RESUMO
Se conoce como gangrena de Fournier a la fascitis necrosante que afecta a la región perineogenital. Es un cuadro poco frecuente, pero con un índice de mortalidad alto. Su incidencia es mayor en pacientes varones de mediana edad con factores predisponentes, como la diabetes mellitus y el alcoholismo crónico. Suele aparecer como complicación de una enfermedad perirrectal o urogenital que no ha sido correctamente tratada. El diagnóstico de esta entidad es fundamentalmente clínico y el tratamiento debe ser precoz para mejorar el pronóstico; en él se incluyen: antibióticos de amplio espectro, desbridamiento quirúrgico precoz y agresivo y soporte vital de mantenimiento. Se trata de una patología grave que debe ser derivada de forma urgente al hospital
Fournier's gangrene is the term applied to necrotizing fascitis affecting the perineal-genital area. It is an uncommon condition with a high mortality rate. It is more frequent in male and middle aged patients with predisposing factors like diabetes mellitus and chronic alcohol consumption. It arises from perirectal or urologic abscess that has not been correctly treated. Diagnosis of this entity is basically clinical and treatment should be prompt in order to improve the prognosis. This includes broad spectrum antibiotics, early and aggressive surgical debridement and intensive care. Since it remains a surgical emergency, patients should be referred to the hospital for adequate management

Introducción: La rotura diafragmática es conocida desde 1541, inicialmente relacionada con el trauma militar; tras la invención del automóvil y el crecimiento de la violencia civil se observa con una frecuencia creciente en nuestro medio. Métodos: Estudio retrospectivo a lo largo de 20 años que incluye a 38 pacientes afectos de traumatismos diafragmáticos confirmados quirúrgicamente. Resultados: El agente etiológico más frecuente fueron los accidentes de circulación con el 60,5 por ciento de los casos y, globalmente, el traumatismo toracoabdominal cerrado fue el mecanismo más frecuente de trauma con el 63,15 por ciento de los casos. La clínica contribuyó poco al diagnóstico y la radiografía de tórax fue la prueba más rentable. El hemidiafragma izquierdo, con el 81,6 por ciento de los casos, fue el afectado con mayor frecuencia. Todos los pacientes tenían lesiones asociadas, siendo el trauma esplénico el más habitual. Un 52,63 por ciento de los pacientes sufrió algún tipo de complicación y 5 pacientes (13,15 por ciento ) fallecieron. Conclusiones: La rotura diafragmática es una patología poco habitual, de diagnóstico preoperatorio difícil y cuyo tratamiento debe llevarse a cabo lo antes posible para disminuir la morbimortalidad de estos enfermos (AU)

Two mouse sera against outer membrane proteins from a pathogenic Neisseria meningitidis strain and a commensal N. lactamica strain and two human sera from patients recovering from meningococcal meningitis were used to identify antigens common to pathogenic and commensal Neisseria species. Two major antigens of 55 kDa and 32 kDa, present in all N. meningitidis and N. lactamica strains tested, were demonstrable with all the sera used; the 55-kDa protein was iron-regulated. Demonstration of other common antigens was dependent on the serum used: a 65-kDa antigen was visualised with the human and the mouse anti-N. lactamica sera; a 37-kDa antigen identified as the meningococcal ferric binding protein (FbpA) was only detected with the mouse sera, and two antigens of 83 kDa and 15 kDa were only shown with the mouse anti-N. meningitidis serum. The results demonstrate the existence of several outer membrane antigens common to N. lactamica and N. meningitidis strains, in agreement with the hypothesis that natural immunity against meningitis is partially acquired through colonisation by commensal species, and open new perspectives for the design of vaccine formulations and the development of strategies for vaccination against meningitis.
