RESUMO

Despite tremendous progress in cell-based therapies for heart repair, many challenges still exist. To enhance the therapeutic potential of cell therapy one approach is the combination of cells with biomaterial delivery vehicles. Here, we developed a biomimetic and biodegradable micro-platform based on polymeric microparticles (MPs) capable of maximizing the therapeutic potential of cardiac progenitor cells (CPCs) and explored its efficacy in a rat model of chronic myocardial infarction. The transplantation of CPCs adhered to MPs within the infarcted myocardial microenvironment improved the long-term engraftment of transplanted cells for up to one month. Furthermore, the enhancement of cardiac cellular retention correlated with an increase in functional recovery. In consonance, better tissue remodeling and vasculogenesis were observed in the animals treated with cells attached to MPs, which presented smaller infarct size, thicker right ventricular free wall, fewer deposition of periostin and greater density of vessels than animals treated with CPCs alone. Finally, we were able to show that part of this beneficial effect was mediated by CPC-derived extracellular vesicles (EVs). Taken together, these findings indicate that the biomimetic microcarriers support stem cell survival and increase cardiac function in chronic myocardial infarction through modulation of cardiac remodeling, vasculogenesis and CPCs-EVs mediated therapeutic effects. The biomimetic microcarriers provide a solution for biomaterial-assisted CPC delivery to the heart. STATEMENT OF SIGNIFICANCE: In this study, we evaluate the possibility of using a biomimetic and biodegradable micro-platform to improve cardiovascular progenitor therapy. The strategy reported herein serves as an injectable scaffold for adherent cells due to their excellent injectability through cardiac catheters, capacity for biomimetic three-dimensional stem cell support and controllable biodegradability. In a rat model of chronic myocardial infarction, the biomimetic microcarriers improved cardiac function, reduced chronic cardiac remodeling and increased vasculogenesis through the paracrine signaling of CPCs. We have also shown that extracellular vesicles derived from CPCs cultured on biomimetic substrates display antifibrotic effects, playing an important role in the therapeutic effects of our tissue-engineered approach. Therefore, biomimetic microcarriers represent a promising and effective strategy for biomaterial-assisted CPC delivery to the heart.

We combine broadband dielectric spectroscopy (BDS) with 1H and 2H nuclear magnetic resonance (NMR) to study molecular dynamics in mixtures of ε-polylysine with H2O or D2O. In BDS, four relaxation processes can be attributed to molecular dynamics. While the fastest process P1 obeys the Arrhenius law, the slowest process P4 shows prominent non-Arrhenius behavior typical of structural α relaxation. For the intermediate processes P2 and P3, the temperature dependence changes at the glass transition temperature Tg. The 1H and 2H NMR results yield insights into the molecular origins of these relaxation phenomena. In these NMR analyses, we exploit, in addition to the isotope selectivity of the method, the possibility to distinguish between various types of motion based on their respective line-shape effects and the capability to single out specific molecular moieties based on different spin-lattice relaxation behaviors. In this way, we reveal that process P1 results from the rotation of side and end groups of the peptide, while process P2 is caused by a reorientation of essentially all water molecules, which are quasi-isotropic and survive well below Tg. As for the peptide backbone dynamics, we find evidence that rotational motion of polar groups is involved in process P3 and that nonpolar regions show a dynamical process, which is located between P3 and P4. Thus, the NMR analyses do not yield evidence for coexisting fast peptide-decoupled and slow peptide-coupled water species, which contribute to BDS processes P2 and P3, respectively, but minor bimodality of water motion may remain undetected. Finally, it is demonstrated that the proton/deuteron exchange needs to be considered when interpreting experimental results for molecular dynamics in aqueous peptide solutions.

