RESUMO

BACKGROUND: The orchestration of jaw relations in edentulous scenarios poses a significant challenge, demanding acumen, and expertise from the clinician. A myriad of devices, substances, and methodologies are employed to gauge the transverse, vertical, and horizontal spectrums of jaw relations. Traditionally, a synthesis of assorted devices and techniques has been requisitioned to chronicle the spatial dynamics between the maxilla and mandible. Yet, the absence of a solitary apparatus capable of concurrently documenting the triad of jaw relations has been a notable lacuna in prosthodontic rehabilitation. PURPOSE: This discourse expounds on an innovative contrivance, termed the Precise Jaw Relation Recorder. This pioneering instrument is adept at capturing the tripartite jaw relation processes: the occlusal plane's alignment in parallelism to the ala-tragus line, the vertical dimension at repose, and the occlusion as well as the centric relation. The device's strategic utility lies in its ability to facilitate the creation of prostheses that are not only functionally superior but also aesthetically more pleasing. CONCLUSION: Its implementation is a stride toward refining the accuracy of prosthodontic outcomes, thereby elevating the standard of patient care in dental practice.

Background and Purpose: During complete denture fabrication making Interocclusal records plays an integral role to obtain the condylar guidance value. The study was conducted to compare the protrusive condylar guidance registration by two interocclusal recording materials-Quick setting plaster and Luxabite®(bisacrylic composite) in semi adjustable articulator for completely edentulous patients. Methodology: Maxillary and Mandibular casts of the completely edentulous patients were mounted in Hanau®Wide Vue articulator. The Quick setting plaster and Luxabite®(bisacrylic composite) was used as interocclusal recording materials to program the protrusive condylar guidance angles in the articulators. Results: The protrusive condylar guidance values registered in the articulator for the different interocclusal records were tabulated and statistically analyzed.. The mean protrusive condylar guidance values registered in the articulator was compared with two parameters obtained from the radiographic tracings -- the protrusive condylar path angle obtained with Quick setting plaster and Luxabite with the inclination of articular eminence to the Frankfort horizontal plane. Conclusion: From the study it was concluded that the Luxabite®(bisacrylic composite) material demonstrated better reproducibility of protrusive condylar guidance registration.than the Quick setting plaster.


Résumé Contexte et objectif: Pendant la fabrication complète de la prothèse de prothèse, la fabrication des enregistrements d'interoccluse joue un rôle essentiel pour obtenir la valeur de guidage condylien. L'étude a été réalisée pour comparer l'enregistrement de guidage condylien en saillie par deux matériaux d'enregistrement interocclusal - Platter en plâtre et Luxabite® (composite bisacrylique) dans un articulateur semi-réglable pour les patients complètement édentés. Méthodologie: Les moulages maxillaires et mandibulaires des patients complètement édentés ont été montés dans l'articulateur de Hanau® à large de Vue. Le plâtre à réglage rapide et Luxabite® (composite bisacrylique) ont été utilisés comme matériel d'enregistrement interocclusal pour programmer les angles de guidage condyliennes en saillie dans les articulateurs. Résultats: Les valeurs de guidage condylien en saillie enregistrées dans l'articulateur pour les différents enregistrements interocclusaux ont été tabulées et analysées statistiquement. Les valeurs de guidage condyliennes en protruments moyennes enregistrées dans l'articulateur ont été comparées à deux paramètres obtenus à partir des tracages radiographiques - le protruments de l'angle de trajectoire condylien obtenu avec un plâtre rapide et luxabite avec l'inclinaison d'une éminence articulaire au plan horizontal de Frankfort. Conclusion: D'après l'étude, il a été conclu que le matériau Luxabite® (composite bisacrylique) avait démontré une meilleure reproductibilité de l'enregistrement de guidage condylien en saillie que le plâtre de réglage rapide. RESUMO

One of the important ways of social interaction is through verbal and nonverbal communication. The human face is capable of eliciting multi-response according to the situation; amongst them, a smile plays a significant effect in relaying a positive communication that can immensely influence societal outcomes. An important part of dental treatment is to restore the tooth to functionality and to esthetically rehabilitate it, which forms the core of esthetic dentistry. Modern advancements have led us to various esthetic treatment options. Recently, due to the boom of information technology, we are progressing into the digital age where everything has almost been made through computers and the internet. In the dental field, advanced software is being used to modify and create smiles, thereby completely revolutionizing esthetic dentistry. Digital smile design is a recently introduced concept and software which analyzes the smile of an individual through various input scanners and photographs. They provide a plethora of solutions and predictions as to how the smile can be designed, to the point it can pinpoint minor corrections. Here, we discuss the importance of smiles and the analysis using digital smile design.

