ABSTRACT

The communication between professional health caregivers, which mainly composed of medical staff, and patients and their families plays an important role in hospice care. The effective doctor-patient communication can guide terminal patients and their families to establish a correct medical view, reduce excessive medical treatment, and improve the quality of life for terminal patients. This paper briefly described the communication context of hospice care, the communication subject and the current situation of hospice care, analyzed the problems and challenges encountered by professional health caregivers, which mainly including medical staff, in the communication context of hospice care diagnosis and hospice care, and made the prospects of the development of effective communication measures.

ABSTRACT

Hypoxia-inducible factor (HIF-1α), a core transcription factor responding to changes in cellular oxygen levels, is closely associated with a wide range of physiological and pathological conditions. However, its differential impacts on vascular cell types and molecular programs modulating human vascular homeostasis and regeneration remain largely elusive. Here, we applied CRISPR/Cas9-mediated gene editing of human embryonic stem cells and directed differentiation to generate HIF-1α-deficient human vascular cells including vascular endothelial cells, vascular smooth muscle cells, and mesenchymal stem cells (MSCs), as a platform for discovering cell type-specific hypoxia-induced response mechanisms. Through comparative molecular profiling across cell types under normoxic and hypoxic conditions, we provide insight into the indispensable role of HIF-1α in the promotion of ischemic vascular regeneration. We found human MSCs to be the vascular cell type most susceptible to HIF-1α deficiency, and that transcriptional inactivation of ANKZF1, an effector of HIF-1α, impaired pro-angiogenic processes. Altogether, our findings deepen the understanding of HIF-1α in human angiogenesis and support further explorations of novel therapeutic strategies of vascular regeneration against ischemic damage.

ABSTRACT

With the prevalence of myopia in Asia, the prevalence of high myopia is gradually increasing, and high myopia has undoubtedly become a public health problem in Asia and even around the globe. As a relatively common pathological change in the eyes of patients with high myopia, optic disc tilt may also become a risk factor for diseases such as glaucoma and macular degeneration, thereby increasing the risk of visual impairment. However, the mechanism of optic disc tilt in high myopia and the role of optic disc tilt in the aggravation of high myopia complications still needs to be further explored. Therefore, this article collects and organizes relevant literatures on optic disc tilt, and makes a comprehensive discussion on the mechanism of optic disc shape changing caused by high myopia and the impact on various complications, so as to provide a certain basis for clinical diagnosis and treatment of high myopia and its complications.

ABSTRACT

ObjectiveTo investigate whether the whole intestinal microbiota transplantation in Alzheimer's disease (AD) model mice has more significant effects on ileum intestinal microenvironment in normal mice under the guidance of the theory of traditional Chinese medicine that "interior-exterior relationship exists between the heart and small intestine". MethodsThe whole intestinal microbiota of fourteen 6-month-old specific pathogen free male APP/PS1 double-transgenic AD model mice was transplanted into the gut of six normal C57BL/6J mice of the same age and background treated with mixed antibiotics for 14 days. Then, after 14 days of normal rearing, the mice were sacrificed. Next, the pathological changes in the ileum and colon were observed, and the composition and diversity of the ileal and colonic microbiota was analyzed by sequencing. ResultsAfter the whole intestinal microbiota of AD mice was transplanted into normal mice, pathological analysis showed that only the ileum tissue had mucosal damage and crypt gland epithelial cell degeneration, necrosis, and shedding. Moreover, the microbiota analysis found that only the number of genera (P<0.01), Chao1 index (P<0.01) and Simpson index of ileal microbiota in normal mice decreased (P<0.01), and the composition of intestinal microbiota was quite similar to that of AD model mice. ConclusionUnder the effect of whole gut microbiota transplantation in AD mice, the diversity and composition of ileal microbiota change more than that of colonic microbiota in normal mice, and at the same time, it results in pathological damage to the ileal mucosa, indicating that the ileal microenvironment may be more closely related to the occurrence and development of AD, which is highly consistent with the traditional Chinese medicine theory of "interior-exterior relationship between heart and small intestine".

