BACKGROUND: Nowadays, percutaneous procedures are expanding in use, and this comes with complications associated with the procedure itself. Cardiac tamponade is rare but may be life threatening since it can involve hemodynamic instability. It is known that after pleural effusion during a percutaneous procedure, pericardiocentesis should be used as drainage of the cavity. However, that does not achieve hemostasis in some cases, and in those patients who are hemodynamically unstable, a sealing agent to promote hemostasis might be useful, like thrombin. CASE PRESENTATION: We present a case report of 89-year-old patient with history of melanoma undergoing treatment with pembrolizumab, who attended the emergency department referring chest pain (intensity 5/10) and palpitations that have lasted hours. He had TnTUs 554/566 ng/L and an echocardiogram that showed dilated right chambers, hypertrophy and global hypokinesia of the left ventricle, increased filling pressures of the left ventricle and pulmonary hypertension. Myocarditis associated with pembrolizumab was suspected, so high dose steroids were initiated and endomyocardial biopsy was conducted, resulting in iatrogenic cardiac tamponade. To determine the etiology of the suspected myocarditis, an endomyocardial biopsy was performed. Unfortunately, an intraprocedural complication arose: pleural effusion resulting in iatrogenic cardiac tamponade, leading to hemodynamic instability. It required immediate pericardial drainage via subxiphoid puncture, obtaining a 550 mL hematic debit. Clinical manifestations raised suspicion of tamponade, prompting a bedside echocardiogram for a definitive diagnosis. Despite these efforts, the patient remained hemodynamically unstable, and due to the elevated surgical risk, intrapericardial thrombin was employed to achieve successful hemostasis. CONCLUSIONS: Cardiac tamponade is a life-threatening condition that can sometimes be induced iatrogenically, resulting from percutaneous interventions. Despite limited evidence regarding this therapeutic strategy, in patients experiencing iatrogenic cardiac tamponade with hemodynamic instability and high surgical risk, the administration of intra-pericardial thrombin could be contemplated.

Arthroscopic posterior bone block procedure using cortical screws has been reported since 2012 for management of recurrent posterior instability associated with bone defects. To avoid screw complications, we describe a full arthroscopic technique based on cortical button fixation. With 4 portals (posterior, poster lateral, anterior, and anterolateral), we use a specific glenoid guiding system (to perform 2 tunnels) and 2 cortical buttons fixation. With a specific glenoid guide, the iliac crest bone graft (ICBG) is secured to the posterior glenoid rim by fixation with 2 cortical buttons. The ICBG is positioned protruding outside the joint through the space, and the posterior capsule is repaired while maintaining an extra-articular bone graft. The protruding bone graft increases the concavity of the glenoid with a progressive bony remodeling to improve the posterior stability of the shoulder in our clinical experience.

OBJECTIVES: To inform about contemporary PCI practice of in-stent (IS) chronic total occlusions (CTO) from a large international registry in Latin America. BACKGROUND: IS-CTO represent a distinctive challenge for PCI, but literature is limited and restricted to high-resource regions of the world. METHODS: Patients undergoing CTO PCI enrolled in the LATAM CTO registry from 42 centers in eight countries were included. We analyzed demographics, angiographic, procedure technique, success and postprocedural outcomes between IS-CTO and non-IS-CTO PCI. RESULTS: From 1,565 patients IS-CTO was present in 181 patients (11.5%). IS-CTO patients had higher prevalence of diabetes and hypertension than patients without IS-CTO. IS-CTOs had less calcification (32.5 vs. 46.7%, p < .001), lower prevalence of a proximal branch (36.3 vs. 50.1%, p < .001), more likely to be ostial (24.4 vs. 18.1%, p = .042), were longer (28.5 vs. 25.2 mm, p = .062), and had less interventional collaterals (49.1 vs. 57.3%, p = .038) compared with non-IS-CTO. CTO complexity scores were similar between both groups. There was no statistically significant difference in the initial or successful strategy between IS-CTO and non-IS-CTO PCI. Technical success rates remained high in IS-CTO (86.7%) and non-IS-CTO (83.1%, p = .230). There was no independent association between IS-CTO and technical success in multivariable analysis. There were no differences between IS-CTO and non-IS-CTO groups for in-hospital clinical outcomes. CONCLUSION: In a contemporary, multicenter, and international registry from Latin America, IS-CTO PCI is frequent and has comparable technical success and safety profile compared to non-IS-CTO PCI.

