ABSTRACT

The colonic epithelial cells represent a border between the colon luminal content, containing notably bacteria and a complex mixture of compounds, and the"milieu interieur"as defined by the French physiologist Claude Bernard.The physical-chemical composition of the luminal content, including luminal pH and bacterial metabolite, that obviousl y is not constant, is modified for instance according to the diet. Data obtained recently indicate that physical exercise may also modify the colonic luminal content. Evidence has indicated that modification of the luminal content characteristics has, indeed, consequences for the colonic epithelial cells, notably in terms of energy metabolism and DNA integrity. Although such alterations impact presumably the homeostatic process of the colonic epithelium renewal and the epithelial barrier function, their contribution to pathological processes like mucosal inflammation, pre-neoplasia, and neoplasia remains partly elusive. Open questions remain regarding the individual and collective roles of luminal changes, particularly in a long-term perspective. These questions are related particularly to the capacity of the bacterial metabolites to cross the mucus layer before entering the colonocytes, to the concentrations of metabolites in proximity of the colonic crypt stem cells, and to the capacity of colonocytes to detoxicate deleterious compounds, to take up and utilize beneficial compounds.

ABSTRACT

The present study was conducted on six healthy early neonatal and six prepubertal buffalo calves to study the location, gross morphology, histomorphology and histochemistry of lymphoglandular complexes in proximal colon. In very proximal part of colon of buffalo calves, an irregular oval mucosal lymphoid patch was found grossly as a proximal colon (PC) patch. Histologically, in proximal colon patch of early neonates (3-4 weeks), an extensive invasion of mucosal glands was observed towards lymphoid nodules that were present in submucosa. The structure as a whole thus formed a complex known as lymphoglandular complex (LGC). Large number of such complexes i.e., LGCs were observed in submucosa of proximal colon at this age. At some places, invasion of mucosal glands into lymphoid tissue was restricted to superficial layer of complexes, with the lymphoglandular complexes opening directly into the lumen but some were deep seated. However, by the age of 6 months in buffalo calves i.e., prepubertal period, LGCs were reduced and were present in single layer within the submucosa of the proximal colon. Moreover, some of LGCs were completely encapsulated by their own lamina muscularis mucosae. But some of the complexes still had their mucosal openings into lumen while others had lost their connection with tunica mucosa. Histochemically, the glands that were observed within LGCs contained mucosubstances, glycogen, mucopolysaccharides, and mucin. However, lipids were present around the lymphocytes observed towards the periphery of these LGCs.


El presente estudio se llevó a cabo en seis terneros de búfalo neonatos sanos y seis terneros prepuberales para estudiar la ubicación, morfología macroscópica, histomorfología e histoquímica de los complejos linfoglandulares en el colon proximal. Se observó en un área del colon proximal (AP) de los terneros de búfalo un óvalo linfoide de mucosa irregular en la parte más proximal de éste. Histológicamente, en el área proximal del colon de los terneros neonatos (3-4 semanas), se observó una invasión extensa de las glándulas mucosas hacia los nódulos linfáticos presentes en la submucosa. La estructura en su totalidad formaba un complejo conocido como complejo linfoglandular (CLG). A esta edad se observó un gran número de estos complejos es decir, se observaron CLGs en la submucosa del colon proximal. La invasión de las glándulas mucosas en el tejido linfoide, se limita a la capa superficial de los complejos, los complejos linfoglandulares distribuidos directamente en el lumen, sin embargo otros se encontraban arraigados de manera profunda. En búfalo a los 6 meses de edad, es decir en el período prepuberal, se observó un número reducido de CLGs presentes en una sola capa dentro de la submucosa del colon proximal. Por otra parte, algunos de CLGs estaban completamente encapsulados por su propia lamina muscularis mucosae. Algunos de los complejos mantenían abertura de las mucosas en el lumen, mientras que otros habían perdido su conexión con la mucosa. En análisis histoquímico, las glándulas que se observaron dentro del CLGs contenían mucosustancias, glucógeno, mucopolisacáridos y mucina. Sin embargo, se encontraron lípidos presentes alrededor de los linfocitos hacia la periferia de los CLGs.

