Head and neck cancer accounts for 2.8% of all cancers and a large proportion of these patients have a locally advanced stage of the disease, for which chemotherapy and radiation therapy are potentially curative treatments. Dysphagia is one of the most common chemoradiotherapy-related side effects in head and neck cancer since it can lead to life-threatening complications. Reports from the current literature suggest better swallowing outcomes with intensity-modulated radiotherapy (IMRT) compared to three-dimensional conformal radiotherapy (3DCT). However, in low-/middle-income countries, multiple healthcare access barriers to 3DCT that may lead to higher rates of chemo/radiotherapy related adverse events. This narrative review provides a comprehensive appraisal of published peer-reviewed data, as well as a description of the clinical practice in an otolaryngology referral center in Colombia, a low-income country.

Múltiples estrategias terapéuticas se han desarrollado con el objetivo de optimizar el tratamiento del cáncer de mama, especialmente antes de ir a quirófano, desde la introducción de la quimioterapia preoperatoria por Fisher. Una de estas ha sido la radioterapia, la cual tiene beneficios claros en esa patología, pues reduce a la mitad el riesgo de recurrencia, y disminuye la muerte por cáncer de mama en aproximadamente una sexta parte después de cirugía conservadora. Aunque no se suele considerar antes de la cirugía, el uso de radioterapia preoperatoria en cáncer de mama, no es nuevo, y se ha demostrado que esta alternativa terapéutica es factible, bien tolerada y asociada a una tasa de respuesta patológica completa del 10 % -40 %. Se presenta el caso de una paciente con cáncer de mama metastásico, quien recibió múltiples esquemas de quimioterapia y a quien se le planteó radioterapia preoperatoria, recibiendo la misma con muy buena respuesta, sin aumentar la dificultad técnica para la realización de la intervención quirúrgica, y sin complicaciones asociadas a la herida operatoria. En la actualidad, existen pocos estudios clínicos sobre radioterapia preoperatoria, y su efecto en las pacientes con cáncer de mama localmente avanzado, es controvertido. Al observar en la biopsia definitiva respuesta patológica completa, es un indicio de cómo la radioterapia preoperatoria en cáncer de mama podría ser una opción en casos complejos con poca respuesta a la terapia convencional(AU)
Multiple therapeutic strategies have been developed with the aim of optimizing breast cancer (BC) treatment, especially preoperatively since the introduction of preoperative chemotherapy by Fisher. One of these has been radiation therapy (RT), which has clear benefits in breast cancer, halving the risk of recurrence andreducing breast cancer death byapproxmatelyone-sixth after breast-conserving surgery. Although not often considered for use prior to surgery, the use of preoperativeradiotherapy in localized breast cancer is not new andhas demonstrated that this therapeutic alternative is feasible, well tolerated and associated with a pathologic complete response rate of 10-40%. We present the case of a patient with metastatic breast cancer who received multiple chemotherapy schedules and who was considered preoperative radiotherapy, receiving the same with very good response, without increasing the technical difficulty for the performance of surgery, and without complications associated with the operative wound. At present, there are few clinical studies on preoperative radiotherapy, and its effect in patients with locally advanced breast cancer is controversial. And although this is a single case, observing a complete pathologic response in the definitive biopsy gives us a great indication of how preoperative radiotherapy in breast cancer could be an option in complex cases with little response to conventional therapy(AU)

