ABSTRACT

Monopolar spindle 1, also known as threonine and tyrosine kinase (TTK), is a key component of spindle assembly checkpoint (SAC). It is considered to be a monitoring mechanism to ensure mitotic fidelity and genomic stability. TTK is overexpressed in a variety of malignant tumors, and patients with low expression of TTK tend to have a longer survival time, suggesting that it may be used as a biomarker for diagnosis and prognosis. Abnormal expression of TTK often impairs the function of SAC, resulting in irregular mitosis, increased aneuploidy and mitotic disaster, thus promoting the occurrence of tumors. Current studies have shown that TTK inhibitors can inhibit the proliferation of tumor cells and increase the sensitivity of tumor cells to therapy in combination with chemotherapy or radiotherapy to achieve sensitization and attenuated effects. This article will review the research and application of TTK and its inhibitors in malignant tumors.

ABSTRACT

Objective To explore the CT imaging features of renal mucinous tubular and spindle cell carcinoma(MTSCC).Methods The CT images of 9 cases patients with renal MTSCC confirmed by pathology were analyzed retrospectively,and their size,shape,density,degree of enhancement and enhancement mode were analyzed.Results There were 6 cases of left renal and 3 cases of right renal,with the largest diameter ranging from 1.6 cm to 7.7 cm.The shape of renal MTSCC was round in 4 cases,oblong in 3 cases,and fan-shaped in 2 cases.The long axis of the oblong tumor was parallel to the renal column,the central angle of the fan-shaped tumor was located in the renal medulla,and the arc was located under the renal capsule.Renal MTSCC was mainly located in the renal medulla.There were 6 cases of complete endophytic tumors,5 of which compressed the renal sinus.The tumor density was uniform in 5 cases,and the CT value of the solid component of the tumor was(32.43±4.82)HU,and the difference was not statistically significant compared with that of the renal parenchymal density(P=0.859).After enhancement,the solid component of the tumor showed mild uniform enhancement in the cortical phase,with a CT value of(41.71±6.74)HU.In the parenchymal phase and excretory phase,there was progressive enhancement,and the CT values were(58.23±9.42)HU and(61.81±9.49)HU,respectively.The CT value of each phase of tumor after enhancement was lower than that of renal medulla in the same period,and the differences were statisti-cally significant(P=0.001,P=0.005,P=0.002).Conclusion Renal MTSCC is mainly located in the renal medulla,which is easy to compress the renal sinus.It can be oblong or fan-shaped.Cystic,necrosis and calcification are rare.After enhancement,the tumor shows mild uniform enhancement in the cortical phase,progressive enhancement in the parenchymal phase and the excretory phase,and the CT value of each phase are lower than that of the renal medulla in the same period,which can suggest the diagnosis.

