Abstract Objective: To estimate the incidence of dental caries and associated factors in the school period from six/seven to ten years of age. Material and Methods: A longitudinal study involving 168 children was followed up between 2015 and 2019 in the municipality of Palhoça, Brazil. The dependent variable was the caries incidence rate in the mixed dentition. The independent variables included information regarding demographic and socioeconomic status. Multivariate analyzes were carried out using Poisson Regression with a robust estimator. Variables with p <0.20 in the bivariate analysis were included in the adjusted model. Relative risks were estimated, as well as 95% confidence intervals. Results: Of 168 followed schoolchildren, 32 developed the disease, providing an incidence rate of 19.0%. Female children had a 10% higher risk [RR = 1.10 (95% CI 1.03; 1.18)] of developing caries than males. Also, children born from fathers with ≤ 8 years of schooling at baseline had a 9% higher risk [RR = 1.09 (95% CI 1.01; 1.16)] of developing dental caries compared to fathers with higher education. Conclusion: The incidence of dental caries in a four-year period was 19.0%. Females and children born from fathers with a lower level of education showed higher incidence rates.

Introdução:A enterocolite necrosante é uma doença que pode afetar o trato gastrointestinal de recém-nascidos,cujas manifestações clínicas podem ser caracterizadas por vômitos biliosos, sangue nas fezes, distensão abdominal, além de alterações nos parâmetros hemodinâmicos e instabilidade térmica. As populações mais vulneráveis a essa enfermidade são recém-nascidos de baixo peso,expostos ao ambiente de terapia intensiva neonatal. Objetivos: Identificar o perfil de recém-nascidos e os fatores maternos e neonatais associados à ocorrência de óbitos por enterocolite necrosante, em maternidade de referência do Ceará-Brasil. Metodologia: Trata-se de coorte retrospectiva, estudo que objetiva a descrição da incidência de determinado evento ao longo do tempo, além do estabelecimento de relações causais entre os fatores associados ao acontecimento. Incluíram-serecém-nascidos que tiveram óbitos por enterocolite necrosante entre 2019 e 2021, comficha de investigação de óbitos neonatais preenchida corretamente, não sendo excluído nenhum recém-nascido, totalizando amostra de 29 óbitos.Resultados: Identificou-se que o perfil dos recém-nascidos foi,em maioria, deprematuros e com baixo peso e fatores de risco para outras doenças associadas,como a sepse, o que acarretourealização de procedimentos invasivos e internação em ambiente de terapia intensiva neonatal.Conclusões: A prematuridade e o baixo peso ao nascer foram as variáveis relevantes no estudo e podem estar associadas à piora das condições clínicas do recém-nascido e ao desenvolvimento de enterocolite necrosante (AU).


Introduction: Necrotizing Enterocolitis is a disease that can affect the gastrointestinal tract of newborns, whose clinical manifestations can be characterized by bilious vomiting, blood in stool, abdominal distension, in addition to changes in hemodynamic parameters and thermal instability. The populations most vulnerable to this disease are low birth weight newborns exposed to the neonatal intensive care environment. Objectives: This study aimed to identify the profile of newborns and maternal and neonatal factors associated with the occurrence of deaths from necrotizing enterocolitis in a reference maternity hospital in Ceará, Brazil. Methodology: This is a retrospective cohort study seeking to describe the incidence ofa particular event over time, as well as establish causal relationships between the factors associated with the event. The study population comprised newborns who died from necrotizing enterocolitis between 2019 and 2021, who had neonatal death investigation forms filled out correctly, with no newborns being excluded, totaling a sample of 29 deaths. Results: It was identified that the profile of newborns was mostly premature, of low birth weight and with risk factors for other associated diseases such as sepsis, leading to invasive procedures and hospitalization in a neonatal intensive care environment. Conclusions: Prematurity and low birth weight were relevant variables in the study and may be associated with worsening of the newborn's clinical conditionsand development of necrotizing enterocolitis (AU).


ntroducción:La Enterocolitis Necrotizante es enfermedad que puede afectar el tracto gastrointestinal del recién nacido, cuyas manifestaciones clínicas pueden caracterizarse por vómitos biliosos, sangre en las heces, distensión abdominal, además de cambios en los parámetros hemodinámicos e inestabilidad térmica.Las poblaciones más vulnerables a esta enfermedad son recién nacidos con bajo peso expuestos al entorno de cuidados intensivos neonatales.Objetivos: Identificar el perfil de recién nacidos y los factores maternos y neonatales asociados a la ocurrencia de muertes por enterocolitis necrotizante, en maternidad de referencia en el Ceará-Brasil.Metodología: Estudio de cohorte retrospectivo, para describir la incidencia de determinado evento a lo largo del tiempo, además de establecer relaciones causales entre los factores asociados al evento.Se incluyeron recién nacidos fallecidos por enterocolitis necrotizante entre 2019 y 2021, quienes tuvieron formulario de investigación de muerte neonatal correctamente diligenciado, no excluyéndose ningún recién nacido, totalizando muestra de 29 defunciones.Resultados:El perfil de los recién nacidos fue mayoritariamente prematuro y de bajo peso al nacer y con factores de riesgo para otras enfermedades asociadas, como sepsis, con procedimientos invasivos y hospitalización en ambiente de cuidados intensivosneonatales.Conclusiones:La prematuridad y el bajo peso al nacer fueron variables relevantes en el estudio y pueden estar asociados con empeoramiento de las condiciones clínicas de recién nacidos y desarrollo de enterocolitis necrotizante (AU).