En la actualidad Venezuela se encuentra en una crisis social y económica sin precedentes. La mortalidad materna(MM) es un indicador en salud importante, debido a que permite tener idea de la atención médica de un país; se mide a través de dos indicadores: Razón de Mortalidad Materna (RMM) y Tasa de Mortalidad Materna (TMM). Objetivo: Revisar y compararla evolución de ambos indicadores de MM desde la década de 1930 hasta la década 2000. Métodos: Se realizó una revisión de la literatura y de informes técnicos de organismos gubernamentales y no gubernamentales para el análisis de la situación previa y reciente de esta situación en Venezuela. Los resultados señalan que existe una notable disminución de las cifras de MM como ha de esperarse con el mejoramiento de la tecnología desde la década de 1930 hasta el año 2000; posteriormente ocurre un retroceso de la sanidad pública con cifras comparables a la década de 1960. Concluimos que la MM ha sido desde tiempos pasados un problema constante en la salud pública; al pasar los años y gobiernos, se han implementado numerosas políticas públicas para mejorar esta situación, muchas de estas estrategias han sido fallidas debido a la falta de su continuidad y de su cumplimiento pleno.
Venezuela is currently in an unprecedented socialand economic crisis. Maternal mortality is an important health indicator because it provides an idea of a country's medical care. Maternal mortality is usually measured through two indicators: Maternal Mortality Ratio (MMR) and Maternal Mortality Rate. Objective: Review and compare the evolution of both healthindicators from the 1930s to 2016. Methods: A review of the literature and technical reports from governmental andnon-governmental organizations was carried out to analyze theprevious and recent situation. of this situation in Venezuela. Theresults indicate that there is a notable decrease in the figures ofmaternal mortality, as should be expected with the improvementof technology from the 1930s to the year 2000. Subsequently, there is a decline in public health with figures comparable to the1960s. We conclude that maternal mortality has been a constant problem in public health since ancient times. Over the years and governments, numerous public policies have been implementedto improve this situation. Many of these strategies have beenfailed due to lack of continuity and in the absence of its full compliance.

BACKGROUND: Although tumors of the central nervous system (CNS) are rare, they can cause significant morbidity and mortality. The clinical presentation of patients with non-functional pituitary adenomas (NFPA) ranges from being completely asymptomatic to causing pituitary, hypothalamic, or visual dysfunction due to their large size. Patients usually arrive with large tumors at the time of diagnosis. Objectives: Try to describe the characteristics of NFPA and explain the causes of delayed diagnosis. Methods: We carried out a retrospective study including 58 patients with NFPA and analyzed the tumor volume at the time of diagnosis and its relationship with sociodemographic and health sector variables. Results: Low socioeconomic status (SES) was associated with high tumor volume (SES 1-2 of 17.4 cm3 vs 3-6 of 11.7 cm3, p=0.018), and the time between first consultation and diagnosis was longer in the public sector than in the private sector (13.5 months vs 5.1 months). The time between the first symptom and the first consultation was shorter when they had visual impairment than when they did not (4.1 vs 18.4 months, p=0.006). CONCLUSIONS: On the one hand, citizens should be made aware that a visual deficit should make them go to a medical check-up, and on the other hand, strengthen the health system so that they have the NFPA as a differential diagnosis in patients with some visual alteration. Socioeconomic inequality in our country undoubtedly puts the underprivileged at greater risk.

Background: Parkinson's disease is poorly studied in Colombia. It is pharmacologically managed, but for refractory cases, surgery is a therapeutic option, positively impacting on quality of life. Objective: To determine the impact of deep brain stimulation as management in the control of progression in patients with Parkinson's disease attended our institution between the years 2014 to 2020. Method: Descriptive retrospective study, with patients collected between 2014 and 2020 undergoing deep brain stimulation surgery. The MDS-UPDRS (Movement Disorder Society-Sponsored Revision of the Unified Parkinson's Disease Rating Scale) was applied in the pre- and postoperative period, and the results were compared. Results: 21 patients were included and the UPDRS was applied, finding a decrease in scores in the postoperative period. One patient had an operative site infection. Conclusions: There was an improvement in the MDS-UPDRS score, with a low rate of complications. The procedure time was prolonged from the preoperative evaluation. Deep brain stimulation is the management of choice for refractory Parkinson's disease. The patients in this series showed improvement in their symptoms. Unfortunately, there are limitations to perform this procedure in Colombia, such as the delay in the authorization of the procedure.
Antecedentes: La enfermedad de Parkinson está poco estudiada en Colombia. Es de manejo farmacológico, pero para casos refractarios la cirugía es una opción terapéutica que impacta positivamente en la calidad de vida. Objetivo: Determinar el impacto de la estimulación cerebral profunda como manejo en el control de la progresión en pacientes con enfermedad de Parkinson atendidos nuestra institución entre los años 2014 a 2020. Método: Estudio descriptivo de corte retrospectivo con pacientes recolectados entre los años 2014 y 2020 sometidos a cirugía de estimulación cerebral profunda. Se aplicó la MDS-UPDRS (Movement Disorder Society-Sponsored Revision of the Unified Parkinson's Disease Rating Scale) en el pre- y el posoperatorio, y se compararon los resultados. Resultados: Se incluyeron 21 pacientes y se les aplicó la MDS-UPDRS, encontrando una disminución en las puntuaciones en el posoperatorio. Un paciente presentó infección del sitio operatorio. Conclusiones: Hubo mejoría en la puntuación de la MDS-UPDRS, con baja tasa de complicaciones. El tiempo de realización del procedimiento fue prolongado desde la valoración preoperatoria. La estimulación cerebral profunda es el manejo de elección para la enfermedad de Parkinson refractaria. Los pacientes de esta serie mostraron mejoría en sus síntomas. Desafortunadamente, existen limitaciones para la realización de este procedimiento en Colombia, como el retraso en la autorización del procedimiento.