Objective The aim of this study was to investigate retrorenal colon incidence in percutaneous nephrolithotomy (PNL) interventions made in our clinic. Materials and Methods Clinical data of 804 PNL patients, accumulated over a 7 year period (2006-2012), was surveyed. The patient files were reviewed retrospectively, and only those who had abdominal computed tomography (CT) images before PNL intervention were included in the study. In the CT images, the position of both the ascending and descending colon in relation to the right and left kidneys were evaluated. Results According to our hospital reports, 394 patients with CT images were included in the present study 27 patients (6.9%) had retrorenal colon, of which 18 (4.6%) were on the left side, 4 (1.0%) on the right side and 5 (1.3%) had bilateral retrorenal colons. Colonic perforation complication was seen only in two patients and the colonic perforation rate was 0.3%. These two cases had no CT images. Conclusions PNL, in the process of becoming the standard treatment modality, is a safe and reliable technique for renal stone treatment. Colonic injury should be taken into consideration during PNL interventions of the lower pole of the kidney (especially on the left side) due to the location of retrorenal colon. .

ABSTRACT

The left displacement of the large colon, a condition that commonly occurs in horses, has two clinical manifestations. Different treatments including medical treatment, rolling the horse under general anesthesia or surgical correction have been recommended. The diagnosis can often be made by rectal examination and confirmed by percutaneous ultrasonography. During the period between 2004 and 2009, 11 horses were treated for left displacement of the large colon by rolling the horses under general anesthesia, using a slightly different technique than the ones previously described. The clinical case selection was based on rectal palpation confirmed with ultrasonogram. Nine animals were successfully treated and two had to be submitted to surgery after three attempts of rolling. No short or long term complications were observed after the procedure. Only one animal had a recurrence of the clinical manifestation 10 months after the first treatment and was successfully rolled once again. Despite a bit different from the other rolling procedures, this also proved to be an effective procedure. Rolling a horse even when attempted more than once showed to be a safe procedure, however, we reinforce the need for special attention following the procedure in order to establish proper emergency procedures in case complications occur.


Durante o período de 2004 a 2009, 11 animais foram tratados para o deslocamento à esquerda do cólon maior, por meio da técnica de rolamento sob anestesia geral, com uma técnica distinta das demais previamente descritas. A seleção dos casos foi baseada nos achados da palpação retal e confirmada ultrassonograficamente. Nove animais foram tratados com sucesso e dois foram submetidos ao tratamento cirúrgico após três tentativas de rolamento. Não foram observadas complicações decorrentes do procedimento a curto e a longo prazos. Somente um animal apresentou recidiva do quadro clínico 10 meses após o tratamento e foi novamente submetido ao rolamento. Apesar da diferença com relação às outras técnicas, esta também se mostrou eficaz. O rolamento, mesmo quando realizado mais de uma vez, mostrou ser um procedimento seguro, porém enfatiza-se a necessidade de cuidados especiais aos animais após a sua realização, pois medidas emergenciais podem ser necessárias caso complicações decorrentes desta técnica ocorram.

ABSTRACT

A morfofisiologia relacionada à absorção de ácidos graxos voláteis (AGV) ao longo do trato gastrintestinal de ruminantes não é totalmente caracterizada. Desse modo, os objetivos deste trabalho foram mensurar a extensão da superfície de absorção e determinar o índice mitótico (IM) do abomaso, do ceco e da alça proximal do cólon ascendente (APCA). Dez bovinos mestiços adultos tiveram seu estômago e intestino grosso removidos imediatamente após o abate. A área total da superfície de absorção foi mensurada por meio de digitalização e análise de imagens. Cortes histológicos foram feitos para determinação do IM. A superfície absortiva do abomaso, 0,58m², foi menor (P<0,01) do que a do rúmen, 6,53m², e a do omaso, 2,31m². A superfície absortiva do ceco e da APCA, 0,23m², correspondeu a 3,5% da superfície do rúmen e a 10% da superfície do omaso. O IM observado foi 0,48%; 0,14%; 0,36% e 0,41% para as regiões de pregas espirais, pilórica, ceco e APCA, respectivamente. Observou-se correlação positiva entre a massa tecidual do abomaso e a área de superfície de absorção, aspecto também observado no ceco-APCA. Foi possível estabelecer regressões para facilitar a mensuração da superfície absortiva do abomaso e do ceco-APCA.


The morphology and physiology related to volatile fatty acid (VFA) absorption throughout the gastrointestinal tract of ruminants is not totally characterized. The purpose of this work was to measure the abomasum and cecum-colon absorptive surface extension and determine the mitotic index (MI). Ten adult crossbred bovine had their stomach and large intestine removed after slaughter. The total area of the absorptive surface was measured through image capture and analysis. Histological sections were performed to measure the MI. The abomasum absorptive surface (0.58m²) was lower (P<0.01) than that of the rumen (6.53m²) and omasum (2.31m²). The cecum-colon absorptive surface (0.23m²) corresponded to 3.5% of the rumen and 10% of the omasum. The MI observed was 0.48%; 0.14%; 0.36% and 0.41% for the regions of spiral folds, pyloric, cecum and proximal loop of ascending colon, respectively. A positive correlation between abomasum tissue mass and the abomasum absorptive surface was observed. This phenomenon has also occurred in the cecum and colon. It was possible to establish regressions to facilitate measurements of the absorptive surface of the abomasum and cecum-colon.

