Introducción: La tercerventriculostomía endoscópica (TVE) es una técnica quirúrgica ampliamente reconocida para el tratamiento de la hidrocefalia obstructiva. Al igual que otras técnicas para el manejo de la hidrocefalia, la protocolización del manejo intra y peri-operatorio deviene en mejores resultados. Sin embargo, no existe en la literatura en castellano alguna publicación que exponga en detalle un protocolo de trabajo de TVE. El objetivo de este estudio es presentar los resultados tras la aplicación de nuestro protocolo. Materiales y Métodos: Se realizó una descripción del protocolo para TVE prequirúrgico, intraoperatorio y post-operatorio. Se realizó un análisis prospectivo de una serie de casos. Resultados: Un detallado protocolo fue presentado. Un total de 70 pacientes fueron incluidos para su análisis. La tasa de éxito del procedimiento fue del 82.43%. Hubo una mejoría en la escala de Karnofsky tras la TVE [media de 26.22 puntos (IC 95% 13,77 -36,92)]; siendo esta estadísticamente significativa (p=<0,001). Conclusión: La TVE demostró ser un procedimiento seguro y efectivo para el tratamiento de la hidrocefalia obstructiva. El protocolizar el procedimiento quirúrgico demostró una alta tasa de éxito en nuestra cohorte de pacientes, comparando con los resultados de la literatura internacional
Background. Endoscopic third ventriculostomy (ETV) is a worldwide accepted technique to treat obstructive hydrocephalus. As per many other techniques to treat hydrocephalus, protocols had improved the overall results and reduced the complications rate. However, there are no protocols written in Spanish literature, that illustrate in detailed a step by step on how to perform and follow up patients with ETV.The aim of this study is to present the results after the application of our protocol. Methods. A protocol with surgical technique, management and follow up for ETV in adults was written and a retrospective analysis of several cases was presented. Results. A detailed protocol was presented. Forty-three patients included in the protocol were analysed after ETV. The success rate of the procedure was 82.43%. There was a statistically significant improvement of a mean 26.22 points (IC 95% 13.77 -36.92) in the Karnofsky scale after the ETV (p=<0.001). Conclusion: ETV has shown to be a safe and effective procedure to treat obstructive hydrocephalus. The protocolization of the procedure proved a high success rate in our cohort of patients, comparable with the international literature results

Introducción: Las variantes en la anatomía del piso del tercer ventrículo (PTV) son determinantes en las limitaciones y complicaciones de la tercerventriculostomía endoscópica (TVE). Objetivo: Describir variaciones anatómicas del PTV halladas en casos de hidrocefalia, evaluar incidencia, estratificarlas según tiempo de evolución y correlacionarlas con dificultad quirúrgica. Materiales y Métodos: Análisis retrospectivo de videos intraoperatorios de n=62 casos de hidrocefalia tratados durante el período 2015-2020 en nuestro centro. Incluyéndose n=51 casos donde el PTV se pudo evidenciar claramente en las imágenes. Resultados: Identificamos 10 variedades de PTV de las 14 descriptas en la bibliografía; piso opaco 54,9%, parcialmente borrado 29,4%, intervalo prepontino reducido 23,5%, piso estrecho 17,6%, piso herniado 17,65%. En la hidrocefalia aguda prevalecieron el piso opaco (71,8%) y el piso estrecho (25%). En la crónica predominó el piso delgado (31,5%), el cual se observó exclusivamente en esta categoría. En los pacientes en que se realizó una TVE (43 de 51), el grado de dificultad quirúrgica fue 15 veces mayor en los que presentaban un piso opaco (p 0,00041) y menor en los que presentaban un piso parcialmente borrado (OR de 0,0615; p 0,00092). Conclusión: Las variaciones anatómicas del tercer ventrículo son frecuentes en pacientes hidrocefálicos. Las relacionadas al grosor del piso pueden aumentar el riesgo operatorio. Esta característica no es visualizable en los estudios prequirúrgicos, pero es posible predecirlo conociendo el tiempo de clínica de hidrocefalia y observando la disposición de los cuerpos mamilares en la resonancia(AU)
Background: Variants in the anatomy of the floor of the third ventricle (PTV) are determining factors in the limitations and complications of endoscopic third ventriculostomy (EVT). Objective: Describe anatomical variations of the PTV found in cases of hydrocephalus, evaluate incidence, stratify them according to time of evolution and correlate them with surgical difficulty. Methods: Retrospective analysis of intraoperative videos of n=62 cases of hydrocephalus treated during the period 2015-2020 in our center. Including n=51 cases where the PTV could be clearly evidenced in the images. Results: We identified 10 PTV varieties out of the 14 described in the bibliography; opaque floor 54.9%, partially erased 29.4%, reduced prepontine interval 23.5%, narrow floor 17.6%, herniated floor 17.65%, thin floor 11.76%. In acute hydrocephalus, the opaque floor (71.8%) and the narrow floor (25%) prevailed. In the chronicle, the thin floor predominated (31.5%), which was demonstrated exclusively in this category. In the patients who underwent an EVT (43 of 51), the degree of surgical difficulty was 15 times greater in those who presented an opaque floor (p < 0.00041) and less in those who presented an opaque floor. partially erased (OR of 0.0615; p 0.00092). Conclusion: Anatomical variations of the third ventricle are frequent in hydrocephalic patients. Those related to the thickness of the floor can increase the surgical risk. This characteristic cannot be seen in pre-surgical studies, but it is possible to predict it knowing the time of the hydrocephalus clinic and observing the disposition of the mammillary bodies in the cranial resonance(AU)

