PURPOSE: The prognostic utility and biological correlates of neutrophil to lymphocyte ratio (NLR), a potential biomarker of the balance between immune response and the inflammatory status, are still uncertain in breast cancer (BC). METHODS: We analysed a cohort of 959 women with early breast cancer, mostly treated with neoadjuvant or adjuvant chemotherapy. Clinical and pathological data, survival, NLR (continuous and categorical) and stromal tumor infiltrating lymphocytes (sTIL) were evaluated. RESULTS: NLR was only weakly associated with Ki67, while no association was found for grade, histology, immunohistochemical subtype or stage. Lymphocyte infiltration of the tumor did not correlate with NLR (Rho: 0.05, p = 0.30). These results were similar in the whole group and across the different BC subtypes, with no differences in triple negative BC. Relapse free interval (RFI), breast cancer specific survival (BCSS) and overall survival (OS) changed according to pre-treatment NLR neither in the univariate nor in the multivariate Cox models (RFI: HR 0.948, p = 0.61; BCSS: HR 0.920, p = 0.57; OS: HR 0.96, p = 0.59). CONCLUSION: These results question the utility of NLR as a prognostic biomarker in early breast cancer and suggest the lack of correlation of NLR with tumor microenvironment immune response.

Culicids in Argentinean Patagonia are characterized by low species diversity and adaptation to extreme environmental conditions, yet few studies have been conducted in the region. To further assess the occurrence of Culicidae in Western Patagonia, and in particular the presence of Culex pipiens bioforms at the southernmost extent of their distribution, immature and adult specimens were collected aboveground across various land uses located in shrubland, steppe, and deciduous forest between 38.96 and 46.55°S. Mosquitoes were reported at 35 of the 105 inspected sites. Five species from the genus Culex were identified, all of which were present in the steppe and the forest, while only Cx. apicinus and members of the Cx. pipiens complex were collected in the shrubland. Within the latter, a total of 150 specimens were molecularly identified by PCR amplification of Ace-2 and CQ11 loci. The first-to-date occurrence of bioform pipiens in South America is reported, along with the first records of Cx. quinquefasciatus signatures in Patagonia. In addition, the distribution of Cx. acharistus and Cx. dolosus as south as Santa Cruz province is expanded, and the first record of Cx. eduardoi in Río Negro province is provided. Immature specimens of Cx. pipiens were conspicuous in human-made aquatic habitats (both containers and in the ground), while Cx. acharistus was more prominent in artificial containers and Cx. eduardoi was mainly in ground habitats, either natural or human-made. These findings provide valuable insights into the distribution and ecological roles of these mosquito species in a region of extreme environmental conditions.

Background: Up to 30% of breast cancer (BC) patients treated with neoadjuvant chemotherapy (NCT) will relapse. Our objective was to analyze the predictive capacity of several markers associated with immune response and cell proliferation combined with clinical parameters. Methods: This was a single-center, retrospective cohort study of BC patients treated with NCT (2001-2010), in whom pretreatment biomarkers were analyzed: neutrophil-to-lymphocyte ratio (NLR) in peripheral blood, CD3+ tumor-infiltrating lymphocytes (TILs), and gene expression of AURKA, MYBL2 and MKI67 using qRT-PCR. Results: A total of 121 patients were included. Median followup was 12 years. In a univariate analysis, NLR, TILs, AURKA, and MYBL2 showed prognostic value for overall survival. In multivariate analyses, including hormone receptor, HER2 status, and response to NCT, NLR (HR 1.23, 95% CI 1.01-1.75), TILs (HR 0.84, 95% CI 0.73-0.93), AURKA (HR 1.05, 95% CI 1.00-1.11) and MYBL2 (HR 1.19, 95% CI 1.05-1.35) remained as independent predictor variables. Conclusion: Consecutive addition of these biomarkers to a regression model progressively increased its discriminatory capacity for survival. Should independent cohort studies validate these findings, management of early BC patients may well be changed.