In this paper, a model for Cr (VI) removal and optimization was made using a novel aerogel material, chitosan-resole CS/R aerogel, where a freeze-drying and final thermal treatment was employed to fabricate the aerogel. This processing ensures a network structure and stability for the CS, despite the non-uniform ice growth promoted by this process. Morphological analysis indicated a successful aerogel elaboration process., FTIR spectroscopy corroborated the aerogel precursor's identity and ascertained chemical bonding after adsorption. Owing to the variability of formulations, the adsorption capacity was modeled and optimized using computational techniques. The response surface methodology (RSM), based on the Box-Behnken design using three levels, was used to calculate the best control parameters for the CS/R aerogel: the concentration at %vol (50-90%), the initial concentration of Cr (VI) (25-100 mg/L), and adsorption time (0.3-4 h). Analysis of variance (ANOVA) and 3D graphs reveal that the CS/R aerogel concentration and adsorption time are the main parameters that influence the initial concentration of CS/R aerogel metal-ion uptake. The developed model successfully describes the process with a correlation coefficient of R2 = 0.96 for the RSM. The model obtained was optimized to find the best material design proposal for Cr (VI) removal. Numerical optimization was used and showed superior Cr (VI) removal (94.4%) under conditions of a CS/R aerogel concentration of 87/13 %vol, with an initial concentration of Cr (VI) of 31 mg/L, and an adsorption time of 3.02 h. These results suggest that the proposed computational model can obtain an effective and viable model for CS material processing and for optimization of the uptake of this metal.

Introducción. El angiosarcoma es una neoplasia vascular originada a nivel del endotelio, de baja frecuencia, conocido por su agresividad y crecimiento acelerado. Alcanza solo el 1 al 2 % del total de los sarcomas. La presentación gastrointestinal es infrecuente y su incidencia es poco conocida debido al escaso reporte de casos en la literatura. Suele identificarse en etapas avanzadas debido a la dificultad del diagnóstico histopatológico por sus características morfológicas, siendo necesario aplicar tinciones especiales o estudio inmunohistoquímico. Caso clínico. Por su interés y singularidad, presentamos el caso de un paciente masculino de 54 años, quien consultó con hemorragia digestiva profusa, anemización y requerimiento de terapia transfusional. Resultados. Durante el proceso diagnóstico, en la endoscopia encontraron diversas lesiones multifocales que se extendían por gran parte del tracto gastrointestinal. El estudio histopatológico mostró angiosarcoma gastrointestinal. Discusión. El angiosarcoma del tracto gastrointestinal es extremadamente infrecuente, de difícil diagnóstico y bajas posibilidades de manejo curativo, con opciones terapéuticas limitadas, lo que configura un mal pronóstico a corto plazo
Introduction. Angiosarcoma is a vascular neoplasm originating from endothelial cells, known for its aggressiveness, accelerated growth and reduced frequency. Reach only 1 to 2% of total sarcomas. Gastrointestinal presentation is extremely rare, the true incidence is poorly known, due to the limited reports of this entity in the literature. It is usually identified in advanced stages in view of the difficulty of the histopathological diagnosis, attributable to its morphological characteristics, being necessary to apply special stains or immunohistochemical study. Case report. Due to their interest and uniqueness, we present the case of a 54-year-old male patient, who presented with a profuse gastrointestinal bleeding, anemia, and requirement for transfusion therapy. Results. The endoscopy detected several multifocal lesions that extended most of the gastrointestinal tract. The histopathological study showed gastrointestinal angiosarcoma. Discussion. Angiosarcoma of the gastrointestinal tract is extremely uncommon, difficult to diagnose and has low possibilities of curative management, with limited therapeutic options, which configures a poor prognosis in the short term

