RESUMO

Gut microbiota has been shown to play critical roles in host health. The present study was to determine the toxicological effects of microcystin-LR (MCLR) on gut microbial community and metabolites using 16S rDNA sequencing and gas chromatography-mass spectrometry (GC-MS). MCLR was administered to BALB/c mice by gavage for eight weeks. Results of the microbial alpha-diversity (Sobs, Chao1, ACE and Shannon indexes) decreased in MCLR-treated group versus controls. Phylum Candidatus Saccharibacteria decreased significantly in MCLR-treated group versus controls. Correspondingly, more than thirties genera in relative abundance decreased, especially short chain fatty acid (SCFA)-producing bacteria (e.g., Alistipes and Ruminococcus). These results indicated that the gut microbial community structure was remarkably changed by MCLR. Furthermore, concentrations of SCFAs were significantly decreased after MCLR exposure (P < 0.01), where butyrate decreased as high as 4.9-fold. Consequently, sub-chronic exposure to MCLR could not only alter the microbial composition but metabolites. This study offered novel insights into the toxic mechanism of MCs from gut microbiota, and facilitated further clarification of risks to human health from MCs exposure.

RESUMO

To investigate the genotype and the biological characteristics of Tick-borne encephalitis virus (TBEV) in Charles Hilary endemic foci in the China-Kazakhstan border area in Xinjiang,ticks were collected by flagging during May to June in 2012 and 2014,and were stored in liquid nitrogen.TBEV strains were isolated from tick samples by inoculating BALB/c mice and BHK-21 cells.The FE gene fragments of TBEV-Far and the S gene fragments of TBEV-Sib were detected by RT-PCR from infected mice brain tissue and BHK cells,and then subjected to sequence alignment.Totally 16 TBEV strains were isolated from Ixodes persulcatus and Dermuceuter silvarum,among 13 strains were Far eastern subtype,three strains were Siberian subtype.It was first time that the TBEV-Sib was isolated in China.The Charles Hilary TBE natural foci were in the China-Kazakhstan border area,and both TBEV-Far and TBEV-Sib co-circulated.

Until the recent emergence/re-emergence of human-pathogenic viruses in ticks, tick-borne viruses have been neglected as causative agents of human disease (particularly in China). To gain insight into the diversity of tick-borne viruses in Xinjiang Uygur Autonomous Region (northwestern China), we conducted illumina deep sequencing-based screening for virus-derived small RNAs in field-collected Ixodes persulcatus ticks. We found 32, 631 unique virus-matched reads. In particular, 77 reads mapped to the tick-borne group within the genus of Flavivirus, and covered 3.8%-2.4% viral genomes. In addition, 32 unique reads were specific to the Siberian subtype of tick-borne encephalitis viruses (TBEV-Sib) which have never been reported in Chinese TBE loci. We confirmed the potential existence of TBEV-Sib by amplification (using reverse transcription-polymerase chain reaction) of genomic fragments from the envelope gene or 3' genomic terminus from the pools of examined ticks. Both sequences demonstrated high homology to TBEV-Sib strains attached geographically to southern Siberia with nucleotide identity of 97.2%-95.5% and aminoacid identity of 99.4%-98.3%, respectively. In conclusion, we report, for the first time, detection of TBEV-Sib in the natural TBE loci of China. These novel data may provide genetic information for further isolation and epidemiologic investigation of TBEV-Sib.

OBJECTIVE:To establish a method for the content determination of icariin in Kangguzhi zengsheng tablet,and pro-vide a basis for improving the quality standard. METHODS:HPLC was performed on the column of Hypersil ODS2 C18 with mo-bile phase of acetonitrile- water(27∶73,V/V)at a flow rate of 1.0 ml/min,detection wavelength was 270 nm,column temperature was 25℃,and injection volume was 10μl. RESULTS:The linear range of icariin was 3-60μg/ml(r=0.999 9);limit of quantita-tion was 11.2 μg/ml,limit of detection 4.3 μg/ml;RSDs of precision,stability and reproducibility tests were lower than 2.0%;re-covery was 99.00%-100.67%(RSD=0.63%,n=6). CONCLUSIONS:The method is simple,accurate and reproducible,and can be used for the content determination of icariin in Kangguzhi zengsheng tablet.

