RESUMO

The high morbidity of cholelithiasis and considerable percentage of complications dictates necessity to search new little invasive modes of treatment. In this, one of the perspective directions is the application of dissolution of concrements using contact litholysis. The solubility of concrements directly depends on their composition. The detailed knowledge of composition of gallstones subjected to dissolution is needed for prognosis of solubility of concrements and experimental findings of new solvents. The mode of atomic emission analysis was applied to study mineral composition of 105 gallstones extracted from gallbladders of patients with cholelithiasis operated using laparoscopy. The experimental studies were implemented in vitro on dissolution of concrements depending on their composition and using octane acid. It is established that solubility of gallstones depends on content of calcium and ash composition of concrements. In consideration of relative simplicity of application, absence of complicated preparation of tests for analysis, distinct dependence of solubility of concrements on content of calcium and ash content of gallstones the atomic emission analysis can be recommended for examination of composition of gall concrements in case of search and development of new modes of litholysis. This mode of analysis can be also applied for prognosis of dissolution of concrements under application of contact litholysis.

RESUMO

It was suggested to use contact ultrasonic lithotripsy in lumen of common bile duct during laparoscopic surgery in order to preserve the sphincter apparatus of major duodenal papilla. Waveguide with original structure was used for lithotripsy performing. Waveguide's total length is 400 mm. It has a radially curved 40 degrees broken part with diameter of 6 mm cone rolling in working portion with diameter of 4 mm and length of 60 mm for introduction in common bile duct. There is concave lens on waveguide working portion end with diameter of 1 mm. A lens permits concentrating the waves beam in longitudinal direction avoiding its scattering and minimizing the impact on surrounding tissues. Lithotripsy efficiency was proved in the in vitro (n=68) and in vivo (n=20) experiments. Such structure of waveguide permits to penetrate in lumen of common bile duct through dilated cystic duct or choledochendysis for calculi fragmentation.

RESUMO

The article deals with the results of analysis of specters of short-chained fat acids and cytokines in affected skin of patients with psoriasis. The study revealed the significant decrease of short-chained fat acids level, the shift of cytokine profile in the direction of anti-inflammatory factors (interleukins 1L-1beta, IL-8, tumor necrosis factor TNF-alpha, interferon IFN-alpha) and mytogenetic factors (EGF) on the background of stable values of anti-inflammatory cytokine IL-4. The direct pathogenically significant correlation relationships are established between the IL-4 level and the amount of most analyzed short-chained fat acids. The negative correlation relationships were established between content of C2 and IL-1beta.

RESUMO

High-performance liquid chromatography was used to study the phospholipid composition of spermatozoa and sperm plasma in men with fertility disorders. The changes in the phospholipid spectrum of both germ cells and its surroundings were found to depend on the nature of changes in the spermatogram. The most pronounced shifts were common to subjects with asthenozoo-, oligozoo-, and teratozoospermia, which may be a cause of infertility.

RESUMO

The paper depicts changes in the epidermal fatty-acid spectrum in patients with psoriasis progressing to the stage of dermatosis in various age groups (18-40 and 41-60 years). The parameters studied in healthy individuals showed age-related differences in the pool of saturated and unsaturated fatty acids (SCFA). Heterodirectional changes in the epidermal levels of saturated, polyenic, and monoenic fatty acids were recorded in the lesion foci and visually healthy skin of patients with psoriasis depending on age.

Gas liquid chromatography was used to study the spectrum of short-chain fatty acids (SCFA) in the serum and epidermal corneal layers of abnormal and apparently healthy skin areas in patients with psoriatic disease on an exacerbation and at a progressive stage. Significantly lower SCFA levels were detected in both psoriatic and intact areas, with derangements being mostly manifested in the papulae. There were heterodirectional serum volatile fatty acid spectrum changes that were characterized by a rise in the concentrations of isobutyrate, valeriate, and capronate and by a decrease in the proportion of acetate and propionate.

