Resumo

Purpose: To evaluate the viability of the porcine vas deferens as a realistic microsurgical training model for vasectomy reversal Methods: The model uses swine testicles (vas deferent), which are usually discarded in large street markets since they are not part of Brazilian cuisine. The spermatic cord was carefully dissected, and the vas deferens were isolated, measuring 10 cm in length. A paper quadrilateral with 5 cm2 was built to delimit the surgical training field. The objective of the model is to simulate only the microsurgical step when the vas deferens are already isolated. The parameters analyzed were: feasibility for reproducing the technique, patency before and after performing the vasovasostomy, cost of the model, ease of acquisition, ease of handling, execution time, and model reproducibility. Results: The simulator presented low cost. All models made were viable with a texture similar to human, with positive patency obtained in 100% of the procedures. The internal and external diameters of the vas deferens varied between 0.2-0.4 mm and 2-3 mm, respectively, with a mean length of 9 ± 1.2 cm. The total procedure time was 43.28 ± 3.22 minutes. Conclusions: The realistic model presented proved to be viable for carrying out vasectomy reversal training, due to its low cost, easy acquisition, and easy handling, and providing similar tissue characteristics to humans.

Resumo

A terapia com moxabustão envolve combustão de uma mistura de ervas, composta predominantemente por Artemisia vulgaris. Pode ser aplicada em diversas patologias sendo de grande utilidade para cicatrização de tecidos. O objetivo deste artigo foi abordar o uso da moxabustão em dois cães com feridas contaminadas, em que os tratamentos foram destinados a promover a cicatrização por segunda intenção. É descrito o caso de uma cadela da raça pastor-alemão com deiscência de pontos (caso 1) e o de uma cadela sem raça definida com ferida traumática por mordedura (caso 2). Nos dois casos, o tratamento com a moxabustão, modalidade da Medicina Tradicional Chinesa (MTC), foi efetivo no tratamento das feridas por segunda intenção, melhorando os aspectos de oxigenação, circulação e revitalização de tecidos. Após dois anos no caso 1, o tecido apresenta-se pigmentado e alopécico; no caso 2, a epiderme encontra-se queratinizada, uniforme e com presença de pelos, em menor dimensão há tecido pigmentado.(AU)


Moxibustion therapy involves burning a mixture of herbs, consisting predominantly of Artemisia vulgaris. It can be applied in several pathologies and is very useful for tissue healing. The aim of this article was to present the use of Moxibustion in two dogs with contaminated wounds, whose treatment aimed to achieve healing by second intention. It is presented the case of a German Shepherd dog with dehiscence of points (case 1) and one of a mixed breed dog with traumatic wound by bite (case 2). In both cases, the treatment with Moxibustion, a modality of Traditional Chinese Veterinary Medicine, demonstrated effectiveness in the treatment of wounds by secondary intention, improving aspects of oxygenation, circulation and tissue revitalization. After two years in case 1 the tissue is pigmented and alopecic; in case 2 the epidermis is keratinized, uniform and with the presence of hairs, in a smaller dimension there is pigmented tissue.(AU)

Resumo

A hipospadia é oresultado de falha no desenvolvimento em que não ocorre a fusão normal das dobras genitais e intumescência genital, o que causa desenvolvimento anormal da uretra peniana, do pênis, prepúcio e/ou escroto. O objetivodeste relato é descrever um caso de hipospadia peniana de um canino macho, sem raça definida (SRD), de oito meses deidade, atendido no Hospital Veterinário da Universidade Federal Rural da Amazônia, na cidade de Belém/Pará, detalhando os procedimentos cirúrgico e anestésico utilizados para resolução do caso. Ao chegar ao hospital, o animal foi submetido ao exame físicoinicial no qual observou-se uma deformidade na região perianal por onde drenava a urina e uma abertura na região prepucial. A partir disso, foi solicitada a uretrocistografia retrógrada para complemento diagnóstico da hipospadia e seu comprometimento ao sistema urinário interno. Diante disso, foi realizado procedimento cirúrgico reconstrutivo e orquiectomia terapêutica, alcançando permitindoa resolução do caso e a recuperação completa do animal. Considerando que essa deformidade impossibilita a capacidadesexual generandi, a resolução recomendada é a cirúrgica, na qual o prognóstico é bom, possibilitando qualidade de vida ao animal.(AU)