ABSTRACT Objective: To determine the prevalence and analyze the variables associated with the use of pacifiers and/or bottles by infants up to 6 months of age. Material and Methods: Data on sociodemographic characteristics, intention to offer pacifier and bottle-feeding, pregnancy and breastfeeding (BF) variables were collected at baseline by interviews and a self-administered questionnaire among pregnant women in the 3rd trimester. After delivery, mother-baby binomials were followed by phone calls at the 1st, 3rd, and 6th months of the baby's life (n=467) to gather information on the type of delivery, baby's gender, BF in the first hour of the newborn's life, baby's weight, mother's return to work, and oral habits. Association analyses were performed using logistic regression models with a 5% significance level with the pacifier/bottle-feeding use as the outcome. Results: Most mothers (52.5%) reported their babies used bottle-feeding, 48.2% used pacifiers and 33.4% used both of them throughout the 6 months. Intention to offer pacifier and bottle-feeding was reported by 45.0% and 54.8% of the mothers at the 3rd trimester of the pregnancy, respectively. Not living in one's own residence (OR=1.53; 95%CI: 1.05-2.24) and having the prenatal intention of offering a pacifier (OR=2.50; 95%CI: 1.63-3.83) to the baby were significantly associated with pacifier use. Variables significantly associated with bottle-feeding were mother's return to work (OR=2.48; 95%CI: 1.54-3.97), baby's lower birth weight (OR=1.58; IC95%: 1.07-2.33), and prenatal intention to offer bottle-feeding (OR=2.51; 95%CI 1.56-4.04). Conclusion: About half of the babies used pacifiers or were bottle-fed, which were associated with the mother's prenatal intention to offer them to their babies and socioeconomic factors.

ABSTRACT OBJECTIVE To investigate the occurrence and factors associated with common mental disorders in pregnancy and depressive symptoms in postpartum, as well as the association between both in the Brazilian Western Amazon. METHODS This is a prospective cohort in the MINA-Brazil study with women who received primary health care in the town of Cruzeiro do Sul, Acre State. We performed two clinical evaluations during pregnancy (the first: 16-20 weeks; the second: 28 gestational weeks) and three postpartum evaluations (at 3, 6 and 12 months), in which demographic and socioeconomic, gestational, lifestyle and clinical data were collected. We used the Self-Reported Questionnaire (score ≥ 8) to screen the gestational common mental disorder and the Edinburgh Postnatal Depression Scale (score ≥ 10) to identify postpartum depressive symptoms. We used adjusted ordinal logistic regression to investigate the relationship between the covariates and the occurrence of common mental disorders in pregnancy and postpartum depressive symptomatology. RESULTS A total of 461 women completed the two clinical evaluations in pregnancy; of these, 247 completed the three postpartum evaluations. The occurrence of common mental disorder during pregnancy was 36.2% and 24.5% in the first and second evaluations, respectively, and the cumulative incidence was 9.2%. In addition, 50.3% maintained the disorder between evaluations. During postpartum, approximately 20% of the mothers presented depressive symptoms during the first year of their children's lives. Parity (≥ 2) was associated with common mental disorders, while low maternal education was associated with postpartum depressive symptoms. Women with a common mental disorder in both evaluations during pregnancy were 5.6 times more likely (95%CI: 2.50-12.60) to develop postpartum depressive symptoms. CONCLUSION The occurrence of common mental disorder at any time assessed during pregnancy, but especially its persistence from the second trimester, was strongly associated with depressive symptoms after childbirth. These findings highlight the need for early screening and monitoring of the mental health of pregnant women at the start of prenatal care in order to reduce possible negative impacts on the health of the mother-child binomial caused by such events.


RESUMO OBJETIVO Investigar a ocorrência e os fatores associados com os transtornos mentais comuns na gestação e sintomas depressivos no pós-parto, bem como a associação entre ambos na Amazônia Ocidental Brasileira. MÉTODOS Coorte prospectiva no estudo MINA-Brasil com mulheres atendidas na atenção primária à saúde de Cruzeiro do Sul, Acre. Foram realizadas duas avaliações clínicas na gestação (primeira: 16-20 semanas; segunda: 28 semanas gestacionais) e três avaliações no pós-parto (aos 3, 6 e 12 meses), nas quais foram coletados dados demográficos e socioeconômicos, gestacionais, de estilo de vida e clínicos. Utilizou-se o Self-Reported Questionnaire (escore ≥ 8) para rastreamento do transtorno mental comum gestacional e a escala de depressão pós-natal de Edimburgo (escore ≥ 10) para identificação de sintomas depressivos pós-parto. Foi utilizada regressão logística ordinal ajustada para investigar a relação entre as covariáveis e a ocorrência de transtornos mentais comuns na gravidez e a sintomatologia depressiva no pós-parto. RESULTADOS Um total de 461 mulheres completaram as duas avaliações clínicas na gestação; dessas, 247 completaram as três avaliações pós-parto. A ocorrência de transtorno mental comum durante a gestação foi de 36,2% e 24,5% na primeira e segunda avaliações, respectivamente, e a incidência cumulativa foi de 9,2%. Ademais, 50,3% mantiveram o transtorno entre as avaliações. Durante o pós-parto, aproximadamente 20% das mães apresentaram sintomatologia depressiva ao longo do primeiro ano de vida de seus filhos. A paridade (≥ 2) foi associada ao transtorno mental comum, enquanto a baixa escolaridade materna associou-se com sintoma depressivo pós-parto. Mulheres com transtorno mental comum nas duas avaliações na gravidez apresentaram 5,6 vezes mais chance (IC95% 2,50-12,60) de desenvolverem sintoma depressivo pós-parto. CONCLUSÃO A ocorrência de transtorno mental comum em qualquer momento avaliado durante a gravidez, mas principalmente sua persistência a partir do segundo trimestre, foi fortemente associado ao sintoma depressivo posterior ao parto. Tais achados evidenciam a necessidade de rastreamento precoce e monitoramento da saúde mental de gestantes no início do pré-natal, a fim de reduzir possíveis impactos negativos para a saúde do binômio mãe-filho causados por tais eventos.