Luminal breast cancer (BC) is associated with less immune activation, and the significance of stromal lymphocytic infiltration (sTIL) is more uncertain than in other BC subtypes. The aim of this study was to investigate the predictive and prognostic value of sTIL in early luminal BC. The study was performed with an observational design in a prospective cohort of 345 patients with predominantly high-risk luminal (hormone receptor positive, HER2 negative) BC and with luminal B features (n = 286), in which the presence of sTIL was analyzed with validated methods. Median sTIL infiltration was 5% (Q1-Q3 range (IQR), 0-10). We found that sTIL were associated with characteristics of higher biological and clinical aggressiveness (tumor and lymph node proliferation and stage, among others) and that the percentage of sTIL was predictive of pathologic complete response in patients treated with neoadjuvant chemotherapy (OR: 1.05, 95%CI 1.02-1.09, p < 0.001). The inclusion of sTIL (any level of lymphocytic infiltration: sTIL > 0%) in Cox regression multivariable prognostic models was associated with a shorter relapse-free interval (HR: 4.85, 95%CI 1.33-17.65, p = 0.016) and significantly improved its performance. The prognostic impact of sTIL was independent of other clinical and pathological variables and was mainly driven by its relevance in luminal B BC.

Human health risk in urban areas depends on multiple environmental features. We performed a year-round survey in a highly urbanized district located in temperate Argentina (General San Martín, Buenos Aires) to establish baseline information about environmental hazards associated with health risks. Sampling was stratified into low and high hazardous zones according to estimated indexes previously developed for the area for four hazards: drinking water and air pollution, and mosquito and rodent infestation. Water from wells showed lower concentrations of aluminum, manganese and iron, and higher values of arsenic than tap samples, with the latter showing records above the maximum permitted for arsenic, aluminum and chromium. Benzene concentration in air was higher in summer than in winter, and in areas close to dumps and landfills, gas stations, high traffic pathways and industries with respect to low hazard areas. Adult mosquito collections were more abundant in high hazardous areas, three species from the genus Culex dominated the captures and the proportion of individuals from each species was variable seasonally and spatially. Rodent activity was recorded inside and outside dwellings, and its observed values did not differ between low and high hazardous areas. In the comparison between field data and estimated hazard maps, high accuracy was obtained for air pollution maps, intermediate accuracy for water pollution and mosquito infestation, and poor accuracy for rodent infestation. How to improve field surveys and estimated maps are both discussed, highlighting the need for dynamic feedback between GIS-based models and environmental monitoring.

The geographic distribution of Aedes (Stegomyia) aegypti (L.) in South America has been expanding during the last decades. Herein we present two new distribution records that extend its southern limits towards localities with extremer environmental conditions than reported to date. San Antonio Oeste constitutes the southernmost finding for the continent (40º44'S), whereas Tandil is the infested locality with the coldest mean annual temperature in Argentina (14.17ºC). The projection of a previous distribution model for Ae. aegypti predicts these two cities as positive and suggests several other localities with suitable conditions for vector proliferation beyond its assumed distribution limits.

To assess the occurrence of Cx. pipiens bioforms in the southernmost limit of its distribution, specimens were collected aboveground in southern Buenos Aires Province and east Patagonia, Argentina. Ten larvae and 25 adults were individually processed and identified by polymerase chain reaction (PCR) amplification of Ace-2 and CQ11 loci. Culex quinquefasciatus Say (one larva, two adults), Cx. pipiens f. molestus (one larva, one adult) and one adult of hybrid origin were identified in Buenos Aires Province; only Cx. pipiens f. molestus was recorded in Patagonia (eight larvae, 21 adults). The potential absence of bioform pipiens and its implications in arbovirus enzootic cycles is discussed.

The prognostic impact of neutrophil-lymphocyte ratio (NLR) in metastatic breast cancer (MBC) has been previously evaluated in early and metastatic mixed breast cancer cohorts or without considering other relevant prognostic factors. Our aim was to determine whether NLR prognostic and predictive value in MBC was dependent on other clinical variables. We studied a consecutive retrospective cohort of patients with MBC from a single centre, with any type of first line systemic treatment. The association of NLR at diagnosis of metastasis with progression free survival (PFS) and overall survival (OS) was evaluated using Cox univariate and multivariate proportional hazard models. In the full cohort, that included 263 MBC patients, a higher than the median (>2.32) NLR was significantly associated with OS in the univariate analysis (HR 1.36, 95% CI 1.00-1.83), but the association was non-significant (HR 1.12, 95% CI 0.80-1.56) when other clinical covariates (performance status, stage at diagnosis, CNS involvement, visceral disease and visceral crisis) were included in the multivariate analysis. No significant association was observed for PFS. In conclusion, MBC patients with higher baseline NLR had worse overall survival, but the prognostic impact of NLR is likely derived from its association with other relevant clinical prognostic factors.

