Objective. Threshold determination for improving training and sports performance is important for researchers and trainers, who currently use different methods for determining lactate, ventilatory or muscle oxygenation (SmO2) thresholds. Our study aimed to compare the identification of the intensity at the first and second thresholds using lactate and SmO2data by different mathematical methods in different muscles during a graded cycling test.Approach. Twenty-six cyclists (15 males and 11 females; 23 ±6 years, 1.71 ± 0.09 m, 64.3 ± 8.8 Kg and 12 ± 3 training hours per week) performed a graded test on the cycle ergometer. Power output and saturation of muscle oxygen in four muscles (vastus lateralis, biceps femoris, gastrocnemius and tibialis anterior) were measured, along with systemic lactate concentration.Main Results. Our results showed that any method was reliable for determining the first muscle oxygenation threshold (MOT1) when comparing the lactate threshold in any muscle. However, the best method for determining the second muscle oxygenation threshold (MOT2) was the Exp-Dmax (p< 0.01; ICC = 0.79-0.91) in all muscles. In particular, the vastus lateralis muscle showed the highest intraclass correlation coefficient (ICC = 0.91, CI95% [0.81, 0.96]). However, results varied per sex across all muscles analyzed.Significance. Although the first muscle oxygenation threshold could not be determined using mathematical methods in all the muscles analyzed, the Exp-Dmax method presented excellent results in detecting the second systemic threshold in the vastus lateralis.

Although in recent years near-infrared spectroscopy has been used in many sports to monitor muscle oxygen saturation (SmO2), there is a lack of knowledge about the sex differences in SmO2 during exercise in different muscles. Our study aimed to examine SmO2 differences in muscles between female and male cyclists, during a graded cycling test and at the first and second lactate thresholds. Twenty-five trained cyclists and triathletes (15 males: 23 ± 7 yr, 1.78 ± 0.05 m, 70.2 ± 5.3 kg, and 10 females: 22 ± 5 yr, 1.64 ± 0.06 m, 58 ± 8 kg) performed a graded cycling test on the cycle ergometer. Power output and SmO2 in five muscles (dominant vastus lateralis, tibialis anterior, gastrocnemius medial, biceps femoris, and triceps brachii) were measured. Our mixed regression models showed that the interaction between power output and sex was significant for all the muscles analyzed (P < 0.001), indicating a greater decrease in SmO2 for males as power output increased. Moreover, the statistical parametric mapping analyses showed for females higher SmO2 in the middle of the test in biceps femoris (P = 0.03), gastrocnemius medial (P = 0.02), and tibialis anterior (P = 0.04). Finally, the males presented a lower SmO2 in all muscles where the second lactate threshold occurred, with greater evidence than in the first lactate threshold. In conclusion, females have higher SmO2 in all muscles, and these differences are more noticeable during the graded cycling test, such that males seem to have a greater reliance on oxygen extraction than females for a given relative intensity of exercise.NEW & NOTEWORTHY This study investigated the profiles of muscle oxygen saturation (SmO2) during incremental exercise in females and males. Females presented higher overall SmO2 than males during moderate and heavy intensity domain exercise in all muscles including muscles that are not mainly involved in pedaling (triceps brachii), from those that are stabilizers (medial gastrocnemius, tibialis anterior, and biceps femoris), to those that are related to power output production (vastus lateralis).