Hypospadias is the result of developmental failure where normal fusion of the genital folds and genital tumescence does not occur, which causes abnormal development of the penile urethra, penis, foreskin and/or scrotum. The objective of this report is to describe a case of penile hypospadias in an eight-month-old male mixed breed (SRD) canine, treated at the Veterinary Hospital of the Federal Rural University of Amazônia, in the city of Belém/Pará, detailing the procedures surgical and anesthetic used for resolution of the case. Upon arrival at the hospital, the animal underwent an initial physical examination in which a deformity was observed in the perianal region through which urine drained and an opening in the preputial region. From this, a retrograde urethrocystography was requested to complement the diagnosis of hypospadias and its commitment to the internal urinary system. In view of this, a reconstructive surgical procedure and therapeutic orchiectomy were performed, allowing the resolution of the case and the complete recovery of the animal. Considering that this deformity prevents the sexual capacity generandi, the recommended resolution is surgery, in which the prognosis is good, allowing quality of life for the animal.(AU)


La hipospadias es el resultado de una falla en el desarrollo donde no ocurre la fusión normal de los pliegues genitales y la tumescencia genital, lo que provoca un desarrollo anormal de la uretra peneana, el pene, el prepucio y/o el escroto. El objetivo de este relato es describir un caso de hipospadias peneana en un canino macho mestizo (SRD) de ocho meses de edad, atendido en el Hospital Veterinario de la Universidad Federal Rural de Amazônia, en la ciudad de Belém/Pará, detallando los procedimientos quirúrgicos y anestésicos utilizados para la resolución del caso. A su llegada al hospital, el animal fue sometido a un examen físico inicial en el que se observó una deformidad en la región perianal por donde salía la orina y una abertura en la región prepucial. A partir de ello se solicitó cistouretrografía retrógrada para complementar el diagnóstico de hipospadias y su afectación en el sistema urinario interno. Ante esto, se realizó un procedimiento quirúrgico reconstructivo y orquiectomía terapéutica, que permitió la resolución del caso y la recuperación completa del animal. Teniendo en cuenta que esta deformidad imposibilita la capacidadgeneradi sexual, la resolución recomendada es la cirugía, en la que el pronóstico es bueno, permitiendo calidad de vida para el animal.(AU)

Capybaras are the primary hosts of Amblyomma sculptum tick, vectors of Rickettsia rickettsia bacteria, and the zoonotic agent of Brazilian Spotted Fever (BSF). In this context, contraceptive methods have been suggested for population control in order to reduce the number of free-ranging capybaras cohabiting with humans in urban and rural areas and acting as disease amplifiers. To maintain the group's expected behavior and social hierarchy, sterilization techniques that preserve the gonads are recommended. On 126 female capybaras in the Brazilian state of São Paulo, a new surgical technique named "Passos Nunes" uterine horn ligature was performed after adequate general anesthesia. It achieved effective surgical sterilization, with an incision length of about 3 cm in the periumbilical linea alba, cranial to the pubis. After entering the abdominal cavity, the urinary bladder is pulled laterally to access the uterine horns and the cervix. The uterine horn is folded up, forming a strap; the distal portion of the strap is ligated and its distal end sectioned. The exact process is performed on the opposite horn. After the surgical procedure, the musculature is sutured in a sultan pattern and the subcutaneous tissue with a horizontal mattress pattern. The skin is sutured in a separate simple format, using nylon 2.0 for all steps. The wide exposure of the uterine horns facilitates the confirmation of pregnancy, allowing the surgeon to choose between salpingo hysterectomy or ligature of the uterine horns. The present study presents a new technique of surgical sterilization that can be used in female free-ranging mammals in which maintenance of the gonads is recommended, and births of offspring should not occur.(AU)

