ABSTRACT

Objetivo: Analizar las características epidemiológicas y complicaciones feto/neonatales del embarazo múltiple en el Hospital Militar de Maracaibo. Métodos: Estudio descriptivo y retrospectivo en pacientes ingresadas con embarazo múltiple desde el 01 de enero de 1998 al 30 de mayo de 2013. Se registró edad materna, presentación, tipo de parto, motivo de interrupción del embarazo, corionicidad, complicaciones feto/neonatales, peso y anomalías congénitas. Resultados: De 2 469 embarazadas ingresadas para parto, 31 (1,26 %) fueron múltiples o 1/80 nacimientos. Edad materna promedio 28 años, presentación fetal más frecuente cefálica-podálica en 12 (38,7 %) y se realizó cesárea en 27 (87,1 %) pacientes. La indicación de interrupción del embarazo más frecuente fue la electiva en 12 (38,7 %) y prematuridad en 11 (35,5 %). Se obtuvieron 62 productos y la mortalidad fue 17,7%, con 11 muertes fetales y/o perinatales, de estas 8 (72,7 %) fueron por causa de la monocorionicidad (P < 0,05) y sus consecuencias, en 5 (45,46 %) fetos fue por síndrome de transfusión intergemelar. Se diagnosticó restricción del crecimiento intrauterino en 13 (20,9 %) y en 26 (42 %) crecimiento fetal discordante moderado o severo. En 8 (8,1 %) neonatos se observaron anomalías congénitas predominantemente osteomusculares Conclusión: El embarazo múltiple tiene una elevada morbimortalidad fetal y neonatal dependiente principalmente de las complicaciones derivadas de la corionicidad, amniocidad, prematuridad y de la posibilidad de anomalías congénitas. Realizar el diagnóstico temprano de la corionicidad permite planificar medidas de vigilancia prenatal y garantizar la mejor calidad de vida materno feto/neonatal.


Objective: To analyze epidemiological characteristics and fetal/neonatal complications of twin pregnancy at the Military Hospital of Maracaibo. Methods: A descriptive and retrospective study of patients admitted with twin pregnancies, from January 1st, 1998 to May 30th, 2013. Maternal age, fetal presentations, mode of delivery, reason for interruption of the pregnancy, chorionicity, fetal/neonatal complications, weight and congenital anomalies were registered. Results: There were 31 (1.26 %) twin deliveries (or 1/80) in 2 469 pregnant woman admitted for delivery. Average maternal age was 28. The most common fetal presentation was vertex-breech in 12 (38.7 %) and in 27 (87.1 %) patients cesarean section was performed. The most common indication of pregnancy interruption were the elective delivery in 12 (38.7 %) and prematurity in 11 (35.5 %). 62 fetuses were obtained and mortality was 17.7 % with 11 stillbirth and/or perinatal death, in 8 (72.7 %) were due to monochorionicity (P< 0.05) and its consequences, of these 5 (45.46 %) were for twin-twin transfusion syndrome. Intrauterine growth restriction in 13 babies (20,9 %) and mild or severe discordant fetal growth in 26 (42 %). Congenital anomalies were observed in 8 fetuses (8.1 %) with a musculoskeletal predominance. Conclusion: Multiple pregnancy has high fetal/neonatal morbimortality which is principally dependent of the complications derived from the chorionicity, amnionicity, prematurity and the possibility of congenital anomalies. An early diagnose of the chorionicity allows the planning of antenatal care and guaranteeing the improvement of the mother and fetal/neonatal life and health.

ABSTRACT

OBJETIVO: Pacientes com esclerose mesial temporal (EMT) são clinicamente classificados como concordantes ou discordantes de acordo com a extensão da atividade epileptogênica. O objetivo desse estudo é verificar se as alterações metabólicas no hipocampo são diferentes nos dois grupos. MATERIAIS E MÉTODOS: Foram avaliados 33 pacientes diagnosticados com EMT, 23 concordantes (33±9 anos) e 10 discordantes (33±10 anos), e 28 controles (33±10 anos). Foi obtida espectroscopia por ressonância magnética de ambos os hipocampos (ipsi e contralateral) com aparelho de 3T e com a sequência PRESS de vóxel único com TE/TR=135/1500ms. Os principais metabólitos foram quantificados com o LCModel. Para a comparação entre os grupos foram realizados testes estatísticos com nível de significância de 0,05. RESULTADOS: Para todos os pacientes foi observada redução de NAAipsi e Cr ipsi quando comparado com o lado contralateral, porém esta diferença foi significativa apenas para o grupo de pacientes concordantes. Quando comparado com o grupo controles houve uma redução significativa de Cr ipsi e NAAipsi para os dois grupos e da relação NAA/Cr ipsi para o grupo de pacientes concordantes enquanto que a relação NAA/Cr contra estava diminuída apenas no grupo de pacientes discordantes. CONCLUSÃO: Nossos achados sugerem uma maior diminuição do NAA/Cr ipsi no grupo de pacientes concordantes e da relação NAA/Cr contra no grupo de pacientes discordantes apontando para um padrão diferente de alteração metabólica para os dois grupos. Porém é preciso aumentar o tamanho da amostra para confirmar estes resultados.