Abstract Objective: To assess the incidence of caries in a two-year period among low birth weight (LBW), very low birth weight (VLBW), and extremely low birth weight (ELBW) children considering socioeconomic indicators, dietary factors and oral hygiene. Material and Methods: A convenience sample was formed of 42 low birth weight children aged two to five years at baseline. Two examiners diagnosed caries using the World Health Organization criteria. Birth weight, socioeconomic indicators and diet were collected from medical records and questionnaires. Binomial models were used to estimate unadjusted and adjusted rate ratios (RR) and respective 95% confidence intervals for the factors evaluated. Results: Thirty-six children were re-examined after two years. The incidence of dental caries was 36.7%. The dmft index was 0.44 (±1.25) at baseline and increased to 1.36 (±3.85) at follow-up. VLBW children (1,000 to 1,500 g) (RR=0.23; 95%CI: 0.08-0.72) and LBW children (1,500 to 2,500 g) (RR=0.06; 0.01-0.55) had fewer carious lesions compared to ELBW children (<1,000 g). Carious lesions were more frequent among children with a lower income (RR=6.05; 1.05-34.84) and less frequent among those who did not consume sweetened juice, tea or yogurt (RR: 0.21; 0.07-0.62). Conclusion: An inverse dose-response relation was found between birth weight and the incidence of caries. A lower income and the consumption of sweetened beverages were risk factors for the development of caries (AU).

Introdução: Idosos com depressão e sintomas depressivos apresentam repercussões na funcionalidade, fragilidade e interações sociais, o que se reflete em uma pior qualidade de vida. Na perspectiva da saúde física, a depressão tem associação com multimorbidades, adoecimento cardiovascular e à mortalidade por todas as causas. Com o aumento da prevalência da depressão/sintomas depressivos (DSD) relacionado à mudança demográfica e ao envelhecimento da população global, a demanda de conhecimento aumenta, em especial quanto aos fatores preditivos da DSD, bem como à análise da associação entre esta variável e a mortalidade. Dois objetivos nortearam a presente pesquisa: Primeiro, sintetizar informações obtidas a partir da literatura científica mundial acerca da incidência e fatores preditivos da DSD na população de idosos que vivem na comunidade; segundo, analisar sintomas depressivos (SD) como preditivo de mortalidade em idosos domiciliares em uma cidade de médio porte no Nordeste brasileiro decorridos 138 meses. Método: Diferentes métodos foram utilizados para cada objetivo: Elaboração de Protocolo de Revisão Sistemática e Metanálise acerca dos fatores preditivos do espectro depressivo, registrado na PROSPERO. Estabeleceram-se 6 bases de dados para a aquisição do material a ser avaliado: PubMed, Web of Science, Scopus, Lilacs, Scielo e Cochrane. Todas as etapas foram executadas de maneira pareada. Para o segundo objetivo, realizou-se um uma análise de sobrevivência após 138 meses da participação de uma amostra de idosos com 65 anos e mais, vinculada à Rede FIBRA na cidade de Campina GrandePB, entre os anos de 2008 e 2009. Os dados de mortalidade foram coletados a partir do Sistema de Informação sobre Mortalidade do Município até fevereiro de 2020. A técnica de KaplanMeier (KM) e teste de hipótese com Log Rank foram utilizados na análise bivariada, seguido de Regressão de Cox para o gênero feminino. Resultados: Ao direcionar para obtenção da incidência de DSD na Revisão Sistemática, observaram-se 17.336 documentos encontrados nas bases de dados. Após processo de seleção, foram incluídos 46 artigos. A incidência acumulada (IA) foi sintetizada de acordo com os instrumentos utilizados para avaliação da DSD. Em ordem crescente observou-se que a IA para ferramentas diagnósticas foi 5,24% (IC 95% 4,75-5,77), seguida pela utilização de ferramentas de rastreio: Escala de Depressão do Centro de Estudos Epidemiológicos 10,92% (IC 95% 10,52-11,33), Escala de Depressão Geriátrica 13,24% (IC 95% 13,03-13,45) e outras escalas e 14,35% (IC 95% 14,04-14,67). Na análise de sobrevida, SD em idosos na linha de base não apresentaram um menor tempo de sobrevida decorridos 138 meses, seja por todas as causas de mortalidade (p=0.746), seja por causas cardiovasculares (p=0.3). Conclusão: A despeito da variação de IA anual, observa-se uma frequente ocorrência de DSD no contexto comunitário por parte dos idosos, o que deflagra a necessidade de ações preventivas e de cuidados precoces mais bem direcionados na Atenção Primária a Saúde. Na análise de sobrevivência, a falta de associação pode revelar que outros fatores podem exercer papel mediador na relação encontrada na literatura científica entre a DSD e mortalidade, o que não quer dizer que SD não tenham associação com a mortalidade, visto que alteram comportamentos, estilo de vida e adesão ao tratamento de outras doenças (AU).