Over the last few years, portable Near-Infrared Spectroscopy (NIRS) technology has been suggested for determining metabolic/ventilator thresholds. This systematic review and meta-analysis aimed to assess the reliability of a portable muscle oxygenation monitor for determining thresholds during exercise testing. The proposed PICO question was: Is the exercise intensity of muscle oxygenation thresholds, using portable NIRS, reliable compared with lactate and ventilatory thresholds for exercise intensity determined in athletes? A search of Pubmed, Scopus and Web of Science was undertaken and the review was conducted following PRISMA guidelines. Fifteen articles were included. The domains which presented the highest biases were confounders (93% with moderate or high risk) and participant selection (100% with moderate or high risk). The intra-class correlation coefficient between exercise intensity of the first ventilatory or lactate threshold and the first muscle oxygenation threshold was 0.53 (obtained with data from only 3 studies), whereas the second threshold was 0.80. The present work shows that although a portable muscle oxygenation monitor has moderate to good reliability for determining the second ventilatory and lactate thresholds, further research is necessary to investigate the mathematical methods of detection, the capacity to detect the first threshold, the detection in multiple regions, and the effect of sex, performance level and adipose tissue in determining thresholds.

Patellofemoral Pain Syndrome is characterized by the presence of pain in the front area of the knee, which occurs when performing common activities such as climbing stairs, and bending the knees, among others. The objective of this research was to evaluate the detection capability of infrared thermography in patients with Patellofemoral Pain Syndrome, in the baseline state, as well as after the application of thermal stress. The investigation was conducted in 48 patients, who were subdivided into four groups (n = 12). Two subgroups were healthy patients and two with Patellofemoral Pain Syndrome. For the diagnosis of the syndrome, a manual evaluation was performed using the Zohlen test and Q angle measurement. Subsequently, cold stress was applied for 10 min to a healthy subgroup and an experimental subgroup. The remaining two subgroups were subjected to heat stress for 15 min. Thermographic images of the lower extremities were acquired at seven time points, at baseline, immediately after application of thermal stress and then every 3 min until 15 min were completed. It was observed that patients presented Patellofemoral Pain Syndrome bilaterally. After statistical analysis, it was found that there were no significant differences in baseline temperature between the groups. However, for heat stress, a higher temperature was observed in the group with Patellofemoral Pain Syndrome (p < 0.05) in the recovery period, and in the case of cold stress, only a lower temperature in the left knee immediately after the application. In conclusion, it is not possible to detect patellofemoral syndrome bilaterally in the baseline state by thermography and neither is it evident in cold stress. However, after heat stress, thermal recovery is lower for the PFPS group, so it would be susceptible to detection.

RESUMO

Concepts about motor unit recruitment are important learning contents in exercise physiology and biomechanics classes that are usually taught theoretically. In the last few years, great advances have occurred in the decomposition of surface electromyography, allowing the learning of theoretical contents in an experimental way. In this tutorial paper, we have described the decomposition of surface electromyography methodological aspects and examples to teach motor unit recruitment concepts in exercise physiology and biomechanics practical lessons. This work has the aim to facilitate physiology and biomechanics academics to introduce this technique in practical classes.

Fatigue can be classified as peripheral or central depending on the extent of its effects. Muscle strength reduction, associated with the appearance of fatigue during running, produces kinetics and kinematics modifications which could lead to an increased risk of injury. This study aimed to analyze the effect of peripheral and central fatigue protocols in running kinematics and to investigate the relationship between isokinetic strength and dynamic stability in fatigue related changes. Eighteen male recreational runners participated in the study. The dynamic postural stability index (DPSI) and quadriceps and hamstring isokinetic strength were assessed before the fatigue test. Then, angular kinematics during treadmill running were evaluated in pre- and post-fatigue states (central and peripheral). The results showed that runners with higher hamstring isokinetic strength and better DPSI had lower modifications after central fatigue of stance time, knee flexion, vertical and leg stiffness, and ankle dorsiflexion during the absorption and propulsion phases (r > 0.400, p < 0.05). Moreover, small changes in ankle dorsiflexion at initial contact after peripheral fatigue are related to a better DPSI and higher hamstring isokinetic strength (r > 0.400, p < 0.05). In summary, high values of hamstring isokinetic concentric strength and dynamic stability are related to lower increases of range of movements during running after central and peripheral fatigue. So, fatigue may affect to a lesser extent the running technique of those runners with higher hamstring strength and stability values.