Resumo

Purpose: To evaluate fibrosis formation and number of macrophages in capsules formed around textured implants without and with mesh coverage. Methods: Fibrosis was analyzed through transforming growth factor-beta 1 (TGF-ß1) immunomarker expression and the number of macrophages through CD68 percentage of cells in magnified field. Sixty female Wistar rats were distributed into two groups of 30 rats (unmeshed and meshed). Each group was then subdivided into two subgroups for postoperative evaluation after 30 and 90 days. The p value was adjusted by Bonferroni lower than 0.012. Results: No difference was observed in fibrosis between meshed and unmeshed groups (30 days p = 0.436; 90 days p = 0.079) and from 30 to 90 days in the unmeshed group (p = 0.426). The meshed group showed higher fibrosis on the 90th day (p = 0.001). The number of macrophages was similar between groups without and with mesh coverage (30 days p = 0.218; 90 days p = 0.044), and similar between subgroups 30 and 90 days (unmeshed p = 0.085; meshed p = 0.059). Conclusions: In the meshed group, fibrosis formation was higher at 90 days and the mesh-covered implants produced capsules similar to microtextured ones when analyzing macrophages. Due to these characteristics, mesh coating did not seem to significantly affect the local fibrosis formation.

Resumo

Minimally invasive surgery represented a significant milestone in modern surgery; however, continuous innovation and the emergence of new technologies pose new challenges in terms of surgical learning curves since new interventions are associated with increased surgical complexity and a higher risk of complications. For this reason, surgeons are aware of the beneficial effects of "learning before doing" and the importance of safely implementing new surgical procedures in order to obtain better patient outcomes. Considered the largest Latin American training center in minimally invasive surgery, IRCAD Barretos, São Paulo, Brazil, makes it possible to acquire surgical skills through training in different and the most complex areas of medicine, providing the experience of real and simulated situations, with focus on innovation. The center possesses state-of-the-art infrastructure and technology, with a very high-level teaching staff and an affectionate and hospitable reception. Since its inauguration, in 2011, the center has already qualified numerous professionals and has placed the country in a privileged position in terms of surgical knowledge. The present article describes the activities developed over these ten years of the institute in Brazil as the largest training center for surgeons of the continent in order to address the importance of surgical skills training.

Purpose: To describe the use of endocavitary ultrasound probe as an auxiliary tool when performing partial nephrectomy in cases of endophytic renal tumors, to standardize the method, and to report the preliminary results achieved with this technique. Methods: Fifteen patients diagnosed with completely endophytic underwent partial nephrectomy with the use of an endocavitary ultrasound probe. This article describes the technique involved in partial nephrectomy and details the preparation of the endocavitary ultrasound probe to ensure its safe use. Results: All the patients had a RENAL score between 8 and 11. The median time of warm ischemia was 26 and 18 minutes for laparoscopic or robot-assisted surgery, respectively. The median duration of surgery was 150 minutes, and the median console time was 145 minutes for the laparoscopic and robot-assisted surgery groups, respectively. The median estimate of blood loss was 200 mL. Only three patients in the laparoscopic group had focal positive surgical margins. There were no cases of infection at the site of probe entry. Conclusions: Intraoperative use of an endocavitary ultrasound probe for partial nephrectomy is possible and a safe alternative to the excision of endophytic tumors when neither robotic probes nor laparoscopic probes are available.

Purpose: This study aimed to develop a minimally invasive surgical procedure for laminar lift and posterior cervical laminoplasty via the intermuscular approach using a canine model. Methods: Six Alaskan dogs were used for developing the surgical approach. The bilateral laminae of C3-7 were cut with an ultrasonic osteotome and fixed with bilateral plates to maintain the lamina lifting and reshape a wider spinal canal. The important structures, such as ligaments, supraspinous ligaments, interspinous ligaments, and ligamentum flavum were preserved. The therapeutic effect was evaluated by preoperative and postoperative imaging results and neck mobility. Results: The surgical procedures were all successfully performed in the 6 animals. All the dogs survived well within 1 year of postoperative follow-up. The postoperative neck mobility was as good as the preoperative one. Computed tomography results showed that the anteroposterior diameter of the spinal canal was successfully enlarged and maintained well. Conclusions: The minimally invasive surgical procedure for laminar lift and posterior cervical laminoplasty via the intermuscular approach was feasible in a canine model, which might be applied in clinical practice.