Breed traits seem to influence the dimensions of the cerebral ventricles in dogs. The ratios between the ventricles and the brain are crucial diagnostic criteria for suspected canine cognitive dysfunction (CCD). This study aimed to establish linear computed tomography (CT)-scan measurements of the cerebral ventricles in 55 Poodle dogs aged >7 years. To this end, cross-sectional CT images were evaluated. The measurements in the whole sample were: height of the right ventricle, 6.0 ± 1.6 mm; height of the left ventricle, 5.8 ± 1.6 mm; width of the right ventricle, 6.9 ± 1.4 mm; width of the left ventricle, 7.0 ± 1.3 mm; height of the third ventricle, 3.4 ± 0.8 mm; height of the right cerebral hemisphere, 39.5 ± 2.0 mm; and height of the left cerebral hemisphere, 40.2 ± 2.6 mm. The average ventricular measurements were higher in dogs older than 11 years (p < 0.05). However, the average ratio of the ventricle height to the height of the brain did not reveal differences between age groups, sex, or antimeres. In addition, none of the images showed fused lateral ventricles. Thus, these data can assist in interpreting ventricle size in senile Poodle dogs (aged >7 years).
Características raciais parecem influenciar as dimensões dos ventrículos cerebrais em cães. As proporções entre os ventrículos e o cérebro são cruciais para os casos suspeitos de disfunção cognitiva canina (DCC). Este estudo objetivou estabelecer medidas lineares dos ventrículos cerebrais de 55 cães da raça Poodle com idade acima de sete anos. Para tal, foram avaliadas imagens de cortes transversais obtidas por tomografia computadorizada. As medidas na amostragem total foram: altura do ventrículo direito 6,0 ± 1,6 mm, altura do ventrículo esquerdo 5,8 ± 1,6 mm, largura do ventrículo direito 6,9 ± 1,4 mm, largura do ventrículo esquerdo 7.0 ± 1,3 mm, altura do terceiro ventrículo 3,4 ± 0,8 mm, altura do hemisfério cerebral direito 39,5 ± 2,0 mm e altura do hemisfério cerebral esquerdo 40,2 ± 2,6 mm. As médias das medidas ventriculares foram maiores em cães acima de 11 anos de idade (p < 0,05). Contudo, a relação entre altura ventricular e altura cerebral não revelou diferenças entre grupos etários, sexo ou antímeros. Além disso, nenhuma imagem revelou ventrículos fusionados. Assim, os dados do presente estudo podem auxiliar na interpretação das dimensões ventriculares em cães da raça Poodle acima dos sete anos de idade.

Craniopharyngioma is a rare embryonic malformation, usually benign, of the sellar or parasellar regions. In this study, an uncommon case of third ventricle craniopharyngioma and a succinct review of its prevalence are presented. Even with low mortality rates, these injuries involve a high degree of endocrinological, visual, and neuropsychological morbidities, which have a huge impact on the patient's quality of life. Thus, surgical resection is the preferred therapy for tumors that compromise the flow of cerebrospinal fluid. However, due to the location of the craniopharyngioma, surgical management is accompanied by the risk of injury to important adjacent areas with postoperative repercussions. Therefore, the neurosurgeon's experience and the choice of the best surgical approach, are crucial for increasing prognosis.

BACKGROUND: After kidney transplantation neurologic manifestations may develop, including Parkinson's disease (PD). An enlarged substantia nigra (SN) by transcranial sonography has been recognized as a marker of PD. METHODS: In renal transplant recipients (RTRs = 95) and controls (n = 20), measurement of mesencephalon, SN, third ventricle, spleen and carotid intima-media thickness (cIMT) and middle cerebral artery (MCA), kidney and spleen arteries Doppler resistive index (RI) were performed. RESULTS: RTRs had larger SN, third ventricle and cIMT and higher renal RI than controls. The SN was larger in the CNIs group than in controls and rapamycin group, while the third ventricle was similar between patients but larger than in controls. In RTRs, SN showed a direct linear correlation with spleen and the third ventricle with age, cIMT and RI of the MCA, kidney and spleen. In CNIs group the SN correlated positively with age and cIMT, while the third ventricle reproduced RTRs correlations. Rapamycin group showed a direct linear relationship between the third ventricle and age and RI of the MCA, kidney and spleen; SN showed no correlations. CONCLUSION: RTRs on CNIs present a larger SN area than on rapamycin, probably due to the antiproliferative effect of rapamycin. This finding might be relevant when interpreting TCS in RTRs.