RESUMEN Introducción: La diabetes mellitus (DM) se ha asociado a un incremento en los resultados adversos en pacientes tratados con angioplastia coronaria (ATC), en comparación con los pacientes no diabéticos. Objetivos: Evaluar el riesgo de eventos cardiovasculares mayores en los pacientes diabéticos, estratificados según el tratamiento recibido (no insulinorrequirientes o insulinorrequirientes), en una población de pacientes no seleccionada tratados con angioplastia coronaria. Material y métodos: Análisis de registro, unicéntrico y retrospectivo de pacientes con enfermedad coronaria tratados con ATC desde marzo 2009 a junio 2018, según la presencia de DM estratificada de acuerdo con el tratamiento establecido para el control del desorden metabólico en: DM insulino-requirientes (DM-IR) y DM no insulino-requirientes (DM-NIR). Se aplicó un modelo de regresión de Cox ajustado para evaluar la relación entre la presencia de diabetes y el riesgo de eventos cardiovasculares mayores. Resultados: Se incluyeron 6.313 pacientes (seguimiento promedio 4,1 ± 1,8 años), con una prevalencia global de DM del 22,8% (DM-NIR 19,1%; DM-IR 3,8%). Los pacientes diabéticos presentaron un perfil de riesgo elevado, particularmente los DM-IR. Al seguimiento promedio, el riesgo ajustado de eventos cardiovasculares mayores fue similar entre los pacientes No-DM y los DM-NIR (HR 1,02 [0,81-1,27], p 0,85). En relación con los pacientes DM-IR, se observó un riesgo elevado comparados con los No-DM (HR 1,73 [1,20-2,49], p 0,003) y con los DM-NIR (HR 1,65 [1,10-2,48], p 0,015). Se observó una interacción significativa entre el estado diabético y el riesgo de eventos según la indicación de la angioplastia coronaria al ingreso (pint 0,045). Conclusiones: En nuestra serie de pacientes tratados con angioplastia coronaria y con seguimiento a largo plazo, los pacientes diabéticos presentaron alto riesgo de eventos cardiovasculares mayores. Este riesgo se observó particularmente incrementado en pacientes DM-IR. Sin embargo, no se evidenciaron diferencias significativas en el riesgo de eventos entre los pacientes DM-NIR y los No-DM.


ABSTRACT Background: Diabetes mellitus (DM) has been associated with an increase in adverse outcomes in patients treated with coronary angioplasty, compared to non-diabetic patients. Objective: To evaluate the risk of major adverse cardiovascular events in diabetic patients, stratified according to the treatment (non-insulin dependent or insulin-dependent), in a population of unselected patients treated with coronary angioplasty. Methods: Registry-based analysis of patients with coronary artery disease undergoing percutaneous coronary intervention from March 2009 to June 2018, according presence of DM stratified according to the established treatment for the metabolic disorder: insulin-dependent DM (ID -DM) and non-insulin dependent DM (NID -DM). An adjusted Cox regression model was applied to evaluate the relationship between the diabetic status and the risk of major adverse cardiovascular events. Results: A total of 6313 patients were included (mean follow-up 4.1 ± 1.8 years), with a global prevalence of DM of 22,8% (non-insulin dependent DM 19,1%, insulin-dependent DM 3,8%). Diabetic patients showed a higher risk profile, particularly those with ID-DM. At the average follow-up, the adjusted risk of MACE was similar between Non-DM patients and the NIR-DM patients (HR 1,02 [0,81-1,27], p 0.85). In relation to DM-IR patients, it was observed a higher risk of MACE in comparison to Non-DM (HR 1,73 [1,20-2,49], p 0.003) and NIR-DM (HR 1,65 [1,10-2,48], p 0.015). A significant interaction was observed between the diabetic status and the risk of MACE according to the indication of the percutaneous coronary artery intervention (pint 0.045). Conclusions: In our registry of patients undergoing PCI, with long-term follow-up, DM patients had a higher risk of MACE. The risk of MACE was particularly increased in ID-DM patients. However, there were no significant differences in the risk of MACE between DM-NIR and non-DM patients.