Pigmentary superficial keratitis (PSK) is a chronic corneal disorder with different causes, which may include immune-mediated diseases and reactions to ultraviolet rays. This study aimed to evaluate the use of automated central lamellar superficial keratectomy (ACLSK) in the treatment of chronic pigmentary superficial keratitis (CPSK) in dogs. We enrolled 24 animals with CPSK and loss of visual function even after clinical treatment with preserved post-corneal transparent media, and the potential for recovery of visual function after surgical treatment. The microkeratome was positioned on the eye surface, and the central corneal lamellae were obtained after translation. With ACSLC, 21 animals (87.5%) recovered their visual function. In 3 animals (12.5%), the formation of granuloma and posterior central leukoma in the cornea compromised the transparency of the visual axis. This study concludes that ACLSK is a viable surgical alternative for the restoration of visual function in dogs with severe PSK, thus improving their quality of life. The short execution time of this procedure reduces the costs and anesthetic risks.


A ceratite pigmentar superficial (CPS) é uma doença crônica da córnea, com diferentes causas, que podem incluir doenças imunomediadas e reação aos raios ultravioleta. Este estudo teve como objetivo avaliar a utilização da ceratectomia superficial lamelar central automatizada (CSLCA) no tratamento da ceratite superficial pigmentar crônica (CSPC) em cães. Vinte e quatro animais incluídos apresentaram CSPC e perda da função visual mesmo após o tratamento clínico, porém mantiveram os meios transparentes pós-córnea preservados e o potencial de recuperação da função visual após o tratamento cirúrgico. O microcerátomo foi posicionado na superfície do olho e, após o término de sua translação, foram obtidas as lamelas centrais da córnea. Com a CSLCA, 21 animais (87,5%) recuperaram a função visual. Em três animais (12,5%), a formação de granuloma e leucoma central posterior na córnea comprometeu a transparência do eixo visual. Este estudo conclui que a CSLCA é uma alternativa cirúrgica viável para o restabelecimento da função visual em cães com CPS grave, melhorando, assim, sua qualidade de vida. Seu curto tempo de execução proporciona redução de custos e de riscos anestésicos.

Os efeitos do trauma cirúrgico no organismo, os quais são amplamente estudados pela medicina humana e veterinária, resultam em uma complexa resposta neuroendócrina e imunológica, desencadeadas para restabelecer a homeostase e preservar a vida. A resposta metabólica ao trauma é caracterizada pelo aumento da secreção de hormônios hipofisários, ativação do sistema nervoso simpático e secreção de citocinas pró-inflamatórias. Essa resposta tem por objetivo manter a perfusão tecidual, controlar a proliferação de microrganismos e mobilizar substratos para garantir a integridade das funções celulares e promover reparação tecidual. Diferentes fatores podem influenciar a intensidade da resposta do organismo ao trauma, incluindo magnitude e duração dos estímulos, enfermidades ou eventos traumáticos pré-existentes, protocolos anestésicos e técnicas cirúrgicas utilizadas. Respostas metabólicas exacerbadas ocasionam complicações pós-operatórias, tempo prolongado de recuperação dos pacientes e óbito. Através da identificação dos principais eventos relacionados à resposta metabólica ao trauma cirúrgico, é possível atuar de forma a reduzir os danos associados, melhorando a qualidade e minimizando os riscos do período pós-operatório. Sendo assim, este trabalho tem como objetivo apresentar uma revisão descrevendo a fisiopatologia e as consequências da resposta metabólica ao trauma cirúrgico, em suas diferentes formas e origens, no período perioperatório.


The effects of surgical trauma on the organism, which are widely studied by human and veterinary medicine, result in a complex neuroendocrine and immune responses triggered to restore homeostasis and preserve life. The metabolic response to trauma is characterized by increased secretion of hypophyseal hormones, activation of the sympathetic nervous system, and secretion of pro-inflammatory cytokines. This response aims to maintain tissue perfusion, control the proliferation of microorganisms, and mobilize substrates to ensure the integrity of cellular functions and promote tissue repair. Different factors can influence the intensity of the organism's response to trauma, including magnitude and duration of stimuli, pre-existing infirmities or traumas, anaesthetic protocols, and surgical techniques applied. Exacerbated metabolic responses can cause post-operative complications, prolonged recovery time, and death. By identifying the main events related to the metabolic response to surgical trauma, it is possible to act to reduce the associated damages, improving quality and minimizing the risks of the postoperative period. Thus, the present paper aims to present a review describing the physiopathology and the consequences of the metabolic response to surgical trauma, in its different forms and origins, in the perioperative period.

