC), and other mycotoxins (L); pyrrolizidine alkaloids (H) and other plant toxins (NC); organochlorine pesticides (H) and other pesticides (L); pharmaceutically active substances (M); hormones (NC); polycyclic aromatic hydrocarbons (L), heat-induced processing contaminants (NC), and mineral oils (NC). Results of this study can be used to support risk-based monitoring by food safety authorities and feed-producing companies in Europe.

BACKGROUND: Mathematical models to forecast the risk trend of the COVID-19 pandemic timely are of great significance to control the pandemic, but the requirement of manual operation and many parameters hinders their efficiency and value for application. This study aimed to establish a convenient and prompt one for monitoring emerging infectious diseases online and achieving risk assessment in real time. METHODS: The Optimized Moving Average Prediction Limit (Op-MAPL) algorithm model analysed real-time COVID-19 data online and was validated using the data of the Delta variant in India and the Omicron in the United States. Then, the model was utilized to determine the infection risk level of the Omicron in Shanghai and Beijing. RESULTS: The Op-MAPL model can predict the epidemic peak accurately. The daily risk ranking was stable and predictive, with an average accuracy of 87.85% within next 7 days. Early warning signals were issued for Shanghai and Beijing on February 28 and April 23, 2022, respectively. The two cities were rated as medium-high risk or above from March 27 to April 20 and from April 24 to May 5, indicating that the pandemic had entered a period of rapid increase. After April 21 and May 26, the risk level was downgraded to medium and became stable by the algorithm, indicating that the pandemic had been controlled well and mitigated gradually. CONCLUSIONS: The Op-MAPL relies on nothing but an indicator to assess the risk level of the COVID-19 pandemic with different data sources and granularities. This forward-looking method realizes real-time monitoring and early warning effectively to provide a valuable reference to prevent and control infectious diseases.

To prevent and control foodborne diseases, there is a fundamental need to identify the foods that are most likely to cause illness. The goal of this study was to rank 25 commonly consumed food products associated with Salmonella enterica contamination in the Central Region of Mexico. A multicriteria decision analysis (MCDA) framework was developed to obtain an S. enterica risk score for each food product based on four criteria: probability of exposure to S. enterica through domestic food consumption (Se); S. enterica growth potential during home storage (Sg); per capita consumption (Pcc); and food attribution of S. enterica outbreak (So). Risk scores were calculated by the equation Se*W1 +Sg*W2 +Pcc*W3 +So*W4 , where each criterion was assigned a normalized value (1-5) and the relative weights (W) were defined by 22 experts' opinion. Se had the largest effect on the risk score being the criterion with the highest weight (35%; IC95% 20%-60%), followed by So (24%; 5%-50%), Sg (23%; 10%-40%), and Pcc (18%; 10%-35%). The results identified chicken (4.4 ± 0.6), pork (4.2 ± 0.6), and beef (4.2 ± 0.5) as the highest risk foods, followed by seed fruits (3.6 ± 0.5), tropical fruits (3.4 ± 0.4), and dried fruits and nuts (3.4 ± 0.5), while the food products with the lowest risk were yogurt (2.1 ± 0.3), chorizo (2.1 ± 0.4), and cream (2.0 ± 0.3). Approaches with expert-based weighting and equal weighting showed good correlation (R2  = 0.96) and did not show significant differences among the ranking order in the top 20 tier. This study can help risk managers select interventions and develop targeted surveillance programs against S. enterica in high-risk food products.