Introduction: Older adults with depression and depressive symptoms exhibit impairment in functionality, frailty, and social interactions, which is reflected in a worse quality of life. From a physical health perspective, depression is associated with multimorbidities, cardiovascular disease, and all-cause mortality. With the increasing prevalence of depression/depressive symptoms (DDS) related to demographic change and the aging of the global population, the demand for knowledge increases, especially regarding predictive factors of DDS, as well as the analysis of the association between this variable and mortality. Two objectives guided the present research: First, to synthesize information obtained from the world scientific literature about the incidence and predictive factors of DDS in the community-dwelling older adults' population; second, to analyze depressive symptoms (DS) as a predictor of mortality in homebased older adults living in a medium-sized city in the Northeast of Brazil after 138 months. Method: Different methods were used for each objective: Development of a Systematic Review and Meta-Analysis Protocol about predictive factors of the depressive spectrum, registered in PROSPERO. Six databases were established for the acquisition of the material to be evaluated: PubMed, Web of Science, Scopus, Lilacs, Scielo and Cochrane. All steps were performed in a paired manner. For the second objective, we carried out a survival analysis after 138 months of participation of a sample of older adults aged 65 years and over, linked to the FIBRA Network in the city of Campina Grande-PB, between the years 2008 and 2009. Mortality data were collected from the Municipality Mortality Information System until February 2020. The Kaplan-Meier (KM) technique and hypothesis test with Log Rank were used in the bivariate analysis, followed by Cox Regression for female gender. Results: By directing to only retrieving incidence from the Systematic Review, 17,336 documents found in the databases were observed. After selection process, 46 articles were included. The cumulative incidence (CI) was synthesized according to the instruments used to evaluate DDS. In an increasing order we observed that the CI for diagnostic tools was 5.24% (95% CI 4.75-5.77), followed by the use of screening tools: Center for Epidemiologic Studies Depression Scale 10.92% (95% CI 10.52-11.33), Geriatric Depression Scale 13.24% (95% CI 13.03-13.45) and other scales and 14.35% (95% CI 14.04-14.67). In survival analysis, DS in older adults at baseline did not have a shorter survival time after 138 months, either from all-cause mortality (p=0.746) or from cardiovascular causes (p=0.3). Conclusion: Despite the variation in CI observed, it is evident a frequent occurrence of DDS in the community-dwelling older adults' context, which triggers the necessity of preventive actions and better-targeted early care in Primary Health Care. In survival analysis, the lack of association may reveal that other factors may play a mediating role in the relationship found in the scientific literature between DDS and mortality, which does not mean that DS have no association with mortality, since they alter behaviors, lifestyle, and adherence to treatment of other diseases (AU).

Abstract Objective: To analyze and compare changes of quality of life parameter among dental patients rehabilitated by the implant-supported overdentures with different attachment systems. Material and Methods: Forty-eight patients were recruited as a study cohort. The implant placement procedure was based on the results obtained by CBCT scanning and individualized surgical templates manufactured for correct implant placement. Each individual received two k3Pro Implants (Sure Type with 4.0 or 4.5 mm in diameter) at the intraforaminal area due to standard protocol of implantation provided by the manufacturer under local anesthesia. All patients were distributed between two groups based on the fact of using either Locator- or ball-attachments. Rank correlation was measured using Spearman correlation coefficient, while linear correlation was evaluated by Pearson correlation coefficient. Results: No statistically meaningful differences were noted regarding patients' distribution among groups considering age (p>0.05) and gender (p>0.05). Provided patient-level analysis demonstrated that increase of conventional full denture service time was positively correlated with escalation of OHIP-EDENT scores. The most prominent inter-correspondences were noted specifically between longevity of denture service and elevation of scores within "Functional limitation" (r=0.61; p<0.05), "Physical pain" (r=0.51; p<0.05) and "Physical disability" (r=0.57; p<0.05) subdomains. No statistically argumented regressions were noted between increase tendency of OHIP-EDENT scores and gender (p>0.05) or age (p>0.05) parameters. Conclusion: Significant improvements of quality of life measured with OHIP-EDENT were noted for both types of attachments compared to the pre-treatment situation independently of additionally provided surface electromyography-based alignment.

RESUMO

O surto de um novo coronavírus SARS-CoV-2 (Severe acute respiratory syndrome coronavirus 2), foi identificado em Wuhan, na China no final de 2019. O SARS-CoV-2, se propaga através de gotículas respiratórias de uma pessoa infectada e causa uma doença respiratória denominada Covid-19 (Coronavirus Disease, 2019). Seu comportamento clínico é variável e manifesta-se com sintomas leves ou moderado, podendo desenvolver uma infecção respiratória grave. A cavidade oral foi identificada como uma das portas de entrada para o SARS-CoV-2, e seu possível papel como agravante na infectividade e progressão da infecção viral têm sido controversos. Foi realizado um estudo de coorte retrospectivo com dados obtidos de 274 prontuários de pacientes com COVID-19. Foi coletado dados na admissão do paciente, com sete e quatorze dias para analisar a ocorrência de alterações bucais e relacioná-las com a severidade da infecção. Foram encontrados 154 pacientes com alteração bucal e 120 sem alteração. Dos 154 pacientes que apresentaram alteração bucal na admissão, 39 obtiveram remissão em 7 dias, 54 pacientes obtiveram remissão em 14 dias e 61 mantiveram alteração bucal até o fim do estudo. Dos 120 pacientes que não apresentavam alteração bucal na admissão, apenas 2 desenvolveram alteração bucal em 7 dias e estes foram a óbito. A alteração bucal mais frequente foi a úlcera nos três tempos de coleta ( T0: 43,8%, T1: 33,6% e T2: 21,2%). As análises dos exames complementares alteradas mais prevalentes entre os pacientes foram as hemácias, plaquetas, leucócitos, tempo de sangramento, TGP, TGO, albumina, creatinina, PCR, ureia, d-dímero e razão leucócito-linfócito. Não foram observados fatores preditores para o desenvolvimento de alterações bucais em pacientes com COVID 19. As alterações em mucosa oral foram observadas na maioria dos pacientes com COVID-19 analisados nessa pesquisa. Não foi observada associação entre as comorbidades e a presença de lesões orais nos pacientes avaliados. Os fatores de prognósticos foram avaliados pela análise multivariada de Cox que determinou que não fazer uso de medicação para o aparelho digestivo e metabolismo (HR: 2,18; IC95%: 1,13-4,22) e não apresentar hipertensão arterial sistêmica (HR: 1,78; IC95%: 1,13- 2,79) são fatores prognósticos para remissão de alteração bucal em pacientes com COVID-19 (AU).