GNPDA2 has been associated with human obesity and type-2 diabetes by using a GWAS approach. GNPDA2 is an enzyme involved in the hexosamine biosynthesis pathway, which is known to be important for nutrient sensing in various organism. Its counter enzyme, GFAT, has previously been shown to be important to the development of insulin resistance in diabetes. The implication of GNPDA2 and GFAT in metabolism is scarce and the effect of both enzymes over appetite and glucose homeostasis is unknown. Aim: Identify the role of GNPDA2 and GFAT in nutrient sensing circuits of the CNS that are important for the regulation of both appetite and glucose homeostasis. Methods: Using Long Evans rats, we administered either a GNPDA2 or GFAT antagonist or vehicle in i3vt. Key Findings: GNPDA2 is highly expressed in hypothalamus and adipose tissue, followed by muscle and liver. GNPDA2 is expressed in different hypothalamic nuclei (ARC, DMH, LHA, PVN). GNPDA2 is downregulated in hypothalamus under diet-induced obesity (as previously described), but GFAT expression does not change. Moreover, i3vt infusion of GNPDA2 or GFAT inhibitor resulted in increased c-Fos in areas related to appetite and glucose homeostasis control as PVN and DMH and to a lesser extent in the LHA and ARC. Central inhibition of GNPDA2 does not alter either acute food intake or body weight; however, GFAT inhibition diminished appetite and body weight due to visceral illness. In addition, central administration of the GNPDA2 antagonist, prior to an intraperitoneal glucose tolerance test, resulted in glucose intolerance in comparison to vehicle without altering insulin levels. Significance: These results suggest that central GNPDA2 does not control appetite, but regulates glucose homeostasis.

Cavernous malformations of the third ventricle are rare, deep-seated lesions that pose a formidable surgical challenge due to the rich, surrounding anatomy. Despite the potential morbidity of surgical treatment, the possibility of catastrophic, spontaneous hemorrhage in this location is even more feared and aggressive treatment is warranted, especially if the patient had suffered previous hemorrhages and is currently symptomatic. We demonstrate this approach (Video 1) on a 16-year-old boy who presented with right-sided hemiparesis (power grade 4), intense headaches, difficulties with learning and concentration, and memory loss, mainly affecting short-term memory. The patient had a previous unsuccessful excision at another center 3 months after initial hemorrhage. The absence of hydrocephalus and medial thalamic location favored a modified transcallosal transchoroidal (or subchoroidal) approach. Due to the anatomy of the lesion, no other microsurgical approaches were considered. The surgery at our center (second attempt) was performed 5 months after initial hemorrhage. The head was placed in neutral position, with a slight elevation of the vertex and the midline in a vertical position. A callosotomy had already been performed during the patient's first excision attempt at another center. Although dissection through the tela choroidea is commonly performed medially to the choroidal fissure when one wants to enter the third ventricle, we chose to carefully dissect through this structure laterally, because this thalamic lesion extended almost into the ependymal surface of the third ventricle. This way, the choroidal plexus became a protective cushion for the fornix. On entering the third ventricle, a mulberry-like lesion was readily identified and the cavernoma was located. The central contents of the cavernoma were dissected initially, causing relative deflation of the lesion and more maneuverability to dissect it away from the surrounding structures. Neuromonitoring was used to avoid brainstem injury. Postoperative magnetic resonance imaging showed complete resection with no signs of hemorrhage or ischemia. The patient was discharged on postoperative day 5 with no new neurologic deficits. The patient was also able to return to school after 1 month and showed complete recovery. Unfortunately, neuropsychologic evaluation was unavailable to understand his improvement better. In this paper, the authors discuss the embryology and anatomy of the choroidal fissure, as well as the pathophysiology and treatment of cerebrospinal fluid cysts of this structure. Understanding its anatomical relations to nearby structures plays an essential role during brain surgeries. With the advancement and availability of imaging techniques, lesions of the choroidal fissure are often found incidentally. Patients are usually asymptomatic or exhibit symptoms that do not correlate with anatomical location or do not require surgical treatment. The choroidal fissure is a key landmark used during brain surgery. Therefore, a comprehensive understanding of it and nearby anatomical structures is essential. Choroidal fissure cysts can be found incidentally, and well-known key features will allow one to differentiate them from other lesions. Surgical treatment should be reserved for symptomatic patients while asymptomatic patients should be monitored.