Presentamos un caso de una paciente de 3 años de edad con diagnóstico de retinoblastoma en el ojo izquierdo. El curso de la enfermedad obligó a la enucleación de dicho ojo e implantación de una prótesis. Dos años tras la cirugía, se produjo una extrusión parcial de la prótesis en la cavidad anoftálmica y se decidió emplear un sustituto semisintético de duramadre para cubrir el defecto. El uso de un sustituto semisintético de duramadre en este escenario específico no ha sido comunicado previamente

In glass-forming substances, the addition of water tends to produce the effect of lowering the glass transition temperature, Tg. In a previous work by some of us (Ruiz et al., Sci. Rep., 2017, 7, 7470) we reported on a rare anti-plasticizing effect of water on the molecular dynamics of a simple molecular system, the pharmaceutically active prilocaine molecule, for which the addition of water leads to an increase of Tg. In the present work, we study pure and hydrated prilocaine confined in 0.5 nm and 1 nm pore size molecular sieves, and carry out a comparison with the bulk compounds in order to gain a better understanding of the microscopic mechanisms that result in this rare effect. We find that the Tg of the drug under nanometric confinement can be lower than the bulk value by as much as 17 K. Through the concurrent use of differential scanning calorimetry and broadband dielectric spectroscopy we are able to observe the antiplasticizing effect of water in prilocaine also under nanometric confinement, finding an increase of Tg of up to almost 6 K upon hydration. The extension of our analysis to nanoconfined systems provides a plausible explanation for the very uncommon antiplasticizing effect, based on the formation of water-prilocaine molecular complexes. Moreover, this study deepens the understanding of the behavior of drugs under confinement, which is of relevance not only from a fundamental point of view, but also for practical applications such as drug delivery.

Objetivo: Realizar un análisis cualitativo de las ampollas de filtración que han sido reparadas quirúrgicamente por presentar Seidel tardío. Métodos: Estudio de 10 ampollas de filtración que requirieron reparación quirúrgica mediante la OCT-SA Triton (Topcon(R)). Se analizó, a los 6 meses de la cirugía, el patrón morfológico y las estructuras internas de las mismas, así como el estado del recubrimiento. La obtención de las imágenes fue mediante longitudes de onda de 1.050 nm. Resultados: Según la clasificación de Hirooka, encontramos 3 patrones diferentes en la morfología de la ampolla que pudimos relacionar con la funcionalidad de la misma. En un 70% de los casos el recubrimiento fue completo, presentando quistes subepiteliales difusos (tipo quístico). Dos casos mostraron una retracción conjuntival completa, sin cobertura por Tenon. Las paredes estaban adelgazadas, mostrando una desestructuración de la ampolla (patrón difuso). En un tercer grupo, la imagen obtenida mostraba una retracción conjuntival parcial con cobertura por Tenon. Presentaba algún quiste subepitelial difuso y con paredes siguiendo un patrón laminar. Conclusión: Mediante la OCT-SA es posible estudiar de forma detallada las características de la ampolla y las de su cobertura en el caso de reparación con avance conjuntival por Seidel tardío. Permite visualizar precozmente la retracción de la conjuntiva que en la lámpara de hendidura no sería visible y predecir mediante la morfología de la ampolla la funcionalidad de la misma (AU)


Objective: To provide a qualitative analysis of filtering blebs after being surgically repaired due to late blebs leaks. Methods: Blebs were studied 6 months after surgical reparation using AS-OCT Triton (Topcon(R)). An analysis was made of the morphological pattern and internal structures of blebs, including the covering, in 10 patients. The images were obtained using OCTs at a wavelength of 1050 nm. Results: According to the Hirooka classification, three different patterns were found in the structure of blebs, which made it possible to correlate them with their functionality. A full covering was observed in 70% of the cases, and they showed sub-epithelial cysts (cystoid pattern). Two cases showed a full conjunctival retraction without Tenon's covering. The walls were thin, with a de-structured bleb (diffuse pattern) being visualised. In the third group, the image showed a partial conjunctival retraction with Tenon's covering. There were some sub-epithelial diffuse cysts with walls following a laminar pattern. Conclusion: Using AS-OCT, it is possible to study the bleb's characteristics in detail, as well as the cover, in the case of blebs requiring repair due to late leaks, using conjunctival advancement. It allows for the early visualisation of conjunctival retractions that were not visible in a slit lamp, and to predict the functionality of the blebs by their morphology (AU)