Resumo

A pitiose é uma infecção invasiva ulcerativa piogranulomatosa causada pelo oomiceto Pythium insidiosum, parasita de plantas aquáticas em águas estagnadas. Apesar de não serem raros os relatos de pitiose nas espécies domésticas, a espécie equina é a mais afetada. A enfermidade pode ser adquirida através da colonização de lesões traumáticas e do folículo piloso. Em casos dessa infecção, pode-se encontrar hifas recobertas por células necróticas, formando massas branco-amareladas semelhantes a corais, denominadas de kunkers. As lesões são localizadas prioritariamente nas extremidades distais dos membros e na porção ventral da parede tóraco-abdominal. O diagnóstico da pitiose está relacionado a um prognóstico reservado dependendo do grau de comprometimento anatômico que a enfermidade se encontra. Diante do exposto, este trabalho teve como objetivo relatar um caso de pitiose cutânea em um equino, fêmea da raça Mangalarga Machador. O animal apresentava lesões ulcerativas no membro torácico esquerdo e no membro pélvico esquerdo. O tratamento escolhido foi o cirúrgico e imunoterápico, com intuito de remover todo o tecido lesionado e, posteriormente, coletar material para realização do exame histopatológico. O equino foi submetido a um protocolo pós-operatório com a terapia antitetânica por via intramuscular. A antibioticoterapia sistêmica foi realizada com penicilina benzatina, anti-inflamatório esteroidal, dexametasona foi realizada uma vez ao dia, durante cinco dias. Posteriormente, optou-se pelo uso do anti-inflamatório não esteroidal maxicam uma vez ao dia, durante cinco dias. O animal recebeu quatro doses do imunoterápico PITIUM-VAC por via subcutânea, com intervalo de 14 dias entre uma e outra aplicação, apresentando uma resposta satisfatória ao tratamento.


Pythiosis is an invasive ulcerative pyogranulomatous infection caused by the Oomycete Pythium insidiosum, a parasite of aquatic plants in standing water. Although reports of pythiosis in domestic species are not uncommon, equine species are the most affected. The disease can be acquired through the colonization of traumatic lesions and hair follicles. Hyphae covered by necrotic cells, forming yellowish-white coral-like masses called "kunkers" can be found. The lesions are located primarily on the distal extremities of the limbs and on the ventral portion of the thoracoabdominal wall. The diagnosis of pythiosis is linked to a poor prognosis, depending on the degree of anatomical involvement of the disease. Thus, this work aimed to report a case of cutaneous pythiosis in a female Mangalarga Machador equine. The animal presented ulcerative lesions on the left thoracic limb and the left pelvic limb. The chosen treatment was surgery and immunotherapy to remove all the injured tissue and, later, collect the material for histopathological examination. The horse was submitted to a post-operative protocol with intramuscular anti-tetanus therapy. Systemic antibiotic therapy with benzathine penicillin, steroidal anti-inflammatory, dexamethasone was performed once a day for five days. Subsequently, it was decided to use the non-steroidal anti-inflammatory maxicam once a day for five days. The animal received four doses of the immunotherapy PITIUM-VAC subcutaneously, with an interval of 14 days between applications, presenting a satisfactory response to the treatment.

Resumo

O prolapso uretral, o qual é incomum em cães, se caracteriza pela protusão da mucosa uretral além da extremidade do pênis e do orifício externo da uretra. Pode ocorrer por causas congênitas ou adquiridas, tendo maior incidência em animais jovens não castrados. O objetivo do presente trabalho é relatar um caso de prolapso uretral em um cão não castrado da raça American Bully Terrier, de dois anos, o qual foi atendido na Universidade Estácio de Sá localizada no Rio de Janeiro com histórico de lambedura excessiva do pênis, disúria, sangramento prepucial e protrusão da mucosa uretral após tentativa de cópula. Durante a avaliação física o animal apresentou todos os parâmetros clínicos dentro da normalidade, entretanto demonstrava agitação excessiva e dor na região afetada. Então, o diagnóstico foi definido por meio da inspeção direta da visualização da mucosa uretral protusa. Frente ao quadro clínico e físico do animal, foi adotado o procedimento cirúrgico de ressecção e anastomose da porção uretral prolapsada. Desta forma, conclui-se que esta técnica cirúrgica se mostrou eficaz, visto que o referido paciente apresentou ótima recuperação e cicatrização uretral, sem quaisquer sinais de complicações pós-cirúrgicas.