Objective To study the co-infection of tick-borne pathogens in Ixodes persulcatus collected in Charles Hilary,Xinjiang Uygur autonomous region (Xinjiang).Methods Ticks were collected by flagging in grassy areas in Charles Hilary,Xinjiang from June 2012 to June 2013.The 5S-23S rRNA intergenic region from Borrelia (B.) burgdorferi,16S rRNA gene from Anaplasma,ompA gene from spotted fever group Rickettsia,comI gene from Coxiella (C.) burneti and Nss-rRNA gene from Babesia were amplified with nested polymerase chain reaction (nested PCR),respectively.Results Among 204 lxodes persulcatus,104 were positive for tick-borne pathogens with the positive rate of 50.98%,and among them the positive rates of B.burgdorferi,spotted fever group Rickettsia and Anaplasma phagocytophilum were 34.31% (n =70),28.92% (n =59),9.31% (n =19),respectively.And no C.burnetii and Babesia were detected.The overall co-infection rate was 19.12% (39/204),the co-infection rate was 16.18%(33/204) for B.garinii and spotted fever group Rickettsia,4.90% (10/204) for B.burgdorferi and Anaplasma phagocytophilum,2.94%(6/204) for spotted fever group Rickettsia and Anaplasma phagocytophilum and 2.45% (5/204) for B.burgdorferi,Anaplasma phagocytophilum and spotted fever group Rickettsia.Conclusion The results indicated that the natural co-infections of B.garinii,B.afzelii,Anaplasma phagocytophilum and spotted fever group Rickettsia existed in Charles Hilary Ixodes persulcatus collected in Xinjiang.

<p><b>OBJECTIVE</b>To detect potential mutations in genes related with non-syndromic oculocutaneous albinism I-IV and ocular albinism type I in two couples who had given births to children with albinism.</p><p><b>METHODS</b>All exons of the non-syndromic albinism related genes TYR, OCA2, TYRP-1, MITF, SLC45A2 and GPR143 were subjected to deep sequencing. The results were verified with Sanger sequencing.</p><p><b>RESULTS</b>For the two female carriers, the coding region of the TYR gene was found to harbor a frameshift mutation c.925_926insC, which was also suspected to have been pathogenic. In one of the male partners, a nonsense mutations c.832C>T was found, which was also known to be pathogenic. Another male partner was found to harbor a TYR gene mutation c.346C>T, which was also known to be a pathogenic nonsense mutation.</p><p><b>CONCLUSION</b>The coding region of the TYR gene c.925_926insC (p.Thr309ThrfsX9) probably underlies the OCA1 disease phenotype.</p>

Objective To investigate the diversity of mosquito-borne viruses in Xinjiang , China, and to identify mosquitos-borne viruses of medical importance rapidly .Methods The virus-derived RNAs in mosquitos captured in wild were screened and confirmed by using Illumina deep sequencing approach and reverse transcription PCR , respectively .The alignment analysis was performed by using gene sequences from GenBank.Results One hundred and forty-four Culex Flavivirus ( CxFV, Flavivirus genus, Flaviviridae) specific sequences were identified .The overlapping reads were assembled into 7 uncontinuous viral genomic contigs.The gaps between the contigs were further filled by RT-PCR products, which resulted in reconstruc-tion of viral genomic 5′and 3′terminus (687 nt and 411 nt).Phylogenetic analysis showed that the newly identified CxFV belonged to America/Asian genotype , which specifically clustered into a clade with other CxFV strains from China mainland ,sharing 98.2%-99.5%homologies in nucleotide sequences and 99.5%in amino acids sequences among them .Conclusion Illumina deep sequencing approach was successfully applied to arthropod-borne virus surveillance .The recently emerged Culex Flavivirus was detected for the first time in Xinjiang, China.