Colloid cysts of the third ventricle are rare lesions. To our knowledge, only 23 familial cases of colloid cysts have been reported in the literature. The country of origin of the patients with familial cases had not been previously studied as a group. A 49-year-old female patient from Puerto Rico and her 21-year-old daughter underwent surgical resection for colloid cysts within a period of 5 years. The daughter presented with symptomatic hydrocephalus, while the mother only had mild chronic headaches. The occurrence of a colloid cyst in this family prompted us to perform a literature review and tabulate all the familial cases. This report presents the 24th case of a familial colloid cyst, and the fourth involving a mother and daughter. Australia is the country with the largest amount of reported cases. For smaller countries such as Sweden and Finland, two cases had been reported for each of them. Due to the unlikely probability of familial colloid cyst occurring at random, a genetic component is likely to be involved. The occurrence of several reports from patients from Australia, Finland, Sweden, and Puerto Rico where the population is smaller or more segregated may also suggest a genetic inheritance. Screening of first-degree-related subjects is recommended for families in which two or more members are affected. The presence of a colloid cyst in a twin mandates neuroimaging in the other twin, as there are five familial cases in twins reported in the literature.

Central and peripheral mechanisms that modulate energy intake, partition and expenditure determine energy homeostasis. Thyroid hormones (TH) regulate energy expenditure through the control of basal metabolic rate and thermogenesis; they also modulate food intake. TH concentrations are regulated by the hypothalamus-pituitary-thyroid (HPT) axis, and by transport and metabolism in blood and target tissues. In mammals, hypophysiotropic thyrotropin-releasing hormone (TRH) neurons of the paraventricular nucleus of the hypothalamus integrate energy-related information. They project to the external zone of the median eminence (ME), a brain circumventricular organ rich in neuron terminal varicosities and buttons, tanycytes, other glial cells and capillaries. These capillary vessels form a portal system that links the base of the hypothalamus with the anterior pituitary. Tanycytes of the medio-basal hypothalamus express a repertoire of proteins involved in transport, sensing, and metabolism of TH; among them is type 2 deiodinase, a source of 3,3',5-triiodo-L-thyronine necessary for negative feedback on TRH neurons. Tanycytes subtypes are distinguished by position and phenotype. The end-feet of ß2-tanycytes intermingle with TRH varicosities and terminals in the external layer of the ME and terminate close to the ME capillaries. Besides type 2 deiodinase, ß2-tanycytes express the TRH-degrading ectoenzyme (TRH-DE); this enzyme likely controls the amount of TRH entering portal vessels. TRH-DE is rapidly upregulated by TH, contributing to TH negative feedback on HPT axis. Alterations in energy balance also regulate the expression and activity of TRH-DE in the ME, making ß2-tanycytes a hub for energy-related regulation of HPT axis activity. ß2-tanycytes also express TRH-R1, which mediates positive effects of TRH on TRH-DE activity and the size of ß2-tanycyte end-feet contacts with the basal lamina adjacent to ME capillaries. These end-feet associations with ME capillaries, and TRH-DE activity, appear to coordinately control HPT axis activity. Thus, down-stream of neuronal control of TRH release by action potentials arrival in the external layer of the median eminence, imbricated intercellular processes may coordinate the flux of TRH into the portal capillaries. In conclusion, ß2-tanycytes appear as a critical cellular element for the somatic and post-secretory control of TRH flux into portal vessels, and HPT axis regulation in mammals.

BACKGROUND: The epidermoid cyst as a pineal region tumor is an infrequent pathology and with few descriptions in literature. Its prevalence in the third ventricle is 0,0042% of all intracranial tumors. To achieve a complete exéresis we used an endoscopic supracerebellar-infratentorial approach, surgical technique for pineal region approach. OBJECTIVES: The purpose of this work is to show the surgical technique of this novel approach for the treatment of a very infrequent disease. METHODS: 16 years old male patient with history of diabetes insipidus and chronic headaches (m-RS 1). Brain MRI showed a lesion with hyperintensity in T2WI and hypointensity in T1WI. It presents peripheral enhancement with gadolinium and restricted diffusion pattern in DWI. The pituitary stalk also showed enhancement with contrast. Germ cell tumor was a possible diagnosis but tumor cell markers were negative in CSF samples. We decided to make the purely endoscopic approach to the region to make a biopsy and a possible resection. Intraoperative pathology result informed an epidermoid tumor, so we continued with the complete exeresis of the tumor. RESULTS: We achieved a complete resection of a third ventricle epidermoid cyst with a purely endoscopic supracerebellar-infratentorial approach and a skull base rigid endoscope in a 16 years old male patient. The patient has recovered without any sequelae, headaches free but persistence of diabetes insipidus (m-RS 1).Conclusion: The purely endoscopic supracerebellar-infratentorial approach is a safe option to the surgical management of third ventricle pathologies, in this case, an epydermoid cyst.