OBJETIVO: Medir a espessura das criptas e quantificar o número de células caliciformes comparando a mucosa cólica com e sem trânsito intestinal, relacionando-as ao tempo de exclusão. MÉTODOS: Sessenta ratos Wistar, foram distribuídos em três grupos com 20 animais segundo a operação final para a retirada dos cólons, realizadas em seis, 12 ou 18 semanas. Em cada grupo, 15 animais foram submetidos à derivação do trânsito por colostomia proximal no cólon esquerdo e fístula mucosa distal e cinco apenas à laparotomia (controle). Os cólons com e sem trânsito fecal foram removidos, processados, submetidos a cortes histológicos corados pela hematoxilina-eosina. A altura das criptas colônicas e o número de células caliciformes foram mensurados por morfometria computadorizada. Foram utilizados os testes t de Student e Kruskal-Wallis para comparação e análise de variância, estabelecendo-se nível de significância de 5% (p<0,05). RESULTADOS: A altura das criptas diminui nos segmentos sem trânsito fecal (p=0,0001), reduzindo entre seis e 12 semanas de exclusão (p=0,0003), estabilizando-se após este período. O número de células caliciformes nas criptas é menor nos segmentos sem trânsito após 12 e 18 semanas (p=0,0001), porém aumenta com o decorrer do tempo de exclusão (p=0,04) CONCLUSÃO: A exclusão do trânsito intestinal diminui a espessura das criptas colônicas e o número de células caliciformes nos segmentos sem trânsito. Existe aumento do número de células caliciformes com o decorrer do tempo de exclusão.


OBJECTIVE: To measure the thickness of the crypts and quantify the number of goblet cells of the colonic mucosa with and without intestinal transit, relating them to exclusion time. METHODS: Sixty Wistar rats were divided into three groups of 20 animals each according to the time of the final operation for the removal of the colon, in six, 12 or 18 weeks. In each group 15 animals underwent colonic exclusion by left colon proximal colostomy and distal mucous fistula, and five underwent only laparotomy (control). The colons with and without fecal stream were removed, processed and submitted to histological sections stained with hematoxylin-eosin. The height of the colonic crypts and the number of goblet cells were measured by computerized morphometry. We used the Student t test and Kruskal-Wallis test for comparison and analysis of variance, using a significance level of 5% (p <0.05). RESULTS: The height of the crypts decreased in segments without fecal stream (p =0.0001), reducing from six to 12 weeks of exclusion (p = 0.0003), stabilizing thereafter. The number of goblet cells in the crypts was smaller in segments without transit after 12 and 18 weeks (p = 0.0001), but increased as the time of exclusion progressed (p = 0.04) CONCLUSION: The exclusion of intestinal transit decreases the thickness of the colonic crypts and the number of goblet cells in the segments without transit. There is an increased number of goblet cells in the course of time exclusion.

BACKGROUND/AIMS: The aim of this study was to compare polyethylene glycol (PEG) 4 L, split method of PEG 4 L and PEG 2 L plus sodium phosphate (NaP) in the aspect of bowel preparation quality, safety, patients' compliance and preference. METHODS: Total 249 subjects were prospectively enrolled and received bowel preparation for colonoscopy from August to October in 2010; PEG 4 L (93 subjects), split method of 4 L PEG (74 subjects) and PEG 2 L plus NaP 90 mL group (82 subjects). To investigate the completion, preference for bowel preparation and safety, a questionnaire survey was conducted before colonoscopy. RESULTS: There were no significant intergroup differences in the aspect of completion of preparation, cecal intubation time and success rate. Satisfaction and preference were higher in PEG 2 L plus NaP 90 mL and split method of 4 L PEG compared with PEG 4 L. In the aspect of the bowel preparation quality PEG 4 L showed significantly higher quality in the morning colonoscopy (p<0.001). However, in the afternoon colonoscopy PEG 2 L plus NaP 90 mL showed better result than PEG 4 L (p=0.009). Hyperphosphatemia was most frequently observed in PEG 2 L plus NaP 90 mL, but no severe adverse events occurred (p<0.001). CONCLUSIONS: PEG 4 L showed better result than split method of 4 L PEG or PEG 2 L plus NaP 90 mL in the aspect of bowel preparation quality and safety.