The objective of this study was to evaluate the residues of 42 pesticides subject to public concern in Tieguanyin tea produced in Fujian, China. The presence of 42 pesticide residues in 90 Tieguanyin tea samples were determined. A total of 17 pesticides were detected. At least one pesticide residue was detected in 65 samples (72.2%). The detected pesticides imidacloprid, tolfenpyrad, bifenthrin, acetamiprid were found in 55.6%, 33.3%, 18.9% and 16.7% of samples, respectively. Pesticide residues in Tieguanyin tea varied significantly over the seasons and across the production regions. Based on data obtained, the health risks associated with long-term exposure to those pesticides were assessed and risks of detected pesticides were ranked. The hazard quotients (HQs) among the detected pesticides range from 5 × 10-8 for fenpropathrin to 3 × 10-4 for imidacloprid. The results demonstrated that despite a high occurrence of pesticide residues in Tieguanyin tea, residue levels observed could not be considered as a serious public health problem. The risk ranking scheme showed dicofol and thiacloprid were considered to pose a medium-risk. The suggestions for 17 detected pesticides used in Tieguanyin tea plantation were made, including those already banned from use in China (dicofol and methomyl), recommended for diminished use (thiacloprid and chlorpyrifos), and permitted use with considering the pre-harvest interval (13 other pesticides).

This article compares and contrasts microbial and chemical risk assessment methodologies in order to evaluate the potential for a common framework for ranking of risk of chemical and microbiological hazards, and developments needed for such a framework. An overview of microbial (MRA) and chemical (CRA) risk assessment is presented and important differences are highlighted. Two microbiological and two chemical hazard-food combinations were ranked based on both a margin of exposure and a risk assessment approach. The comparisons illustrated that it is possible to rank chemical and microbiological hazard-food combinations with traditional approaches from each domain and indicated that the rank order but not the absolute measures is similar using either approach. Including severity in the assessment using DALY reduced differences between hazards and affected the outcome more than which approach was used. Ranking frameworks should include assessment of uncertainty as an integral part of the ranking, and be based on assessment of risk, not safety, and expressed in a common health metric such as disease burden. Necessary simplifications to address data gaps can involve the use of default scenarios. Challenges include comparisons of case-based vs. non-case-based health-endpoints, e.g. biomarker concentration, and integration of the severity of health effects into ranking.

BACKGROUND: The presence of Anisakis larvae in fish represents a major public health concern. Effective risk management procedures should be applied to prevent heavily infected products from reaching the market. The aim of the study is to provide preliminary data on parasite exposure and risk classification in frozen fish products by applying a risk categorization scheme (site, abundance, density and epidemiology - SADE) and Fish Parasite Rating (FPR) method. Fish and cephalopods samples (N = 771) from 5 different FAO Atlantic areas were examined and categorized after an accurate visual inspection and a chloro-peptic digestion. RESULTS: In 25 out of 33 fish species parasite larvae were found. 10897 anisakids larvae were collected and identified to genus level. Molva dypterygia, Conger conger, Zeus faber and Aphanopus carbo were shown to be the most highly infected species. SADE and FPR scores were 1 and poor, respectively, for the referred species, because of the disseminated Anisakis infection and commercial rejection. CONCLUSION: SADE/FPR method showed high specificity and accuracy. The information provided in this work could be used in early warning systems for the detection of parasites in fishery products and might help fishing industries in establishing management strategies for infected stocks in terms of cost saving decisions.

Decision making in food safety is a complex process that involves several criteria of different nature like the expected reduction in the number of illnesses, the potential economic or health-related cost, or even the environmental impact of a given policy or intervention. Several multicriteria decision analysis (MCDA) algorithms are currently used, mostly individually, in food safety to rank different options in a multifactorial environment. However, the selection of the MCDA algorithm is a decision problem on its own because different methods calculate different rankings. The aim of this study was to compare the impact of different uncertainty sources on the rankings of MCDA problems in the context of food safety. For that purpose, a previously published data set on emerging zoonoses in the Netherlands was used to compare different MCDA algorithms: MMOORA, TOPSIS, VIKOR, WASPAS, and ELECTRE III. The rankings were calculated with and without considering uncertainty (using fuzzy sets), to assess the importance of this factor. The rankings obtained differed between algorithms, emphasizing that the selection of the MCDA method had a relevant impact in the rankings. Furthermore, considering uncertainty in the ranking had a high influence on the results. Both factors were more relevant than the weights associated with each criterion in this case study. A hierarchical clustering method was suggested to aggregate results obtained by the different algorithms. This complementary step seems to be a promising way to decrease extreme difference among algorithms and could provide a strong added value in the decision-making process.