Strictly intraventricular craniopharyngiomas are a rare topographical variety of craniopharyngiomas. The correct diagnosis is important in order to define the surgical planning, as the surgical access is different for suprasellar tumors with secondary invasion of the third ventricle. An image diagnosis may be difficult, though suggestive patterns exist. The aim of the present case report and literature review is to add to the scarce literature on strictly intraventricular craniopharyngiomas, as well as to remind the neurosurgeon of this rare diagnosis so that the proper treatment is provided.
Craniofaringiomas puramente intraventriculares constituem uma rara variedade topográfica dos craniofaringiomas. O diagnóstico correto é fundamental para a definição do plano cirúrgico, posto que o acesso a este tipo de tumor difere dos tumores suprasselares com invasão secundária do terceiro ventrículo. A confirmação por neuroimagem pode ser difícil, embora existam características sugestivas. A presente descrição de caso, bem como a revisão de literatura, visa contribuir com a escassa literatura a respeito de craniofaringiomas puramente intraventriculares, além de remeter o neurocirurgião a este diagnóstico raro para a adoção da conduta correta de tratamento.

Introducción: los quistes coloideos (QC) son tumores benignos de crecimiento lento, que comprenden menos del 1% de los tumores intracraneales. Se presentan en adultos jóvenes y se ubican más frecuentemente en el techo del tercer ventrículo. El objetivo de este trabajo es presentar una serie de pacientes con QC del tercer ventrículo operados por vía endoscópica, analizar la técnica quirúrgica, ventajas y desventajas. Desarrollo: se realizó una búsqueda retrospectiva de pacientes operados por vía endoscópica, en el Servicio de Neurocirugía del Hospital Italiano de Buenos Aires, de tumores del tercer ventrículo en un período de 2 años (2013-2015), con diagnóstico de QC confirmado por anatomía patológica . Se identificaron cinco pacientes, tres mujeres y dos hombres, cuyo promedio de edad fue de 50 años. No hubo complicaciones perioperatorias y ninguno mostró recidiva en el lapso de observación. Conclusión: la vía endoscópica es una vía técnicamente simple y con muy baja morbilidad. Si bien no siempre puede realizarse una exéresis completa, los trabajos prospectivos permitirán definir si esto resulta suficiente para el control de la enfermedad. (AU)
Colloid cysts are benign, slow-growing tumors, comprising less than 1% of intracranial tumors. They occur in young adults and are more frequently located on the roof of the third ventricle. The objective of this study is to present a series of patients with Colloid cysts operated endoscopically and analyze advantages and disadvantages of this surgical technique. We performed a retrospective review of Colloid Cysts operated on endoscopically, at the Neurosurgical Department of Hospital Italiano de Buenos Aires in a period of 2 years (2013-2015). Five patients were identified, three women and two men whose average age was 50 years. No perioperative complications were observed, with no recurrences during the follow up period. Conclusion: the endoscopic approach is technically simple and has very low morbidity. Although a complete excision can not always be performed, prospective studies will allow us to define whether if is sufficient to control the disease. (AU)

BACKGROUND: Intraventricular cavernous malformations are unusual intracranial vascular malformations; their deep anatomical location complicates their surgical management. Microsurgical approaches are the gold standard approaches for the resection of ventricular lesions, however, they imply considerable neurovascular risks. CASE DESCRIPTION: A 51-year-old patient presented with acute headache, diplopia, vertigo, blurred vision, and a depressed level of consciousness. A ventricular hemorrhage was treated with a ventriculostomy and the patient was discharged without hydrocephalus. After 11 days, he developed ataxia, diplopia, and a depressed level of consciousness. The patient was diagnosed with hydrocephalus secondary to the previous third ventricle hemorrhage. An endoscopic exploration using a 30° rigid ventricular endoscope was performed; after the third ventriculostomy, an intraventricular cavernous malformation located on the floor of the third ventricle and the aqueduct of Sylvius was resected. CONCLUSIONS: Three days after the surgery, magnetic resonance imaging demonstrated a gross total resection and adequate third ventriculostomy flow. One year after the surgery, the patient was asymptomatic. Neuroendoscopy has evolved towards minimally invasiveness, and in selected cases is an equally effective surgical approach to ventricular lesions. It provides minimal cerebral cortex disruption and vascular manipulation.

