RESUMO

The clinical effects of a serious game with electromyography feedback (EMGs_SG) and physical therapy (PT) was investigated prospectively in children with unilateral spastic cerebral palsy (USCP). An additional aim was to better understand the influence of muscle shortening on function. Thirty children with USCP (age 7.6 ± 2.1 years) received four weeks of EMGs_SG sessions 2×/week including repetitive, active alternating training of dorsi- and plantar flexors in a seated position. In addition, each child received usual PT treatment ≤ 2×/week, involving plantar flexor stretching and command strengthening on dorsi- and plantar flexors. Five-Step Assessment parameters, including preferred gait velocity (normalized by height); plantar flexor extensibility (XV1); angle of catch (XV3); maximal active ankle dorsiflexion (XA); and derived coefficients of shortening, spasticity, and weakness for both soleus and gastrosoleus complex (GSC) were compared pre and post treatment (t-tests). Correlations were explored between the various coefficients and gait velocities at baseline. After four weeks of EMGs_SG + PT, there was an increase in normalized gait velocity from 0.72 ± 0.13 to 0.77 ± 0.13 m/s (p = 0.025, d = 0.43), a decrease in coefficients of shortening (soleus, 0.10 ± 0.07 pre vs. 0.07 ± 0.08 post, p = 0.004, d = 0.57; GSC 0.16 ± 0.08 vs. 0.13 ± 0.08, p = 0.003, d = 0.58), spasticity (soleus 0.14 ± 0.06 vs. 0.12 ± 0.07, p = 0.02, d = 0.46), and weakness (soleus 0.14 ± 0.07 vs. 0.11 ± 0.07, p = 0.005, d = 0.55). At baseline, normalized gait velocity correlated with the coefficient of GSC shortening (R = -0.43, p = 0.02). Four weeks of EMGs_SG and PT were associated with improved gait velocity and decreased plantar flexor shortening. A randomized controlled trial comparing EMGs_SG and conventional PT is needed.

Syphilis is an infectious disease that can be diagnosed and treated cheaply. Despite being a curable condition, the syphilis rate is increasing worldwide. In this sense, computational methods can analyze data and assist managers in formulating new public policies for preventing and controlling sexually transmitted infections (STIs). Computational techniques can integrate knowledge from experiences and, through an inference mechanism, apply conditions to a database that seeks to explain data behavior. This systematic review analyzed studies that use computational methods to establish or improve syphilis-related aspects. Our review shows the usefulness of computational tools to promote the overall understanding of syphilis, a global problem, to guide public policy and practice, to target better public health interventions such as surveillance and prevention, health service delivery, and the optimal use of diagnostic tools. The review was conducted according to PRISMA 2020 Statement and used several quality criteria to include studies. The publications chosen to compose this review were gathered from Science Direct, Web of Science, Springer, Scopus, ACM Digital Library, and PubMed databases. Then, studies published between 2015 and 2022 were selected. The review identified 1,991 studies. After applying inclusion, exclusion, and study quality assessment criteria, 26 primary studies were included in the final analysis. The results show different computational approaches, including countless Machine Learning algorithmic models, and three sub-areas of application in the context of syphilis: surveillance (61.54%), diagnosis (34.62%), and health policy evaluation (3.85%). These computational approaches are promising and capable of being tools to support syphilis control and surveillance actions.

BACKGROUND: maladaptive changes in the autonomic nervous system (ANS) have been observed in short and long-term phases of COVID-19 infection. Identifying effective treatments to modulate autonomic imbalance could be a strategy for preventing and reducing disease severity and induced complications. OBJECTIVE: to investigate the efficacy, safety, and feasibility of a single session of bihemispheric prefrontal tDCS on indicators of cardiac autonomic regulation and mood of COVID-19 inpatients. METHODS: patients were randomized to receive a single 30-min session of bihemispheric active tDCS over the dorsolateral prefrontal cortex (2 mA; n = 20) or sham (n = 20). Changes in time [post-pre intervention] in heart rate variability (HRV), mood, heart rate, respiratory rate, and oxygen saturation were compared between groups. Additionally, clinical worsening indicators and the occurrence of falls and skin injuries were evaluated. The Brunoni Adverse Effects Questionary was employed after the intervention. RESULTS: there was a large effect size (Hedges' g = 0.7) of intervention on HRV frequency parameters, suggesting alterations in cardiac autonomic regulation. An increment in oxygen saturation was observed in the active group but not in the sham after the intervention (P = 0.045). There were no group differences regarding mood, incidence and intensity of adverse effects, no occurrence of skin lesions, falls, or clinical worsening. CONCLUSIONS: a single prefrontal tDCS session is safe and feasible to modulate indicators of cardiac autonomic regulation in acute COVID-19 inpatients. Further research comprising a thorough assessment of autonomic function and inflammatory biomarkers is required to verify its potential to manage autonomic dysfunctions, mitigate inflammatory responses and enhance clinical outcomes.

