Surgical site infection (SSI) is a serious complication that can occur in orthopedic surgeries that require the use of implants in horses. Morbidity, prolonged and consequently expensive treatments, and even death are consequences of this complication. This paper retrospectively analyzed horses undergoing osteosynthesis or arthrodesis, which presented SSI in the post-surgical period, treated at the Veterinary Teaching Hospital of FMVZ-USP from 2009 to 2021. Sixty-seven horses met the selection criteria and of these, 13 (19.4%) had SSI in the postoperative period. Escherichia coli, Streptococcus sp, and Enterobacter cloacae complex were the most commonly isolated agents and implant removal was performed in 76.9% (10/13) of patients. Aminoglycosides associated or not with beta-lactams were the classes of antimicrobials used in previous therapy in 84.6% (11/13), and in all cases there was a change in the drugs used after the result of the culture and antibiogram, due to antimicrobial resistance identified. The incidence of SSI was similar as reported in other studies, and implant removal was an efficient strategy when clinical treatment fails to improve. The identification of the agents involved and the antibiogram was decisive for cases management.(AU)

Background: Expanding chronic hematoma is a relatively common postoperative complication in humans, not in dogs, nevertheless, is important understand this condition because it may affect the animal's health. A hematoma could be formed within hours and usually stabilizes spontaneously with the reabsorption of its content. However, occasionally the hematoma slowly expands, becomes encapsulated and forms a mass, causing a chronic expanding hematoma. Possibly skin and adipose tissue are displaced secondary to trauma, causing the formation of blood-filled cysts surrounded by a fibrous capsule. The aim of this document is to present the first report of a postsurgical chronic expanding hematoma in dog. Case: A case of chronic expanding hematoma secondary to excision of recurrent myxoma in a bitch was described. Physical examination showed a hard, firm, non-mobile mass enclosed within the deep muscular layers. This mass was between the sternum and the cranial abdomen, similar to previous lesions excised by another veterinarian. Fine needle aspiration indicated the presence of blood. Radiology demonstrated that the bone-cartilage tissue was not involved. Due to the size of the mass, a skin stretching pre-suturing technique using self-adhesive tape was applied around the mass 48 h before surgery. The mass was removed with wide excision margins and portions of the affected muscles, but a large wound with great tension was created; consequently, a parallel to the wound incision and the thoraco-lumbar vertebrae was made. Six months after surgery, the animal presented recurrence of the same lesion, which was also removed. The bleeding times were within the normal range, but the animal was diagnosed with hyperadrenocorticism. After both surgeries histopathological exam revealed a hematoma, that had thrombosis and granulation tissue with fibrosis and severe panniculitis, in addition of multifocal, chronic, moderate lymphohistiocytic myositis; all these changes were compatible with a chronic expanding hematoma. Discussion: Chronic expanding hematoma is a mass that grows slowly secondary to hemorrhage and does not exhibit elements of malignancy. However, it can occasionally be confused for a soft-tissue neoplasm. It has been reported frequently in humans but not in small animals. The first case was reported in 2002: 5 puppies with a cervical hematoma classified as a chronic expanding hematoma was described. In addition, 3 cases were reported: 1 cat with perirenal chronic expanding hematoma; 1 bitch with intra-abdominal presentation, and 1 cat with this pathology on the right pelvic limb after an intramuscular injection. The etiology of chronic expansive hematoma is unidentified in dogs, nevertheless, it is stipulated that is not an inflammatory process as previously assumed, but rather a neoplastic process. As in other cases, it was difficult to determine the origin of the chronic expanding hematoma. The coagulation times and platelets count were within ranges. Similarly, it is very complicated to relate hyperadrenocorticism with chronic expanding hematoma because dogs with Cushing's disease generally present hypercoagulopathies. Myxomas are associated with endocrinopathies in humans and dogs, therefore, hyperadrenocorticism is possibly related with this neoplasia. Myxomas have an expansive growth pattern, contrasting with myxosarcomas, which are infiltrating; Although in the present case the skin mass appeared to be invasive, both macroscopical and histopathological appearance of it corresponded to a well-shaped capsule compatible with chronic expanding hematoma. In summary, to the best of our knowledge, this is the first report of a post-surgical chronic expanding hematoma in a dog.