Introduction: Normal aging is associated with morphological alte-rations in brain. Ventricular system is located deep inside brain and reflect the overall process of parenchymal atrophy. Once neurode-generative disorders course with more prominent dilatation of brain ventricles, to establish normative volumetric parameters from Brazi-lian healthy old individuals is necessary, and it may be an additional tool on differentiation from the normal to pathological. Objective: To investigate brain ventricular volume changes in Brazilian healthy el-derly people. Methods: Transversal study using magnetic resonance imaging (1.5T) of the brain from 21 elderly healthy volunteers (67±6 years old). Data were assessed with manual segmentation techni-que. Regions of interest were the brain ventricles and intracranial volumes. Old (60-69 years old, 15 women) and Older (>69 years old) groups were created for analysis. Results: Volume of all ventricular compartments significantly increased (p<.001) with age, with excep-tion of the fourth ventricle. The third and lateral ventricles increased between groups 2.1- and 2.8-fold, respectively. Mean total ventricular volume was 1.2±.4% of intracranial volume in Old and 3.2±1.8% in Ol-der group (p<.001), which represents 15±6ml and 40±24ml (p=.001), respectively. We observed a moderate to strong positive correlation between ventricular volume and age, with the best correlation in the third ventricle (r=.710). Total intracranial volume diminished with age, but without statistical significance. Conclusions: Brain ventricles vo-lume increased significantly with age in healthy old individuals, with exception of the fourth ventricle. (AU)
Introdução: O envelhecimento normal está associado a alterações morfológicas do cérebro. O sistema ventricular está localizado pro-fundamente no encéfalo e reflete o processo global de atrofia do pa-rênquima. Uma vez que doenças neurodegenerativas cursam com dilatação mais proeminente dos ventrículos cerebrais, estabelecer parâmetros volumétricos de normalidade em nossa população idosa saudável se faz necessário, podendo ser uma ferramenta a mais para diferenciar o normal do patológico. Objetivo: Investigar alterações volumétricas dos ventrículos cerebrais em brasileiros idosos sau-dáveis. Métodos: Estudo transversal com imagens de ressonância magnética (1,5T) do encéfalo de 21 idosos saudáveis (68±6 anos, 15 mulheres). Os dados foram examinados por técnicas de segmenta-ção manual. As regiões de interesse foram os ventrículos cerebrais e o volume intracraniano. Criamos os subgrupos Idosos (60-69 anos) e Mais idosos (>69 anos) para a análise. Resultados: O volume de todos os ventrículos aumentou com a idade (p<0,001), com exceção do quarto ventrículo. O terceiro e os ventrículos laterais aumentaram 2,1 e 2,8 vezes, respectivamente, entre os grupos. O volume ventri-cular médio foi de 1,2±0,4% do volume intracraniano nos Idosos e de 3,2±1,8% nos mais idosos, o que representa 15±6ml e 40±24ml, respectivamente. Observamos correlação positiva de moderada a forte entre volume ventricular e idade, com a melhor correlação no terceiro ventrículo (r=0,710). O volume intracraniano diminui com a idade, sem significância estatística. Conclusão: os ventrículos cere-brais aumentam significativamente com o envelhecimento em idosos saudáveis, exceto o quarto ventrículo. (AU)

Chordoid glioma is a rare tumor of the third ventricle whose imaging features are difficult to distinguish from other more common lesions in this location. There are only 83 cases described so far in the literature. Although gross total resection (GTR) is the treatment of choice, immediate postoperative mortality with this approach can be as high as 29%, and morbidity among survivors can reach 67%. We report a case of a male patient of advanced age, with a third ventricle mass lesion, who presented with a progressive right temporal hemianopia. Imaging was compatible with craniopharyngioma, meningioma or even metastasis. Chordoid glioma was not considered in the differential diagnosis. The patient underwent surgery and GTR was achieved. There were no postoperative complications, and the patient was discharged from the hospital three weeks later. Unexpectedly, two days afterwards, he suffered a major brainstem hemorrhagic stroke and, unfortunately, died.
O glioma cordoide é um tumor raro do terceiro ventrículo, e as suas características imagiológicas são difíceis de distinguir de outras lesões mais comuns nesta localização. Até a data presente, existem apenas 83 casos de gliomas cordoides descritos na literatura. A remoção macroscópica total destes tumores deve ser o tratamento de escolha; no entanto, a mortalidade pós-operatória imediata pode chegar aos 29%, e a morbilidade pode atingir os 67% entre os sobreviventes. Nós descrevemos o caso de um homem idoso com uma lesão tumoral no terceiro ventrículo, que se manifestou com uma hemianopsia temporal direita progressiva. Os exames de imagem eram compatíveis com craniofaringioma, meningioma ou até metástase. O glioma cordoide não foi considerado como uma das hipóteses no diagnóstico diferencial inicial. O paciente foi submetido a cirurgia, tendo-se obtido a remoção macroscópica total. Não houve qualquer complicação no período pós-operatório, e o paciente teve alta hospitalar após três semanas. Inesperadamente, dois dias após a alta clínica, o paciente sofreu um AVC hemorrágico do tronco cerebral, e acabou por falecer.