The improvement of laboratory diagnosis is a critical step for the reduction of syphilis cases around the world. In this paper, we present the development of an impedance-based method for detecting T. pallidum antigens and antibodies as an auxiliary tool for syphilis laboratory diagnosis. We evaluate the voltammetric signal obtained after incubation in carbon or gold nanoparticle-modified carbon electrodes in the presence or absence of Poly-L-Lysine. Our results indicate that the signal obtained from the electrodes was sufficient to distinguish between infected and non-infected samples immediately (T0') or 15 min (T15') after incubation, indicating its potential use as a point-of-care method as a screening strategy.

Biofeedback based on electromyograms (EMGs) has been recently proposed to reduce exaggerated postural activity. Whether the effect of EMG biofeedback on the targeted muscles generalizes to - or is compensated by - other muscles is still an open question we address here. Fourteen young individuals were tested in three 60 s standing trials, without and with EMG-audio feedback: (i) collectively from soleus and medial gastrocnemius and (ii) from medial gastrocnemii. The Root Mean Square (RMS) of bipolar EMGs sampled from postural muscles bilaterally was computed to assess the degree of activity and postural sway was assessed from the center of pressure (CoP). In relation to standing at naturally, EMG-audio feedback from soleus and medial gastrocnemii decreased plantar flexors' activity (∼10 %) but at the cost of increased amplitude of tibialis anterior (∼5%) and vasti muscles (∼20 %) accompanied by a posterior shift of the mean CoP position. However, EMG-audio feedback from medial gastrocnemii reduced only plantar flexors' activity (∼5%) when compared to standing at naturally. Current results suggest the EMG biofeedback has the potential to reduce calf muscles' activity without loading other postural muscles especially when using medial gastrocnemii as feedback source, with implications on postural training aimed at assisting individuals in activating more efficiently postural muscles during standing.

Since the COVID-19 pandemic emerged, vaccination has been the core strategy to mitigate the spread of SARS-CoV-2 in humans. This paper analyzes the impact of COVID-19 vaccination on hospitalizations and deaths in the state of Rio Grande do Norte, Brazil. We analyzed data from 23,516 hospitalized COVID-19 patients diagnosed between April 2020 and August 2021. We excluded the data from patients hospitalized through direct occupancy, unknown outcomes, and unconfirmed COVID-19 cases, resulting in data from 12,635 patients cross-referenced with the immunization status during hospitalization. Our results indicated that administering at least one dose of the immunizers was sufficient to significantly reduce the occurrence of moderate and severe COVID-19 cases among patients under 59 years. Considering the partially or fully immunized patients, the mean age is similar between the analyzed groups, despite the occurrence of comorbidities and higher than that observed among not immunized patients. Thus, immunized patients present lower Unified Score for Prioritization (USP) levels when diagnosed with COVID-19. Our data suggest that COVID-19 vaccination significantly reduced the hospitalization and death of elderly patients (60+ years) after administration of at least one dose. Comorbidities do not change the mean age of moderate/severe COVID-19 cases and the days required for the hospitalization of these patients.

(1) Background: Maternal syphilis (MS) and congenital syphilis (CS) are serious public health problems worldwide due to their high morbidity and mortality rates. (2) Objective: Evaluating the applicability of Clinical Protocols and Treatment Guidelines on case incidence trends in Brazil and Portugal. (3) Methods: The review was done through bibliographic research in two public databases and government websites from both countries, published between 2007 and 2022. All guidelines that contained CS and MS were selected. (4) Results and discussion: After evaluation, we found that Brazil and Portugal have adequate protocols for screening and treating congenital and maternal syphilis. (5) Conclusion: The results suggest that CS and MS incidence are notably higher in Brazil than in Portugal due to economic, cultural, and social disparities and the differences in territory size. Therefore, these demographic and socioeconomic factors could strongly influence efforts to fight against syphilis and thus control the infection.