This study aims at examining the contamination of coliform bacteria, Escherichia coli, Listeria monocytogenes, Vibrio vulnificus, and Vibrio cholerae, which carry extremely serious risks to the consumer health, in 700 seafood belonging to 4 different (raw sea fish, raw mussels, raw shrimp, and raw squid) categories. The total number of samples was determined as 700. When the obtained results were viewed in total, they were found to be 48.14%, 18.71%, 8.57%, and 3.42% for coliform bacteria, E. coli, L. monocytogenes, and V. vulnificus, respectively. V. cholerae, one of the factors studied, was not found. Conventional microbiological cultivation methods were used in the analysis stage as well as the real-time PCR method. This study aims at making a risk ranking modeling for consumer health based on product category and pathogens by interpreting the results of the analysis with statistical methods. According to the statistical analysis, significantly binary correlations were determined among some parameters that stimulate one another for reproducing. In the light of the obtained results of the study, it has been concluded that the studies of the most detailed examinations of the microbiological risks associated with seafood, forms of microbial pollution and microorganisms that cause deterioration in seafood and threaten consumer health and the path that their epidemiologies follow, are of primary importance to both protecting consumer health and obtaining safe and quality seafood.

Burden of disease metrics are increasingly established to prioritize food safety interventions. We estimated the burden of disease caused by seven foodborne pathogens in Denmark in 2017: Campylobacter, Salmonella, Shiga toxin-producing Escherichia coli, norovirus, Yersinia enterocolitica, Listeria monocytogenes, and Toxoplasma gondii. We used public health surveillance data and scientific literature to estimate incidence, mortality, and total disability-adjusted life year (DALY) of each, and linked results with estimates of the proportion of disease burden that is attributable to foods. Our estimates showed that Campylobacter caused the highest burden of disease, leading to a total burden of 1709 DALYs (95% uncertainty interval [UI] 1665-1755), more than threefold higher than the second highest ranked pathogen (Salmonella: 492 DALYs; 95% UI 481-504). Campylobacter still led the ranking when excluding DALYs attributable to nonfoodborne routes of exposure. The total estimated incidence was highest for norovirus, but this agent ranked sixth when focusing on foodborne burden. Salmonella ranked second in terms of foodborne burden of disease, followed by Listeria and Yersinia. Foodborne congenital toxoplasmosis was estimated to cause the loss of ∼100 years of healthy life, a burden that was borne by a low number of cases in the population. The ranking of foodborne pathogens varied substantially when based on reported cases, estimated incidence, and burden of disease estimates. Our results reinforce the need to continue food safety efforts throughout the food chain in Denmark, with a particular focus on reducing the incidence of Campylobacter infections.

The study aims at taking risk assessment of pesticide residues in ginseng and high risk pesticides were picked up in order to provide a scientific basis for the establishment of maximum residue limits(MRLs) for pesticides in ginseng. Residues of 246 pesticides in 80 ginseng samples collected from different place were detected by GC-MS/MS and LC-MS/MS method. Acute and chronic intake risks were evaluated by using deterministic approach, and the matrix ranking developed by the Veterinary Residues Committee of the United Kingdom was referred to assess risk score of pesticides. The 25 kinds of pesticide residues were detected in ginseng samples, the detection rate of quintozene(PCNB) was 78%, which was the most frequently detectable pesticide. The chronic dietary intake risks of 25 pesticide residues expressed as %ADI were 0.00%-2.6%, and their acute dietary intake risks expressed as %ARfD were 0.00%-104.2%. Among them the acute dietary intake risks of PCNB was 104%,which was the highest. The 25 pesticides were divided into 3 groups by risk score, high risk group(4 pesticides), medium risk group(6 pesticides), and low risk group(15 pesticides). Hexachlorobenzene, phorate, PCNB and BHC were classified as high risk group. It is necessary and effective to establish the limit of residual organic chlorine in ginseng from Chinese Pharmacopoeia(2015 edition). MRLs for PCNB and hexachlorobenzene in ginseng were proposed to be revised based on the results of risk assessment.