Urethral prolapse, which is uncommon in dogs, is characterized by protrusion of the urethral mucosa beyond the tip of the penis and the external orifice of the urethra. It can occur due to congenital or acquired causes, with a higher incidence in young animals not castrated. The present study aims to report a case of urethral prolapse in a non-castrated dog of 2 years old (American Bully Terrier), which was treated at the Estácio de Sá University located in Rio de Janeiro city, with a history of excessive penis licking, dysuria, prepucial bleeding, and protrusion of the urethral mucosa after an attempt of copulation. During the physical evaluation, the animal presented all clinical parameters within the normal range; however, it showed excessive agitation and pain in the affected region. Then, the diagnosis was defined through direct inspection of the visualization of the protruding urethral mucosa. Given the animal's clinical and physical condition, the surgical procedure of resection and anastomosis of the prolapsed urethral portion was adopted. Therefore, it is concluded that this surgical technique proved to be effective since the patient presented excellent recovery and urethral healing, without any signs of post-surgical complications.

A urolitíase é uma condição rara, mas considerada de emergência na rotina de equinos. Este trabalho relata um caso de correção de urolitíase vesical em equino pela técnica cirúrgica de laparocistotomia parainguinal a campo. Para isso, foi realizado atendimento de um equino macho, sem raça definida, com histórico de disúria e hematúria exacerbada após exercício. Após realização de exame clínico, a suspeita diagnóstica de urolitíase vesical foi confirmada por meio de ultrassonografia transretal, sendo indicado o tratamento cirúrgico. Diante da impossibilidade de realização do procedimento em centro cirúrgico, optou-se por operá-lo em condições a campo. Para o acesso à cavidade abdominal, optou-se pela laparotomia parainguinal. A bexiga foi localizada e esvaziada por punção. Procedeu-se cistotomia para remoção do urólito, seguida da lavagem da bexiga com solução fisiológica para remoção de possíveis detritos. A cistorrafia foi realizada em dois planos de sutura do tipo Schmieden-Cushing, com fio absorvível sintético, seguida pela laparorrafia. O pós-operatório consistiu em curativos diários da ferida cirúrgica, duchas e administração de antimicrobiano e anti-inflamatório. Conclui-se que o exame ultrassonográfico foi fundamental para o correto diagnóstico e que a escolha da técnica cirúrgica de laparocistotomia parainguinal realizada a campo oferece resultados satisfatórios para o tratamento de equino com urolitíase vesical.


Urolithiasis is a rare condition, but it is considered an emergency in the equine routine. The present work reports a correction case of vesical urolithiasis in equines by the surgical technique of parainguinal laparocystotomy in the field. A male horse, of mixed breed, with a history of dysuria and exacerbated hematuria after exercise was treated. After performing the clinical examination, the diagnostic suspicion of bladder urolithiasis was confirmed through transrectal ultrasonography and the surgical treatment was indicated. Given the impossibility of performing the procedure in the operating room, it was decided to operate the animal in field conditions. For access to the abdominal cavity, parainguinal laparotomy was chosen. The bladder was located and emptied by puncture. A cystotomy was performed to remove the urolith, followed by washing the bladder with saline solution to remove possible debris. The cistorrhaphy was performed in two planes of Schmieden-Cushing suture, with synthetic absorbable suture, followed by laparorrhaphy. The postoperative period consisted of daily dressings of the surgical wound, showers, and administration of antimicrobial and anti-inflammatory. It is concluded that the ultrasound examination was essential for the correct diagnosis and that the choice of the surgical technique of parainguinal laparocystotomy performed in the field offers satisfactory results for the treatment of horses with bladder urolithiasis.