INTRODUCTION: Syphilis is a sexually transmitted disease (STD) caused by Treponema pallidum subspecies pallidum. In 2016, it was declared an epidemic in Brazil due to its high morbidity and mortality rates, mainly in cases of maternal syphilis (MS) and congenital syphilis (CS) with unfavorable outcomes. This paper aimed to mathematically describe the relationship between MS and CS cases reported in Brazil over the interval from 2010 to 2020, considering the likelihood of diagnosis and effective and timely maternal treatment during prenatal care, thus supporting the decision-making and coordination of syphilis response efforts. METHODS: The model used in this paper was based on stochastic Petri net (SPN) theory. Three different regressions, including linear, polynomial, and logistic regression, were used to obtain the weights of an SPN model. To validate the model, we ran 100 independent simulations for each probability of an untreated MS case leading to CS case (PUMLC) and performed a statistical t-test to reinforce the results reported herein. RESULTS: According to our analysis, the model for predicting congenital syphilis cases consistently achieved an average accuracy of 93% or more for all tested probabilities of an untreated MS case leading to CS case. CONCLUSIONS: The SPN approach proved to be suitable for explaining the Notifiable Diseases Information System (SINAN) dataset using the range of 75-95% for the probability of an untreated MS case leading to a CS case (PUMLC). In addition, the model's predictive power can help plan actions to fight against the disease.

Prosopis juliflora is an invasive plant distributed throughout the world and presents metabolites of interest for cosmetology. The aim of this work was to develop a new polysaccharide-based ingredient from P. juliflora and analyze its application in a solid core formulation that upon contact with water instantly forms a gel to improve moisturizing and anti-aging skin properties. Purified extracts by gel chromatography were characterized by NMR and LC-DAD-MS-MS. The in vitro and in vivo safety, antioxidant activity, formulation development and clinical evaluation were performed. The extract was characterized as containing an α-glucan and phenolics. It was non-cytotoxic, non-phototoxic and no skin reactions were observed in vivo. Antioxidant activity were present through different mechanisms. Clinical evaluation reinforced the potential of P. juliflora in skin hydration and microrelief improvement. This innovative form proved to be a prototype of a new product and the first study of an α-glucan as a cosmetic ingredient.

Although some studies have reported significant changes in autonomic responses according to the perspective-taking during motor imagery [first person perspective (1P) and third person perspective (3P)], investigations on how the strategies adopted to mentally simulate a given movement affect the heart rate variability (HRV) seem so far unexplored. Twenty healthy subjects mentally simulated the movement of middle-finger extension in 1P and 3P, while electrocardiogram was recorded. After each task, the level of easiness was self-reported. Motor imagery ability was also assessed through the revised version of Movement Imagery Questionnaire (MIQ-R) and a mental chronometry index. The traditional measures of HRV in the time- and frequency-domain were compared between 1P and 3P tasks by using Student's t-test for dependent samples. The MIQ-R results showed that subjects had the same facility to imagine movements in 1P or 3P. The mental chronometry index revealed a similar temporal course only between 1P and execution, while the 3P strategy had a shorter duration. Additionally, the subjective report was similar between the experimental tasks. Regarding the HRV measures, the low frequency component, in log-transformed unit, was significantly higher (p=0.017) in 1P than 3P, suggesting a higher activity of the sympathetic system during 1P. This log-transformed HRV parameter seems to be more sensitive than normalized values for the assessment of the motor imagery ability, together with questionnaires, scales and mental chronometry.

During standing, age-related differences in the activation of ankle muscles have been reported from surface electromyograms (EMGs) sampled locally. Given though activity seems to distribute unevenly within ankle muscles, the local sampling of surface EMGs may provide a biased view on how often and how much elderly and young individuals activate these muscles during standing. This study aimed therefore at sampling EMGs from multiple regions of individual ankle muscles to evaluate whether the distribution of muscle activity differs between aged and young subjects during standing. Thirteen young and eleven aged, healthy subjects were tested. Surface EMGs were sampled at multiple skin locations from tibialis anterior, soleus and medial and lateral gastrocnemius muscles while subjects stood at ease. The root mean square amplitude of EMGs was considered to estimate the duration, the degree of activity and the size of the region where muscle activity was detected. Our main findings revealed the medial gastrocnemius was active for longer periods in aged (interquartile interval; 74.1-98.2%) than young (44.9-81.9%) individuals (P = 0.02). Similarly, while tibialis anterior was rarely active in young (0.7-4.4%), in elderly subjects (2.6-82.5%) it was often recruited (P = 0.01). Moreover, EMGs with relatively higher amplitude were detected over a significantly wider proximo-distal region of medial gastrocnemius in aged (29.4-45.6%) than young (20.1-31.3%) subjects (P = 0.04). These results indicate the duration and the size of active muscle volume, as quantified from the spatial distribution of surface EMGs, may discriminate aged from young individuals during standing; elderlies seem to rely more heavily on the active loading of ankle muscles to control their standing posture than young individuals. Most importantly, current results suggest different conclusions on the active control of standing posture may be drawn depending on the skin location from where EMGs are collected, in particular for the medial gastrocnemius.