An outbreak of a new SARS-CoV-2 coronavirus (Severe acute respiratory syndrome coronavirus 2) was identified in Wuhan, China in late 2019. SARS-CoV-2, spreads through respiratory droplets from an infected person and causes a respiratory disease called Covid-19 (Coronavirus Disease, 2019). Its clinical behavior is variable and manifests with mild or moderate symptoms, and may develop a severe respiratory infection. The oral cavity has been identified as one of the gateways for SARS-CoV-2, and its possible role as an aggravating factor in the infectivity and progression of viral infection has been controversial. A retrospective cohort study was carried out with data obtained from 274 medical records of patients with COVID-19. Data was collected on the admission of the patient, with seven and fourteen days to analyze the occurrence of oral changes and relate them to the severity of the infection. A total of 154 patients with oral alterations and 120 without alterations were found. Of the 154 patients who presented oral changes on admission, 39 achieved remission within 7 days, 54 patients achieved remission within 14 days, and 61 maintained oral changes until the end of the study. Of the 120 patients who did not present oral changes on admission, only 2 developed oral changes within 7 days and these died. The most frequent oral alteration was an ulcer in the three collection times (T0: 43.8%, T1: 33.6% and T2: 21.2%). The most prevalent abnormal exam analyses among patients were red blood cells, platelets, leukocytes, bleeding time, TGP, TGO, albumin, creatinine, CRP, urea, d-dimer, and leukocyte-lymphocyte ratio. No predictors were observed for the development of oral alterations in patients with COVID 19. Alterations in the oral mucosa were observed in most patients with COVID-19 analysed in this study. There was no association between comorbidities and the presence of oral lesions in the patients evaluated. Prognostic factors were evaluated by Cox's multivariate analysis, which determined that they did not use medication for the digestive system and for metabolism (HR: 2.18; 95%CI: 1.13- 4.22) and did not have systemic arterial hypertension (HR: 1.78; 95%CI: 1.13-2.79), these are prognostic factors for remission of oral changes in patients with COVID-19 (AU).

Introduction: Head and check squamous cell carcinomas (HNSCC) affects the Brazilian population with a high incidence and usually has a poor prognosis. Objective: To evaluate the risk factors for survival of patients diagnosed with HNSCC and investigate the influence of epidemiological and clinical factors on the prognostic of HNSCC in southeastern Brazil. Materials and Methods: Clinical records of 211 patients with head and neck squamous cell carcinomas diagnosed and treated between 2010 and 2018 at a reference hospital for oncology, were selected. Clinical and pathological characteristics at diagnosis and for 5 years follow up were collected. The Kaplan-Meier Curve with the Log-Rank test assessed survival, and forward stepwise multivariate logistic regression model was performed to determine the factors affecting HNSCC survival. Results: The 5-year overall survival was 30.0%. Laryngeal cancer was the most prevalent (34.1%), followed by oropharynx (33.6%) and oral cavity (24.2%). About 64% of patients had locally advanced tumors (T3 and T4) and 75.4% of the patients were diagnosed as being in the advanced clinical stages (III and IV). In the multivariate analysis, the locally advanced tumors (OR=2.748; 95%CI:1.310- 5.765), palliative chemotherapy (OR=15.757; 95%CI:5.868-42.309) and metastasis during oncological follow-up (OR=11.602; 95%CI:1.380-97.507) were associated with a poor prognosis. Conclusion: The survival rate was considered low when compared with the literature. Locally advanced tumors, palliative chemotherapy, and the appearance of metastases during follow-up were considered the most important risk factors associated with a low HNSCC survival.


Introdução: O carcinoma de células escamosas (CCE) de cabeça e pescoço possui alta incidência na população brasileira e normalmente está associado a um prognóstico desfavorável. Objetivo: Avaliar os fatores de risco para sobrevida de pacientes diagnosticados com CCE de cabeça e pescoço e investigar a influência de fatores clínicoepidemiológicos no prognóstico do CCE. Materiais e Métodos: Foram selecionados prontuários de 211 pacientes com CCE de cabeça e pescoço diagnosticados e tratados entre 2010 e 2018 em um hospital de referência em oncologia. Foram coletadas as características clínico-patológicas no momento do diagnóstico e nos 5 anos de acompanhamento. A curva de Kaplan-Meier com o teste Log-Rank avaliou a sobrevivência e o modelo de regressão logística multivariada progressiva foram realizados para determinar os fatores que afetaram a sobrevivência do CCE. Resultados: A sobrevida global em 5 anos foi de 30,0%. O câncer de laringe foi o mais prevalente (34,1%), seguido de orofaringe (33,6%) e cavidade oral (24,2%). 64% dos pacientes apresentavam tumores localmente avançados (T3 e T4) e 75,4% dos pacientes foram diagnosticados em estádios clínicos avançados (III e IV). Na análise multivariada, os tumores localmente avançados (RC = 2,748; IC 95%: 1,310-5,765), quimioterapia paliativa (RC = 15,757; IC 95%: 5,868-42,309) e metástases durante o acompanhamento oncológico (RC = 11,602; IC 95%: 1,380-97,507) foram associados a um pior prognóstico. Conclusão: A taxa de sobrevida foi considerada baixa. Tumores localmente avançados, quimioterapia paliativa e aparecimento de metástases durante o seguimento foram considerados os fatores de risco mais importantes associados a uma baixa sobrevida.