ABSTRACT

Estudaram-se os efeitos da infecção toxoplásmica sobre a morfometria da parede intestinal, a distribuição de fibras colágenas e a dinâmica de mucinas secretadas no cólon de frangos. Foram utilizados 16 frangos machos de linhagem comercial, com 26 dias de idade. As aves foram distribuídas, aleatoriamente, em três grupos (G). As do G1 não receberam inóculo e se caracterizaram como grupo-controle; nas do G2, foram inoculados cistos teciduais da cepa ME49 de Toxoplasma gondii; e nos G3, oocistos da cepa M7741 de T. gondii. Após 60 dias da inoculação, os animais foram sacrificados para coleta do cólon, o qual foi submetido à rotina de processamento histológico. Em G2 e G3, observou-se hipertrofia da parede do cólon, contudo não houve alteração na proporção do número de células caliciformes e de enterócitos presentes no epitélio intestinal.


The effects of toxoplasmosis on the intestinal wall morphometry, the distribution of collagen fibers, and the dynamic of mucins secreted in the chicken colon were analyzed. Sixteen 26-day-old male Cobb chicks (Gallus gallus) were randomly distributed into three groups (G1, G2, and G3). G1 received no inoculums and characterized the control group, G2 was inoculated with tissue cysts of ME49 strain of Toxoplasma gondii, and G3 was inoculated with oocytes of M7741 strain of T. gondii. After 60 days of inoculation, the animals were slaughtered and had their colon collected and submitted to histological processing. Transversal cuts (4μm) were stained with Hematoxylin-Eosin (HE), Periodic Acid Schiff (PAS), Alcian Blue pH 2.5, Alcian Blue (AB) pH 1.0, and Azan. G2 and G3 animals showed hypertrophy of the colon wall, but the proportion between the number of goblet cells and enterocytes present in the intestinal epithelium was not altered.

El tránsito colónico es la medición objetiva del tiempo en el que trascurre el contenido intestinal a través del colon y representa un adelanto muy importante en la evaluación de pacientes con trastornos en el hábito intestinal. Valores normales de tránsito colónico (TC). Determinar valores normales de TC en voluntarios sanos en la ciudad de Caracas. En el Hospital Universitario de Caracas (HUC). Estudio descriptivo, transversal; se empleó anillos metálicos de 8Fr (2.7mm), como marcadores radioopacos, introducidos en capsulas de gelatina tamaño 00 largo (24 anillos/cápsula), se tomaron al azar 30 voluntarios sanos, personal médico y de enfermería del HUC, con edades entre 18 y 50 años, agrupados por género: 15 masculino y 15 femenino. Se realizó radiografías de abdomen simple en posición decúbito dorsal a las 72, 96 y 120 horas posteriores a la ingesta de la cápsula, cuantificando la cantidad de marcadores eliminados. Para el análisis de los resultados se aplicó la prueba no paramétrica U de Mann-Whitney; con un nivel de significación del 95%. No se realizó modificaciones en la dieta durante el estudio. 21 voluntarios (70%) evacuaron más del 80% de los marcadores radiopacos a las 96 horas. 20% (6 voluntarios) registraron la eliminación total de los marcadores antes de las 96 horas. 70% presentó la eliminación de más del 80% de los marcadores de TC a las 96 horas. No se evidenció diferencias estadísticas significativas entre los grupos etarios ni género. Los valores obtenidos son inferiores a los aceptados como normales, reportados en la literatura mundial (eliminación del 80% de los marcadores antes de las 120 horas), y superiores en comparación con un estudio mexicano semejante (eliminación del 80% de los marcadores a las 48 horas)...


The colonic transit is the objective measurement of the time lag in the intestinal contents through the colon and represents an important advancement in the evaluation of patients with intestinal disorders of habit. Normal colonic transit (CT). To determine normal values of CT in healthy volunteers in the city of Caracas. Hospital Universitario de Caracas (HUC). A descriptive cross-functional study; 8Fr (2.7mm) metal rings were used as radiopaque markers, placed into gelatin capsules of 00 long (24 rings / capsule), 30 healthy volunteers, medical and nursing staff of HCU, aged between 18 and 50 years, matched by gender were randomly taken: 15 male and 15 female. A simple abdominal radiography was performed on dorsal decubitus position at 72, 96 and 120 hours after ingesting the capsule, quantifying the amount of markers removed. For the analysis of the results we used the nonparametric Mann-Whitney U test, with a significance level of 95%. No changes were made in the diet during the study. 21 volunteers (70%) evacuated more than 80% of the radiopaque markers at 96 hours. 20% (6 volunteers) reported the total elimination of the markers prior to 96 hours. 70% eliminated over 80% of the markers of the CT at 96 hours. No statistically significant differences were found between age groups or gender. The values obtained are below the ones accepted as normal, reported in the literature (80% elimination of the markers before the 120 hours) and higher compared to a similar Mexican study (80% elimination of the markers at 48 hours)...