<p><b>OBJECTIVE</b>To monitor and discover medically important mosquito-borne viruses circulating in Xinjiang, China.</p><p><b>METHODS</b>Mosquitoes were collected from Abbey Lake wetland in Bortala, in Northern Xinjiang. Viral isolates were obtained through inoculating and serial passaging into susceptible mammalian host cells (BHK-21), identified by cytopathogenic effect (CPE) observation and plague forming assay. Genetic identification of viral isolates was conducted by RT-PCR, sequencing and phylogenetic analysis.</p><p><b>RESULTS</b>A virus strain which causing CPE on BHK-21 cells, was isolated from the predominant Culex modestus (36.6%) and tentatively designated as Abbey Lake virus. Information on molecular identification revealed that Abbey Lake virus belonged to Orthobunyavirus genus within Bunyaviridae. Partial sequences (651 bp and 980 bp) of viral genomic S and M segment showed that Abbey Lake virus was phylogenetically related to Germiston virus that uniquely found in South Africa with 90.6% nucleotides and 95.0% amino acids similarities in S segment. However, viral M segment displayed much variability with 78.6% nucleotides and 86.1% amino acid similarities, suggesting a new member of Orthobunyavirus genus was discovered in the area.</p><p><b>CONCLUSION</b>In this study, Abbey Lake virus was isolated and characterized indicating its potential circulation nature of this newly-emerged mosquito-borne virus.</p>

<p><b>BACKGROUND</b>The peculiar and highly variable C2 anatomy can make screw fixation more challenging and prone to potential vertebral artery or neurologic injury. Conventional C-arm fluoroscopy has several drawbacks. The aim of this research was to evaluate the accuracy of posterior C2 screw fixation using intraoperative three-dimensional fluoroscopy-based navigation (ITFN) and assess the perioperative complication rate related to screw placement.</p><p><b>METHODS</b>A retrospective review identified patients who underwent operative management with C2 instruments using ITFN at our hospital between January 2006 and December 2012. Clinical data were obtained from medical records and final screw positions were graded according to a modified classification of Gertzbein and Robbins. Grade A and B screws were considered well positioned.</p><p><b>RESULTS</b>The study included 99 patients (53 males and 46 females) who underwent posterior C2 screw fixation using ITFN. The mean Japan Orthopedic Association score improved from (6.7 ± 1.9) points before surgery to (12.5 ± 2.7) points at 6-month follow-up (z = +8.628, P < 0.01). The mean visual analogue scale improved from (4.1 ± 1.2) points before surgery to (0.7 ± 0.9) points at 6-month follow-up, with an improvement of 83.7% (z = 8.638, P < 0.01). Of the 196 screws analyzed using computed tomography and chart review, 126 transarticular, 64 pedicle, and 6 pars screws were placed with 82.5% (104/126), 89.1% (57/64), and 100% (6/6) accuracy (grade A), respectively; 98.5% (193/196) of screws were grade A or B (grade C, 1.5% (3/196)), and no neurologic injuries occurred. In normal C2 cases, 93 transarticulars and 47 pedicles were placed with high accuracy rates of 90.3% (84/93) and 93.6% (44/47) (grade A), respectively. However, in cases with C2 deformity, 33 transarticular, 17 pedicle, and 6 pars screws were placed with only 60.6% (20/33), 76.5% (13/17), and 100% (6/6) accuracy (grade A), respectively.</p><p><b>CONCLUSION</b>ITFN is a safe, accurate, and effective tool for posterior C2 fixation.</p>

<p><b>OBJECTIVE</b>To compare the acute toxicity and content of daphnoretin among extracts of unprocessed indian string-bush root and its two different processed products, and to provide a basis for discussion of the mechanism of two processed methods.</p><p><b>METHOD</b>Extracts of unprocessed indian stringbush root and processed indian stringbush root with "sweat" and "artificial sweat" were prepared. The mice were intragastrically administrated once with these three extracts, the mortalities of mice were observed, and the median lethal dose (LD50) of different extracts were calculated with Bliss method. The determination of daphnoretin in these three samples was performed by high performance liquid chromatography.</p><p><b>RESULT</b>The LD50 of indian stringbush root extracts, indian stringbush root processed with "sweat" and with "artificial sweat" were 46.678, 72.190, 67.953 g x kg(-1), respectively. The contents of daphnoretin in unprocessed indian stringbush root, indian stringbush root processed with "sweat" and with "artificial sweat" were 0.189%, 0.407% and 0.345%, respectively.</p><p><b>CONCLUSION</b>The toxicity of indian stringbush root processed with both "sweat" and "artificial sweat" is lower than that of the original rude drug. But the decreasion of toxicity of processed products is not by the reduced daphoretin content.</p>