O trauma é frequentemente relatado na rotina clínica de pequenos animais, podendo gerar fraturas de componentes ósseos e lesões de tecidos moles adjacentes. No presente trabalho, objetivou-se relatar o caso de um canino, macho, sem raça definida, de sete meses de idade, vítima de trauma craniomaxilofacial decorrente de mordedura, diagnosticado com fratura e deslocamento de osso zigomático, além de importante laceração de pele. O tratamento estabelecido baseou-se no debridamento da ferida, estabilização cirúrgica de arco zigomático com fios de Nylon e na sutura dos ferimentos de pele causados. Em um segundo tempo cirúrgico, foi realizada enucleação e recobrimento do defeito na órbita com malha cirúrgica de polipropileno, além de remoção de tecido exuberante e confecção de retalho subdérmico de avanço facial lateral para recobrimento do defeito cutâneo. A complicação evidenciada na primeira intervenção cirúrgica foi a deiscência das suturas de pele, enquanto no segundo tempo cirúrgico, não houve sinais de complicações. No pós-operatório, o paciente apresentou evolução clínica satisfatória, recebendo alta médica 21 dias após o segundo procedimento cirúrgico realizado. Tendo em vista a adequada evolução clínica, bem como os reduzidos efeitos colaterais, sugere-se que a conduta clínica e técnicas cirúrgicas adotadas para tratamento do paciente em questão foram efetivas.


Trauma is frequently reported in the small animal clinics, and can lead to fractures of cranial bone components and injuries to the adjacent soft tissues. In the present study, the objective was to report the case of a seven-month-old male mongrel dog, that had a craniomaxillofacial trauma resulting from a bite, characterized by zygomatic bone fracture and displacement, in addition to a major skin laceration. The stablished treatment was based on wound debridement, surgical stabilization of the zygomatic arch with nylon threads and in the suturing of skin wounds. In a second surgical procedure, enucleation was performed and a surgical polypropylene mesh was applied to cover the orbital defect, exuberant tissue was removed and a subdermal advancement flap was used to cover the skin defect. The complication observed in the first surgical intervention was dehiscence of the skin sutures, while in the second surgical procedure, there were no signs of complications. Postoperatively, the patient had a satisfactory clinical recovery, being discharged 21 days after the second surgical procedure. Considering the adequate clinical evolution and the reduced complications, it issuggestedthat the clinical conduct and surgical techniques adopted for the treatment of the patient in question were effective.

Umbilical disorders, which frequently occur in calves, are among the major causes of economic losses in herds. Antibiotic therapy alone is effective for some infectious cases, but surgical intervention is often indicated. This review aims to provide an overview of the clinical and surgical perspectives of the principal umbilical disorders in calves. The umbilicus may be affected by infectious or non-infectious conditions, including hernia, persistent urachus, omphalitis, urachitis, omphalophlebitis, and omphaloarteritis. Infectious varieties can culminate in sepsis and sometimes even involve other organs. Under these conditions, the chief complaint of calves tends to be apathy and visible swelling in the umbilical region. When surgery is indicated, the veterinary surgeon needs to have a thorough understanding of umbilical disorders, which coupled with careful clinical examination, will enable the formulation of an effective surgical plan. Surgical principles linked to pre-, trans-, and post-operative care must be considered, as recovery depends upon the surgery performed, pathological factors, and the patient's individual response. Some of the most common surgeries performed include herniorrhaphy, urachus, and umbilical vein resection. Several technological resources, such as ultrasonography and laparoscopy, are new tools that can help improve the performance of these procedures.