ABSTRACT

O abaixamento endoanal do cólon (AEC) introduzido em 1998 por De la Torre & Ortega tem tornado obsoleta as cirurgias realizadas em dois ou mais tempos, na correção da moléstia de Hirschsprung (MD). Esta técnica é preconizada principalmente em recém-nascido e lactente. O objetivo deste trabalho é descrever nossa casuística demonstrando os aspectos clínicos, cirúrgicos, complicações, seguimento e medida manométrica do canal anorretal no pré e pós-operatório. Foi um estudo retrospectivo de 6 anos (2001 a 2007) onde foram estudadas 36 crianças com MD operados pela AEC. Os dados clínicos, cirúrgicos e seguimento ambulatorial foram coletados dos prontuários médicos. Foram 36 crianças (26 meninos e 10 meninas). A mediana da idade por ocasião do diagnóstico foi de 36 dias (1 a 2507 dias). Enema opaco revelou zona de transição em 32 pacientes. A manometria demonstrou ausência de reflexo retoesfincteriano em 35 pacientes. A biópsia retal por sucção em 16 pacientes, coradas com acetilcolinesterase confirmou a doença em 12 e em 4 mostrou padrão equívoco. No pré-operatório a média de pressão de repouso foi de 72,8 ± 26,3 mmHg sem diferença estatística com grupo controle (74,5 ± 25,2 mmHg). Na manometria pós-operatória a média foi de 69,7 ± 24,6 mmHg, sem diferença estatística, quando comparada com o pré-operatório. A mediana de idade na cirurgia foi de 154 dias (3 a 2.855 dias). O tempo médio de cirurgia foi de 200,1 minutos (55 a 310 minutos). A média de comprimento ressecado foi de 28,4 cm (11 a 48 cm). Houve 2 pacientes com necessidade de conversão para laparotomia. O tempo médio de internação foi de 6,6 ± 2,1 dias (2 a 48 dias). Evacuação entre as primeiras 48 horas ocorreu em 35 pacientes. Introdução da dieta nas primeiras 48 horas de pós-operatório foi em 35 pacientes. Realizou-se a dilatação anal em 14 pacientes por estenose anal em 14 pacientes por estenose anal. O controle esfincteriano foi observado em 25 pacientes maiores de 3 anos...

PURPOSE: To validate Peyer's patch as an anatomical repair landmark for colorectal surgery in rats and to measure the collagen content in segments of the colon containing or not containing Peyer's patch. METHODS: The distance between Peyer's patch and the peritoneal reflection was measured in forty-five Wistar rats. The colon and rectum were resected for quantification of collagen content by means of computer-assisted image analysis in regions of the colon with and without Peyer's patch. RESULTS: There was great variation in the distance between Peyer's patch and the peritoneal reflection when the male and female rats were considered as a single group (p=0.04). Comparison between the genders showed that the distance between the patch and the peritoneal reflection was greater in female than in male rats (p=0.001). The colonic segment containing Peyer's patch was observed to have lower tissue collagen content than the segment in which this structure was not present (p=0.02). CONCLUSION: Peyer's patch can be indicated as an anatomical repair landmark, and there is a need to study the healing of colorectal anastomoses in rats based on differing quantities of tissue collagen existing in the colonic wall with or without this structure.


OBJETIVO: Validar a placa de Peyer como reparo anatômico para a cirurgia colorretal em ratos e mensurar a quantidade de colágeno em segmentos da parede cólica que contém ou não a placa de Peyer. MÉTODOS: Foi aferida a distância entre a placa de Peyer e a reflexão peritoneal em 45 ratos Wistar. O cólon e o reto foram ressecados, para a quantificação do colágeno, por meio de análise de imagem assistida por computador, em regiões do cólon que continham ou não a placa de Peyer. RESULTADOS: Existe grande variação entre a distância da placa de Peyer e a reflexão peritoneal quando se consideraram os animais de ambos os gêneros como grupo único (p= 0.04), sendo a distância entre a placa e a reflexão peritoneal maior entre as fêmeas (p=0.001). Constatou-se que o segmento cólico que contém a placa de Peyer apresenta conteúdo menor de colágeno quando comparado ao segmento onde a estrutura não estava presente (p=0.02). CONCLUSÃO: A placa de Peyer pode ser indicada como reparo anatômico e no estudo da cicatrização de anastomoses colorretais em ratos, baseado nas diferentes quantidades de colágeno tecidual existente na parede cólica que contém ou não esta estrutura.