As a highly hydrated gel-type food, soybean curd is perishable and the development of spoilage bacteria has been described. Whole soybean curd (WSC), an innovative soy product retains the most nutrients in raw ingredients and exhibits more nutritive value compared with conventional soybean curd (CSC). However, the risk assessment of WSC is not well evaluated up to now. In this study, the same species of spoilage microorganism were separated from WSC and CSC. Two main spoilage strains were separated and identified as B. subtilis and B. cereus. The risk ranking scores of WSC was higher than that of CSC but still within medium risk range. In summary, we reported the presence of B. subtilis and B. cereus in WSC for the first time. Further, application of the risk ranger tool confirmed that the risk profile of WSC was medium and acceptable.

Inhalation exposure to elongated cleavage fragments occurring at mineral and rock mining and crushing operations raises important questions regarding potential health effects given their resemblance to fibers with known adverse health effects like amphibole asbestos. Thus, a major goal for establishing a toxicity profile for elongate mineral particles (EMPs) is to identify and characterize a suspected hazard and characterize a risk by examining together results of hazard and exposure assessment. This will require not only knowledge about biokinetics of inhaled EMPs but also about underlying mechanisms of effects induced by retained EMPs. In vitro toxicity assays with predictive power for in vivo effects have been established as useful screening tools for toxicological characterization of particulate materials including EMPs. Important determinants of physiological/toxicological mechanisms are physico-chemical and functional properties of inhaled particulate materials. Of the physico-chemical (intrinsic) properties, size, shape and surface characteristics are well known to affect toxicological responses; functional properties include (i) solubility/dissolution rate in physiological fluid simulants in vitro and following inhalation in vivo; (ii) ROS-inducing capacity in vitro and in vivo determined as specific particle surface reactivity; (iii) bioprocessing in vivo. A key parameter for all is the dose and duration of exposure, requiring to establish exposure-dose-response relationships. Examples of studies with fibrous and non-fibrous particles are discussed to illustrate the relevancy of evaluating extrinsic and intrinsic particle properties for predicting in vivo responses of new particulate materials. This will allow hazard and risk ranking/grouping based on a comparison to toxicologically well-characterized positive and negative benchmarks. Future efforts should be directed at developing and validating new approaches using in vitro (non-animal) studies for establishing a complete risk assessment for EMPs. Further comparative in-depth analyses with analytical and ultra-high resolution technology examining bioprocessing events at target organ sites have proven highly successful to identify biotransformations in target cells at near atomic level. In the case of EMPs, such analyses can be essential to separate benign from harmful ones.

RESUMO

This study aimed to critically review methods for ranking risks related to food safety and dietary hazards on the basis of their anticipated human health impacts. A literature review was performed to identify and characterize methods for risk ranking from the fields of food, environmental science and socio-economic sciences. The review used a predefined search protocol, and covered the bibliographic databases Scopus, CAB Abstracts, Web of Sciences, and PubMed over the period 1993-2013. All references deemed relevant, on the basis of predefined evaluation criteria, were included in the review, and the risk ranking method characterized. The methods were then clustered-based on their characteristics-into eleven method categories. These categories included: risk assessment, comparative risk assessment, risk ratio method, scoring method, cost of illness, health adjusted life years (HALY), multi-criteria decision analysis, risk matrix, flow charts/decision trees, stated preference techniques and expert synthesis. Method categories were described by their characteristics, weaknesses and strengths, data resources, and fields of applications. It was concluded there is no single best method for risk ranking. The method to be used should be selected on the basis of risk manager/assessor requirements, data availability, and the characteristics of the method. Recommendations for future use and application are provided.