The relative content of higher fatty acids (FA) was studied in lachrymal fluid of 10 virtually healthy subjects and of 9 patients with maculodystrophy aged 30 to 40. The FA methyl ethers were assayed with the "Chrystall-2000 M" gas chromatograph. The content of C(15:0), C(15:1), C(16:0) and C(16:1) as well as the content of such oversaturated FA as C(18:3omega6) and C(20:4omega6) was found to be decreased in patients with macoludystrophy. Simultaneously, a higher level of FA with a longer carbon chain (C(17:0), C(18:0), C(18:1), C(18:3omega3) and C(20:3omega6) was registered in these patients. The results testify to changes in the composition of lachrymal fluid in ophthalmopathy and point out the expedience of further research in the discussed field.

RESUMO

43 patients and 20 healthy children, aged 11-15, were examined. Increased contents of primary, secondary, intermediary and final products of lipid peroxidation (LPO) in serum lipids as well as an intensified antioxidant activity were registered in children with erosive gastroduodenitis. Ventramine, when used in the treatment of erosive gastroduodenitis in children, arrests rapidly the clinical signs of the disease and normalizes LPO.

The paper presents the data of examination of three groups of patients with pulmonary tuberculosis: 25 with a relatively favorable course of the disease, 26 with acutely progressive tuberculosis, pneumoperitoneum along with chemotherapy was used in complex therapy in 10 of them. The parameters of lipid peroxidation were estimated in the condensate of the expired air from patients before and 3 months after inpatient therapy. Irrespective of the type of the course of the disease, patients with tuberculosis were found to have a marked increase in the elimination of lipid peroxidation products, as well as deficiency of antiradical defense factors, which also preserved after therapy. The use of pneumoperitoneum in the complex therapy of patients with acutely progressive tuberculosis reduced the level of lipid peroxidation products more significantly.

Changes of the lipid peroxidation processes (LP) in the skin (corneal layer of the epidermis) and in blood, as observed in 61 patients with local psoriasis at the progressing stage and in 55 patients with local psoriasis at remission, were investigated. The quantity of diene conjugates, ketodienes, and of conjugate trienes in the heptanoic and isopropanol phases was decreasing both in the affected and intact skins versus the controls. The test based on thiobarbituric acid (TBA) was used to establish that the content of the TBA-positive products was also decreasing. The examined values did not reach the control figures at remission. The content of the studied indices in blood was reliably higher than the control figures in all lipid extract phases at exacerbation of the dermal affection; it did not decrease after the main dermal lesion was eliminated. The correlation analysis revealed the presence of correlations between the studied parameters in blood and skin, which were changing with respect to a stage of the dermal affection.

RESUMO

The efficacy of 1% emoxipin solution (eye drops) in the nonproliferative stage of diabetic retinopathy was studied. The inhibition of lipid peroxidation and an enhancement of antioxidizing guard were observed both after the therapy by the above preparation and 3 months later. It was concluded that emoxipin was effective for leveling the biochemical violations in the "lipid peroxidation--antioxidants" system of lachrymal fluid in patients with the nonproliferative stage of diabetic retinopathy. The prescription of emoxipin eye drops can enhance the efficacy of therapy at the early stages of the disease.

Pain dysfunction syndrome of the temporomandibular joint (TMJ) in women is often accompanied by mammary and reproductive disorders associated with endocrine disturbances. 30 female patients aged 19-35 years suffering from of the pain dysfunction syndrome TMJ combined with intact dentition and orthognathic occlusion were studied. Dental examination was followed by gynecologic examination. According to the treatment the patients were divided into 2 groups. Group I included women who were administered conventional treatment (orthopedic treatment, drug and physiotherapy). In group II patients conventional therapy was also combined with monophase oral contraceptive "Femoden". Efficacy of treatment in group I was 46.67%; while group II patients showed a 100% favourable effect. Besides, combination of dental and gynecologic therapy was found to exert a 2.1 times more stable clinical effect us compared to conventional treatment. Hormone correction in women with reproductive pathology results in prolonged control of the pain dysfunction syndrome TMJ due to steroid normalization (hypothalamic, hypophysial, ovary steroid normalization) and has no side effects.