ABSTRACT

Progressive functional deterioration in the cochlea is associated with age-related hearing loss (ARHL). However, the cellular and molecular basis underlying cochlear aging remains largely unknown. Here, we established a dynamic single-cell transcriptomic landscape of mouse cochlear aging, in which we characterized aging-associated transcriptomic changes in 27 different cochlear cell types across five different time points. Overall, our analysis pinpoints loss of proteostasis and elevated apoptosis as the hallmark features of cochlear aging, highlights unexpected age-related transcriptional fluctuations in intermediate cells localized in the stria vascularis (SV) and demonstrates that upregulation of endoplasmic reticulum (ER) chaperon protein HSP90AA1 mitigates ER stress-induced damages associated with aging. Our work suggests that targeting unfolded protein response pathways may help alleviate aging-related SV atrophy and hence delay the progression of ARHL.

ABSTRACT

Network pharmacology, molecular docking, and in vivo and in vitro experiments were employed to study the molecular mechanism of Blaps rynchopetera Fairmaire in the treatment of non-small cell lung cancer(NSCLC). The components of B. rynchopetera were collected by literature review, and the active components were screened out through the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP). PharmMapper was used to obtain the targets of the active components. The targets of NSCLC were obtained from DrugBank, GeneCards, OMIM, TTD, and PharmGKB. The Venn diagram was drawn to identify the common targets shared by the active components of B. rynchopetera and NSCLC. The "drug component-target" network and protein-protein interaction(PPI) network were constructed by Cytoscape, and the key targets were screened by Centiscape. Gene Ontology(GO) annotation and Kyoto Encyclopedia of Genes and Genomes(KEGG) enrichment of the above key targets were performed by DAVID. AutoDock and PyMOL were used for the molecular docking between the key targets and corresponding active components. A total of 31 active components, 72 potential targets, and 11 key targets of B. rynchopetera against NSCLC were obtained. The active components of B. rynchopetera had good binding activity with key targets. Further, the serum containing B. rynchopetera was prepared and used to culture human lung adenocarcinoma A549 cells. The CCK-8 assay was employed to determine the inhibition rates on the growth of A549 cells in blank control group and those exposed to different concentrations of B. rynchopetera-containing serum, cisplatin, and drug combination(B. rynchopetera-containing serum+cisplatin) for different time periods. The cell migration and invasion of A549 cells were detected by cell scratch assay and Transwell assay, respectively. Western blot was employed to determine the expression levels of B-cell lymphoma-2(Bcl-2), Bcl-2-associated X(Bax), caspase-3, cell division cycle 42(CDC42), proto-oncogene tyrosine-protein kinase SRC, and vascular endothelial growth factor(VEGF) in A549 cells. C57BL/6 mice were inoculated with Lewis cells and randomly assigned into a model control group, a B. rynchopetera group, a cisplatin group, and a drug combination(B. rynchopetera+cisplatin) group, with 12 mice per group. The body weight and the long diameter(a) and short diameter(b) of the tumor were monitored every other day during treatment, and the tumor volume(mm~3) was calculated as 0.52ab~2. After 14 days of continuous medication, the mice were sacrificed for the collection of tumor, spleen, and thymus, and the tumor inhibition rate and immune organ indexes were calculated. The tissue morphology of tumors was observed by hematoxylin-eosin(HE) staining, and the positive expression of Bax, Bcl-2, caspase-3, CDC42, SRC, and VEGF in the tumor tissue was detected by immunohistochemistry. The results indicated that B. rynchopetera and the drug combination regulated the expression levels of Bax, Bcl-2, caspase-3, CDC42, SRC, and VEGF to inhibit the proliferation, migration, and invasion of A549 cells and Lewis cells, thus playing a role in the treatment of NSCLC via multiple ways.