Lung cancer is a leading cause of cancer-related deaths in Latin America, with non-small cell lung cancer (NSCLC) being the most prevalent. The current study aimed to report real-world data on epidermal growth factor receptor (EGFR) mutational testing and treatment regimens at diagnosis and progression in patients with metastatic NSCLC across four Latin American countries (Argentina, Chile, Colombia and Uruguay). A retrospective, multicenter, observational study was conducted in patients with NSCLC using medical records from participating countries. The study population was categorized into two cohorts: Cohort 1 comprised of newly diagnosed, treatment-naïve patients with stage IV NSCLC; and cohort 2 comprised of stage IV NSCLC EGFR mutation (EGFRm)-positive patients who had progressed after first- or second-generation EGFR-tyrosine kinase inhibitor (TKI) treatment. Measures included demographic variables, health characteristics, treatment regimen, molecular testing rate and turnaround time at diagnosis and at progression for cohorts 1 and 2, respectively. Descriptive statistics were used to summarize all study measures. Of the 462 patients enrolled, 431 were newly diagnosed or treatment naïve with metastatic NSCLC. In cohort 1, the majority of patients with private health insurance (57.31%) underwent molecular diagnosis while only 41.3% of patients within the public sector had access to testing. The average molecular testing rate in cohort 1 varied across countries, with Argentina having the highest testing rate (79%) and Uruguay the lowest (27.63%). EGFRm was observed in 22% of patients. Cohort 2 comprised 31 patients who had progressed after first- or second-generation EGFR-TKI treatment and of these, only 22 (70.97%) underwent testing after progression. Access to molecular testing is still a challenge impacting the choice of first-line treatment in Latin American patients with NSCLC. These findings underline the unmet needs of ensuring early diagnosis, molecular profiling and use of correct treatment to alleviate NSCLC burden in the region.