Objetivo: Describir las características de la neovascularización coroidea (NVC) tipo 1 en pacientes con degeneración macular asociada a la edad (DMAE), utilizando la angiografía por tomografía de coherencia óptica (A-OCT) secuencialmente durante el transcurso de un protocolo estándar de 3 inyecciones intravítreas de fármaco anti-VEGF. Métodos: Seis ojos con DMAE neovascular no tratados previamente fueron incluidos. Se obtuvieron imágenes por A-OCT empleando AngioPlex Cirrus HD-OCT 5000 (Carl Zeiss Meditec, Inc., Dublin, EE. UU.) y DRI OCT Triton SS-OCT Angio (Topcon, Medical Systems, Inc. Oakland, NJ, EE. UU.). El área estudiada comprende un escáner macular de 3×3mm. Diferentes patrones morfológicos y aspectos cuantificables de las membranas neovasculares han sido evaluados con imágenes en proyección en face, que fueron tomadas en distintos tiempos del seguimiento de los pacientes. Resultados: El grado de respuesta al tratamiento fue estimado empleando criterios de actividad de NVC para A-OCT. Puntuaciones más altas en los ítems de actividad antes del tratamiento resultaron en mayores reducciones del área de las membranas. Los resultados finales de reducción de área oscilaron entre el 83,5 y el 1,4%. Las A-OCT realizadas a la semana de tratamiento revelaron los mayores porcentajes de reducción. Conclusiones: La A-OCT ofrece la posibilidad de analizar en profundidad las características morfológicas y estructurales en NVC de tipo 1. Los criterios de actividad permiten guiar decisiones terapéuticas y evaluar la respuesta al tratamiento. Con esta técnica puede obtenerse información útil tanto cualitativa como cuantitativa. Sin embargo, son necesarios avances en el desarrollo del software y en investigación para poder definir el papel de esta herramienta en la práctica diaria (AU)

The estimated net decline in area ranged from 83.5% to 1.4%. The OCT-A performed one-week after treatment revealed the greatest area reductions. Conclusions: OCT-A provides new possibilities for the non-invasive assessment of features of neovascular networks and CNV structural morphology. Newly described activity criteria can also guide therapeutic decisions, and help in evaluating responses. Quantitative and qualitative information can be provided with this technique. However, further software development and future investigation are essential to define the role of this tool on a daily basis (AU)

Caso clínico: Niño de 8 años, sin enfermedades, con pérdida brusca de la agudeza visual (AV) en el ojo izquierdo (OI). Exploración: AV 1 en el OD y 0,1 en el OI, discreto DPAR izquierdo. Biomicroscopía normal. Fundoscopía: papila congestiva, tortuosidad venosa, hemorragias peripapilares con edema macular en el OI. El estudio sistémico únicamente reveló polimorfismo C46T del gen F12 de la coagulación. Ocho meses después: AV de 1 con fundoscopía normal. Discusión: La papiloflebitis es una oclusión de la vena central de la retina (VCR) inflamatoria no isquémica, oftalmoscópicamente semejante a las trombosis de VCR. El estudio sistémico es fundamental para descartar enfermedades subyacentes (AU)

Discussion: Papillophlebitis is an inflammatory and non-ischaemic central retinal vein occlusion, ophthalmoscopically similar to central retinal vein thrombosis. The systemic study is essential to rule out underlying diseases (AU)

OBJECTIVES: This case study describes the implementation of an academic institution's disaster management plan. STUDY DESIGN: Case study. METHODS: USFQ's Medical School developed a six-phase disaster relief plan consisting of: induction, establishing a base camp, crisis management and mental health aid, creation of multidisciplinary teams and multi-agency teams, and reconstruction. Each phase uses a community-oriented approach to foster survivor autonomy and recovery. RESULTS: Our methodology facilitated the successful implementation of multidisciplinary interventions to manage the earthquake's aftermath on the personal, community and regional levels, treated and prevented psychological and physical morbidity among survivors and promoted healthy living conditions and independence. CONCLUSIONS: A multidisciplinary response team that addresses medical needs, mental health, education, food, nutrition and sanitation is highly effective in contributing to timely, effective relief efforts. The short- and long-term solutions we describe could be applicable to other academic centres' interventions in future disaster scenarios around the world.