The distribution of active pharmaceutical ingredients (APIs) in prescription medicines for human consumption in Japan was estimated using the public database of the National Database of Health Insurance Claims and Specific Health Checkups of Japan (NDB). From the latest NDB, 2058 APIs were identified, and the prescription weight exceeded 1 tonne/year for 711 APIs. Of these, 298 APIs were selected for further analysis after removing 413 APIs that were not covered by current environmental risk assessment (ERA) directives or were combination products. Among the 298 APIs, 43 were relatively newly branded APIs that have been available on the Japanese market since 2001 or later and have no generic drugs, and only 5 of the branded APIs are used by more than 1% of the population. When prescription data from the 47 prefectures in Japan were analyzed, prescription weights for 257 of the 298 APIs were the highest in Tokyo, probably because of its large population. Though it has both advantages and limitations, this novel method based on a non-profit public database can provide a transparent, unbiased and cost-effective solution for the estimation of the environmental exposure of generic and branded human medicines distributed with prescriptions in Japan.

This study examined how experts and laypeople using both a deliberative and a survey method prioritized 28 hazards related to food and everyday items. To enable them to make deliberative decisions, participants received detailed descriptions of the hazards. The participants prioritized the hazards before and after a group discussion, in which the group's average prioritization was discussed. The rankings of the hazards before and after the group discussion were highly correlated. However, laypeople and experts differed significantly in their rankings for 18 of the 28 hazards. Trust and confidence were important predictors for laypeople's risk rankings. To test the influence of the deliberative method (e.g., providing detailed information about each hazard), data from a second group of laypeople were collected with a no-information survey. This group did not receive specific information about the hazards. The risk rankings of the laypeople who received information were highly correlated with the risk rankings of laypeople who did not receive information. Overall, the results suggest that deliberative methods of risk-ranking or no-information survey methods with no information about hazards provide similar results among laypeople. The conclusion is that government agencies should not only base their risk prioritization on evidence from risk assessments but also need to consider laypeople's hazard rankings. This procedure may result in an efficient and publicly accepted risk management strategy.

Environmental risk assessment (ERA) is a commonly used, effective tool applied to reduce adverse effects of environmental risk factors. In this study, ERA was investigated using the Bayesian network (BN) model based on a hierarchical structure of variables in an influence diagram (ID). ID facilitated ranking of the different alternatives under uncertainty that were then used to evaluate comparisons of the different risk factors. BN was used to present a new model for ERA applicable to complicated development projects such as dam construction. The methodology was applied to the Gabric Dam, in southern Iran. The main environmental risk factors in the region, presented by the Gabric Dam, were identified based on the Delphi technique and specific features of the study area. These included the following: flood, water pollution, earthquake, changes in land use, erosion and sedimentation, effects on the population, and ecosensitivity. These risk factors were then categorized based on results from the output decision node of the BN, including expected utility values for risk factors in the decision node. ERA was performed for the Gabric Dam using the analytical hierarchy process (AHP) method to compare results of BN modeling with those of conventional methods. Results determined that a BN-based hierarchical structure to ERA present acceptable and reasonable risk assessment prioritization in proposing suitable solutions to reduce environmental risks and can be used as a powerful decision support system for evaluating environmental risks.

Antibiotics are widely used in aquaculture and therefore may be present as a dietary risk in cultured aquatic products. Using the Tai Lake Basin as a study area, we assessed the presence of 15 antibiotics in 5 widely cultured aquatic species using a newly developed dietary risk ranking approach. By assigning scores to each factor involved in the ranking matrices, the scores of dietary risks per antibiotic and per aquatic species were calculated. The results indicated that fluoroquinolone antibiotics posed the highest dietary risk in all aquatic species. Then, the total scores per aquatic species were summed by all 15 antibiotic scores of antibiotics, it was found that Crab (Eriocheir sinensis) had the highest dietary risks. Finally, the most concerned antibiotic category and aquatic species were selected. This study highlighted the importance of dietary risk ranking in the production and consumption of cultured aquatic products around Tai Lake.