Climate change can be associated with variations in the frequency and intensity of extreme temperatures and precipitation events on the local and regional scales. Along coastal areas, flooding associated with increased occupation has seriously impacted products and services generated by marine life, in particular the biotechnological potential that macroalgae hold. Therefore, this paper analyzes the available information on the taxonomy, ecology and physiology of macroalgae and discusses the impacts of climate change and local stress on the biotechnological potential of Brazilian macroalgae. Based on data compiled from a series of floristic and ecological works, we note the disappearance in some Brazilian regions of major groups of biotechnological interest. In some cases, the introduction of exotic species has been documented, as well as expansion of the distribution range of economically important species. We also verify an increase in the similarities between the Brazilian phycogeographic provinces, although they still remain different. It is possible that these changes have resulted from the warming of South Atlantic water, as observed for its surface in southeastern Brazilian, mainly during the winter. However, unplanned urbanization of coastal areas can also produce similar biodiversity losses, which requires efforts to generate long-term temporal data on the composition, community structure and physiology of macroalgae.

Introdução: A distrofia muscular descreve um grupo de miopatias geneticamente determinadas com curso progressivo. A distrofia muscular de Duchenne (DMD) é a mais comum na infância, sendo uma desordem recessiva ligada ao cromossomo X. O presente estudo buscou avaliar a força muscular respiratória e o peak flow em pacientes com DMD submetidos à ventilação não invasiva (VNI) e hidroterapia. Metodologia: Participaram 6 voluntários do sexo masculino, entre 13 e 19 anos, divididos em dois grupos; controle (tratados com hidroterapia) e experimental (tratados com hidroterapia associada à VNI), os quaisforam avaliados antes e após a 10ª e a 20ª sessão. Resultados: Os resultados mostraram uma diferença significativa (p<0,05) quando comparamos a PEmáx entre os grupos controle e experimental após a 10ª (p=0,025) e a 20ª sessão (p=0,005). Conclusão: O estudo demonstrou que a VNI foi capaz de influenciar um aumento de sua expectativa de vida, segundo relato dospróprios pacientes, e que a hidroterapia foi uma terapêutica favorável na melhora da musculatura expiratória em pacientes com DMD e que não são portadores de escoliose.


Introduction: Muscular dystrophy describes a group of genetically determined myopathy with a progressive course. Duchennemuscular dystrophy (DMD) is the most frequent type in childhood and is a recessive disorder related to the X chromosome. The present study is an effort to evaluate the respiratory muscle force and the peak flow in patients with DMD exposed to non-invasive ventilation (NIV) and hydrotherapy. Methodology: Six male volunteers between the ages of 13 and 19 weredivided into two groups: control group (treated with hydrotherapy) and experimental group (PImáx < 60cmH2O, treated with hydrotherapy combined with NIV). The volunteers were analyzed after the 10th and 20th session. Results: Results show asignificant difference (p < 0.05) when comparing PEmáx in control and in experimental group after the 10th (p = 0.025) and 20th (p = 0.005) session. Conclusion: The study demonstrated that the NIV was able to promote an increase in the patients’ life expectancy, according to their own reports. It also showed that hydrotherapy was a suitable therapeutics for the improvement of the expiratory musculature in patients with DMD and not carriers of scoliosis.

Os distúrbios Osteomusculares Relacionados ao Trabalho (DORT) vêm crescendo substancialmente nas últimas décadas e constituem as mais importantes causas de ausência e incapacitação ao trabalho. O objetivo deste estudo consistiu-se em avaliar os DORT dos trabalhadores da FADE-UFPE. A amostra foi constituída por 60 indivíduos, que foram submetidos à avaliação fisioterapêutica e posteriormente preencheram um questionário para análise das queixas. Observou-se a organização estrutural do ambientte e realizou-se a análise ergonômica e postural durante o trabalho. Verificou-se que 92 dos sujeitos relataram uma ou mais queixas, sendo as mais freqüentes a dor na coluna (24), nos trapézios (22) e no punho (11). A maioria dos profissionais (34) relatou o início do aparecimento dos sintomas no trabalho e utilizavam para o alívio das queixas o alongamento e a mobilização (30) e a massagem (17). As condições que agravavam os sintomas estavam relacionadas à elevação dos membros (22), ao uso contínuo do teclado(25), do mouse (15) e escrita (14). As principais causas de aparecimento destes sintomas relacionavam-se ao movimento repetitivo exigido pela tarefa, a adoção de posturas globais e articulares inadequadas e a ausência de pausas distribuídas ao longo da jornada de trabalho.