Resumen Introducción: la anemia es la complicación más frecuente en enfermedad inflamatoria intestinal (EII). El objetivo de este estudio fue determinar la prevalencia, asociaciones y tratamiento de la anemia en EII en nuestro medio. Materiales y métodos: se analizaron de forma retrospectiva datos de pacientes adultos con EII del Hospital Pablo Tobón Uribe en Medellín, Colombia, que consultaron desde 2001 hasta febrero de 2019. Se emplearon frecuencias absolutas y relativas. Para comparar dos proporciones se utilizó la prueba de chi cuadrado de independencia y se estimó el Odds ratio (OR). Resultados: se documentaron 759 pacientes con EII, de los cuales 544 (71,6 %) presentaron colitis ulcerativa (CU); 200 (26,3 %), enfermedad de Crohn (EC), y 15, EII no clasificable (1,9 %). En total, 185 (24,4 %) pacientes con EII presentaron anemia, que es menos frecuente en CU que en EC (22,2 % frente a 32,5 %, respectivamente; OR: 0,684; IC: 0,456-0,96; p = 0,03). Los pacientes con CU extensa (54,1 %) tenían más anemia que aquellos con CU no extensa (46,3 %) (OR: 4,4; IC: 2,6-7,4; p = 0,001); lo mismo se observó al comparar la CU grave (66,1 %) con la no grave (32,3 %) (OR: 4,95; IC: 2,87-8,51; p = 0,000). En la EC, los pacientes con comportamiento no inflamatorio (B2, B3: 73,9 %) presentaron más anemia en comparación con aquellos con comportamiento inflamatorio (B1: 26,2 %) (OR: 0,35; IC: 0,18-0,67; p = 0,000). 44,3 % de los pacientes no recibió tratamiento, 19,5 % recibió hierro oral, 20,0 % recibió hierro intravenoso y 16,2 % fue transfundido. Conclusiones: la anemia es una complicación frecuente de la EII en nuestro medio (24,4 %). A pesar de las guías internacionales, el tratamiento es subóptimo en nuestro medio.
Abstract Introduction: Anemia is the most frequent complication of inflammatory bowel disease (IBD). This study aims to determine the prevalence, connection, and treatment of anemia in IBD in a local context. Materials and Methods: This retrospective study was conducted at The Pablo Tobon Uribe Hospital, in Medellín (Colombia) with adult patients who (were admitted) came for consultation from 2001, until February 2019. Absolut and relative frequencies were calculated. The Chi square test of independence was applied for comparing two proportions and the odds ratio (OR) was estimated. Results: A total of 759 IBD patients were enrolled, 544 (71.6%) with ulcerative colitis (UC); 200 (26.3%) Crohn's disease (CD), and 15 (1.9%) with non-classifiable IBD. In total, 185 (24,4 %) IBD patients had a diagnosis of anemia, that is less frequent in UC patients that in CD patients (22,2 % versus 32,5 %, respectively; OR: 0,684; CI: 0,456-0,96; p = 0,03). Extensive UC patients (54,1 %) had a more recurrent level of anemia than non-extensive UC (46,3 %) (OR: 4,4; CI: 2,6-7,4; p = 0,001); the same result was observed when severe UC (66,1 %) was compared with UC non-severe (32,3 %) (OR: 4,95; CI: 2,87-8,51; p = 0,000). In the analysis of CD, patients with a non-inflammatory response (B2, B3: 73,9 %) had a more recurrent level of anemia than patients with an inflammatory response (B1: 26,2 %) (OR: 0,35; CI: 0,18-0,67; p = 0,000). 44,3 % of the total number of patients received treatment, 19,5 % received oral iron, 20,0 % received intravenous iron, and 16,2 % received a blood transfusion. Conclusions: In our context, Anemia is a common complication in IBD cases (24,4 %). Despite the existence of international guidelines, the treatment in our context is not optimal.

The removal of Cu(II) ions from aqueous solutions at a pH of 5.0 was carried out using fixed-bed columns packed with alginate-chitosan (Alg-Ch) or alginate-chitosan sulfate (Alg-ChS) hydrogel beads. The effect of the initial Cu(II) concentration, flow rate, pH, and height of the column on the amount of Cu removed by the column at the breakpoint and at the exhaustion point is reported. The pH of the solution at the column's exit was initially higher than that at the entrance, and then decreased slowly. This pH increase was attributed to proton transfer from the aqueous solution to the amino and COO- groups of the hydrogel. The effect of operating conditions on the mass transfer zone (MTZ) and the length of the unused bed (HLUB) is reported. At the lower flow rate and lower Cu(II) concentration used, the MTZ was completely developed and the column operated efficiently; by increasing column height, the MTZ has a better opportunity to develop fully. Experimental data were fitted to the fixed-bed Thomas model using a non-linear regression analysis and a good correspondence between experimental and Thomas model curves was observed.
RESUMO
ABSTRACT: This article aims to demonstrate how perversion, as a subject, activates investigation. As a starting point, it explores Freud's idea surrounding fetishism as a paradigm of itself; a necessary interval between neurosis and psychosis. This analysis is followed by a presentation of various notions developed by Lacan in his teachings. Subsequently, the article analyzes Haneke's 2001 film "The piano teacher", here considered as a case of substitute perversion that adds to the few documented cases of this nature. The movie contributes elements that allow to think about and further develop the notion of substitute perversion in psychosis. In it, "the teacher" demonstrates the separation between love, desire, juissance and what Lacan calls "false hole" through borromean rings. Lastly, questions are posed regarding perversion; organized by answers, opposite the two aforementioned structures.
Resumo: O artigo ocupa-se de mostrar como a perversão é uma temática que ativa a pesquisa. Explora, inicialmente, a ideia de Freud sobre o fetichismo como paradigma desse operador clínico, intervalo necessário entre a neurose e a psicose. Depois apresenta noções formuladas por Lacan em seus ensinamentos. Examina, posteriormente, o filme A professora de piano, de Haneke (2001), considerando-o material que contribui diante da escassez de casos sobre perversão. Encontra no filme elementos para pensar e indagar a suplência perversa na psicose. A professora ensina a propósito da amarração do nó borromeano entre amor, desejo, gozo e do estabelecimento do que Lacan chama: "falso furo". O artigo, finalmente, formula questões sobre essa estrutura clínica que, diferentemente das outras duas, se organiza não em certezas, mas adiantando respostas.