Airborne bio-aerosol content (mainly pollen and spores) depends on the surrounding vegetation and weather conditions, particularly wind direction. In order to understand this issue, maps of the main land cover in influence areas of 10km in radius surrounding pollen traps were created. Atmospheric content of the most abundant 14 pollen types was analysed in relation to the predominant wind directions measured in three localities of SW of Iberian Peninsula, from March 2011 to March 2014. Three Hirst type traps were used for aerobiological monitoring. The surface area for each land cover category was calculated and wind direction analysis was approached by using circular statistics. This method could be helpful for estimating the potential risk of exposure to various pollen types. Thus, the main land cover was different for each monitoring location, being irrigated crops, pastures and hardwood forests the main categories among 11 types described. Comparison of the pollen content with the predominant winds and land cover shows that the atmospheric pollen concentration is related to some source areas identified in the inventory. The study found that some pollen types (e.g. Plantago, Fraxinus-Phillyrea, Alnus) come from local sources but other pollen types (e.g. Quercus) are mostly coming from longer distances. As main conclusions, airborne particle concentrations can be effectively split by addressing wind with circular statistics. By combining circular statistics and GIS method with aerobiological data, we have created a useful tool for understanding pollen origin. Some pollen loads can be explained by immediate surrounding landscape and observed wind patterns for most of the time. However, other factors like medium or long-distance transport or even pollen trap location within a city, may occasionally affect the pollen load recorded using an air sampler.

The systemic study is essential to rule out underlying diseases.

Las intoxicaciones son susceptibles de cambios a lo largo del tiempo, en parte debido a la propia evolución de la población atendida. El objetivo de este trabajo es analizar las diferencias existentes en dos periodos de tiempo en las intoxicaciones atendidas en el Servicio de Urgencias de un hospital Universitario durante los años 2003-04 y 2009-10. Los datos se obtuvieron a través de la revisión de las historias clínicas. Se produjeron un total de 1017 intoxicaciones en el periodo 2003-04 y 989 en el periodo 2009-10. Con respecto al primer periodo, en 2009-10 se observó un aumento en la edad media (30,9 (11) vs 33,4 (13); p < 0,001) de los pacientes intoxicados, así como en el porcentaje de varones (65,6% vs 71,5%; p = 0,004) y de inmigrantes (24,0% vs 42,7%; p<0,001), sobre todo procedentes de Europa del Este (3,5% vs 18,2%; p<0,001). Asimismo, se produjo un descenso en las intoxicaciones producidas en lugares públicos (83,4% vs 75,8%; p<0,001), aumentó la intoxicación por alcohol (39,0% vs 57,9%) y disminuyeron las intoxicaciones debidas a drogas de abuso (60,8 % vs 35,6%) (AU)


Poisoning features are susceptible to changes over time, in part due to the evolution of the population attended. The objective of this study is to analyze the differences in the characteristics of poisonings treated in the Emergency Department of a university hospital between two periods. The data were obtained reviewing the medical records of 2003-04 and 2009-10 years. A total of 1017 and 989 poisonings were attended in 2003-04 and 2009-10 periods, respectively. We observed an increase in the mean age of the poisoned patients [30.9 (11) vs 33.4 (13); p <0.001] and in the percentage of males (65.6% vs 71.5%; p = 0.004) in 2009- 10 period. Moreover, an increase in immigrant population attended was observed (42.7% in 2009-2010 vs 24.0% in 2003-2004; p <0.001), especially from Eastern Europe (18.2% vs 3.5%; p <0.001). A decrease in poisonings produced in public places (83.4% vs 75.8%; p <0.001) was observed too. Regarding type of intoxication, in the second period alcohol intoxications were higher (39.0% vs 57.9%) while intoxications due to drugs of abuse decreased (60.8% vs 35.6%) (AU)