ABSTRACT

ObjectiveTo investigate the effect of Huangqisan pellets (HQS) on the phosphatidylinositol-3 kinase/protein kinase B/mammalian target of rapamycin (PI3K/Akt/mTOR) signaling pathway and autophagy in the kidney of diabetic nephropathy (DN) rats. MethodDN rat model was established through high-fat diet combined with intraperitoneal injection of streptozotocin (35 mg·kg-1). DN rats were randomly assigned into model group, irbesartan (0.027 g·kg-1) group, low-dose HQS (0.54 g·kg-1) group and high-dose HQS (1.08 g·kg-1) group. The levels of 24 h urinary total protein (UTP), serum albumin (Alb), serum creatinine (SCr), urea nitrogen (BUN), triglyceride (TG) and total cholesterol (TC) were measured after 12 weeks of continuous administration. The pathological changes of renal tissue were observed via hematoxylin-eosin (HE) staining. The expression of podocyte split diaphragm proteins nephrin and podocin in the renal tissue were detected by immunohistochemistry. The protein levels and phosphorylation of key proteins in PI3K/Akt/mTOR signaling pathway, as well as the expression of yeast Atg6 homolog (Beclin1) and microtubule-associated protein 1 light chain 3 (LC3) in the renal tissue were analyzed by Western blot. ResultCompared with the control group, the model group showcased increased 24 h UTP, SCr, BUN, TG, and TC levels and decreased Alb level (P<0.01). After modeling, the rats showed granulosity of epithelial cells of renal tubules, thickening of capillary basement membrane, proliferation of mesangial cells, and sclerosis of glomerulus. Furthermore, modeling down-regulated the expression of nephrin and podocin in the podocyte hiatus of glomerulus (P<0.01) as well as the protein levels of p-PI3K, p-Akt, and p-mTOR and the autophagy markers LC3 and Beclin1 in renal tissue (P<0.01). Compared with model group, irbesartan and HQS decreased the 24 h UTP, Cr, BUN, TG, and TC levels, increased the Alb level, and alleviated the pathological damage of kidney. Moreover, they up-regulated the expression of Nephrin and Podocin in the podocyte hiatus of glomerulus, as well as the protein levels of p-PI3K, p-Akt, p-mTOR, LC3, and Beclin1 in renal tissue (P<0.05, P<0.01). ConclusionHQS may inhibit the PI3K/Akt/mTOR signaling pathway to enhance podocyte autophagy and protect the glomerulus, thus slowing down the development of DN.

ABSTRACT

Multi-template molecularly imprinted solid phase extraction not only has the advantages of high selectivity, large adsorption capacity, easy preparation, reuse and low environmental pollution, but also can realize the enrichment and separation of many kinds of compounds. It has attracted wide attention in the extraction and separation of traditional Chinese medicine components. This study summarizes the latest development of multi-template molecularly imprinted solid phase extraction. At the same time, based on the classification of active components of traditional Chinese medicine (flavonoids, alkaloids, phenylpropanol, terpenes, etc.), the latest application of multi-template molecular imprinting solid phase extraction in multi-component separation of traditional Chinese medicine was reviewed, with a view to better application of multi-template molecularly imprinted polymer in active multi-component extraction and separation of traditional Chinese medicine and provide reference for the material basic research of the efficacy of traditional Chinese medicine.

ABSTRACT

OBJECTIVE@#To investigate the mechanism of Radix Kansui (RK) stir-fried with vinegar (VRK) decreased hepatotoxicity in mice.@*METHODS@#According to a random number table, 40 mice were randomly divided into negative control group (0.5% carboxymethylcellulose sodium, 20 mL/kg), positive control group (0.1% mixture of carbon tetrachloride in soybean oil, 20 mL/kg), RK group (the ethyl acetate extracts of RK, 250 g crude drug/kg) and VRK group (the ethyl acetate extracts of VRK, 250 g crude drug/kg) with 10 mice per group. All mice were administered orally by gavage daily for 7 continuous days. The morphology of liver tissues was examined to assess the liver injury by a transmission electron microscope. Hepatocyte apoptosis in vivo was determined by terminal deoxynucleotidyltransferase-mediated dUTP-biotin nickend labeling (TUNEL) assay. Immunohistochemical technique was adopted to detect the expression of particular antiapoptotic and proapoptotic proteins in the mitochondrial pathways, including B-cell lymphoma (Bcl-2) and caspase-3, as well as the expression of inflammatory mediators, including nuclear factor kappa B (NF- κ B) and intercellular adhesion molecule-1 (ICAM-1).@*RESULTS@#Liver injury and hepatocyte apoptosis were observed in RK mice, and the liver injury were significantly reduced in VRK-treated mice. In immunohistochemistry study, compared with the negative control group, RK inhibited dramatically the Bcl-2 protein expression and significantly increased the expression of caspase-3, NF- κ B and ICAM-1 (all P<0.01). Compared with the RK group, VRK group induced significant increase on Bcl-2 protein expression, and decreased the caspase-3, NF- κ B and ICAM-1 protein expression (P<0.05 or P<0.01).@*CONCLUSION@#The mechanism of reduced hepatotoxicity of VRK may be associated with the reduced inflammation, regulation of antiapoptotic and proapoptotic mediators in the mitochondrial pathway.