Unplanned urbanization increases the exposure of people to environmental hazards. Within a landscape ecology framework, this study is a diagnosis of human health risk in San Martín, an urban district of Buenos Aires, Argentina. Risk was estimated by combining four hazard indexes (water and air pollution, and mosquito and rodent infestation) and a vulnerability index. Each index was obtained by integrating environmental and socio-demographic layers in a Geographic Information System. Spatial autocorrelation was assessed for each hazard, vulnerability and risk indexes using Moran's tests. Also, spatial associations between pairs of variables were addressed by means of Geographically Weighted Regressions. The robustness of hazard and vulnerability indexes was checked by a sensitivity analysis. In General San Martín district, 83.3% of the population is exposed to relatively high levels of at least one hazard; 7.4% is exposed to relatively high levels of all hazards (11.5% of the total area) and only 16.7% lives in areas of relatively low levels of all hazards (15.4% of the total area). Areas where hazard intensity was relatively high corresponded to those areas where the most vulnerable population lives, enhancing human health risk. The models for hazards and vulnerability were reasonably robust to changes in the weights of the variables considered. Our results highlight the spatially heterogeneous nature of human health risk in an urban landscape, and reveal the location of critical risk hotspots where reduction or mitigation actions should be focused.

La leishmaniasis cutánea es una enfermedad parasitaria granulomatosa crónica que se transmite por la picadura de flebótomos infectados, esta patología afecta a todos los grupos etarios y los menores de 12 años constituyen únicamente el 10%, las manifestaciones clínicas dependerán de factores como el estado inmune y la especie de Leishmania, la forma clínica de leishmaniasis cutánea representa el 99% en los pacientes pediátricos siendo así la más frecuente, el diagnóstico se basa en criterios clínicos, epidemiológicos y de laboratorio. El tratamiento en la edad pediátrica puede hacerse con antimoniato de meglumina con buena respuesta como en los casos que presentamos a continuación


Cutaneous leishmaniasis is a chronic granulomatous parasitic disease that is transmitted by the bite of infected sandflies, this pathology affects all age groups and those under 12 years constitute only 10%, the clinical manifestations depend on factors such as the immune status and the species of leishmania, the clinical form of cutaneous leishmaniasis represents 99% in pediatric patients being the most frequent, the diagnosis is based on clinical, epidemiological and laboratory criteria. Treatment in the pediatric age can be done with meglumineantimonate with good response as in the cases presented below.

The control of the mosquito vector Aedes aegypti L. (Diptera: Culicidae) is the main action against dengue, chikungunya, and Zika. The excessive use of conventional insecticides has promoted the development of other control methods and strategies with lower environmental impact. We evaluated the effectiveness of applying triflumuron 1 ppm and emptying water-filled containers in a field trial in temperate Argentina. Both control methods were implemented either individually or combined and regularly from the beginning of the mosquito reproductive season or once it reached peak abundance. The impact on a non-target midge of the genus Chironomus was also tested. The highest reductions of Ae. aegypti were achieved in treatments which included triflumuron. This effect was stronger when applied from the beginning of the reproductive season, with < 1.3% of positive containers throughout the entire season. No enhancing effects were obtained when combining both control methods. Treatments with triflumuron were not completely innocuous for the non-target species, with Chironomus sp. more susceptible to treatments including triflumuron applied from the beginning of the reproductive season than all others. Sharp reductions of mosquito populations in urban environments with high density of water-filled containers are possible with minimum container management efforts, by applying triflumuron 1 ppm every 6 weeks. In temperate urban settings, better results can be obtained when applications begin early in the reproductive season of the mosquito vector Aedes aegypti.

PURPOSE: Therapeutic exploitation of angiogenesis in breast cancer has been limited by the lack of reliable biomarkers. Circulating small-sized endothelial microparticles (sEMP) are likely to play a significant role as messengers of angiogenesis. Higher levels of EMP have been observed in cancer patients, but their prognostic value in breast cancer is unknown. Our aim was to determine the value of circulating sEMP as a marker of response to chemotherapy in breast cancer. METHODS: We included patients with breast cancer treated with neoadjuvant or first-line chemotherapy. Baseline and post-treatment circulating sEMP (CD144+) were quantified using a flow cytometer approach specifically designed for analysis of small-sized particles (0.1-0.5 µm). Small-sized EMP response was defined as a post-treatment decrease of sEMP larger than the median decrease of sEMP after chemotherapy. Baseline and post-chemotherapy VEGFA levels were determined with ELISA. RESULTS: Forty-four breast cancer patients were included (19 with metastatic and 25 with locally advanced disease). Median levels of sEMP decreased after chemotherapy (P = 0.005). Response to chemotherapy showed a non-significant trend to associate with sEMP response (P = 0.056). A sEMP response was observed in 51% of patients and was associated with better overall survival (HR 0.18; 95% CI 0.04-0.87; P = 0.02) and progression free survival (HR 0.30; 95% CI 0.09-0.99; P = 0.04) in the group of women with metastatic disease. Post-chemotherapy decrease of VEGFA levels was not associated with breast cancer prognosis. CONCLUSIONS: Our results did not support sEMP as a marker of response to chemotherapy. However, our exploratory analysis suggests that in patients with metastatic breast cancer, the decrease of sEMP levels after chemotherapy is associated with better overall and disease free survival and might be superior to VEGFA levels as an angiogenesis-related prognostic marker.