ABSTRACT

Objective:To explore and analyze the application of polysomnographic sleep monitor in patients with schizophrenia and the monitoring effect of that on sleep quality and sleep structure of them.Methods:A total of 90 patients with schizophrenia admitted to the First Affiliated Hospital of Xinjiang Medical University from March 2021 to May 2023 were selected as the observation group,and 80 healthy volunteers were selected as the health control group.All subjects were monitored by polysomnographic sleep monitor.Pittsburgh Sleep Quality Index(PSQI)score,sleep quality index of polysomnographic sleep monitor,the indicators of sleep structure and spindle wave index between two groups were compared.At the same time,Pearson correlation analysis was used to explore the correlation between PSQI score and sleep parameters.Results:PSQI score of the health control group was(5.36±0.65)scores,and that of the observation group was(14.24±3.58)scores,and the PSQI score of the observation group was significantly higher than that of the health control group,and the difference was statistically significant(t=23.115,P<0.05).Compared with the health control group,the observation group had shorter total sleep time,longer sleep latency,shorter rapid eye movement(REM)period and more awakening times,with statistical significances(t=15.136,40.355,36.620,24.226,P<0.05),respectively.There was no significant difference in REM latency between the observation group and the control group before treatment(P>0.05).Compared with the observation group before treatment,the observation group after treatment had longer total sleep time,shorter sleep latency,longer REM period and less awakening times,with statistical significances(t=3.145,12.021,8.668,9.101,P<0.05),respectively.Compared with health control group after treatment,the observation group after treatment had shorter total sleep time,longer sleep latency,shorter REM period and more awakening times,and the differences of them between two groups were statistically significant(t=9.704,14.781,15.899,9.901,P<0.05).Compared with the observation group before treatment,the N1%value was higher,the N2%value was higher and the N3%value was lower in the health control group before treatment,and the differences were statistically significant(t=10.163,9.483,10.065,P<0.05),respectively.There were no significant differences in REM%between the health control group and the observation group before and after treatment(P>0.05),respectively.Compared with the observation group before treatment,that after treatment had lower N1%value and N2%value,and higher N3%value(t=10.163,9.483,10.065,P<0.05),respectively.Compared with the health control group after treatment,the observation group after treatment had higher N1%value and N2%value,and lower N3%value,and the differences of them between two groups were statistically significant(t=7.628,4.210,7.153,P<0.05),respectively.Compared with the observation group before treatment,that after treatment had higher spindle wave density,amplitude and time.Compared with the health control group after treatment,the observation group after treatment had lower spindle wave density,amplitude and time,and the differences of them between two groups were significant(t=2.514,2.665,2.014,P<0.05),respectively.Pearson correlation analysis showed that PSQI score appeared significantly negative correlation with total sleep time,REM period,N3%value,spindle wave density and spindle amplitude,and appeared significantly positive correlation with sleep latency,awakening times,N1%value and N2%value,respectively,and the differences were statistically significant(r=-0.612,-0.269,-0.812,-0.778,-0.841,r=0.382,0.226,0.654,0.778,P<0.05).Conclusion:Abnormal sleep quality and structure,as well as abnormal sleep spindle wave activity,of patients with schizophrenia can be observed by using polysomnographic sleep monitor,which indicators is closely related to PSQI.

Introducción: El hiatus glótico longitudinal es un signo descrito en la práctica clínica a partir de las fibrolaringoscopias de pacientes con patologías laríngeas. Objetivo: Describir la prevalencia de patologías laríngeas orgánicas y funcionales mediante videolaringoestroboscopia, y evaluar la frecuencia de hiatus longitudinal y su asociación con patología orgánica en pacientes adultos disfónicos. Material y Método: Se realizó un estudio retrospectivo y descriptivo durante 2018-2019 en un hospital de alta complejidad. Se incluyeron las videolaringoestroboscopia de pacientes mayores de 14 años que consultaron por disfonía. Las patologías laríngeas se categorizaron en orgánicas o funcionales. La asociación entre patología orgánica y el hiatus longitudinal se analizó mediante un análisis multivariado ajustado por confundidores. Resultados: Se incluyeron 310 pacientes de los cuales se analizaron 269. La media de edad fue 43,9 ± 7,09 años y el porcentaje de sexo femenino 65,8% (n: 177). La prevalencia de disfonía orgánica fue del 84,01% y de disfonía funcional, del 13,38%. La prevalencia de hiatus longitudinal fue del 23,79% (n: 64). Se observó una asociación, estadísticamente significativa entre la presencia de hiatus longitudinal y la presencia de patología orgánica con OR de12,64 (1,60-99,42; p: 0,01) ajustada por edad, sexo, tabaquismo y antigüedad de la disfonía. Conclusión: La prevalencia de patologías laríngeas orgánicas es mayor a la de funcionales y el hiatus longitudinal se asocia a patología orgánica ajustada por confundidores.