PURPOSE: Patients with mesial temporal sclerosis (MTS) are clinically classified as concordant or discordant according to the extent of epileptogenic activity. The aim of this study is to determine whether the metabolic changes in the hippocampus are different in the two groups. MATERIALS AND METHODS: 33 patients diagnosed with MTS, 23 concordant (33±9 years old) and 10 discordant (33±10 years old) and 28 controls (33±10 years old) were evaluated. We obtained magnetic resonance spectroscopy of both hippocampi (ipsilateral and contralateral) on a 3T scanner with single voxel PRESS sequence with TE/TR=135/1500ms. The main metabolites were quantified with LC Model. For comparison between groups statistical tests were performed with a significance level of 0.05. RESULTS: In all patients a reduction of NAAipsi and Cr ipsi was observed, when compared to the contralateral side, but this difference was only significant for the group of concordant patients. In comparison with controls a significant reduction of Cr ipsi and NAAipsi was observed for both groups and for Naa/Cr ipsi in the group of concordant patients while NAA/Cr contra was reduced only in the group of discordant patients. CONCLUSION: Our findings suggest a stronger decline of NAA/Cr ipsi in the group of concordant patients and of NAA/Cr contra in discordant patients, suggesting a slightly different metabolic pattern for both groups. However, we need to increase the sample size to confirm these findings.

Introducción. El objetivo de la terapia antirretroviral es lograr un descenso rápido de la carga viral y un aumento en el recuento de las células CD4- Sin embargo, en la respuesta discordante sólo se cumple uno de los objetivos de la terapia: disminución de la carga viral o aumento de linfocitos CD4. Métodos. Estudio retrospectivo de pacientes pediátricos que acuden a la Clínica de Inmunodeficiencias del Hospital Infantil de México Federico Gómez. La respuesta inmunológica/virológica a terapia antirretroviral se clasificó en 4 clases: óptima, falla, respuesta discordante con éxito viral y respuesta discordante con fracaso viral. Se analizaron los datos demográficos y las determinaciones de carga viral y cuenta de linfocitos CD4 al ingreso, a los 6 y 12 meses posteriores al inicio del tratamiento. Resultados. Se analizaron 56 pacientes, con edad promedio a su ingreso de 30.5 meses. La frecuencia de respuesta discordante al tratamiento, después de 6 y 12 meses fue de 45 y 53.5%, respectivamente. Conclusión. La respuesta discordante en nuestra población pediátrica es mayor de 50%, similar a lo reportado en otros estudios pediátricos. A pesar de ser un escenario común, la repercusión clínica aún es incierta, por lo que este tipo de respuesta no debe de interpretarse, en primera instancia, como una mala respuesta a la terapia antirretroviral.


Introduction. The goal of antiretroviral (ART) therapy is to control human immunodeficiency virus replication and to increase CD4 T-cell count. Discordant response to ART occurs when only one of these two objectives is achieved. Methods. Pediatric patients who attended the Clínica de Inmunodeficiencias at Hospital Infantil de Mexico Federico Gomez were studied retrospectively. Response to a Pibased highly active antiretroviral therapy (HAART) was classified as optimal, failure, discordant response with viral failure and discordant response with viral success. Demographic data, viral load and CD4 T-cell count (baseline, 6-month and 12-month follow-up determinations) were analyzed. Results. Fifty six patients were included. Mean age of patients was 30.5 months. Discordant response was seen in 45 and 53.5% at 6- and 12-months follow-up, respectively. Conclusions. Discordant response was seen in >50% of our study population and is a common scenario; clinical implications are still unknown. Discordant response should not be interpreted, especially during the first 12 months of therapy, as a failure of HAART.