The aim of the study was to trace hormone shifts in women suffering from different manifestation of the TMJ pain dysfunction syndrome. Levels of some blood steroid hormones (oestradiol, progesterone, testosterone, cortisol) were determined in 193 female patients aged 14-70. The object of study was blood serum collected in different menstrual phases. It was found that when pain dysfunction syndrome of the TMJ was accompanied by intact dentition and orthognathic occlusion blood steroid levels demonstrated shifts since puberty. In pain dysfunction syndrome of the TMJ accompanied by occlusal disharmony steroid shifts were first registered an early reproductive age. In patients without pain dysfunction syndrome of the TMJ steroid values remained close to those of healthy control. Shifts in steroid production may be indicative of impaired steroidogenesis in women with pain dysfunction syndrome of the TMJ.

RESUMO

The fatty acid spectrum of an expired air condensate was studied in the workers of the Kharanor coal stripping and in the dwellers of Chita (a control group). Regularities in the changes of the expired air fatty acid profile were assessed by the degree of exposure to coal dust and by the length of service. The findings indicated the relationship of profile changes with the degree of exposure to occupational noxious agents: the proportion of saturated fatty acids decreases much more significantly in workers who were directly exposed to the dust than in those were indirectly done. It is concluded that the fatty acid composition of an expired air condensate adequately reflects the changes occurring in the respiratory system upon exposure to dust.

The fatty acid (FA) spectrum of lung tissue fragments was studied versus distance of malignant cells from lesion focus in lung cancer patients. Decreased polyunsaturated FA levels in all samples were attributed, chiefly, to linoleic, (linoleic-new) and eicosatrienoic acid deficiency. Both in tumor tissue and areas of intensive cell proliferation, accumulation of monounsaturated C18:1 and C15:1 fatty acids, low C20:4(6 and C 20:5(3 and an FA with 19 carbon atoms were detected. The total concentration of (-6 and (-3 series fatty acids was relatively low, too, the deficit of the latter being remarkably low. Those changes are likely to be related to certain peculiarities of the structure and functions of tumor cells.

The evaluation of local peroxide status was studied in 42 patients with psoriasis depending on the disease stage in different age periods (from 18 to 67 years) with disease duration from 2 months to 20 years. It was established that the development of psoriatic process is accompanied with the marked changes in local peroxide status in the epidermis. It is manifested by the decrease in intensity of lipid free-and-radical oxidation. Decrease in the products of lipoperoxidation occurs in epidermis--both in the affected and intact areas in all investigated patients independently of the stage of psoriatic process. It is more expressed during the aggravation period. The minimal levels of the intermediators being studied are recorded in the psoriatic plaques.

Phospholipid spectrum of lung tissue fragments was studied in the dependence on the presence of malignant cells located at different distance from the focus of affection. Elevation of sphingomyelin (SM) level and lowering of phosphatidylcholine (PC) was noted in all analyzed patterns in pulmonary tissue. Alteration of growth malignant fields with extensive necrosis zones predominated in the same simples. The accumulation of phosphatidylinositol (PI) fraction and phosphatidylglycerol (PGL) deficiency was found. A similar tendency was supplemented with lysophosphatidylcholine (LPC) maximum on line of the extensive growth. It might be the result of invasive characteristic manifestation by tumor cells.

RESUMO

As shown by investigations of alkaline phosphatase, plasma levels of calcium, non-organic phosphorus, coefficient Ca/P in 58 patients with osteoarthrosis deformans before and after radon baths, mud applications or their combination, the highest biochemical effect was achieved in a group of patients on mud therapy.