Introduction: Spindle-shaped glottic chink is a sign described in clinical practice from fibrolaryngoscopy of patients with laryngeal pathologies. Aim: to describe the prevalence of organic and functional laryngeal pathologies by videolaringostroboscopy, and to evaluate the frequency of spindle-shaped glottic chink and its association with organic pathology in dysphonic adult patients. Material and Method: A retrospective and descriptive study was conducted during 2018-2019 in a high complexity hospital. Patient's videostroboscopy older than 14 years old who consulted for dysphonia were included. Laryngeal pathologies were categorized into organic or functional diseases. The association between organic pathology and spindle-shaped glottic chink was analyzed using a multivariate analysis adjusted for confounders. Results: 310 patients were included of whom 269 patients were analyzed. The mean age was 43.9 ± 7.09 years and the percentage of female sex was 65.8 % (n: 177). The prevalence of organic dysphonia was 84.01% and of functional dysphonia, 13.38%. The prevalence of spindle-shaped glottic chink was 23.79% (n:64) (18.67-28.91%). A statistically significant association was observed between the presence of spindle-shaped glottic chink and the presence of organic pathology with OR of 12.64 (1.60- 99.42; p: 0.01) adjusted for age, sex, smoking status and history of dysphonia. Conclusion: The prevalence of organic laryngeal pathology is higher than functional and spindle-shaped glottic chink is associated with organic pathology adjusted for confounders.

ABSTRACT

Leprosy is a chronic granulomatous disease caused by Mycobacterium leprae. Histoid leprosy is considered an uncommon multibacillary form of leprosy characterized by presence of histoid lepromas which present as erythematous round or oval shaped glistening nodules. Most commonly this form of leprosy is seen in patients on irregular therapy with MDT. In this series we report a total of three cases with their clinical, histopathological and dermoscopic findings.

@#Objective Exposure to nocturnal traffic noise at the same levels can produce different effects on sleep and psychological health in different populations. This study compared the electroencephalographic characteristics during sleep of people with insomnia and those with normal sleep quality under the same levels of nocturnal traffic noise exposure to elucidate the different effects of noise pollution on different populations,and to provide individualized precise treatment plans for people with insomnia who are sensitive to noise pollution. Methods We selected people who lived near highways,railways,and aircraft routes for more than six months where noise levels were relatively fixed(50-60 decibels,10-15 seconds per time) during the nighttime sleep period (22:00-6:00). We included 26 people with normal sleep quality and 30 people with insomnia based on the Pittsburgh Sleep Quality Index. The Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were administered. Polysomnography (PSG) was used to monitor the spindle density index during sleep. The sleep quality and psychological status of the two groups were compared. Results There were no significant differences in demographic characteristics between the two groups under the same nocturnal noise exposure conditions. The incidence of anxiety and depression was significantly lower in the people with normal sleep quality than in those with insomnia(P<0.01). Among the 26 people with normal sleep quality,5(19.23%) had mild anxiety,and no one had moderate to severe anxiety or depression. Among the 30 people with insomnia,6(20.00%) had mild anxiety,15 (50.00%) had moderate anxiety,and 4(13.33%) had severe anxiety;3(10.00%) had mild depression,9(30.00%) had moderate depression,and 3(10%) had severe depression. PSG showed that the sleep spindle index of the normal sleep group was significantly higher than that of the insomnia group(25.43 vs 16.60,P<0.05),with no significant differences in the average duration of sleep spindles during sleep between the two groups (P>0.05). Conclusion Under the same levels of nocturnal traffic noise exposure,people with insomnia have a higher incidence of anxiety and depression than people with normal sleep quality. Patients with insomnia with a reduced spindle density index have worse sleep quality under exposure to noise pollution.

ABSTRACT

In November 2019, we received and treated a patient with MTSCC of the left kidney. The tumor was located at the upper pole of the left kidney, with a size of 23.3 cm×18.0 cm×21.8 cm. She underwent transperitoneal radical nephrectomy. There was no local recurrence or distant metastasis during the follow-up of 3 years and 2 months. MTSCC of the kidney is a rare subtype of renal carcinoma, with slower disease progression, a clear and smooth rim, and fewer invasion or metastasis. Its final diagnosis should depend on pathology examination. Surgical treatment is the only effective intervention for this disease at present.