Objective By systematically analyzing the past literature on medical quality evaluation and the principles of selecting indicators and establishing the evaluate system in comprehensive hospitals in China,we offered valuable references for the establishment of the indicator system for evaluating medical quality.Methods According to the requirements of systematic analysis,we searched all the literatures published in the past decades within CNKI and analyzed them in accord with the inclusive criteria.Results It established 13 items medical quality key evaluation indications including the bed utilization ratio,the average hospitalization days of patients,the yearly average rate of in-patient cares per doctor,the turnover of beds,the degree of nursing effect,the success rate in rescuing critically ill patients,the incidence of nosocomial infections,the curing and improvement rate,the accordant diagnostic rate,the eligibility of basic nursing,the satisfactory ratio of patients,the income per capita,the ratio of drug income in the total revenues,the average medical expense per inpatient.Conclusion China's general hospitals already have in place their principles of KPI selection and their indicator systerm.The methodology to screen quality indicators has also taken shape.This study earmarked 13 key performance indicators for quality of care in its analysis,yet roadblocks still remain in building China' s KPIs for quality of care.

Objective To study the clinical outcome of percutaneous vertebroplasty (PVP) for the s disease who had been suffering severe back pain even after conservative therapy for months were treated with PVP. Their preoperative CT images indicated nonunion of factures and "vacuum signs". Dynamic X-ray films demonstrated formation of pseudoarthrosis in the involved vertebral bodies in some eases. Their back pain was evaluated with visual analogue scale (VAS). Their preoperative and postoperative VAS scores and radiological indexes were compared. Results The mean VAS scores were 7.0±1.2 preoperatively, but 3.1±1.5 at the follow-up (P < 0.05) . The height of anterior margin of involved vertebral body was (2.1±0.3) cm pre-operatively, but (2.3±0.2) cm at the follow-up (P < 0.05). The ratio of anterior margin height to posterior margin height of the involved vertebral body was 0. 67±0. 10, but 0.84±0.08 at the follow-up (P<0.05), The focal kyphosis angle was 27.3°± 6.4° preoperatively but 20.7°±5.0° at the follow-up (P < 0.05). No pulmonary embolisms or neurological injuries happened. Conclusion PVP is an effective method for anterior margin of the involved vertebral body partially, and decrease the focal kyphosis.

Objective: To investigate the effects of Kangguzhe Formula(KGZF) on promoting union of experimental rat fracture.Methods: The experimental rat fracture models were reproduced by cannon born injure,continually treated with KGZF 21d by oral administration.The X-ray,pathological histology and anti-fracture intension of fracture union tissue were detected to investigate the effects of KGZF.Results: KGZF can significantly improve the osteotylus rebuild,trabeculation of bone and concentrated osteoblasts of fracture tissue;X-ray detection also showed that it can increase the grade of X-ray and anti-fracture intension of fracture union tissue;Compared with model group,it had significant difference.Conclusion: KGZF can obviously improve the union of experimental rat fracture.

RESUMO

Vertebral compression fractures, the most common complication of the osteoporosis, results in significant morbidity of prolonged and intractable pain in some patients. Vertebroplasty procedure that involves percutaneous injection of bone cement into a collapsed vertebra has recently been introduced to treat osteoporotic patients who have prolonged pain following vertebral compression fracture. To determine the details of the procedure and to gather information on its safety and efficacy, we performed a MEDLINE search using the terms ‘vertebroplasty’. We reviewed reports of these procedures in patients with osteoporosis. Several reports suggest that the vertebroplasty is associated with pain relief in 67% to 100% of the cases. Short-term complications, mainly the result of extravasation of cement, can increase pain and damage from heat or compression to the spinal cord or nerve roots. Proper patient selection and good technique can decrease such complications so that the decompression surgery is rarely needed. But long-term complications, which include foreign-body reaction at the cement-bone interface, the wear of the cement, and increased risk of fracture in the adjacent vertebrae due to changes of mechanical stresses, have not been fully evaluated. In short, there is still a long way to go for the vertebroplasty.