ABSTRACT

OBJECTIVE@#To observe the clinical effect of high suspension and low incision (HSLI) surgery on mixed haemorrhoids, compared with Milligan-Morgan haemorrhoidectomy.@*METHODS@#A multi-centre, randomized, single-blind, non-inferiority clinical trial was performed. Participants with mixed haemorrhoids from Xiyuan Hospital of China Academy of Chinese Medical Sciences, Beijing Rectum Hospital, Air Force Medical Center of People's Liberation Army of China, and Puyang Hospital of Traditional Chinese Medicine were enrolled from September 2016 to March 2018. By using a blocked randomization scheme, participants were assigned to two groups. The experimental group was treated with HSLI, while the control group was treated with Milligan-Morgan haemorrhoidectomy. The primary outcome was the clinical effect evaluated at 12 weeks after operation. The secondary outcomes included the number of haemorrhoids treated during the operation, pain scores, use of analgesics, postoperative oedema, wound healing, incidence of anal stenosis, anorectal manometry after operation, as well as surgical duration, length of stay and total hospitalization expenses. A safety evaluation was also conducted.@*RESULTS@#In total, 246 eligible participants were enrolled, with 123 cases in each group. There was no significant difference in the clinical effect between the two groups (100.00% vs. 99.19%, P>0.05). Compared with the control group, the number of external haemorrhoids treated during the operation and the pain scores after operation were significantly reduced in the experimental group (P0.05). The surgical duration and length of stay in the experimental group were significantly longer than those in the control group, and the total hospitalization expense was significantly higher than that in the control group (all P<0.05). No adverse events were reported in either group during the whole trial or follow-up period.@*CONCLUSION@#HSLI had the advantages of preserving the skin of anal canal completely, alleviating postsurgical pain and promoting rapid recovery after operation. ABSTRACT

Objective@#Cytoreductive surgery followed by adjuvant chemotherapy is a standard frontline treatment for epithelial ovarian cancer (EOC). We aimed to develop an ovarian cancer risk score (OVRS) based on the expression of 10 ovarian-cancer-related genes to predict the chemoresistance, and outcomes of EOC patients. @*Methods@#We designed a case-control study with total 149 EOC women including 75 chemosensitives and 74 chemoresistants. Gene expression was measured using the quantitative real-time polymerase chain reaction. We tested for correlation between the OVRS and chemosensitivity or chemoresistance, disease-free survival (DFS), and overall survival (OS), and validated the OVRS by analyzing patients from the TCGA database. @*Results@#The chemosensitive group had lower OVRS than the chemoresistant group (5 vs.15, p≤0.001, Mann-Whitney U test). Patients with disease relapse (13 vs. 5, p60 months) of patients with OVRS ≥10 were significantly shorter than those of patients with OVRS <9). The high OVRS group also had significantly shorter median OS than the low OVRS group in 255 patients in the TCGA database (39 vs. 49 months, p=0.046). @*Conclusions@#Specific genes panel can be clinically applied in predicting the chemoresistance and outcome, and decision-making of epithelial ovarian cancer.

ABSTRACT

Mineral medicine is an indispensable part of traditional Chinese medicine and has a long history of application. Among them, mineral-based hemostatics have been widely applied for the treatment of various hemorrhagic diseases with extensive clinical experience and established efficacy. Gypsum Fibrosum (GF), a commonly used mineral medicine in clinical, can clear away heat, and relieve anxiety and thirst. Gypsum Ustum (GU) is the processed product of GF after calcining at high temperature. It is mainly composed of anhydrous calcium sulfate (CaSO4) with the functions of moisturizing, promoting muscle growth, astringent sores and hemostasis. GU is often used externally to treat ulcer, itching, eczema, water and fire scalds, trauma bleeding, etc. Studies on the mechanism of hemostasis have shown that Ca2+ (coagulation factor Ⅳ) is involved in many key processes of the internal and external coagulation cascades and can prevent bleeding by regulating platelet activation and aggregation, and promoting the production of insoluble fibrin and the ultimate formation of a blood clot. GF and GU both contain Ca2+ which provide an important material basis of hemostatic effect for both compounds, but GU has a significant hemostatic effect, while GF has no hemostatic effect. After processing, the taste and efficacy of the GF have been obviously changed which reflects the characteristics of processing, but the processing mechanism of GU has not been fully clarified. Therefore, based on studies of GF before and after calcining, this paper focused on these aspects including calcining process, crystal form comparison, element content, efficacy comparison, and summarized various aspects of Ca2+ involved in hemostasis. In addition, the hemostatic properties of other calcium-containing mineral medicines and new calcium-containing hemostatic materials such as calcium alginate, mesoporous calcium silicate and nanogel hemostatic materials were also discussed. The paper aimed to provide a reference for elucidating processing mechanism and clinical dialectical use of GU, also to promote development of new calcium-containing hemostatic materials.