Background: Mammary tumors, for which mastectomy is the main treatment, are the most common neoplasms in bitches. Mastectomy is painful and, in order to reduce the pain stimulus in the transoperative period, tumescent local anesthesia is associated with general inhalation anesthesia. However, despite the numerous benefits of tumescence, intraoperative hypothermia is the most common complication. In Medicine, especially in plastic and dermatological surgery, it is common to use a heated tumescence solution to prevent intraoperative hypothermia; however, in Veterinary medicine, no previous study has examined the advantages and disadvantages of using heated tumescence solution. Thus, this study aimed to investigate the transanesthetic cardiorespiratory effects of heated tumescence solution in bitches submitted to radical unilateral mastectomy. Materials, Methods & Results: Eight animals were treated with 0.1% lidocaine solution, warmed to 37-42°C, using a Klein’s cannula for administration. Chlorpromazine (0.3 mg/kg) and meperidine (3 mg/kg) were used as pre-anesthetic medication intramuscularly, and induction was performed with intravenous propofol and maintenance with isoflurane. The data collection times were as follows: 15 min after starting isoflurane administration (M1), 5 min after tumescence (M2), after beginning of surgical incision (M3), during breast pullout (M4), after clamping of the superficial caudal epigastric vein, and artery (M5), after the beginning of the approximation of the subcutaneous tissue (M6), after the beginning of the intradermal suture (M7), and at the end of the surgical procedure (Mfinal). Background: Postoperative care after thoracolumbar hemilaminectomy in dogs generally includes rest, physical therapy, and analgesics such as opioids. Currently, there is no established standard for the management of postoperative pain inpatients undergoing hemilaminectomy. Ideally, an analgesic protocol should provide adequate pain relief with limited sedation, low adverse effects, and postoperative patient comfort. Therefore, this study aimed to evaluate postoperative analgesiausing transdermal fentanyl or intramuscular methadone in dogs undergoing thoracolumbar hemilaminectomy associatedwith intervertebral disc fenestration (HT) for the treatment of intervertebral disc extrusion (IVDE).Materials, Methods & Results: Eight dogs from the Department of Neurology and Neurosurgery of a Veterinary TeachingHospital, submitted to HT for the treatment of IVDE, were included. The dogs were randomly distributed into 2 groups ofequal numbers, namely the transdermal fentanyl (FT) group and the intramuscular methadone (IM) group. At the end ofthe surgical procedure, a fentanyl adhesive patch was applied to the animals in the FT group, which remained there for 72h. In the IM group, analgesia was induced by intramuscular administration of methadone at intervals of 6 h until 72 h aftersurgery. The animals were evaluated using the short form of the Glasgow Composite Measure Pains Scale (CMPS-SF).Evaluations of physiological parameters, side effects, and pain were performed by 2 assessors who had experience usingthe pain scale and were blinded to the analgesic protocol. Pain evaluations were performed every 2 h (from T4) until 24h after the surgical procedure. Evaluations were performed every 4 h from 24 h to 48 h after the surgical procedure and...

Purpose: Simultaneous pancreas-kidney transplantation (SPKT) brings several benefits for insulin-dependent type-1 diabetic patients associated with end-stage renal disease (ESRD). However, data on psychological outcomes for the waiting list and the transplanted patients are still lacking. Methods: Using the psychological Beck inventories of anxiety (BAI) and depression (BDI), 39 patients on the waiting list were compared to 88 post-transplanted patients who had undergone SPKT. Results: Significant differences were found regarding depression (p = 0.003) but not anxiety (p = 0.161), being the pretransplant patients more vulnerable to psychological disorders. Remarkable differences were observed relative to the feeling of punishment (p < 0.001) and suicidal thoughts (p = 0.008) between the groups. It was observed that patients who waited a longer period for the transplant showed more post-transplant anxiety symptoms due to the long treatment burden (p = 0.002). Conclusions: These results demonstrated the positive impact of SPKT on psychological aspects related to depression when comparing the groups. The high number of stressors in the pretransplant stage impacts more severely the psychosocial condition of the patient.

Purpose To analyze the effectiveness of vertical gastrectomy in the treatment of obese patients, adherence to clinical follow-up and the influence of factors such as gender and age. Methods This is a retrospective, observational and descriptive study, conducted with patients undergoing vertical gastrectomy, operated at Hospital São Domingos, between January 2016 and July 2018. Results Most patients undergoing vertical gastrectomy were female (n = 193, 72.28%) and had a mean age of37.11 ± 8.96 years old. The loss of follow-up was 56.18%. Among adherent patients (n = 117; 43.82%), most patients were female (n = 89; 76.07%) and had a mean age of 37.92 ± 9.85 years old. The mean body mass index (BMI) of the adherents in the preoperative was 37.85 ± 3.72 kg/m2. Both BMI and excess weight (EW) showed a statistically significant difference between pre- and postoperative period. Percentage of excess weight loss (% EWL) was satisfactory for 96.6% of adherent patients. Older patients had a statistically significant lower % EWL compared to the other groups. Conclusions Vertical gastrectomy was effective in the treatment of obese patients, with significant weight loss.(AU)