This study aimed to assess the effect of a marathon running at a hot environmental temperature on the baseline skin temperature (Tsk) of the posterior day and to analyze the relationship between Tsk response and muscle damage markers variation. The Tsk, creatine kinase, and lactate dehydrogenase of 16 marathon runners were assessed four times before (15 days and 45 min) and after (24 h and 6 days) a marathon in a hot environment (thermal stress index = 28.3 ± 3.3 °C and humidity ~81%). The Tsk of thirteen different body regions of both right and left lower limbs were analyzed. Higher values after the marathon were observed than 45 min before in creatine kinase (174.3 ± 136.4 UI/L < 1159.7 ± 699.7 UI/L, p < 0.01 and large effect size) and lactate dehydrogenase (362.6 ± 99.9 UI/L < 438 ± 115.5 UI/L, p = 0.02 and moderate effect size). Generally, Tsk was higher the day after the marathon than at the other three moments (e.g., rectus femoris region, 6 days before vs. the day after, 95% confidence interval of the difference (0.3, 1.6 °C), p = 0.04 and large effect size). No relationship or correlation was observed between the variation of Tsk and muscle damage markers (p > 0.05). In conclusion, performing a marathon in a hot environmental condition results in a higher Tsk the day after the marathon. This increase in Tsk could be because of the heat generated by the marathon and its subsequent physiological processes (e.g., increase in endothelial nitric oxide, glycogen resynthesis, or increase of systemic hormones), which would be reflected in the Tsk due to the peripheral vasodilation promoted by the hot environment. However, among these processes, muscle damage does not seem to be of great importance due to the lack of an observed relationship between Tsk and muscle damage markers.

Amateur runners usually run carrying implements in their hands (keys, a mobile phone, or a bottle of water). However, there is a lack of literature about the effects of different handloads on impact accelerations. Thus, this study aimed to analyse the effects of carrying different objects in the hand on impact accelerations during running. Nineteen male recreational runners (age 24.3 ± 6.8 years, training volume of 25 ± 7.38 km/week) performed twenty minutes of running on a treadmill at 2.78 m/s with four different conditions: no extra weight, with keys, with a mobile phone, and with a bottle of water. Impact acceleration and spatio-temporal parameters were analysed through a wireless triaxial accelerometry system composed of three accelerometers: two placed in each tibia and one placed on the forehead. A higher tibia acceleration rate in the dominant leg was observed when participants ran holding both a mobile phone (p = 0.027; ES = 0.359) and a bottle of water (p = 0.027; ES = 0.359), compared to no extra weight. No changes were observed in peak acceleration, acceleration magnitude, and shock attenuation in any other conditions. Likewise, neither stride frequency nor step length was modified. Our results suggest that recreational runners should not worry about carrying objects in their hands, like a mobile phone or a bottle of water, in short races because their effect seems minimal.

Delayed onset muscle soreness (DOMS) indicates the presence of muscle damage and impairs force production and control. Monitorization of DOMS is useful to improving recovery intervention plans. The magnitude of DOMS may relate to muscle fatigue, which can be monitored by surface electromyography (EMG). Additionally, growing interest has been expressed in determining whether the skin temperature over a muscle group during exercise to fatigue could be a non-invasive marker for DOMS. Here we determine whether skin temperature and manifestations of muscle fatigue during exercise are correlated and can predict DOMS after concentric-eccentric bicep curl exercises. We tested 10 young adults who performed concentric-eccentric bicep curl exercises to induce muscle damage in the biceps brachialis to investigate the relationship between skin temperature and fatigue during exercise and DOMS after exercise. Muscle activation and skin temperature were recorded during exercise. DOMS was evaluated 24 h after exercise. Data analysis was performed using Bayesian regression models with regularizing priors. We found significant muscle fatigue and an increase in skin temperature during exercise. DOMS was observed 24 h after exercise. The regression models showed no correlation of changes in skin temperature and muscle fatigue during exercise with DOMS 24 h after exercise. In conclusion, our preliminary results do not support a relationship between skin temperature measured during exercise and either muscle fatigue during exercise or the ability to predict DOMS 24 h after exercise.