El objetivo del presente trabajo ha sido llevar a cabo un análisis de las urgencias atendidas en un hospital urbano de tercer nivel que se han relacionado con el consumo de cocaína. Para ello se ha realizado un estudio retrospectivo, con exclusión del área de pediatría y ginecología, durante los años 2005 y 2006. Se revisaron todas las asistencias clínicas que consultaron por intoxicación por cocaína. Los datos fueron analizados con el paquete estadístico SPSS 15.0 para windows. Durante el periodo de estudio se produjeron 1.531 intoxicaciones (1,2% del total de las urgencias). De éstas, 327 eran por cocaína (21,4% de las intoxicaciones). La edad media fue de 31 (8) años y la moda de 26 años. Un 76,5% eran varones. El 69,1% ingresaron en ambulancia. La mayor afluencia se produjo en fin de semana (46,8%) y en horario nocturno (54,1%). El 53,2% eran consumidores habituales y el 25,4% esporádicos. Un 55,7% asociaron más de 2 drogas: el 53,5% alcohol, el 31,2% opiáceos, el 17.4% benzodiacepinas, el 17,1% cannabis, el 11,3% GHB y el resto otras drogas. El 77,1% consumieron en un lugar público. Tenían antecedentes de intoxicaciones previas un 59,3%. El 93% presentaban síntomatología: (52,6% neurológicos, 48% conductuales, 28,4% cardiovasculares, 19% respiratorios y 8,3% digestivos). Se administraron antídotos a un 20,5%. El destino final fue alta en las primeras 12 horas en el 60,9%, con solo un 5,7% de ingresos. Se realizó valoración psiquiátrica en el 21,1%. En conclusión, el perfil del usuario que consulta por intoxicación por cocaína es un varón de 26 años traído en ambulancia de un lugar público, en fin de semana, en horario nocturno, consumidor habitual, con sintomatología cardiológica y neurológica y dado de alta en menos de 12 horas. La intoxicación aguda por cocaína no es pura. Destaca la asociación de alcohol, opiáceos y benzodiacepinas. Menos de una cuarta parte fueron valorados por Psiquiatría (AU)


The main of this study was to analyze all patients with cocaine intoxication attended in the emergency department of a tertiary-care hospital. For this, a retrospective study for the years 2005 and 2006, excluding peadiatric and gynecological areas, has been carried out. All cases in which the main reason for seeking urgent medical care was cocaine intoxication were reviewed. Data were analyzed with the SPSS statistical package (version 15.0) for Windows. During the study period, a total of 1531 subjects with drug of abuse intoxication were attended, which accounted for 1.2% of all patients attended in the emergency department. Cocaine was the drug of abuse responsible for the intoxication in 327 cases (21.4% of all intoxications). The mean (SD) age of the patients was 31 (8) years (mode 26 years). A total of 76.5% were men, and 69.1% arrived to the emergency department in ambulance. A large percentage of subjects were attended on weekends (46.8%) and during the night shift (54.1%). A total of 53.2% were regular cocaine consumers and 25.4% consumed the drug sporadically. In 55.7% of the cases, more than two drugs were concomitantly consumed, including alcohol in 53.5% of cases, opioids in 31.2%, benzodiazepines in 17.4%, cannabis in 17.1%, gamma hydroxybutyric acid in 11.3%, and other drugs in the remaining cases. Drugs were consumed in a public place in 77.1% of cases. History of previous episodes of intoxication was recorded in 59.3% of cases. Symptoms were present in 93% of subjects (neurological in 52.6%, behavioural in 48%, cardiovascular in 28.4%, respiratory in 19%, and gastrointestinal in 8.3%). Antidotes were administered in 20.5% of cases. A total of 60.9% of subjects were discharged from the emergency department within the first 12 hours and only 5.7% were admitted to the hospital. Twenty-one percent of subjects underwent psychiatric assessment. In conclusion, the profile of a subject with cocaine intoxication attended in the emergency department is a man of 26 years of age, transferred in ambulance from a public place, on weekends and at night, regular cocaine consumer, presenting with neurological and cardiological symptoms, and discharged from the emergency department in less than 12 hours after admission. Cocaine intoxication occurred frequently in association with alcohol, opioids, and benzodiazepine use. Less than one fourth of subjects underwent psychiatric assessment (AU)