PURPOSE: Limited information is available on multiple myeloma (MM), chronic lymphocytic leukemia (CLL), and non-Hodgkin lymphoma (NHL) management in Latin America. The primary objective of the Hemato-Oncology Latin America (HOLA) study was to describe patient characteristics and treatment patterns of Latin American patients with MM, CLL, and NHL. METHODS: This study was a multicenter, retrospective, medical chart review of patients with MM, CLL, and NHL in Latin America identified between January 1, 2006, and December 31, 2015. Included were adults with at least 1 year of follow-up (except in cases of death within 1 year of diagnosis) treated at 30 oncology hospitals (Argentina, 5; Brazil, 9; Chile, 1; Colombia, 5; Mexico, 6; Panama/Guatemala, 4). RESULTS: Of 5,140 patients, 2,967 (57.7%) had NHL, 1,518 (29.5%) MM, and 655 (12.7%) CLL. Median follow-up was 2.2 years for MM, 3.0 years for CLL, and 2.2 years for NHL, and approximately 26% died during the study observation period. Most patients had at least one comorbidity at diagnosis. The most frequent induction regimen was thalidomide-based chemotherapy for MM and chlorambucil with or without prednisone for CLL. Most patients with NHL had diffuse large B-cell lymphoma (DLBCL; 49.1%) or follicular lymphoma (FL; 19.5%). The majority of patients with DLBCL or FL received rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone. CONCLUSION: The HOLA study generated an unprecedented level of high-quality, real-world evidence on characteristics and treatment patterns of patients with hematologic malignancies. Regional disparities in patient characteristics may reflect differences in ethnoracial identity and level of access to care. These data provide needed real-world evidence to understand the disease landscape in Latin America and may be used to inform clinical and health policy decision making.

ABSTRACT Context: There is an increasing interest in the standardization of the evaluation of imaging exams, especially enterography in Crohn's disease. Objective: To compare the quality of the conventional report with the structured report in computed tomography enterography in Crohn's disease. Method: Nine CT enterographies of patients with Crohn's disease with conventional reports were performed; after the standardization of the structured report by the radiology department of a university hospital that is a reference in the care of patients with inflammatory bowel diseases, the same exams were reevaluated, and new reports were issued, without the radiologists having access to the previous report; the 18 reports underwent evaluation of five specialists in Inflammatory Bowel Diseases, resulting in 90 analyses of the report; the specialists evaluated the following criteria: clarity of the information of the report, ease in characterizing the extension of the disease, ease in defining the presence of complications, usefulness of the exam to define the behavior, and ease in defining the activity of the disease, with the use of a 0-10 score. The statistical analysis was perfrmed with Wilcoxon's non-parametric test, considering a significance level of 5%. Results: The mean scores assigned to the exam reports, using both reports (unstructured and structured) ranged from 7.98 to 9.16; however, the scores of the structured reports were significantly higher (p < 0.003). Conclusion: In the specialists' evaluation, the structured report was shown to be of better quality than the unstructured one in the CT enterography of patients with Crohn's Disease.
RESUMO Contexto: Há cada vez maior interesse em padronização na avaliação de exames de imagem, em especial a enterografia na Doenca de Crohn. Objetivo: Comparar a qualidade do laudo convencional com o estruturado na enterografia por tomografia computadorizada na Doença de Crohn. Método: Foram realizados nove enterografias por TC de pacientes com Doença de Crohn com laudos convencionais; após a padronização do laudo estruturado pelo serviço de radiologia um hospital universitário, referência no atendimento de pacientes com doenças inflamatórias intestinais, os mesmos exames foram reavaliados e novos laudos foram emitidos, sem que os radiologistas tivessem acesso ao laudo anterior; os 18 laudos foram submetidos a avaliação de cinco especialistas em Doenças Inflamatórias Intestinais, resultando em 90 análises de laudo; os especialistas avaliaram os seguintes critérios: clareza das informações do laudo, facilidade em caracterizar a extensão da doença, facilidade em definir a presença de complicações, utilidade do exame para definir a conduta e facilidade em definir a atividade da doença, conferindo uma pontuação de 0 a 10. A análise estatística foi realizada com o teste não paramétrico de Wilcoxon, considerando-se nível de significância de 5%. Resultados: os escores médios atribuídos aos laudos do exame, usando ambos os relatórios (não estruturado e estruturado), variaram de 7,98 a 9,16; todavia, os escores dos laudos estruturados foram significativamente maiores (p < 0,003). Conclusão: Na avaliação dos especialistas o laudo estruturado mostrou-se de melhor qualidade que o não estruturado na enterografia por tomografia computadorizada de pacientes com Doença de Crohn.