As such, there is a high number of post-operative complications, which include stump pyometra, accidental ligation of the ureter, remnant ovary syndrome, formation of inflammatory granulomas, and organ adhesions. Ovarian stump granulomas are usually formed by suture which has been improperly used, or to infection. Such granulomas can lead to adhesions, also affecting organs close to the surgical site. This report refers to a female dog that presented ovarian granulomas with adhesions to the pancreas and to several organs after ovary-hysterectomy. In order to solve the case, an exploratory celiotomy was performed to assess the abdominal cavity, removing the adhesions of the structures and granulomatous formations, which required the performance of a partial pancreatectomy for the thorough removal of the granuloma. After surgery, the patient developed pancreatitis, a very common complication in pancreatic surgery. Even so, the result was considered satisfactory, as the pancreatitis was resolved and the granulomas removed and until the last post-operative review, the patient was clinically well. OVH is one of the most commonly performed surgeries in the veterinary medical routine, yet it is neglected in several aspects. Therefore, there is a need for the surgical technique and its possible complications to be performed correctly and effectively.(AU)

This survey evaluated mombin leaves (Spondias mombin L.) decoction efficiency as an antiseptic during post-surgery period on cats submitted to orchiectomy and ovariosalpingohisterectomy. For this purpose, 45 castrated mongrels cats were divided into three groups, the first group as a positive control using 0.5% chlorhexidine-alcohol solution, the second a negative control group using sterile distilled water and, finally, the test group using mombin leaves decocted with a concentration of 100 mg/mL. All animals, independent of age and sex, had visibly healed in most cases in a similar time. Animals treated with mombin leaves decoction presented a significant reduction of bacterial growth. In addition, the animals treated in the test group had better surgical wound healing. All isolated bacterial strains presented inhibition halo for chlorhexidine and for Spondias mombin L. Thus, the decoction of Spondias mombin L. leaves proved antiseptic efficacy in the surgical wounds of cats submitted to orchiectomy and ovariosalpingohisterectomy.(AU)

Purpose To assess the effect of antibiotic prophylaxis on surgical site infection (SSI) rates in women undergoing breast cancer surgery in two tertiary hospitals in Brazil. Methods This was a randomized, double-blind, placebo-controlled, parallel-group clinical trial. A total of 124 women without independent risk factors for SSI were randomly assigned to receive either cefazolin (antibiotic group, n = 62) or placebo (control group, n = 62) as preoperative prophylaxis. After surgery, all surgical wounds were examined once a week, for four weeks, according to the Centers for Disease Control and Prevention definitions and classifications for SSI. Results Baseline characteristics were homogeneous between the two groups. Only one patient in the antibiotic group developed SSI, which was classified as superficial incisional. The overall SSI rate was low, with no significant difference between groups. Conclusion Antibiotic prophylaxis had no significant effect on reducing SSI rates in women without independent risk factors for SSI undergoing breast cancer surgery.(AU)

Background: Ovariohysterectomy (OHE) is the most commonly performed elective surgical procedure in companionanimals. OHE offers benefits of control of population and decreased risk of potentially life-threatening diseases such asmammarian tumours and pyometra. Traditional OHE intervention causes inflammation and pain due to trauma duringorgan manipulation. The purpose of this study was to compare the effect of intraperitoneal and incisional administrationof bupivacaine (BP) or levobupivacaine (LP) on postoperative analgesia in dogs undergoing the OHE procedure.Materials, Methods & Results: A total of 24 mix-breed bitches aged between 1 - 3 years and weighed 19 - 20 kg wereused in this study. The animals were divided into three groups as control (n = 8), BP (n = 8) and LP group (n = 8). Theanimals were kept under surveillance at the hospitalisation unit of the animal hospital for one day before the elective OHE.The dogs were fasted for 12 h before the surgery, with adlib water consumption. Atropine sulphate 0.045 mg/kg was administered subcutaneously approximately 30 min before general anaesthesia. Midazolam (0.3 mg/kg) was intravenouslyinjected into all dogs for pre-anaesthetic medication. After sedation, anaesthesia was induced with propofol (4 mg/kg, IV)and then the dogs were orotracheally intubated using cuffed endotracheal tubes. General anaesthesia was maintained byadministration of 2% isoflurane. The ventral abdomen was prepared aseptically for OHE following the general anaesthesia. All animals were operated on by the same surgeon. During surgery, sprayed bupivacaine 0.5% (4.4 mg/kg) with an equalvolume of saline in BP group, levobupivacaine 0.5% (4.4 mg/kg) with an equal volume of saline in LP were then appliedover the ovaries, uterine broad ligaments and cervix uteri. After removal of the uterine body, either LP or BP was sprayedto left and right, or cranial and caudal parts of the abdominal cavity...