ABSTRACT

Objective::To observe the effect of modified Tongqiao Huoxuetang combined with stent thrombectomy in treating acute cerebrovascular occlusion. Method::A total of 120 cases were randomly divided into control group and observation group, with 60 cases in each group. The control group received stent thrombectomy, while the observation group received modified Tongqiao Huoxuetang combined with stent thrombectomy. The clinical symptoms [health neural function defect scale stroke scale scores(NIHSS), activities of daily living(ADL), mini mental state examination(MMSE)], hemodynamic indexes [high-shear reduction viscosity(HSRV), medium-shear reduction viscosity(MSRV), low-shear reduction viscosity(LSRV), whole blood plasma viscosity(WBV)], vascular endothelial functions [serum nitric oxide synthase(NOS), endothelin-1(ET-1), vascular endothelial growth factor(VEGF)] were observed. The clinical efficacy and the incidence of adverse reactions were compared between two groups. The blood routine, urine routine, heart, liver and kidney function, incidence of cerebral hemorrhage in two groups were observed during the treatment. Result::There were 4 cases were shed in control group and 2 cases in observation group during the study period. The total effective rate in observation group was 94.8%(55/58), which was higher than 83.9% of the control group(47/56)(P<0.05). The NIHSS in observation group was lower than that in control group(P<0.05), whereas ADL and MMSE were higher than those in control group(P<0.05). The hemorheological indexes in observation group were lower than those in control group(P<0.05), NOS and VEGF in observation group were higher than those in control group(P<0.05), and ET-1 score was lower than that in control group(P<0.05). During the study period, the incidence of cerebral hemorrhage was 7.14%(4/56) in control group and 8.6%(5/58) in observation group, with no significant difference. Conclusion::Modified Tongqiao Huoxuetang combined with stent thrombectomy could significantly improve the clinical symptoms, hemorheology and vascular endothelial function of patients with acute cerebrovascular occlusion, and so is worthy of clinical promotion and application.

ABSTRACT

Objective:To compare the complications, degree of satisfaction and quality of life among breast cancer patients treated with different reconstruction methods after postoperative radiotherapy, aiming to explore the optimal combination of reconstruction and radiotherapy.Methods:105 breast cancer patients treated with postoperative radiotherapy after reconstruction surgery in Tumor Hospital from 2014 to 2019 were enrolled. According to the type of reconstruction, all patients were divided into group A (autologous reconstruction group, n=54) and group B (implant reconstruction group, n=51). Patients in group B were further divided into group B1(one-stage reconstruction group, n=30) and group B2(two-stage reconstruction group, n=21) according to the timing of reconstruction. The incidence of complications and BREAST-Q score were statistically compared between groups A and B, groups B1 and B2, respectively. The influencing factors of BREAST-Q score were identified by multiple linear regression analysis. Results:The incidence of long-term complications and the total incidence of complications in group A were significantly lower than those in group B (1.9% vs. 37.3%, P<0.001 and 9.3% vs.43.1%, P<0.001), and the scores of psychosocial well-being and degree of satisfaction with breasts in group A were significantly higher than those in group B (71(15) vs. 66(22), P=0.027 and 53(8) vs. 53(8), P=0.032)). There was no significant difference in the incidence of complications and BREAST-Q scores between groups B1 and B2(both P>0.05). Breast volume and complications were the predictors of BREAST-Q score (both P<0.001). Conclusions:Radiotherapy after autologous reconstruction of breast cancer yields fewer complications and better BREAST-Q score than the implantation reconstruction. The incidence of postoperative radiotherapy complications and BREAST-Q scores are equivalent between one-stage and two-stage reconstruction. The BREAST-Q score is lower in patients with large breasts or complications.