Abstract The objective was assess the influence of untreated caries and socioeconomic status (SES) on school dropout among adolescents. A six-year cohort study was conducted with random sample of adolescents (12 years-old) who had been evaluated initially in 2012 from Santa Maria, Brazil. Sex, socioeconomic status (mother's education and household income), and untreated caries were collected at the baseline. The outcome variable was collected at the follow-up through self-report and was divided into three categories: adolescents who only studied, who studied and employed, and who school dropouts. A multinomial regression model was performed to assess the influence of oral disease and SES on school leaving, through relative risk ratio (RRR) and 95% confidence interval (95% CI). From of 1,134 adolescents evaluated at the baseline, 768 participants with a mean age of 17.5 years were re-evaluated at follow-up (67.8% response rate). Male (RRR: 2.31; 95%CI: 1.19-4.48) and adolescents with untreated caries at the baseline had an increment in school-leaving (RRR: 2.26; 95%CI: 1.12-4.56). Mothers with low education (RRR: 2.24; 95%CI: 1.09-4.61) had a higher probability of having children who leave school. Untreated caries and low SES in early adolescence can influence the tendency to school dropouts.


Resumo O objetivo foi avaliar a influência da cárie dentária não tratada e do status socioeconômico (SES) sobre o abandono escolar dos adolescentes. Foi realizado um estudo de coorte de seis anos de acompanhamento com uma amostra aleatória de adolescentes (12 anos de idade) avaliados inicialmente em 2012 em Santa Maria, Brasil. Sexo, status socioeconômico (educação da mãe e renda familiar) e cárie não tratada foram coletados no início do estudo. A variável de desfecho foi coletada no acompanhamento por meio do autorrelato e foi dividida em três categorias: adolescentes que apenas estudavam, aqueles que estudavam e trabalhavam e quem tinha abandonado a escola. Um modelo de regressão multinomial foi realizado para avaliar a influência da doença bucal e do SES sobre o abandono escolar, por meio da razão de risco relativo (RRR) e intervalo de confiança de 95% (IC95%). Dos 1.134 adolescentes avaliados no início do estudo, 768 participantes com idade média de 17,5 anos foram reavaliados no acompanhamento (taxa de resposta de 67,8%). Meninos (RRR: 2,31; IC 95%: 1,19- 4,48) e adolescentes com cárie não tratada no início do estudo tiveram um incremento no abandono escolar (RRR: 2,26; IC 95%: 1,12-4,56). Mães com baixa escolaridade (RRR: 2,24; IC95%: 1,09-4,61) tiveram maior probabilidade de ter filhos que abandonaram a escola. Cárie dentária não tratada e baixo SES no início da adolescência pode influenciar a tendência de evasão escolar.

ABSTRACT OBJECTIVE To investigate the association between child care attendance since birth and development in two-years-old Brazilian children. METHODS The study used longitudinal data from the 2015 Pelotas Birth Cohort. The childhood development (cognitive, fine and gross motor skills, and language) at two-years-old children was assessed using INTER-NDA (INTERGROWTH-21st Neurodevelopment Assessment). The child care attendance was measured at ages one and two years and categorized as: a) never attended child care; b) attended some child care (one or two years); and c) always attended child care (one and two years). Demographic, socioeconomic, health, and child stimulation variables were considered as confounders. Crude and adjusted analyses of child care attendance and development were carried out using linear regression. RESULTS Out of the 3,870 infants included in the analyses, around 1/3 attended center-based child care. In crude analyses, attending center-based child care was associated with positive developmental outcomes, except in motor domains. In adjusted analyses, compared to those children that have never attended child care, children who did attend presented higher scores for cognitive development (always in child care: β: 2.44, 95%CI: 0.83-4.05; some child care: β: 1.35, 95%CI: 0.17-2.53). CONCLUSIONS This study suggests that center-based child care may help improve child cognitive development in the Brazilian context. Furthermore, the association was higher for early and continued attendance. Considering the low prevalence of children in external care, it is recommended to improve child care opportunities in early childhood.

Abstract: This study aimed to verify, through a longitudinal follow-up, the pre-, peri- and postnatal factors associated with the eruption of deciduous teeth. The study was nested in a Birth Cohort Study conducted in Pelotas, a Southern Brazilian city. Mothers were followed prenatally and their children were followed-up perinatally, at 3 and 12 months of age. The outcome was the number of teeth at 12 months, and exploratory variables included maternal habits and characteristics, anthropometric measures of children and mothers, and socioeconomic and demographic information. Data were collected through interviews with the mother and children's clinical exam. Poisson regression models were used for the analysis. Participants included 4,014 children with a mean number of erupted teeth at 12 months of 5.50. After adjustments, a lower mean number of teeth was observed in children from non-white mothers, early preterm children, and shorter children at birth and at 12 months. A higher number of teeth was observed for mothers with excessive weight gain during pregnancy, mothers who smoked during pregnancy, weightier children at birth and at 12 months, and for children with larger heads at birth and at 12 months. Our findings suggest that maternal and children characteristics influenced primary tooth eruption.

ABSTRACT Objective: To assess the influence of early childhood malocclusion on oral health-related quality of life (OHRQoL). Methods: 7-year cohort study involving 639 preschoolers (1 to 5 years) who had been evaluated initially with a survey conduced in 2010. Children completed the Brazilian version of the Child Perception Questionnaire (CPQ8-10) to assess OHRQoL during the follow-up period. Exploratory variables were collected at baseline, including the presence and severity of malocclusion (overjet and lip coverage). Socioeconomic characteristics, oral health behavior, and patterns of dental attendance were also investigated. A multilevel Poisson regression model was used to fit the association between malocclusion and OHRQoL. With this approach, incidence rate ratio (IRR) and 95% confidence intervals (95% CI) were calculated. Results: A total of 449 children were re-evaluated (follow-up rate, 70.3%). The prevalence of accentuated overjet and inadequate lip coverage was 13.5% and 11.9%, respectively. The mean (±SD) CPQ8-10 score was 10.57±10.32. The presence of inadequate lip coverage was associated with higher overall mean CPQ8-10 scores (IRR 1.51; 95% CI 1.29-1.77), and social well-being, emotional well-being, and functional limitation domains. Children with accentuated overjet (>3mm) also demonstrated higher overall scores on the CPQ8-10 than their normal counterparts. The presence of this condition also influenced the oral symptom (IRR 1.29; 95% CI 1.08-1.53) and emotional well-being (IRR 1.30; 95% CI 1.02-1.66) domains. Conclusion: Results of the present study suggest that early childhood malocclusion is a risk factor for low OHRQoL in future.