It is important to collect information about owners of dogs with mammary tumor, since the owner must take care of the animal after surgery, given that the treatment of choice is a mastectomy. This study interviewed eleven owners of female dogs submitted to mastectomy. Most respondents have high school and/or college education, live in their own homes with incomes between one to five minimum salaries. Only one person said they did not know that dogs could have mammary cancer. However, most do not know how to prevent mammary cancer in dogs, demonstrating the need for educational campaigns on the disease. The most frequent way of acquiring the animal was adoption, which demonstrates the growth of responsible ownership among Brazilian owners. Three owners stated that they used contraceptive drugs on their dogs several times. This fact is quite worrisome since such drugs have several adverse effects, including mammary tumor. Eight owners reported that they had never been to a veterinarian before about the tumor, although the tumor was evident. Most owners only sought help for the dog because participation in the project was free of charge. Financial difficulties are one of the main reasons for owners to delay consultations. However, when asked why the dogs had not been treated before, no owner mentioned financial difficulties. It can be concluded that some breast tumors were caused by the use of contraceptive drugs and that ignorance regarding the disease is still high among the interviewed tutorsd.(AU)

Background: Mammary tumors are the most common neoplasms in female dogs. Surgical removal of the mammary glandchain is considered the standard treatment and is usually performed along with ovariohysterectomy (OH) to suppressovarian hormonal influence. Mastectomies cause moderate to severe pain and require preferential multimodal analgesicprotocols. The aim of this study was to compare the postoperative analgesic efficacy and the recovery times on femaledogs undergoing mastectomies and OH between those treated with epidural levobupivacaine alone and those treated withassociated doses of tramadol and anesthetized with propofol.Materials, Methods & Results: Eighteen female dogs were pretreated with acepromazine (0.03 mg/kg), using propofol(4 mg/kg) for induction and anesthesia maintenance. The dogs were randomly divided into three groups (n = 6) treatedwith epidural anesthesia with levobupivacaine alone at 1.5 mg/kg (GL) or associated with tramadol at doses of 2 mg/kg(GLT2) or 4 mg/kg (GLT4). After anesthesia, the mean propofol infusion rate for each group were calculed. During thepostoperative period, the degree of analgesia according to the University of Melbourne Pain Scale for 6 hours were determined, considering seven time points for evaluation (M30, M60, M90, M120, M180, M240, and M360). Supplementalanalgesia with morphine (0.5 mg/kg) to those dogs with scores ≥13 were provided. During the recovery period, the timeintervals between the end of anesthesia, and the following events: extubation (EX), the first head movement (MC), and theestablishment of sternal (PE) and quadrupedal (PQ) positions were measured. Data were subjected to the Friedman testfor analysis of non-parametric variables within the same group and to the Mann–Whitney test for independent variables, comparing the mean scores between groups (statistical significance was set at P < 0.05). In the GL group, a higher meanpropofol infusion rate than in either the ...

The objective of this study was to report the cesarean section procedure in a case of dystocic delivery in goats. A four-year-old Anglo-Nubian goat, weighing 40kg, was attended at the Veterinary Hospital Antônio Francisco Fonzar of the Faculty of Agrarian Sciences of Andradina, presenting difficulty in parturition. On physical examination, the animal was restless, with a present appetite and good body condition. The vaginal examination revealed vaginal prolapse, rectal, insufficient cervical dilatation, but without fetal observation and fetal membranes. In the clinical examination it was verified that the rectal temperature and heart and respiratory rate were within the limits considered normal for the species. A uterine ultrasound was performed observing twin pregnancy, fetal viability and abnormal fetal presentation due to the transverse position of one fetus. The therapeutic plan chosen for cesarean section, the postoperative treatment consisted of administration of Flunixin meglumine (2 mg/kg body weight for 3 days), Oxytetracycline 20% (20 mg/kg body weight in a single application) and Benzathine Penicillin (20,000 IU/kg every 48 hours, totaling 5 applications). For the local dressing was used ointment based on gentamicin, silver sulfadiazine, vitamin A and ointment. After two weeks the animal already had full recovery, it is concluded that the surgical procedures and conduct were successful.