ABSTRACT

Effects of protein and B-complex vitamin deficiency were assessed with respect to the morphometry of myenteric neurons in the descending colon of adult rats. Sixteen animals were divided into two groups: Control Group (CG, n=8) and Experimental Group (EG, n=8). The CG received 22 percent protein chow and the EG received 4 percent protein chow for 120 days. The descending colon was submitted to NADH- and NADPH-diaphorase technique in order to evidence nervous cells in the whole mounts preparations. In the EG, NADH-d positive neurons presented reduced nuclei, while NADPH-d positive neurons showed atrophy of the soma area (~41.7 percent) inducing an increase of the proportion occupied by the nucleus inside in the soma of these cells.


Esta investigación buscó evaluar los efectos de la desnutrición proteica y vitamínica delcomplexo B sobre aspectos morfométricos del plexo mientérico del colon descendente de ratones adultos. Dieciséis animales fueron distribuidos en dos grupos: control que recibieron ración comercial con 22 por ciento de proteína y experimental alimentados con ración de tenor proteico reducido para 8 por ciento, durante 120 días. Neuronas del plexo mientérico presentes en preparados totales fueron evidenciados a través de la técnica histoquímica de la NADH-diaforasa y de la NADPH-diaforasa. En el grupo experimental, las neuronas NADH-d positivos sufrieron reducción del núcleo celular, ya las neuronas NADPH-d sufrieron atrofia de 41,7 por ciento de la superficie de su pericarion, lo que hizo con que el núcleo celular pasase a ocupar una mayor proporción de la región trófica de las neuronas.

ABSTRACT

El estudio tuvo por objetivo describir la irrigación de la flexura cólica derecha, con la finalidad de proporcionar conocimientos anatómicos más amplios respecto del tema, y que éstos puedan ser utilizados para mejorar la práctica quirúrgica. Fueron utilizadas 40 piezas anatómicas de abdomen de cadáveres humanos, de ambos sexos y diferentes edades, fijados en formaldehido al 10 por ciento, pertenecientes al Departamento de Morfología de la Universidad Federal de Alagoas, Brasil. Se observó que la flexura cólica derecha estaba irrigada: en la mayoría de los casos (42,5 por ciento) por la arcada anastomótica, formada por la rama ascendente de la arteria cólica derecha con la rama derecha de la arteria cólica media; en el 27,5 por ciento de los casos por la anastomosis entre la rama derecha de la arteria cólica media y la rama ascendente de la arteria íleo cólica; en el 22,5 por ciento de los casos por las ramas ascendente y descendente de la arteria cólica derecha y en el 7,5 por ciento de los casos por la anastomosis entre las ramas derecha e izquierda de la arteria cólica media. Podemos concluir que la arteria cólica media fue el vaso con mayor presencia (77,5 por ciento) en la irrigación de la flexura cólica derecha. La anastomosis predominante en la flexura cólica derecha ocurrió entre la rama ascendente de la arteria cólica derecha y la rama derecha de la arteria cólica media.


This study has as objective present data about the irrigation of the right colic flexure, in order to proportionate an ample anatomic understanding about this subject, and that can be useful to perfect the surgical exercise. Was utilized 40 abdominal anatomic pieces of human corpses, of both sexes and variable ages, fixed in formaldehyde at 10 percent, belonging to Departament of Morphology of the Universidade Federal de Alagoas, Brazil. Was observed that the right colic flexure was presented in the majority of the cases (42.5 percent) irrigated by the anastomotic arcade composed by ascending branch of right colic artery with the right branch of the middle colic artery; following by the anastomosis between the right branch of middle colic artery and the ascending branch of ileocolic artery, in 27.5 percent of the cases. The ascendant and descendant branches of the right colic artery nourished the right colic flexure in the 22.5 percent of cases; at the present the anastomosis between the right and left branches of the middle colic artery irrigated the right colic flexure in 7.5 percent of the cases. We concluded that the middle colic artery was the most present vase (77.5 percent) in irrigation of the right colic flexure. The most prevalent anastomosis in the right colic flexure occurred between the ascending branch of right colic artery and the right branch of middle colic artery.