Aging is a gradual biological process characterized by a decrease in cell and organism functions. Gingival wound healing is one of the impaired processes found in old rats. Here, we studied the in vivo wound healing process using a gingival repair rat model and an in vitro model using human gingival fibroblast for cellular responses associated to wound healing. To do that, we evaluated cell proliferation of both epithelial and connective tissue cells in gingival wounds and found decreased of Ki67 nuclear staining in old rats when compared to their young counterparts. We next evaluated cellular responses of primary gingival fibroblast obtained from young subjects in the presence human blood serum of individuals of different ages. Eighteen to sixty five years old masculine donors were classified into 3 groups: "young" from 18 to 22 years old, "middle-aged" from 30 to 48 years old and "aged" over 50 years old. Cell proliferation, measured through immunofluorescence for Ki67 and flow cytometry for DNA content, was decreased when middle-aged and aged serum was added to gingival fibroblast compared to young serum. Myofibroblastic differentiation, measured through alpha-smooth muscle actin (α-SMA), was stimulated with young but not middle-aged or aged serum both the protein levels and incorporation of α-SMA into actin stress fibers. High levels of PDGF, VEGF, IL-6R were detected in blood serum from young subjects when compared to middle-aged and aged donors. In addition, the pro-inflammatory cytokines MCP-1 and TNF were increased in the serum of aged donors. In old rat wound there is an increased of staining for TNF compared to young wound. Moreover, healthy gingiva (non injury) shows less staining compared to a wound site, suggesting a role in wound healing. Moreover, serum from middle-aged and aged donors was able to stimulate cellular senescence in young cells as determined by the expression of senescence associated beta-galactosidase and histone H2A.X phosphorylated at Ser139. Moreover, we detected an increased frequency of γ-H2A.X-positive cells in aged rat gingival tissues. The present study suggests that serum factors present in middle-aged and aged individuals may be responsible, at least in part, for the altered responses observed during wound healing in aging.

Resumen La presente entrevista se realiza en el contexto de una investigación en curso y da cuenta de tres aspectos centrales que se presentan cuando se realizan procesos de investigación en el ámbito temático de la subjetividad: lo epistemológico, lo metodológico y lo que por tradición se denomina "lo operativo", pero que aquí se nombra como procesos de relación dinámicos propios del método. Se muestran las tensiones que se presentan al interior y entre cada uno de los aspectos referidos, explicitando la perspectiva histórico cultural que el entrevistado ha desarrollado y su propuesta de epistemología cualitativa.
Abstract This interview has been conducted within the context of a current research. It reflects three general common aspects in Subjectivity research processes: the epistemological aspect, the methodological aspect and, what traditionally has been called "operative aspect". In this interview this aspect is called dynamic relation processes inherent to the method. Here the tensions within the mentioned aspects are shown providing evidence the historical and cultural perspective that the interviewee has been developed as well as his qualitative epistemology proposal.
RESUMO
This paper presents the technical-economic and environmental assessment of four lignin extraction processes from two different raw materials (sugarcane bagasse and rice husks). The processes are divided into two categories, the first processes evaluates lignin extraction with prior acid hydrolysis step, while in the second case the extraction processes are evaluated standalone for a total analysis of 16 scenarios. Profitability indicators as the net present value (NPV) and environmental indicators as the potential environmental impact (PEI) are used through a process engineering approach to understand and select the best lignin extraction process. The results show that both economically and environmentally process with sulfites and soda from rice husk presents the best results; however the quality of lignin obtained with sulfites is not suitable for high value-added products. Then, the soda is an interesting option for the extraction of lignin if high quality lignin is required for high value-added products at low costs.