Although strength imbalances using isokinetic dynamometer have been examined for injury risk screening in soccer players, it is very expensive and time-consuming, making the evaluation of new methods appealing. The aim of the study was to analyze the agreement between muscular strength imbalances and skin temperature bilateral asymmetries as well as skin temperature differences in the hamstrings and quadriceps. The skin temperature of the anterior and posterior thigh of 59 healthy male soccer athletes was assessed at baseline using infrared thermography for the identification of hamstrings-quadriceps skin temperature differences and thermal asymmetries (>0.5 °C). Subsequently, concentric and eccentric peak torque of the quadriceps and hamstrings were considered in the determination of the ratios, as well as muscular asymmetries (>15%). When considering the torque parameters, 37.3% (n = 22) of the players would be classified as high risk for injuries. The percentage of those presenting skin temperature imbalances superior to 0.5 °C was 52.5% (n = 31). The skin temperature assessment showed sensitivity (22%) and specificity (32.2%) to identify torque asymmetries, demonstrating the inability to identify false negatives (15.3%) and false positives (30.5%) from all soccer athletes. In conclusion, skin temperature differences between hamstrings and quadriceps could be more related to thermoregulatory factors than strength imbalances.

OBJECTIVE: Measurement of skin temperature using infrared thermography has become popular in sports, and has been proposed as an indicator of exercise-induced muscle damage after exercise. However, the relationship between skin temperature and exercise-induced muscle damage is still unclear. Here we set out to investigate the relationship between skin temperature and exercise-induced muscle damage. APPROACH: Twenty untrained participants completed a protocol of exercise for calf muscles. Before and after exercise blood samples were collected to determine creatine kinase and acetylcholinesterase activity. Thermal images were recorded from the exercised muscles to determine skin temperature. Delayed onset muscle soreness was quantified. Correlations between skin temperature and exercise-induced muscle damage were analyzed considering thermal data, creatine kinase and acetylcholinesterase activity at different time moments. MAIN RESULTS: We found delayed onset muscle soreness and an increased creatine kinase activity 48 h after exercise (P < 0.01). Skin temperature parameters (average, maximal, amplitude and difference pre- and post-exercise, immediately after and 48 h after) did not correlate with the creatine kinase responses (P > 0.05). Acetylcholinesterase activity remained stable (P = 0.59). SIGNIFICANCE: We recommend caution when considering changes in skin temperature as dependent on the level of localized and symmetric muscle damage considering calf muscles in untrained participants.

BACKGROUND: Padel is a relatively new sport, with steady participation increases over the past 50 years. However, there is a need for data examining injury and associated risk factors. The study examined how intrinsic (e.g. age and sex) and extrinsic factors (e.g. padel participation and equipment) were associated with injury in recreational padel players. METHODS: A self-administered questionnaire completed by 80 recreational padel players between September 2014 and March 2015 acquired data on: respondent demographics and padel-related activities; the number of injuries within the past year; characteristics of the last injury sustained; and extrinsic factors including respondents' evaluations of equipment (e.g., racquet control/power, footwear cushioning, flexibility, traction). χ2 tests compared the proportion of respondents reporting injuries in the past year among those reporting and not reporting equipment-related issues. RESULTS: Most respondents were male (57.5%), aged 30-40 years (46.3%), and played more than 2 padel sessions/week (48.8%). Overall, 40.0% reported sustaining at least one injury during the past year. Sex, age, frequency of padel sessions/week, and years of padel experience were not associated with the reporting of injury within the past year (P>0.05). The proportion of padel players reporting injuries in the past year was greater among those that wore padel-specific footwear than those that wore another kind of sports footwear (50.0% vs. 23.3%; P=0.02). CONCLUSIONS: Recreational padel players wearing padel-specific footwear were more at risk for injury than those not. Our findings highlight the need for additional research regarding injury risk and footwear.