Objective:To investigate the clinicopathological features,immunophenotype,and prognosis of head and neck spindle cell squamous cell carcinoma(SCSCC)to improve the understanding and diagnosis of this tumor.Methods:Clinicopathological data collected from January 2012 to December 2022 at the First Affiliated Hospital,Air Force Military Medical University from 20 patients with head and neck SCSCC were retrospectively reviewed for histological morphology and immunophenotype.In situ and fluorescence in situ hybridization were performed to detect EBV-encoded ribonucleic acid(EBER)status and MDM2 gene amplification,respectively.Results:The median age among the 20 SCSCC cases was 67 years with a male-to-female ratio of 4∶1.Tumor locations were laryngeal(35.0%)and sinonasal(30.0%).SCSCC presented as polypoid or exogenous growths(61.5%),often with surface ulceration(90.0%).Histologically,sarcomatoid growth pat-terns were exhibited in 75.0%of the patients(n=15),while the remainder showed granular tissue-like or angiosarcomatoid patterns.Most tumors(65.0%)displayed components of conventional squamous cell carcinomas,with a predominant occurrence of high to moderate dif-ferentiation(91.7%).In terms of immunohistochemistry,AE1/AE3 was expressed in 83.3%(15/18)of cases,while p63 and p40 expression rates were 62.5%and 66.7%,respectively.All cases were negative for EBER.The Ki-67 proliferation index ranged 10%-70%.Overall,33.3%(1/3)of the cases showed MDM2 gene amplification.Among these,the median follow-up time for 18 patients was 18.3 months(range:1-92 months),with 6 survivors and 12 deaths.Conclusions:Head and neck SCSCC is more prevalent among elderly male smokers,predominantly exhibits a polypoid growth pattern,and does not display human papillomavirus or Epstein-Barr virus infection.Diagnosis requires a compre-hensive evaluation of clinical and pathological features and immunophenotype.Surgical resection is the primary treatment method.

ABSTRACT

Objective:To explore the clinicopathological features and prognosis of renal mucinous tubular spindle cell carcinoma (MTSCC).Methods:The clinical data of 16 patients with MTSCC admitted to the First Affiliated Hospital of Zhengzhou University from July 2013 to July 2022 were retrospectively analyzed. There were six male cases and ten female cases. The mean age was (56.4±11.4) years old. Among them, 10 cases were asymptomatic, two complained of hematuria, three complained of lumbar pain, and one complained of lower limb pain. Twelve cases underwent preoperative enhanced CT examination, 6 cases of ultrasound examination, 3 cases of MRI examination, and 1 case of bone scan. Imaging manifestations showed that the masses were round or round-like with clear borders. Two cases combined with hemorrhage and three cases combined with calcification. Five cases showed exophytic growth, 10 cases partially exophytic, and 1 case completely endophytic. The maximum diameter of the tumor was (65.7±27.4) mm. The tumors were located in the left kidney in 11 cases and in the right kidney in 5 cases. The tumors were mildly delayed-enhancing under enhanced CT, long/short T1 signal mixed with long/short T2 signal under MRI, and diffusion-limited high signal under DWI. The tumors were hypoechoic masses without obvious blood flow signals under ultrasound. Twelve cases were diagnosed as renal occupying neoplasms, 2 cases were suggested as lack of blood supply renal tumor, and one was considered renal tumor rupture and bleeding. In one case, a bone scan suggested metastasis to the thoracic spine and pelvis. The metastatic renal tumor was diagnosed, and a renal puncture was performed to clarify the pathology. Eleven patients underwent laparoscopic radical nephrectomy, and 4 patients underwent partial nephrectomy. One case was metastasized without surgery and treated with apatinib mesylate and zoledronic acid.Results:The postoperative pathological specimens showed grayish, grayish-yellow, or grayish-red masses with a soft or medium texture. No perinephric, ureteral, or adrenal invasion was seen in all tumors. Microscopically, the tumor cells were round and ovoid. The tumor cells were arranged in tubular and striated shapes, and mucus pools were locally visible. No sarcomatous component was seen in all tumors. There were 9 patients with pT 1N 0M 0, 6 patients with pT 2N 0M 0, and 1 patient with pT 1N 0M 1. After operation, 2 patients with pT 2N 0M 0, who underwent laparoscopic radical nephrectomy were treated with pazopanib and sunitinib, respectively. All patients were followed up for a median of 50.7(25.8, 75.0)months, 15 patients were free of recurrent metastases, and 1 patient with pre-puncture metastasis died due to tumor progression of multiple pulmonary and bone metastases, with a survival of 16.9 months. Conclusions:Renal MTSCC is rare, mostly found on physical examination, with female patients predominantly, and imaging shows a lack of blood supply tumor. Surgery is the primary treatment method. Partial nephrectomy or radical nephrectomy could be chosen according to the tumor stage, kidney function, and patient's underlying condition, and patients have a good prognosis.