PURPOSE: Positron emission tomography (PET) analysis of clinical studies is mostly restricted to qualitative evaluation. Quantitative analysis of PET studies is highly desirable to be able to compute an objective measurement of the process of interest in order to evaluate treatment response and/or compare patient data. But implementation of quantitative analysis generally requires the determination of the input function: the arterial blood or plasma activity which indicates how much tracer is available for uptake in the brain. The purpose of our work was to share with the community an open software tool that can assist in the estimation of this input function, and the derivation of a quantitative map from the dynamic PET study. METHODS: Arterial blood sampling during the PET study is the gold standard method to get the input function, but is uncomfortable and risky for the patient so it is rarely used in routine studies. To overcome the lack of a direct input function, different alternatives have been devised and are available in the literature. These alternatives derive the input function from the PET image itself (image-derived input function) or from data gathered from previous similar studies (population-based input function). In this article, we present ongoing work that includes the development of a software tool that integrates several methods with novel strategies for the segmentation of blood pools and parameter estimation. RESULTS: The tool is available as an extension to the 3D Slicer software. Tests on phantoms were conducted in order to validate the implemented methods. We evaluated the segmentation algorithms over a range of acquisition conditions and vasculature size. Input function estimation algorithms were evaluated against ground truth of the phantoms, as well as on their impact over the final quantification map. End-to-end use of the tool yields quantification maps with [Formula: see text] relative error in the estimated influx versus ground truth on phantoms. CONCLUSIONS: The main contribution of this article is the development of an open-source, free to use tool that encapsulates several well-known methods for the estimation of the input function and the quantification of dynamic PET FDG studies. Some alternative strategies are also proposed and implemented in the tool for the segmentation of blood pools and parameter estimation. The tool was tested on phantoms with encouraging results that suggest that even bloodless estimators could provide a viable alternative to blood sampling for quantification using graphical analysis. The open tool is a promising opportunity for collaboration among investigators and further validation on real studies.

Se presenta una conversación con el docente investigador Álvaro Díaz Gómez, la cual a través de una autobiografía intelectual da cuenta del desarrollo de la psicología política en Colombia desde sus precariedades y sus potencialidades. Lo anterior se enmarca en su devenir latinoamericano mostrando diversos autores, enfoques, perspectivas y problemas de investigación. Así, se reconocen cuatro características de la psicología política: 1.- es psicología en cuanto aborda procesos psicosociales que en cuanto disciplina ha investigado y documentado; 2.- es un área de investigación y actuación de la psicología en los procesos políticos; 3.- es una tradición que se ha institucionalizado permitiendo la existencia de pioneros, enfoques, comunidades académicas, formas de difusión; 4.- es plural dados los campos sobre los cuales puede actuar. Desde allí, se propone que la psicología política asuma como campo de indagación lo político y de manera específica la subjetividad política.
This paper presents a conversation with the researcher professor Alvaro Diaz Gomez, which through an intellectual autobiography, talks about the development of political psychology in Colombia since its precariousness and potentialities. This is framed in the Latin-American perspective presenting different authors, approaches, views and research problems. Four characteristics about Political Psychology: 1- It is Psychology in terms of facing psychosocial processes, does research and designs documents about it. 2. - It is research and performing psychological area in regards to political processes. 3. - It is a tradition that allows the existence of pioneers, approaches, academic communities, and dissemination ways. 4. - It is plural due to the performing fields. It was proposed that the political psychology assumes politics as an inquiry field and the political subjectivity in specific.