ABSTRACT

Estudos recentes demonstraram que a distensão luminal do cólon menor, apesar de ter promovido redução da irrigação da camada seromuscular resultando em inflamação intensa, não acarretou em lesões histopatológicas aparentes na mucosa. O objetivo do presente estudo foi avaliar, por meio de histomorfometria e microscopia eletrônica de varredura (MEV), os efeitos da distensão intraluminal sobre a mucosa do cólon menor. Utilizaram-se 16 eqüinos. No Grupo distendido (GD), nove deles foram submetidos a quatro horas de distensão intraluminal do cólon menor por um balão inflado com pres são de 40mm de Hg. No Grupo instrumentado (GI), o balão foi introduzido no lume, mas sem provocar distensão. Colheram-se amostras intestinais antes e ao fim da obstrução e após 1,5 e 12 horas de reperfusão. À MEV observou-se em GD que a superfície da mucosa adquiriu um aspecto liso, aplainado, após a distensão, retornando ao aspecto rugoso após descompressão. Ao fim do experimento, a superfície da mucosa se apresentava com aspecto um pouco mais irregular, com pontos de fragmentação. À histomorfometria observou-se em GD uma redução da espessura da mucosa em relação ao controle após a distensão, retornando a valores semelhantes aos basais após descompressão. Concluiu-se que a mucosa, apesar de ter seu aspecto alterado e sua espessura reduzida, conseguiu suportar a distensão promovida pelo aumento de pressão intraluminal, retornando às suas características originais sem apresentar lesões significativas.


Recently it has been shown that experimental distention of the small colon of horses promotes reduction of microvascular circulation and inflammation of the seromuscular layer associated with neutrophil accumulation in the lungs. However this model was not sufficient to induce evident histophatological changes in the mucosal layer. The aim of this study was to evaluate the mucosa subjected to that model of small colon distention by histomorphometry and scan electronic microscopy (SEM). Sixteen horses were used. In the distended group (DG), nine of them were subjected to distention of the small colon by a surgically implanted intraluminal balloon that was inflated with a pressure of 40mm Hg during 4 hours. In the sham-operated group (SG), the balloon was implanted but not inflated. Full-thickness intestinal samples were collected before and after obstruction and after 1.5 and 12 hours of decompression. By SEM, it was observed that the mucosa turned flat and smooth after distention and returned to the wrinkled original appearance after decompression. Twelve hours after decompression the mucosa had a more irregular appearance with points of fragmentation. There was a reduction in mucosa thickness after distention, returning to basal values after decompression. Instead of the fact that there were changes in appearance and thickness, it was concluded that the mucosa could borne up the compression caused by distention returning to the original characteristics without major lesions.

ABSTRACT

Foi realizado estudo morfométrico do cólon descendente de 15 eqüinos hígidos, nove fêmeas e seis machos, submetidos a enterorrafias aposicionais com fio de poliglactina 910 ou com cianoacrilato. Os animais foram distribuídos aleatoriamente em cinco grupos de três animais cada e sacrificados aos três, sete, 14, 35 e 70 dias de pós-operatório para colheita dos segmentos intestinais operados e posterior processamento histológico de rotina. A análise morfométrica foi feita em microscópio óptico adaptado a um sistema de análise de imagens, onde se quantificou o infiltrado inflamatório, mediante contagens de neutrófilos, eosinófilos, macrófagos, linfócitos e plasmócitos, não sendo observadas diferenças (P>0,05) entre as enterorrafias. Houve diferenças significativas entre os dias de pós-operatório para as contagens de neutrófilos, eosinófilos, macrófagos e linfócitos. Também se mensuraram as espessuras das regiões operadas (cicatriz) e das áreas contíguas não operadas (controle), registrando-se diferenças entre os dias de pós-operatório e as regiões (P<0,05), mas não entre as enterorrafias. O fio de poliglactina 910 e o cianoacrilato apresentam a mesma eficácia para serem usados em enterorrafias aposicionais do cólon descendente de eqüinos.