@#Spindle Cell Carcinoma of the Ovary is arate form of cancer with controversial histogenesis. It shares the histologic, cytologic, and molecular properties of both epithelial and mesenchymal differentiation of ovarian neoplasms, which makes diagnosis very challenging among pathologists. Endometrioid cystadenoma is a benign ovarian neoplasms classified under epithelial ovarian tumors.Malignant transformation of benign ovarian neoplasms is known as a rare complication, occurring in approximately 0.9% of patients with ovarian endometriosis. Clear cell adenocarcinoma is the most common endometriosis-associated ovarian cancer followed by endometrioid cancer. This is the case of a 56-year old post-menopausal patient initially presenting with increasingabdominal girth. Whole abdominal ultrasound revealed a large pelvo-abdominal mass. Transvaginal and transabdominal ultrasound findings of bilateral ovarian new growth with benign sonologic features. The patient underwent bilateral salpingo-oophorectomy. Histopathologic findings of the specimen submitted revealed high-grade spindle cell carcinoma arising in an endometrioid cystadenoma of the right ovary, and endometrioid cystadenofibroma with focal epithelial proliferation.

ABSTRACT

Spindle assembly checkpoint (SAC) is a protective mechanism for cells to undergo accurate mitosis. SAC prevented chromosome segregation when kinetochores were not, or incorrectly attached to microtubules in the anaphase of mitosis, thus avoiding aneuploid chromosomes in daughter cells. Aneuploidy and altered expression of SAC component proteins are common in different cancers, including lung cancer. Therefore, SAC is a potential new target for lung cancer therapy. Five small molecule inhibitors of monopolar spindle 1 (MPS1), an upstream component protein of SAC, have entered clinical trials. This article introduces the biological functions of SAC, summarizes the abnormal expression of SAC component proteins in various cancers and the research progress of MPS1 inhibitors, and expects to provide a reference for the future development of lung cancer therapeutic strategies targeting SAC components.

Introduction: Hyperpigmentation is one of the most common reac?on to inflammatory, benign and malignant lesions of the skin. These disorders comprise heterogeneous group of diseases of epidermal and dermal hyperpigmenta?on divided into various types according to e?ology and pathology. Correct diagnosis of these hyperpigmented lesions is linked to histopathologic examina?on of skin biopsies with clinical correla?on. Aim: To study the spectrum of hyperpigmented skin lesions with reference to age and sex distribu?on. Materials and Methods: This prospec?ve cohort study was conducted at Department of Pathology, at Alluri Sitaramaraju Academy of Medical Sciences, Eluru, Andhra Pradesh, India, which included 80 pa?ents who were clinically diagnosed with hyperpigmented skin lesions in all age groups from July 2014 to August 2016. Frequency and percentage sta?s?cs was used to present the results. Results: Out of 80 cases, 34 cases of inflammatory lesions, 23 cases of benign lesions and 23 cases of malignant lesions were reported. Among the post inflammatory lesions the majority were classical Lichen planus. Conclusion: Most common lesion was lichen planus and its variants with highest incidence in females and age group greater than 60 years. Histopathological diagnosis with clinical correla?on aids in effec?ve management of the pa?ents.

ABSTRACT

We report a case of a 49-year-old male patient suffering from an intraspinal tumor in the lumbar vertebra. The neoplasm was composed of mono-morphic spindle cells, arrayed in a patternless pattern in a background of prominent myxoid hyaline stroma with perivascular collagen rings in hyper-cellular regions. Instead, aggregated collagen fibers arranged into nodules and apparent calcium deposition were found in hypo-cellular regions. The tumor cells showed immunopositivity with S100 and CD34, whereas lacked SOX10 expression, which were reminiscent of a group of S100 and CD34 co-expression soft tissue spindle cell lesions having recurrent fusions including RAF1, BRAF, NTRK1/2/3, and RET genes. Interestingly, a novel anaplastic lymphoma kinase (ALK)- echinoderm microtubule-associated protein-like 4 (EML4) gene fusion was revealed. To our best knowledge, it was the first time to identify such gene fusion in the Orientals among this mentioned group, and it expands the molecular genetic spectrum of this specific group. The clinical relevance of this novel fusion requires further investigations.