RESUMO Objetivo: Avaliar a influência da má oclusão na primeira infância na qualidade de vida relacionada à saúde bucal (QVRSB). Métodos: Este estudo de coorte de 7 anos envolveu 639 pré-escolares (1 a 5 anos) que foram avaliados inicialmente em um levantamento transversal conduzido em 2010. As crianças completaram a versão brasileira do Child Perception Questionnaire (CPQ8-10) para avaliar sua QVRSB no período do acompanhamento. Variáveis exploratórias foram coletadas na linha de base, incluindo a presença e severidade de má oclusão (sobressaliência e cobertura labial). Características socioeconômicas, hábitos de saúde bucal e padrões de assistência odontológica também foram investigados. Um modelo de regressão de Poisson multinível foi utilizado para medir a associação entre má oclusão e QVRSB. Com essa abordagem, calculou-se a razão de taxa de incidência (IRR, incidence rate ratio) e o intervalo de confiança de 95% (IC 95%). Resultados: No total, 449 crianças foram reavaliadas (taxa de acompanhamento de 70,3%). A prevalência de sobressaliência acentuada e cobertura labial inadequada foi de 13,5% e 11,9%, respectivamente. A média±DP de pontuação do CPQ8-10 foi 10,57±10,32. A presença de selamento labial inadequado foi associada com maiores médias na pontuação total do CPQ8-10 (IRR 1,51; IC 95% 1,29-1,77) e nos domínios de bem-estar social, bem-estar emocional e limitação funcional. Crianças com sobressaliência acentuada (>3 mm) também demonstraram médias nas pontuações do CPQ8-10 total maiores do que suas contrapartes normais. A presença dessa condição também influenciou os domínios de sintomas bucais (IRR 1,29; IC 95% 1,08-1,53) e bem-estar emocional (IRR 1,30; IC 95% 1,02-1,66). Conclusão: Os resultados do presente estudo sugerem que a má oclusão na primeira infância é um fator de risco para baixa QVRSB no futuro.

Abstract Most of the Brazilian population is covered by the Family Health Strategy (FHS), however no longitudinal study has assessed the impact of the FHS on child oral health-related quality of life (OHRQoL). The objective of the study was to evaluate the longitudinal impact of the FHS on the OHRQoL. This study followed up 459 children aged 2 to 5 years for 2 years. OHRQoL was assessed by the Brazilian version of the Early Childhood Oral Health Impact Scale (ECOHIS) at baseline (April to November 2016) and follow-up (April to December 2018). Children's parents answered a questionnaire regarding sociodemographic information, FHS service, and dental service. Participants were clinically examined for dental caries. Multilevel Poisson regression was used to assess the associations between FHS variables at baseline and overall/domain-specific of the ECOHIS scores over time. A total of 365 children were reassessed for OHRQoL (follow-up rate: 79.5%). The absence of FHS coverage from the child's first year of age was associated with higher scores in the family function domain [rate ratio (RR) = 2.42; 95% confidence interval (CI) 1.28-4.58)]. Home visits by the FHS team members were associated with higher psychological domain scores (RR = 1.60; 95%CI 1.01-2.57). Children not covered by the FHS since the first year of age reported worse OHRQoL over time. This fact highlights the importance of an integrated health approach to promote children's health.

ABSTRACT Objective: To describe the sociodemographic profile of elderly people with temporomandibular disorder and depression in Parkinson's disease cases. Material and Methods: It is a cross-sectional cohort cut study, which used secondary data from 124 elderly people with Parkinson's disease (PD). To diagnose temporomandibular disorder (TMD), the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire was used, and the variables studied were: age, sex, race, education, marital status, family income, stage of the disease, and depression. The statistical analysis performed was descriptive and used absolute numbers and a frequency count. Results: After the application of the eligibility criteria, the patient records of 81 elderly people with Parkinson's disease were selected; 15 presented associated TMD and depression. The average age was 69 years old; 66.67% were male; 46.67% had more than 9 years of schooling; 60% were married or had a partner; 53.33% received between 1 and 2 minimum wages; 33.33% stated they were brown; 80% were in the moderate stage of the disease; 46.66% were in group II according to the RDC/TMD, presenting disk displacement; and 53.33% presented severe depression. Conclusion: It was verified that the elderly people with Parkinson's and associated TMD and depression were male, married or with a partner, on a low income, with nine or more years of schooling, and were in the moderate stage of the disease.

ABSTRACT Objective: To estimate the prevalence and associated factors of dental trauma in 6-year-old school children in the city of Palhoça, Brazil. Material and Methods: Cross-sectional study nested in a cohort study. It involved a representative sample of school children born in 2009, residing in the municipality and regularly enrolled in public and private schools (n = 1,102). Clinical data were obtained through oral exams. To evaluate the incisal overjet, the distance in millimeters was measured horizontally from the labial surface of the lower central incisor to the labial surface of the upper incisor. Bivariate analyses were performed using the Chi-square test, with a p-value <0.05 considered statistically significant. Multivariate analyses using Poisson Regression were performed to identify independent associations between the prevalence and the independent variables studied. Results: Prevalence of dental trauma was 4.2% (95% CI 3.0-5.4). Dental trauma was statistically associated with studying in a private school (PR = 1.03; 95% CI 1.01-1.06) (p=0.016) and with inadequate lip coverage (PR = 1.08; 95% CI 1.01-1.14) (p=0.016). Conclusion: The prevalence of 4.2% of dental trauma in six-year-old children, associated with inadequate and greater lip coverage in children from private schools should be taken into account, since most teeth at this stage are newly erupted.