BACKGROUND: A large percentage of patients with non-ST-segment acute coronary syndrome (NSTE-ACS) present with acute total occlusion (TO) of some major epicardial vessel that does not generate electrocardiographic changes. Ongoing research into the methods of accurately predicting acute TO have not yielded great success. HYPOTHESIS: High-sensitivity cardiac troponin T (hs-cTnT) has a good predictive value for the presence of acute TO of the culprit artery in patients with NSTE-ACS. METHODS: A single-center retrospective study of 1011 patients diagnosed with NSTE-ACS who underwent coronary angiography and hs-cTnT measured on admission. The predictive value of hs-cTnT in the presence of acute TO was assessed by the area under the ROC curve. RESULTS: The mean age of the population was 67.12 ± 13.18 and 74.1% were male. 7.3% of the patients presented with acute TO. The AUC for hs-cTnT to predict acute TO was 0.95. A hs-cTnT value of 1006 ng/L (71.8 fold of the URL) best predicted the presence of acute TO, with a sensitivity of 86% and specificity of 95% positive predictive value (PPV): 86% and negative predictive value (NPV): 94%. CONCLUSIONS: Hs-cTnT was a good predictor of acute TO in patients with NSTE-ACS. Hs-cTnT values greater than 1006 ng/L were highly predictive of acute TO of a major coronary vessel.

Apocal hypertrophic cardiomyopathy (AHCM) is a phenotypic variant within hypertrophic cardiomyopathies, in which ventricular repolarization alterations are present. These electrocardiographic disturbances can mimic an anterior infarction which triggers a series of studies and treatments that may be unnecessary. The aim of this study was to describe and compare electrocardiographic differences in a series of patients with AHCM and apical non-ST segment elevation myocardial infarction in patients (NSTEMI) with T-wave changes. We conducted an observational and retrospective study, including patients with diagnosed AHCM (N = 19) and apical NSTEMI (N = 19) with negative T waves in V1 and V6 lead of the EKG. Those with AHCM presented higher T-wave voltage (7 mV vs. 5 mV, p = 0.001) and peak voltage (29 mV vs. 17 mV, p = 0.003), higher R-waves (25 mV vs. 10 mV, p = 0.0001), and a maximum voltage of R and T sum (R + T) significantly higher (33 vs. 14, p = 0.00001). They also showed a greater T-wave asymmetry, with a TiTp / TpTf ratio > 1. At a cut-off value of 26.5 mV for the R + T variable, 68% sensitivity and 100% specificity were obtained to diagnose AHCM. This study shows the existence of major differences in electrocardiographic presentation of AHCM and apical NSTEMI.

La miocardiopatía hipertrófica apical (MCHA) es una variante fenotípica dentro de las miocardiopatías hipertróficas que presenta alteraciones de la repolarización ventricular. Estos cambios electrocardiográficos pueden simular en muchos casos un infarto anterior, lo que dispara una serie de estudios y tratamientos que pueden ser innecesarios. El objetivo del trabajo fue describir y comparar las diferencias electrocardiográficas en una serie de pacientes con MCHA e infarto sin elevación del ST apicales (IAMSESTa) que presenten cambios tipo T en el electrocardiograma (ECG). Se realizó un estudio observacional y retrospectivo, incluyendo pacientes con diagnóstico de MCHA (n = 19) e IAMSESTa (n = 19) con ondas T negativas en ECG de ingreso en derivaciones V1-V6. Se excluyeron aquellos con MCHA y enfermedad coronaria asociada. Se analizaron las características clínicas y electrocardiográficas entre ambos grupos. Los pacientes con MCHA presentaron mayor voltaje de ondas T (7 mV vs. 5 mV; p = 0.001) y sumatoria de voltaje de las mismas (29 mV vs. 17 mV; p = 0.003), mayor voltaje de ondas R (25 mV vs. 10 mV; p = 0.0001), con una sumatoria de máximo voltaje de R y T (R+T) significativamente mayor (33 vs. 14; p = 0.00001). Presentaron además mayor asimetría de las ondas T negativas, objetivado mediante una relación TiTp/TpTf > 1. Con un valor de corte de 26.5 mV para la variable R+T, se obtuvo un 68% de sensibilidad y 100% de especificidad para diagnosticar MCHA. El presente trabajo demuestra la existencia de diferencias en el patrón del ECG en MCHA e IAMSESTa.