RESUMO

The practice of evidence-based dentistry has assumed an integral part in today's world. It allows us to achieve self-motivated, problem-based learning, which eventually leads to acquiring clinically sound and relevant information that has a strong backing of evidence on a scientific basis. This would enhance the diagnosis but also the prognoses and treatment that are administered with the highest of ethical standards. Due to the advancement in material studies, dentistry, especially prosthodontics, is getting more intricate and complex due to the dynamic state in the development of new dental materials and equipment. The best evidence is sought through vast empirical literature consisting of controlled trials and reviews. Modern clinical practice should be concurrent with the latest scientific evidence that brings high standards to the treatment options and patient's values. This review highlights the evidence based dentistry on prosthodontics while also addressing the issues it poses in modern day dental practice.

AIM: The aim of the systematic review was to assess the influence of dental prostheses on cognitive functioning in elderly population. MATERIALS AND METHODS: This systematic review was conducted according to the PRISMA guidelines. The initial electronic search was conducted using the following search databases: MEDLINE (PubMed), Cochrane Library, Google Scholar, and EMBASE. The search was limited to English language using the search items/keywords: "dental prostheses and cognitive functioning," "dental prostheses and brain function," "Tooth loss and cognitive loss," "mastication and prefrontal activity," and "prostheses on mental state." The search strategy was followed using the PICOS framework. RESULTS: A total of 19 studies were selected according to the selection criteria. Out of 19 studies, 15 studies were included and 4 studies were excluded from the review. CONCLUSION: With the available evidence in the literature, it can be concluded that dental prostheses have a very significant role in preventing the cognitive impairment and act as a protective factor in enhancing the cognitive function in patients with dementia-related diseases and neurodegenerative diseases.

BACKGROUND: Rosa damascena Mill, commonly known as Damascus rose and garland rose, belongs to Rosaceae family, which has around 200 different types of rose species. R. damascena Mill is famous for its fragrance and beauty, and hence it is highly cultivated all over the world. In addition, this plant is also used for its medicinal properties. OBJECTIVES: The objectives of this study are to prepare mouth wash and gel from the oil of R. damacena Mill, to evaluate the effectiveness of the medicinal properties of prepared mouth wash and gel, to evaluate its anti-inflammatory and analgesic properties, and to evaluate the adverse effects. MATERIALS AND METHODS: At the JSS College of Pharmacy, Ooty, the oil of R. damacena Mill was converted into mouth wash and gel. To evaluate the effectiveness of the prepared rose gel and mouthwash, 24 Wister and 24 Albino rats were subjected to acetic acid induced writhing, hot plate method, and Carragennin induced paw edema test. RESULTS: Acetic acid induced writhing in mice and hot plate method were carried out. Both methods proved presence of analgesic activity, and Carragennin induced paw edema test showed the anti-inflammatory property of R. damascena mill. CONCLUSION: These two in vivo studies proved that the presence of analgesic and anti-inflammatory properties in prepared R. damascena mill mouth wash and gel.

INTRODUCTION: An accurate and passive fit of implant framework prosthesis, as well as the successful surgical operation is suggested as one of the critical requirements for long-term implant success. OBJECTIVE: The purpose of this in vitro study was to evaluate the accuracy of the master cast using open tray impression technique with conventional and novel splinting materials. METHODOLOGY: A mandibular reference model with four ADIN implants was done. Ten custom trays were fabricated using the light curable resin sheets. Medium body polyether impression material was used. These trays were randomly divided between the two groups, with five trays in each group. Impression techniques were divided into two groups namely: Group A: Direct impression technique with open tray impression copings splinted with autopolymerizing acrylic resin (GC pattern resin). Group B: Direct impression technique with open tray impression copings splinted with Pro-temp TM 4 (bis-GMA) syringable temporization material. Thus, final impressions were made. Total of 10 master casts were fabricated. Evaluation of casts using Dynascope-Vision Engineering, TESA microhite two- dimension and coordinate measuring machine were used. RESULTS: Statistical comparisons were made using ANOVA test and post-hoc test. Same amount of deviation values obtained with resin splinted and bis-GMA splinted impression copings. CONCLUSION: The master cast obtained by both the splinting material exhibits no difference from the reference model. So bis-GMA can be used, which is easy to handle, less time consuming, less technique sensitive, rigid, and readily available material in clinics.

RESUMO

The mainstay of pediatric dental practice is the successful esthetic rehabilitation of a preschooler with advanced carious lesions. Loss of masticatory efficiency, compromised esthesis, mispronunciation of labiodentals sounds, and development of abnormal oral habits are compromises arising due to the loss of primary anterior teeth at an early age either due to trauma or due to caries. Parental desire is the most decisive factor for the placement of an anterior esthetic appliance. This unique case report highlights the fabrication of simple, Groper's appliance in a 5-year-old child with early childhood caries.