Given their medical and veterinary relevance, the members of the Pipiens Assemblage are a worldwide target of ecological research. The distribution of Culex pipiens s.s. and Cx. quinquefasciatus converge in Buenos Aires, Argentina, where hybrids have been detected. Each member of the assemblage exhibits a distinct eco-physiological behaviour that can affect its efficiency in pathogen transmission. Our aim was to identify the environmental drivers for the spatio-temporal distribution of each member, focusing on latitudinal and urbanisation gradients. Immatures of mosquitoes were surveyed in artificial containers found within 11 public cemeteries, raised up to the adult stage and identified by their male genitalia. The distribution of each member was associated with the environment in a Generalized Linear Model. The variable accounting for most of the heterogeneity was latitude; Cx. quinquefasciatus was collected more frequently at northern cemeteries, whereas Cx. pipiens and hybrids were more likely at the southern extreme. The urbanisation gradient was also associated with the occurrence of Cx. quinquefasciatus and hybrids at the high and low end, respectively. Other relevant variables were cemetery total area, the proportion with graves and the presence of plastic flowers in the containers. The spatial distribution of the members of the Pipiens Assemblage within the sympatric region in South America is driven by environmental features. The information presented herein provides essential baseline data for surveillance programs and control activities.

Se resumen en este trabajo las comunicaciones más relevantes y con mayor impacto clínico presentadas en el Simposio Internacional de Cáncer de Mama, celebrado en San Antonio en diciembre de 2015. Desde el punto de vista translacional, las principales aportaciones han sido las referidas a la dinámica clonal y a la heterogeneidad tumoral, así como a la posibilidad de estudiar mutaciones emergentes relacionadas con resistencia mediante biopsia líquida y técnicas de secuenciación masiva, como demuestran los datos de los ensayos BELLE-2 y BOLERO-2. Desde el punto de vista clínico, los resultados finales del estudio BCIRG-006 y los intermedios del estudio ExteNET en cáncer de mama HER2 modifican el panorama de la adyuvancia en este grupo de pacientes. Los resultados del estudio ABCSG-18 con denosumab adyuvante en enfermedad luminal apoyan los resultados previos con bifosfonatos. En enfermedad triple negativa, se han aportado datos interesantes acerca de la utilización de carboplatino y nab-paclitaxel en esquemas neoadyuvantes. El estudio CREATE-X con capecitabina adyuvante plantea la posibilidad de mejorar el pronóstico de las pacientes que no alcanzan respuesta completa tras neoadyuvancia, especialmente en el grupo triple negativo. La posibilidad de reducir la intensidad del tratamiento en algunos subgrupos de pacientes, las controversias sobre la irradiación nodal y el manejo quirúrgico de la axila, y los datos iniciales sobre inmunoterapia completan este resumen (AU)


We summarise the most interesting and clinically relevant works presented at the 38th Annual San Antonio Breast Cancer Symposium (December 2015). In the field of translational research, the main contributions were made in clonal dynamics and tumour heterogeneity, as well as in the possibility of studying emerging resistance mutations with liquid biopsy and next-generation sequencing, as shown by data from the BELLE-2 and BOLERO-2 trials. From the clinical point of view, the final analysis of BCIRG-006 trial and the intermediate analysis of the ExteNET trial will probably change the adjuvant treatment of HER2 breast cancer patients. The results of the ABCSG-18 trial with adjuvant denosumab in luminal breast cancer are consistent with previous data from trials of adjuvant bisphosphonate therapy. In triple-negative breast cancer, interesting data were reported supporting the addition of carboplatin and nab-paclitaxel to neoadjuvant chemotherapy regimens. The CREATE-X trial is the first study showing a survival benefit (larger in triple negative disease) of adjuvant chemotherapy with capecitabine in those patients without pathologic complete response after neoadjuvant chemotherapy. Finally, other problems addressed during the meeting were treatment de-escalation in some patient subgroups, the controversies surrounding and integration of nodal irradiation and axillary surgery, and early clinical results with immune therapy (AU)