Introducción Para las fracturas de húmero diafisario de manejo quirúrgico, los abordajes por mínima invasión mejoran desenlaces. Se han descrito varios. Sin embargo, para el abordaje posterior no existe claridad entre la relación anatómica del nervio radial y las incisiones o la posición de la placa. El objetivo de este estudio es describir la relación anatómica del nervio radial tanto con las incisiones del abordaje posterior por mínima invasión como con la punta distal de la placa. Materiales y métodos se realizó un estudio descriptivo en cadáveres sin traumatismo de miembros superiores, en prono con 45° de abducción de hombro y 90° de flexión de codo, tras deslizar una placa de 2,7 mm. Se consignaron en milímetros las distancias del nervio radial respecto a los puntos de referencia del abordaje y la punta distal de la placa. Resultados Por término medio, la longitud humeral fue 286,6 mm; la distancia del epicóndilo lateral al nervio radial, 155,1 mm; la aponeurosis tricipital al nervio radial, 138,9 mm, y la punta distal de la placa al nervio radial, 155,6 mm. Discusión La fijación con placa por mínima invasión por abordaje posterior puede ser segura para las fracturas de húmero diafisario respecto a la lesión del nervio radial, al ubicar los tornillos de la placa por fuera de 128,5-169,5 mm medidos desde la punta de la placa aunque se requieren estudios clínicos para demostrar la seguridad de este abordaje. Background It is well known that the various minimally invasive approaches described improve outcomes for the surgical fixation of diaphyseal humerus fractures. However, there is a lack of information between the anatomical relationship of the radial nerve for the required incisions or for the position of the plate when a posterior approach is used. The objective of the study is to describe the anatomical relationship of the radial nerve with both incisions of the posterior minimally invasive approach, and with the distal tip of the osteosynthesis plate. Materials and methods A descriptive study was performed on cadavers without trauma of upper limbs, in prone with 45° of abduction of shoulder and 90° of elbow flexion. After sliding a plate of 2.7 mm, the distances of the radial nerve with respect to the reference points of the approach and distal tip of the plate were recorded in millimetres. Results A mean humeral length of 286.6 mm was found. The mean distance from the lateral epicondyle to the radial nerve was 155.1 mm. The mean distance from the tricipital aponeurosis to the radial nerve was 138.9 mm, and from the distal tip of the plate to the radial nerve was 155.6 mm. Discussion Plate fixation using minimal invasive technique using a posterior surgical approach may be safe for diaphyseal fractures of the humerus with respect to radial nerve injuries, as long as the plate screws are located outside the range of 128.5 mm to 169.5 mm measured from the tip of the plate. Clinical studies are required to demonstrate the safety of this approach. Evidence level IV.

RESUMO

La hemiartroplastia de cadera constituye una opción terapéutica en el manejo de las fracturas intracapsulares de cadera en pacientes ancianos, especialmente en aquellos con comorbilidades y poca demanda funcional. Se trata de un procedimiento que, como cualquier otro, no está exento de riesgos tales como el sangrado, la infección, la luxación, el protrusio acetabular y, menos frecuente, la ruptura del implante. De esta complicación existen pocos casos reportados en la literatura. En este artículo se reportan dos casos de pacientes tratados en el Hospital San Vicente de Paúl con falla de hemiartroplastia por ruptura de tallo femoral y se presenta una revisión de la literatura pertinente sobre el tema.