The characteristics of a treadmill and the environment where it is based could influence the user's gaze and have an effect on their running kinematics and lower limb impacts. The aim of this study was to identify the effect of visual focus on spatio-temporal parameters and lower limb kinematics during treadmill running. Twenty six experienced runners ran at 3.33ms-1 on a treadmill under two visual conditions, either looking ahead at a wall or looking down at the treadmill visual display. Spatio-temporal parameters, impact accelerations of the head and tibia, and knee and ankle kinematics were measured for the final 15s of a 90s bout of running under each condition. At the end of the test, participants reported their preference for the visual conditions assessed. Participants' stride angle, flight time, knee flexion during the flight phase, and ankle eversion during contact time were increased when runners directed visual focus toward the wall compared to the treadmill display (p<0.05). Whilst head acceleration was also increased in the wall condition (p<0.05), the other acceleration parameters were unaffected (p>0.05). However, the effect size of all biomechanical alterations was small. The Treadmill condition was the preferred condition by the participants (p<0.001; ESw=1.0). The results of the current study indicate that runners had a greater mass centre vertical displacement when they ran looking ahead, probably with the aim of compensating for reduced visual feedback, which resulted in larger head accelerations. Greater knee flexion during the flight phase and ankle eversion during the contact time were suggested as compensatory mechanisms for lower limb impacts.

The importance of using infrared thermography (IRT) to assess skin temperature (tsk) is increasing in clinical settings. Recently, its use has been increasing in sports and exercise medicine; however, no consensus guideline exists to address the methods for collecting data in such situations. The aim of this study was to develop a checklist for the collection of tsk using IRT in sports and exercise medicine. We carried out a Delphi study to set a checklist based on consensus agreement from leading experts in the field. Panelists (n = 24) representing the areas of sport science (n = 8; 33%), physiology (n = 7; 29%), physiotherapy (n = 3; 13%) and medicine (n = 6; 25%), from 13 different countries completed the Delphi process. An initial list of 16 points was proposed which was rated and commented on by panelists in three rounds of anonymous surveys following a standard Delphi procedure. The panel reached consensus on 15 items which encompassed the participants' demographic information, camera/room or environment setup and recording/analysis of tsk using IRT. The results of the Delphi produced the checklist entitled "Thermographic Imaging in Sports and Exercise Medicine (TISEM)" which is a proposal to standardize the collection and analysis of tsk data using IRT. It is intended that the TISEM can also be applied to evaluate bias in thermographic studies and to guide practitioners in the use of this technique.

While different studies showed that better fitness level adds to the efficiency of the thermoregulatory system, the relationship between muscular effort and skin temperature is still unknown. Therefore, the present study assessed the relationship between neuromuscular activation and skin temperature during cycle exercise. Ten physically active participants performed an incremental workload cycling test to exhaustion while neuromuscular activations were recorded (via surface electromyography - EMG) from rectus femoris, vastus lateralis, biceps femoris and gastrocnemius medialis. Thermographic images were recorded before, immediately after and 10 min after finishing the cycling test, at four body regions of interest corresponding to the muscles where neuromuscular activations were monitored. Frequency band analysis was conducted to assess spectral properties of EMG signals in order to infer on priority in recruitment of motor units. Significant inverse relationship between changes in skin temperature and changes in overall neuromuscular activation for vastus lateralis was observed (r<-0.5 and p<0.04). Significant positive relationship was observed between skin temperature and low frequency components of neuromuscular activation from vastus lateralis (r>0.7 and p<0.01). Participants with larger overall activation and reduced low frequency component for vastus lateralis activation presented a better adaptive response of their thermoregulatory system by showing fewer changes in skin temperature after incremental cycling test.