ABSTRACT

Neurofibromas of the penis occur uncommonly and their solitary occurrence is even rarer. We report a case of a ten year old child presenting with a solitary neurofibroma located on penis. A provisional clinical diagnosis of penile retention cyst was made and histopathological examination releaved a benign spindle cell lesion and few close differential diagnosis were made, which on application of immunohistochemistry was diagnosed as neurofibroma of penis. Surgical excision of the lesion was done and there was no recurrence after a year of follow up.

Spindle cell sarcoma of heart are the least reported primary cardiac tumours. We present a case of a 60-year-old man reported to us following successful resuscitation after cardiac arrest. This patient presented with symptoms of dyspnoea on exertion. The echocardiography showed features of cardiac tamponade. CT scan chest+ Abdomen + Pelvis confirmed echocardiography findings, and showed significant pericardial effusion with early cardiac tamponade. Patient continued to suffer dyspnoea even after pericardiocentesis and was unstable in the intensive care unit, hence he was shifted to operating room for re-exploration. The mass was excised in a piecemeal without instituting cardiopulmonary bypass. The total weight of the mass was approximately 500gms. The macroscopic examination of the specimen revealed a cystic mass with solid grey brown tissue. Following surgical debulking, chest X -ray in Intensive Care Unit showed improvement. The patient visited the outpatient clinic after 15 days of surgery. 2-D echocardiography revealed minimal pericardial effusion and patient was comfortable.

ABSTRACT

Abstract Aim: The present study aimed to compare the strength performance and the neuromuscular activity during one maximum repetition test (1RM), and the maximum voluntary isometric contractions (MVIC) performed with whole-body vibration (WBV), local vibration (LV), and no vibration (NV). Methods: The sample consisted of 15 males, experienced in strength training for at least 6 months, which performed all strength tests in the barbell curl exercise across randomized trials on the following conditions: NV, WBV, and LV. During all tests, the normalized root means square values of the electromyographic signals (EMGRMS) of the biceps brachii and brachioradialis were recorded and compared between the conditions. The one-way ANOVAs with repeated measures were used to compare the results of 1RM and MVIC tests and the normalized EMGRMS between the conditions. When necessary, a post hoc Scott-Knott test was used to identify the differences reported in the ANOVAs. The significance level adopted was α < 0.05. Results: The EMGRMS response of the biceps brachii and brachioradialis muscles during the 1RM and MVIC tests presented significantly higher values at LV compared to WBV, and NV (p < 0.001). The 1RM tests, and the MVIC results were similar between conditions (p = 0.9803; p = 0.061, respectively). Conclusion: These results indicate that the application of MV was not sufficient to increase strength performance.

ABSTRACT

【Objective】 To analyze the clinical and pathological characteristics of mucinous tubular and spindle cell carcinoma (MTSCC) in the kidney so as to evaluate the value of radiotherapy in local treatment of refractory MTSCC. 【Methods】 A case of MTSCC with recurrent recurrence and metastasis was reported, and the existing literature on MTSCC was reviewed and analyzed to explore the value of radiotherapy in the treatment of MTSCC. 【Results】 After three operations, postoperative pathology of the patient showed mucinous tubular and spindle cell carcinoma of the kidney. Radiotherapy was performed in the first recurrence area, and long-term follow-up in the tumor bed area showed no recurrence. 【Conclusion】 MTSCC, a rare renal epithelial tumor, has the low-grade malignance in most cases, and rarely recurs and has metastasis. This patient had repeated recurrence and metastasis before surgery, but after tumor bed radiotherapy after the first recurrence no lesion was found, suggesting that radiotherapy plays a certain role in preventing local recurrence. It is necessary to further explore the value of radiotherapy in the treatment of MTSCC.