O acesso ao terceiro ventrículo constitui verdadeiro desafio ao neurocirurgião. Nesse contexto, estudos anatômicos e morfométricos são úteis para estabelecer as limitações e as vantagens de determinado acesso cirúrgico. O acesso transcorioideo é versátil e promove exposição adequada da região média e posterior do terceiro ventrículo. Entretanto, a coluna do fórnice limita a exposição da região anterior do terceiro ventrículo. Há evidências de que a secção ipsilateral da coluna do fórnice tenha pouca repercussão na função cognitiva. Esta tese compara a exposição anatômica proporcionada pelo acesso transforniceal transcorioideo com o do acesso transcorioideo e realiza avaliação morfométrica de estruturas relevantes e comuns aos dois acessos. Material e métodos: A exposição anatômica proporcionada pelos acessos transcaloso transcorioideo e transcaloso transforniceal transcorioideo foram comparadas em oito cadáveres não submetidos à conservação, utilizando o sistema de neuronavegação (Artis, Brasília, Brasil), para aferir a área de trabalho, a área de exposição microcirúrgica, a exposição angular no plano longitudinal e transversal de dois alvos anatômicos (túber cinéreo e aqueduto cerebral). Adicionalmente, foram quantificados a espessura do parênquima do lobo frontal direito, a espessura do tronco do corpo caloso, o diâmetro longitudinal do forame interventricular, a distância de trabalho da superfície cortical ao túber cinéreo e a distância de trabalho da superfície cortical até o aqueduto cerebral. Os valores obtidos foram submetidos a análise de estatística utilizando o teste de Wilcoxon. Approaches to the third ventricle constitute a formidable challenge to the neurosurgeon and, in this context, anatomical and morphometric studies are useful to establish the limitations and advantages of certain surgical approaches. The transchoroidal approach is a versatile one that promotes adequate exposure of the middle and posterior regions of the third ventricle. However, the column of fornix limits the exposure of the anterior third ventricle region. There is evidence that the ipsilateral section of the column of fornix has little effect on the cognitive function. This thesis compares the anatomical exposure using the transchoroidal transforniceal technique with the transchoroidal approach, and performs morphometric assessment of relevant structures common to both approaches. Material and methods: The anatomical exposure achieved through the transchoroidal transcallosal approach and transchoroidal transforniceal transcallosal were compared in 8 fresh cadavers using the neuronavigation system (Artis, Brasilia, Brazil), to assess the working area, microsurgical exposure area, to quantify the angular exposure in the longitudinal and cross-sectional planes to two anatomical targets (tuber cinereum and cerebral aqueduct), to measure the thickness of the right frontal lobe parenchyma, corpus callosum body thickness, longitudinal diameter of the interventricular foramen, working distance from the cortical surface to the tuber cinereum and working distance from the cortical surface to the cerebral aqueduct. The values obtained were submitted to statistical analysis using Wilcoxon's test. INTRODUCTION: Solitary fibrous tumors are mesenchymally derived masses most commonly originating from the lung pleura. CASE REPORT: Herein, we report a 6-month-old presenting with syndrome of inappropriate antidiuretic hormone secretion (SIADH) and a suprasellar mass. The mass proved to be a solitary fibrous tumor. This case and salient literature are reviewed. CONCLUSIONS: To our knowledge, this is the youngest patient to be described with a mass of this type within the central nervous system.