El objetivo del estudio fue comparar la presencia curricular de las unidades de aprendizaje de Psicología de la Actividad Física en los planes de estudio de las licenciaturas (pregrado) y posgrado de Educación Física versus Psicología en las universidades que se ofertan en la República Mexicana. Estudio descriptivo y transversal que tuvo de universo 144 programas de estudio de psicología y 68 de educación física; encontrando que en los Posgrados de Psicología el peso porcentual es un poco mayor (16.36) que en los Posgrados de Educación Física (14. 33). Pero al unir pregrado y posgrado se triplica el peso porcentual de las unidades de aprendizaje en las disciplinas de Educación Física (3.13) a diferencia que en psicología (0.93). Estos resultados permiten concluir que, como eje de formación curricular, la psicología de la actividad física y del deporte tiene mayor presencia en Educación Física que en Psicología, lo que significa la necesidad interdisciplinar y transdisicplinar de nuestro objeto de estudio; la unión e interconexión de la psicología y la actividad física y el deporte. Un problema y necesidad enfrentada fue la distinta nomenclatura de los programas de formación curricular tanto de los planes de estudio como de la unidad de aprendizaje, que pueden llevar a la necesidad nacional e internacional de acordar nombres, niveles, carga horaria y competencias (u objetivos) profesionales que pueden facilitar este tipo de análisis, pero sobretodo que faciliten el intercambio académico de estudiantes y profesores.


O presente estudo teve como objetivo comparar o peso percentual das unidades de aprendizagem de psicologia da atividade física na estrutura curricular para a formação profissional inicial e de pós-graduação nos currículos de Educação Física versus Psicologia, em universidades mexicanas. De caráter descritivo e transversal, e tendo como universo 144 currículos de Psicologia e 68 de Educação Física, o estudo mostrou que nos Programas de pós-graduação em Psicologia o peso percentual é um pouco maior (16.36) que nos Programas de Pós-graduação em Educação Física (14.33). Entretanto, ao considerarmos graduação e pós-graduação conjuntamente, o peso percentual das unidades de aprendizagem é três vezes superior nas disicplinas de Educação Física (3.13) em relação às de Psicologia (0.93).


The aim of this study was to compare the percentage of Learning Units in Psychology of Physical Activity in the curriculum structure for the training of the degree (undergraduate) and graduate curricula in Physical Education versus Psychology offered in universities in Mexico. This study was descriptive and cross-sectional, it had an universe of 144 programs of study in Psychology and 68 in Physical Education. We found that percentage in graduate programs of Psychology is slightly higher (16.36) than Graduate programs of Physical Education (14.33). But by combining Undergraduate and Graduate, the percentage of Learning Units in the disciplines of Physical Education (3.13) triples the percentaje weight in Psychology (0.93).These results allow the conclusion that, as the axis of training curriculum, Psychology of Physical Activity and Sport has a greater presence in Physical Education than in Psychology, which means the need interdisciplinary and transdisciplinary of our object of study; the connection and interconnection of the Psychology and the study of Physical Activity and Sport. A problem found during this study which needs to be faced was the nomenclature of the different training programs from both the curriculum plans of study and the learning units, which can lead to national and international agreements on names, levels, load time and professional competences (or targets) to provide this type of analysis, but especially to facilitate academic exchange programs of students and teachers.

Used vehicle tires are a source of mosquito vectors and a means of their introduction and expansion. With the aim of assessing the effects of urbanisation on the main mosquito vectors in temperate Argentina, the infestation levels of Aedes aegypti (L.) and Culex pipiens L. were studied in used tires from highly urbanised cities to low-urbanised small towns in Buenos Aires. Immatures of both species accounted for 96% of the 9,722 individuals collected; the total individuals collected represented seven species. The percentage of water-filled tires containing mosquitoes [container index (CI)] was 33% and the percentage of infested sites [site index (SI)] was 65.2%. These indexes decreased significantly from low to high urbanisation levels for both mosquito species. The relative abundance (RA) of Ae. aegypti immatures was slightly higher toward large cities, but showed no difference for Cx. pipiens. The CI of shaded tires was significantly higher than the CI of exposed tires for both mosquito species. There was no difference in RA values between shaded and sunlit tires. The CI and the SI were highest during the summer across the urbanisation levels, except for Cx. pipiens, which continued to increase during the autumn in small towns. Results related to urbanisation gradient, sunlit exposure and seasonality are discussed.