ABSTRACT This study describes body weight changes among participants of the NutriNet Brasil cohort (n = 14,259) during the covid-19 pandemic. We analyzed data reported before the pandemic onset (01/26/2020 to 03/18/2020) and about six months after (09/14/2020 to 10/19/2020). Our results show that 19.7% of the participants gained ≥ 2 kg. Weight gain was directly associated with male gender, lower education, and previous presence of overweight, and inversely associated with age. In turn, 15.2% lost ≥ 2kg, being directly associated with male gender and previous presence of overweight and inversely associated with age.


RESUMO Este estudo descreve modificações no peso corporal de participantes da coorte NutriNet Brasil (n = 14.259) ocorridas durante a pandemia de covid-19. Foram analisados dados informados em período anterior ao início da pandemia (26/01/2020 a 18/03/2020) e cerca de 6 meses após (14/09/2020 a 19/10/2020). O ganho de peso ≥ 2 kg alcançou 19,7% dos participantes, mostrando-se diretamente associado ao sexo masculino, à menor escolaridade e à presença prévia de excesso de peso, sendo inversamente associado à idade. A perda de peso ≥ 2kg alcançou 15,2% dos participantes, mostrando-se diretamente associada ao sexo masculino e à presença prévia de excesso de peso, sendo inversamente associada à idade.

ABSTRACT Objective: To study the influence of the first thousand days of life on establishing determinant behaviors for dental caries in childhood. Material and Methods: Longitudinal retrospective study involving 664 children born in 2009 living in a southern Brazilian municipality was carried out. Data was collected through interviews with mothers and through child's health card. Dependent variables were 1) tooth brushing onset after two years of age; 2) absent tooth brushing or once a day; 3) tooth brushing without adult supervision; 4) not having gone to the dentist until the age of 6; and 5) seek for dental services due to toothache at 6 years of age, over the last 30 days. Bivariate and multivariate analyses were performed using Poisson Regression to estimate relative risks (RR) and respective confidence intervals at the 95% accuracy level. Results: Mothers with lower schooling at child's birth presented a higher risk of "child's tooth brushing onset only after two years of age", "brushing the teeth once a day only or not brushing" and with "child not having gone to the dentist until the age of 6". Mothers who did not have a job when the child was born were associated with "not having gone to the dentist until the age 6". Children with gastroesophageal reflux who underwent hospitalization for more than 2 days under age 2 were associated with "seeking dental services because of pain at 6 years of age over the last 30 days". Conclusion: Higher risks of some determinant behaviors related to dental caries were associated with variables of the period of the first thousand days of life.

ABSTRACT OBJECTIVE To examine the effects of physical activity (PA) and sedentary behaviour (SB), in isolation and combination, on all-cause mortality in older adults. METHODS Prospective, population-based cohort study. The data were collected from first wave in 2015 and the follow-up continued until 2020. The sample consisted of 332 older adult people aged ≥ 60 years-old, out of which 59 died. The level of PA and SB was assessed by the International Physical Activity Questionnaire (IPAQ). The older adults were divided into PA categorized as sufficiently active and insufficiently active and into high and low SB. We built four combinations of PA and SB. Also, we used the Cox proportional hazards regression with a 95% confidence interval with hazard ratio estimate so as to verify the mortality risks between PA, SB, and the combinations of PA and SB. RESULTS Insufficiently active individuals had higher risks of mortality compared to sufficiently active people. We observed no associations between SB and mortality separately; however, when evaluated in a combined way, insufficiently active individuals and with a high SB time had a higher chance of mortality compared to active individuals with a low SB time. CONCLUSION Our isolated analyses demonstrate that complying with PA recommendations reduces the risk of mortality; however, no association was found between the time of PA exposure with the time of SB. When analysing the combination, being physically inactive and with a long time of SB showed higher mortality rates, with SB being an enhancer of this risk. The results of this study show the interdependence of SB for PA performed at moderate to vigorous intensity. The understanding of this interrelation must be considered in the formulation of public health guidelines.

ABSTRACT OBJECTIVE To describe feeding practices and the risk factors for the mixed breastfeeding and early weaning in the neonatal period. METHODS Cohort study, which we collected socioeconomic, demographic, health care and feeding data from 415 mother/child binomials born in four public maternity hospitals in Natal/Brazil. They were followed-up at 48 hours, 7 and 28 days after birth. The association was established using Pearson's Chi-square test and Poisson's regression, after adjusting it to other variables. RESULTS The prevalence of mixed breastfeeding in the first 2 days was 47,2% and early weaning in 7 and 28 days was 8,4% and 16,2% in that order. The main reasons for mixed breastfeeding and early weaning were: colostrum deficiency (33.8%), difficulty in latching/sucking (23.5%) and "little milk" (70.0%). The use of formula/milk/porridge remained associated with maternal age ≤ 20 years (RR = 0.64; 95%CI: 0.47-0.86), age 20-29 years (RR = 0,70; 95%CI: 0,57-0,87), primiparity (RR = 1.37; 95%CI: 1.11-1.60) and cesarean delivery (RR = 1.20; 95%CI: 1.00-1.45) at 2 days; absence of paternal support (RR = 4.98; 95%CI: 2.54-9.79) and pacifier use (RR = 3.21; 95%CI: 1.63-6.32) at 7 days; and only pacifier use (RR = 2.48; 95%CI: 1.53-4.02) at 28 days. CONCLUSIONS Early weaning was associated with maternal and health care factors, thus suggesting the need to readjust good practices and educational actions to achieve the exclusive offer to the maternal breast in the neonatal period.