Objective To introduce a two-stage operative method to treat ma lu nion after the distal femoral fracture. Methods From Aug. 1992 to Aug. 2000, 12 patients underwent osteotomy and secondary fixation due to malunion after the di stal femoral fracture. There were 10 closed fractures and 2 open ones. The prima ry treatment was conservative for 2 patients, and operative for 10. We first per formed the osteotomy along the primary fracture line where the deformity was the most severe. When the roentgenogram confirmed that the deformity had been corre cted satisfactorily, a secondary operation of ORIF (open reduction with internal fixation) was performed. Results On average, all the 12 patients gained bone un ion 5.1 months postoperatively. The mean follow-up duration was 7.2 years (rang e: 3.5 to 11.5 years). None of the patients was afflicted by infection or loosen ing of the internal fixator. According to the Schatzker scoring scale system, al l the patients were evaluated as poor preoperatively, but as excellent in 7, goo d in 4 and fair in 1 at the last follow-up. Conclusion Osteotomy and secondary fixation is not only simple and accessible but also can lead to confirmed clinic al results. However, it is still very important to reduce and fix the acute frac tures stably to avoid malunion.

Objective To introduce a new bilateral triceps brachii approach for the treatment of intercondylar fractures of the humerus, and explore the possibility for the operation without injuring the mechanism of extension of the elbow. Methods With fresh cadaver specimens, the triceps brachii was stripped off from the distal end of the humerus, the muscle belly was elevated and retracted bilaterally, then the height was recorded, and the exposure of the distal humerus was observed, especially to the trochlear region when the elbow were flexed at 15? , 30? , 45? , 60? , 80? respectively. Results Through the cadaver specimen observation and the clinical application, the reduction and fixation of the intercondylar fractures of humerus should be performed when the elbow is flexed at 45?- 60?, at 15?- 30? flexion, fracture over the supracondylar can be treated and finally at 80? flexion, the reduction of the trochlear region can be examined. Conclusion This bilateral approach through the triceps brachii is suitable for the treatment both of the intercondylar and epicondylar fractures.

Objective To observe the therapeutic effect of Gusongbao capsule for osteoporosis. Methods Experimental osteoporosis rat models were established by extirpation of bilateral ovaries of female rats. Bone morphological indexes, blood biochemical indexes, and daily output of calcium and phosphorus in urine in the Gusongbao treatment group and control group were determined. Results Compared with the control, the average width of bone trabecula, relative bone volume, average thickness of bone cortex, bone calcium, and the average gray degree of femoral head and femoral shaft were high, and the serum calcitonin (CT) and blood calcium increased markedly in the treatment group. High-dose and moderate-dose Gusongbao increased number of osteocytes in per unit area of bone trabecula, decreased serum alkaline phosphatase (ALP) level and reduced the daily output of urine calcium. Conclusion Gusongbao capsule can obviously inhibit the excessive bone transformation and promote the re-construction of bone and exerts an antagonistic effect on osteoporosis in ovariectomized rats.

Objective To explore the technique outcome of fixing the ventral stress pedicle screws into the injured vertebrae, as a method to enhance the posterior internal fixation. Methods From March 2002 to March 2005, 33 single thoracolumbar fractures were studied retrospectively. Among which, 16 cases were treated with the above method(group A), and the other 17 were treated with traditional two-level fixation(group B). Group A involved 11 males and 5 females, aged 48 years averagely(range, 32-74 years); and group B included 12 males and 5 females, aged from 21 to 61 years(mean, 40 years). All the patients underwent the operation within up to 3 weeks after fracture. For the injured vertebral bodies, their pedicles were intact on either unilateral or bilateral side, and their lower half and endplate were free from split. In group A, the pedicle screws in the injured vertebrae were used to achieve the ventral press vertical to the distraction for the stress neutralization, and also with the routine distraction and lordosis restoration, simultaneously. The mean follow-up period was 11 months with a range from 6 to 24 months. Results After operation, the optimal Cobb angle and anterior column restoration were achieved through the ventral reduction from the injured vertebral body, which was the contribution from the vertical stress pedicle screw.