OBJECTIVE@#By observing the effect of inner-heating acupuncture on the expression of Wnt1, Axin and β-catenin in the intervertebral disc of rats with lumbar degenerative disease and to explore the regulatory mechanism of inner-heating acupuncture on the apoptosis of annulus fibrosus cells in rats with lumbar degenerative disease.@*METHODS@#Forty SD rats were randomly divided into normal group, model group, inner heating acupuncture group and acupuncture group with 10 rats in each group. Except for normal group, all rats were modeled. Bilateral were selected for inner-heating acupuncture and acupuncture treatment. Western-bolt assay was used to detect the expression of Wnt1, Axin and β-catenin in rat intervertebral discs.@*RESULTS@#Compared with the model group, Wnt1 and β-catenin expression in annulus fibrosus cells of rats in the inner heating acupuncture group and the acupuncture group were significantly decreased(0.05).@*CONCLUSION@#Inner heating acupuncture can down regulate the expression of Wnt1 and β-catenin and up regulate the expression of Axin in annulus fibrosus of rats with lumbar degenerative desease. It is suggested that the mechanism may be to regulate the expression of related factors in the β-catenin signaling pathway, so as to achieve the goal of treating lumbar degenerative diseases.

ABSTRACT

Mitochondria, as the key passway of neuronal apoptosis after ischemia, is closely related to cerebral ischemia-reperfusion injury. Remote ischemic post-conditioning can alleviate cerebral ischemia-reperfusion injury, and its mechanism is related to alleviating mitochondrial injury and improving its dysfunction. ABSTRACT

Objective@#To differentiate the inflow and outflow channels of gastric varices in cirrhotic portal hypertension patients using multi-slice spiral CT (MSCT), and to assess the relationship between calculable CT volume of gastric varices and the amount of tissue adhesive.@*Methods@#97 cases with cirrhotic gastric varices who were admitted from November 2013 to August 2017 were selected. The type and shape of gastric varices were observed before tissue glue injection treatment by MSCT. The correlation between CT volume of gastric varices and the amount of tissue adhesive was evaluated by Spearman rank correlation coefficient and Univariate linear regression analysis.@*Results@#MSCT showed that Le, g type had the highest proportion (54.6%), followed by Le, g, Lg (20.6%). Le, Lg and Lgf type accounted for 17.5%, and 5.2%, respectively, while Lgf+b accounted for 2.1%. On MSCT, varices of the gastric fundus were in the direction from bottom to top, and 75% of the fundus had a large curved side varices combined with gastric and renal shunt. Under the gastroscopy, varices in the small curved side of the gastric fundus from near to far were formless. In addition, varices in the large curved side of the gastric fundus when observed from different angles to the direction of blood flow (reverse gastroscope) were 72.7% (near and far) or 20.5 % (far and near). There was a positive correlation between CT volume (R = 0.97, P < 0.001) and the amount of tissue adhesive (Y1 = 0.35 + 0.65X1, Univariate linear regression equation; ρ = 0.89, P < 0.001, Spearman correlation analysis).@*Conclusion@#MSCT can recognize the vascular shape and inflow and outflow channels of gastric varices. A positive correlation between CT volume and the amount of tissue adhesive, suggested that the CT volume measurement before treatment could be used as one of the method to predict the amount of tissue adhesive.

ABSTRACT

Hepatic fibrosis is a repair process of the liver in response to various chronic injuries. The unbalanced proliferation and decomposition of fibrous tissue in the liver causes the excessive deposition of fibrous connective tissue in the liver. Hepatic fibrosis is a common intermediate link in a variety of chronic liver diseases. In this pathological process, the interaction between cells and cytokines leads to the disorder of liver extracellular matrix metabolism, which results in excessive deposition of fibrous tissue in the liver, and eventually causes liver fibrosis. After continuous deterioration, it would gradually develop into liver cirrhosis and even liver cancer. Because liver fibrosis and liver cirrhosis can be reversed in the early stage, it is very important to control the reversible process of liver fibrosis for the prevention and treatment of liver cirrhosis and liver cancer. Cordyceps, as one of valuable medicinal fungi in China, has been given widespread attention from scientific workers, with broad pharmacological actions. Cordyceps polysaccharide is the highest content of bioactive substances in Cordyceps. Recent studies have found that it has an anti-hepatic fibrosis effect. With the application of molecular biology technology, researches on the anti-liver fibrosis mechanism of traditional Chinese medicine at home and abroad have been gradually deepened into the cellular and molecular levels. This paper will investigate the preventive effect of Cordyceps polysaccharide in terms of cell basis, cytokines, collagenase and lipid peroxidation level that are related to liver fibrosis, so as to clarify the specific action mechanism on anti-hepatic fibrosis, and provide a reference for the development of anti-hepatic fibrosis drugs.