Patients with a tracheostomy stoma experience compromised speech and function due to the associated changes in airflow patterns. Rehabilitation of a patient with tracheal stoma is a highly challenging task. The main objective is to design an inexpensive, easily fabricated stomal prosthesis for postlaryngectomy patients who require prolonged tracheotomy. This clinical case report describes a 29-year-old male patient who underwent for tracheotomy 3 months before for respiratory distress following a suicidal attempt. Hence tracheotomy was done, and the patient has been with the tracheostomal tube since surgery for the past 3 months. Laryngoscopy examination reported as restricted bilateral vocal cord movements, and the cords were in the adducted position with minimal glottic chink. No history of difficulty in swallowing. On examination, no scar or ulceration is seen around the stoma. The skin around the stoma is healthy. The patient was referred to the oral and maxillofacial Prosthodontics Department from the Department of ENT. The patient's old tracheostomal tube was used as the dimensions of the custom made tracheal prosthesis without making a functional impression of the mature stoma. A tracheal button was made with 2 mm polyethylene urethane sheet to maintain the airway patency of the mature stoma. Width and length of the old tracheostomal tube were measured and customized with polyurethane sheet by directly flaming over heat. The finished product was thin, flexible, maintains enhanced tear strength, require no tapes or adhesives and less technique sensitive. These properties of the prosthesis make more advantageous than the commercially available tracheal buttons. The result in this patient was excellent with no postoperative complications. An innovative approach for fabrication of tracheostomal prosthesis was discussed to increase its successful use in tracheostomal patients. The patient's old tracheostomal tube was used as the dimensions of the custom made tracheal prosthesis without making a functional impression of the mature stoma. The finished product was thin, flexible, maintains enhanced tear strength, require no tapes or adhesives and less technique sensitive. These properties of the prosthesis makes more advantageous than the commercially available tracheal buttons. The result in this patient was excellent with no postoperative complications.

The periodontal health of abutment teeth and the durability of fixed partial denture depends on the marginal adaptation of the prosthesis. Any discrepancy in the marginal area leads to dissolution of luting agent and plaque accumulation. This study was done with the aim of evaluating the accuracy of marginal fit of four unit crown and bridge made up of Ni-Cr and Cr-Co alloys under induction and centrifugal casting. They were compared to cast fixed partial denture (FPD) and soldered FPD. For the purpose of this study a metal model was fabricated. A total of 40 samples (4-unit crown and bridge) were prepared in which 20 Cr-Co samples and 20 Ni-Cr samples were fabricated. Within these 20 samples of each group 10 samples were prepared by induction casting technique and other 10 samples with centrifugal casting technique. The cast FPD samples obtained were seated on the model and the samples were then measured with travelling microscope having precision of 0.001 cm. Sectioning of samples was done between the two pontics and measurements were made, then the soldering was made with torch soldering unit. The marginal discrepancy of soldered samples was measured and all findings were statistically analysed. The results revealed minimal marginal discrepancy with Cr-Co samples when compared to Ni-Cr samples done under induction casting technique. When compared to cast FPD samples, the soldered group showed reduced marginal discrepancy.

Large cranial defects of complex geometric shapes are challenging to reconstruct. The cranial implants has to be fabricated prior to the cranioplastic surgery. The ideal material for cranial implant has to be inert, light weight, easy to fit and adaptable to the defect, offering the best aesthetic and functional results. Here is a clinical case report of a patient who was operated for osteomyelitis in the parieto-temporal region. The defect was reconstructed with heat cure polymethylmethacrylate (PMMA). Operative closure of the defect was facilitated with ligature titanium wires with minimal prosthesis contouring. The heat cure PMMA cranial implant is a safe, easy and economic alternative with great adaptability to cranial vault defects. The cosmetic results in this patient was excellent. No post-operative complications occurred.

"Identification through forensic science is an art of giving the corpse a name A real life detective work that would put even Sherlock Homes to shame." Forensic dentistry deals with proper handling and examination of dental evidence and proper evaluation and presentation of dental findings in interest of justice. Denture marking or labeling is not a new concept in either Prosthetic or Forensic dentistry and its routine practice has been urged by Forensic dentists internationally for many years. Denture marking is accepted as a means of identifying dentures and persons in geriatric institutions or post mortem during war, crimes, and civil unrest, natural and mass disasters. Prosthodontists are playing very important role in forensic dentistry as they are concerned with fabrication of various prostheses which can serve as an important tool for identification. Identification is essential requirement of any medico-legal investigation because a wrong identity may pose a problem in delivering justice. The main objective of this article is to discuss the various methods of denture marking and to emphasize the importance of denture marking for person identification in medico legal investigations.