A morphometric study was carried out in descending colon of 15 healthy horses, nine females and six males, submitted to appositional suture techniques with polyglactin 910 or with cyanoacrilate. The animals were randomly distributed in five groups of three animals each and sacrificed on postoperative days 3, 7, 14, 35 or 70 in order to collect the operated intestinal segments. The morphometric evaluation was done with an optic microscope adapted to an image analysis system, where was counted the inflammatory infiltrated, through neutrophils, eosinophils, macrophages, lymphocytes and plasmacytes counting. No difference (P>0.05) was observed between the two suture techniques. There were significant differences between postoperative days related to the counting of neutrophils, eosinophils, macrophages and lymphocytes. It was also measured the thickness of operated regions (scar) and non operated nearby areas (control); it registered differences between the postoperative days and the regions (P<0.05), but not between the sutures. The polyglactin 910 and the cyanoacrilate showed the same efficacy to be used in appositional enterorrhaphies in equine descending colon.

ABSTRACT

The purpose of this study was to determine the spectrum of colonic wall changes in patients with cirrhosis and to assess its clinical significance by CT. 80 patients clinically diagnosed as liver cirrhosis underwent triphasic CT study of the liver. They were evaluated for the degree and extent of colonic involvement. Other features of cirrhosis including portal hypertension, ascites and splenomegaly were also recorded. Colonic wall thickening >6mm was seen in the 41% [33/80] of the patients. The ascending colon and ceacum were involved in 17 cases. The ascending and transverse colon were involved in 11 cases while the ascending, transverse and descending colon were involved in 5 cases. Portosystemic collaterals were present in 33 cases of colonic wall thickening. Splenomegaly, was present in all the examined cases. Ascites was present in 20 cases, all of them has colonic wall thickening. Colonic wall thickening was limited predominantly to the right colon which could be explained due to portal hypertension and can be used as one of the indicators of poor prognosis in cirrhotic patients. CT can be used as one of the indicators of poor prognosis in patients of liver cirrhosis

ABSTRACT

Avaliou-se ação da solução isotônica poliônica enteral da sene mais fluidoterapia intravenosa e da fluidoterapia intravenosa no tratamento da compactação no cólon maior em eqüinos. Foram utilizados 20 animais divididos em quatro grupos. Os animais dos grupos tratados eram portadores de compactação no cólon maior induzida experimentalmente. O grupo C, controle, não foi tratado, o grupo E8 foi tratado com solução isotônica poliônica enteral (8ml/kg/hora/48horas), o grupo SE recebeu sene (20mg/kg, duas doses de 24/24h) mais ringer lactato intravenoso (10ml/kg/hora/12horas, durante dois dias) e o grupo RL recebeu ringer lactato intravenoso (16ml/kg/hora/12horas, durante dois dias). O tratamento E8 provocou hipernatremia, hipercloremia e hiperglicemia. No grupo SE foi observada maior concentração dos valores de uréia, creatinina, osmolalidade e lactato plasmático. O tratamento RL não resultou em alteração no equilíbrio hidroeletrolítico e foi o mais eficiente para a correção dos valores bioquímicos em eqüinos com compactação do cólon maior, seguido pelo E8. O SE foi o menos eficiente na normalização das variáveis bioquímicas.

ABSTRACT

OBJETIVO: Avaliar, do ponto de vista histológico, a ação do extrato aquoso da aroeira-do-sertão na cicatrização da anastomose colônica, em ratos Wistar. MÉTODOS: Foram utilizados 48 ratos da linhagem Wistar, machos, com peso médio de 320g, distribuídos em dois grupos, com 24 animais, cada. Todos os animais foram submetidos à secção transversa completa do cólon descendente, seguida de anastomose colônica. Os ratos do grupo A receberam diariamente no pós-operatório enemas de veículo à base de carboximetilcelulose (CMC). Os animais do grupo B receberam no pós-operatório enemas de extrato aquoso de aroeira-do-sertão a 10 por cento em veículo à base de CMC, até a data da eutanásia. Nas datas 3, 7, 14 e 21 do experimento, seis ratos de cada grupo foram submetidos à remoção de segmento colônico, incluindo a anastomose, destinado à apreciação histológica e análise qualitativa da resposta celular inflamatória e cicatricial. RESULTADOS: A análise morfológica revela diferença significante entre os grupos, no dia 7 (ñ < 0,05), quando o grupo estudado apresentou menor escore que o grupo controle. A deposição do colágeno no grupo aroeira foi menor, nos dias 3, 7 e 14 (ñ < 0,05), em relação ao grupo controle. CONCLUSÃO: O extrato aquoso da aroeira-do-sertão tem ação antiinflamatória. Também possui efeito inibitório na deposição do colágeno nas fases inflamatória e de fibroplasia no processo cicatricial de anastomoses colônicas, em ratos Wistar, porém em fases avançadas (dia 21) a cicatrização se dá de forma semelhante aos animais do grupo controle.