ABSTRACT

Objective To investigate the expression and clinical significance of vasohibin-1 (VASH-1) in patients with type 2 diabetes mellitus.Methods From November 2015 to December 2017,four hundred and twenty-two patients with T2DM in Chaoyang Central Hospital were selected and divided into three groups according to the urinary albumin excretion rate (UACR):normal albuminuria group (UACR <30 mg/g,149 cases),microalbuminuria group (30 ≤ UACR ≤ 300 mg/g,143 cases),clinical albuminuria group(UACR>300 mg/g,130 cases).Three hundred healthy subjects in Chaoyang Central Hospital during the same period were selected as control group.The levels of serum VASH-1 and transforming growth factor-β1 (TGF-β1) in each group were determined by enzyme linked immunosorbent assay.Results VASH-1 and TGF-β1 in diabetic group and control group were (579±236) ng/L,(350±141) ng/L and (18.92 ±2.21) μg/L,(5.69 ± 0.70) μg/L,respectively.There were significant differences between the two groups (t =4.721,6.142,P<0.01).The serum VASH-1 levels in normal albuminuria group,microalbuminuria group and massive albuminuria group were (450+182) ng/L,(571±194) ng/L,(786±201) ng/L,and the serum TGF-β1 levels were (11.71 ± 1.81) μg/L,(14.02 ± 2.91) μg/L,(19.32 ± 1.97) μg/L,respectively.Compared with the normal albuminuria group,the serum VASH-1 and TGF-β1 levels in the microalbuminuria group and the massive albuminuria group were significantly higher (P<0.05),and the serum VASH-1 and TGF-β1 levels in the massive albuminuria group were significantly higher than those in the microalbuminuria group,with significant difference between the two groups (P<0.05).Multivariate linear regression analysis showed that serum VASH-1 level was positively correlated with TGF-β1 in diabetic patients (r=0.554,P=0.000).Conclusion With the progress of DN microangiopathy,the serum VASH-1 concentration in type 2 diabetes mellitus increased significantly through negative feedback to regulate and control the formation of neovascularization and delay renal interstitial fibrosis,so as to achieve the protection of the kidney.

OBJECTIVE@#To investigate the efficacy of multiple protein expressions and clinical features on the threapeutic effect and prognosis of patients with newly diagnosed diffuse large B-cell lymphoma (DLBCL).@*METHODS@#The clinical data of 68 DLBCL patients were collected and analyzed retrospectively. The clinical staging was performed according to Ann Arbor staging; the risk grading was performed by IPI index; the DLBCL typing (germinal center and non-germinal center) was performed according to B cell source; the expression of Ki67,BCL-2, BCL-6, C-MYC, MUM1 and CD10 protein was detected by immunohistochemistry method; the patients were divided into R-CHOP group(50 cases) and CHOP group(18 cases) according to chemotherapy regimen of using rituximab or not; finally, the related factors affecting the prognosis of patients(PFS and OS) were analysed statistically by using SPSS 22.0 software according to sex, age, erythrocyte sedimentation rate (ESR), lactate dehydrogenase(LDH) and use of rituximab or no, as well as the above-mentioned clinical indicators.@*RESULTS@#IPI grade high-risk, elevated LDH, positive expression of BCL-2 protein and negative expression of BCL-6 protein were independent prognostic factors for progression-free survival (PFS); elevated LDH and negative expression of BCL-6 protein were independent prognostic factors for overall survival time (OS); multivariate analysis showed that elevated LDH and positive expression of BCL-2 protein were independent prognostic factors for progression-free survival (PFS). The overall survival time (OS) associated with ESR, IPI classification and BCL-6 protein expression.@*CONCLUSION@#The expression of BCL-2 and BCL-6 protein and some clinical features can be used as predictors of clinical efficacy for DLBCL. Choosing the treatment regimen combined with ritu-ximab can further improve the survival and prognosis of DLBCL patients.