OBJETIVO: Desenvolver peças anatômicas que simulem uma condição real de ventriculomegalia para serem utilizadas como uma ferramenta no treinamento dos neurocirurgiões nas técnicas de neuroendoscopia e viabilizar estudo anatômico dos ventrículos. MÉTODO: Foram utilizadas vinte peças anatômicas de encéfalo de cadáveres de indigentes, com a aprovação do Comitê de Ética em pesquisa da FMUSP sob o número 046/10. As peças foram retiradas da base do crânio com a persistência da superfície óssea (parte da calvária) para serem submetidas aos seguintes procedimentos: canulação do IV ventrículo por meio da abertura mediana do IV ventrículo (forame de Magendie); tomografias pré-experimento e injeção de água destilada no sistema ventricular. A água injetada estava à temperatura ambiente e os cérebros foram resfriados até 4º C e, após 12 horas, foram congelados a uma temperatura de 0º C (no estado sólido) por 24 horas. Esses procedimentos foram realizados na frequência de três vezes. Após o experimento, foram realizadas tomografias pós-experimento e procedimentos neuroendoscópicos ventriculares. Foram excluídos encéfalos com lesões traumáticas ou antecedentes de enfermidades transmissíveis. Não foram critérios de exclusão o sexo e a idade. Foram avaliadas, nas imagens tomográficas, a variação pré e pós-experimento dos seguintes parâmetros: coeficiente corno frontal/diâmetro interno; índice de Evan; e tamanho do corno temporal. As análises estatísticas foram realizadas no programa SPSS (Statistical Package for the Social Sciences) versão 13, para ambos os grupos. RESULTADOS: A avaliação da relação Corno frontal/Diâmetro interno, antes e pós-experimento apresentou média de 11,98% e 19,46%, respectivamente. Estudo estatístico (t Student) mostrou diferença estatística (t= -5142, gl =19; p < 0,01). PURPOSE: To develop anatomical models which simulate real conditions of ventriculomegaly and to use them as tools to train neuroendoscopic techniques and allow the study of the ventricles. METHODS: A total of twenty brains, with the approval of the Ethics in Research Committee from FMUSP (046/10) were used to perform this research. The brains were separated from the skull base, but keeping part of the calvaria, and then underwent the following procedures: cannulation of the fourth ventricle through the median open of the fourth ventricle (foramen of Magendie); CT scans performed before the experiment; and then injection of distilled water into the ventricular system. The water was injected at room temperature, and then the brains were cooled to 4ºC. After 12 hours, they were then frozen at 0ºC for 24 hours.These procedures were repeated three times. After the experiment,CT scans were performed after the injections and neuroendoscopic procedures. Brains, which had traumatic injuries or history of infectious diseases, were excluded. Gender and age were not exclusion criteria. The statistical analysis was performed with the SPSS program (Statistical Package for the Social Sciences) version 13, for both groups. To assess the variation of the frontal horn (FH)/internal diameter (ID) coefficient, such as Evan's index's variation, and to analyze the temporal horn (TH) size, thet-Student test was used. RESULTS: The results of the t-Student test showed that the FH/ID, with an average of 11.98% before the experiment, had significant difference (t = -5.142, gl = 19; p < 0.01) after the experiment, with an average of19.46%. The management of hydrocephalus associated with posterior fossa tumors in children has always been controversial. Studies show that 71%-90% of children with posterior fossa tumors already presented hydrocephalus on admission. Of these cases, 10%-62% will have persistent hydrocephalus after surgical treatment of the tumor. The shunt, although it is an important alternative for preoperative treatment in these cases is not without complications. Among the numerous complications identified by the literature, we highlight the most rare, intratumoral hemorrhage, upward transtentorial herniation and dissemination of neoplastic cells by the peritoneum. Medulloblastoma is considered the most common malignant pediatric tumor, comprising 15%-20% of all intracranial tumors in childhood. In addition, 5%-6% of primary or recurrent medulloblastomas may be associated with spontaneous bleeding and rapid deterioration. The bleeding presents with tumor swelling, extending to the ventricular system through the tumor capsule, increased intracranial pressure and upward transtentorial herniation. Tumoral hemorrhage associated with ventricular drainage for treatment of hydrocephalus in patients with medulloblastoma represents an extremely rare phenomenon. The endoscopy third-ventriculostomy (ETV) is known to be very useful in the treatment of intracranial hypertension preoperatively and prevent persistent postoperative hydrocephalus in cases associated with posterior fossa tumors, with efficacy superior to traditional ventricular shunts. We describe a case of intratumoral hemorrhage and upward transtentorial herniation associated with endoscopic third-ventriculostomy (ETV).
O manejo da hidrocefalia associada a tumores de fossa posterior em crianças sempre foi controverso. Trabalhos mostram que 71%-90% das crianças com tumores de fossa posterior já se apresentam hidrocefálicos na admissão. Desses casos, 10%-62% terão hidrocefalia persistente após o tratamento cirúrgico do tumor. A derivação ventricular, embora consista em importante alternativa de tratamento pré-operatório nesses casos, não está isenta de complicações. Dentre as inúmeras complicações apontadas pela literatura atual, particularmente nos casos de meduloblastoma que necessitaram de derivação ventricular por hidrocefalia, destacamos as mais raras: hemorragia tumoral, herniação transtentorial ascendente e disseminação de células neoplásicas pelo peritônio. O meduloblastoma é considerado o tumor pediátrico maligno mais comum, compreendendo 15%-20% de todas as neoplasias intracranianas na infância. Além disso, 5%-6% dos meduloblastomas primários ou recorrentes podem estar associados à hemorragia espontânea e rápida deterioração. A hemorragia cursa com aumento de volume do tumor, extravasamento para o sistema ventricular através da cápsula tumoral, aumento da pressão intracraniana e herniação transtentorial ascendente. Hemorragia tumoral associada à drenagem ventricular, para tratamento de hidrocefalia em portadores de meduloblastoma, representa um fenômeno extremamente raro. A terceiroventriculostomia endoscópica (TVE) é reconhecida por ser bastante útil no tratamento da hipertensão intracraniana no pré-operatório e por prevenir a hidrocefalia persistente pós-operatória nos casos associados a tumores de fossa posterior, com eficácia superior às derivações ventriculares tradicionais. Descrevemos um caso de hemorragia tumoral e herniação transtentorial ascendente associados a terceiro-ventriculostomia endoscópica (TVE).