As afecções umbilicais são frequentes em bezerros e causam prejuízos econômicos e produtivos nos rebanhos. Apesar da antibioticoterapia ser eficaz em muitos casos infecciosos, o tratamento cirúrgico pode ser indicado. Pela incipiência de revisão relacionada a temática, objetivou-se produzir uma revisão sobre a abordagem clínica e cirúrgica das principais afecções umbilicais de bezerros. O umbigo pode apresentar infecções, como onfalite, uraquite, onfaloflebite, onfaloarterite, paratopias cirúrgicas e persistência de úraco. As infecções podem progredir para quadros septicêmicos com acometimentos de outros órgãos. A queixa principal dessas afecções consiste na protrusão da região umbilical e apatia dos bezerros. Quando a indicação terapêutica for cirúrgica, uma adequada compreensão das afecções umbilicais aliada a um bom exame clínico possibilitará ao cirurgião veterinário estabelecer um bom plano cirúrgico. Princípios cirúrgicos relacionados ao pré, trans e pós-operatório devem ser levados em consideração, pois a recuperação do paciente está diretamente relacionada à tratamento cirúrgico executada, fatores patológicos e resposta individual do paciente. Dentre as cirurgias mais comuns realizadas estão as herniorrafias, cirurgias de úraco e de veias umbilicais. Vários recursos tecnológicos trazem novidades nesses procedimentos, como a ultrassonografia, com destaque para videocirurgia.

A descorna cirúrgica a campo ainda e uma prática comum em animais de produção, apesar deste procedimento na maioria ainda se realizado por leigos, ou realizada em animais com menos de um ano de idade com ferro candente (avermelhado), esta conduta geralmente é efetuada na propriedade, sendo executada pelo próprio proprietário ou funcionário. O presente experimento usando anestesia geral e bloqueio local do nervo córneo e circularmente na base do corno com abraçadeira de naylon para sutura de pele, associada a ligadura da artéria e veia cornual mostrou ser eficiente reduzindo o tempo cirúrgico a campo e promovendo uma prevenção antecipada de hemorragia que é frequente para este procedimento.(AU)


The surgical dehorning the field and still a common practice in farm animals, although this procedure in most still held by lay people, or performed on animals less than one year old with red-hot iron (red), this conduct is usually done on the property, being executed by the owner himself or employee. El quirúrgica descorne el campo quieto y una práctica común en los animales de granja, aunque este procedimiento en la mayoría todavía en manos de los laicos, o lleva a cabo en animales de menos de un año de edad con hierro al rojo vivo (rojo), este comportamiento se realiza generalmente en la propiedad, los trabajos realizados por el propietario o el propio empleado. Este experimento usando anestesia bloques general y local de los nervios de la córnea y alrededor de la base del cuerno con naylon pinza de sutura de la piel, asociados con la ligadura de la vena y la arteria cornual fue eficiente que reduce el tiempo quirúrgico el campo y la promoción de una prevención temprana sangrado que es común para este procedimiento.(AU)

Purpose: To evaluate hemostasis of the ovarian arteriovenous complex (OAVC) in relation to surgical time, practicality and feasibility in three ovariohysterectomy (OH) techniques for queens. Methods: The experiment was performed on 21 female cats aged between six months and seven years, randomly arranged into three groups in a completely randomized design. Group one was spayed using the conventional three-clamp technique, group two using the OAVC knotting technique, and group three using the ovarian pedicle rotation technique. The students t-test and Tukeys test were used to compare the mean surgical times. Results: The conventional technique, which uses thread wires, was more laborious and required longer execution time compared to the other two techniques. The OAVC knotting technique was the fastest and had the least blood loss. Conclusion: The use of techniques that do not use synthetic materials for OAVC hemostasis was proven to be appropriate in castration projects, provided that the surgical team has sufficient training.(AU)

ABSTRACT Purpose To evaluate the use of barbed sutures over the surgical time, the leukogram, the tissue thickness in which the sutures were employed (ultrasonography), the costs, and the possible complications in bitches with pyometra submitted to ovariohysterectomy (OH). Methods Convectional 2.0 polyglyconate suture was used in the control group (CG n = 10) and 2.0 barbed polyglyconate suture in the barbed group (BG n = 10) to perform celiorrhaphy (simple continuous pattern) and subcutaneous closure (continuous intradermal pattern). Data were assessed using paired (leukogram between 24 and 48 h within the same group) and unpaired (leukogram, surgical time, tissue thickness, and costs) Student's t-test. The Fisher exact test was used to assess the occurrence of seroma between groups (p < 0.05). Results are shown as mean ± standard error of mean. Results The time spent to perform the celiorrhaphy (195.30 ± 17.37 s vs. 204 ± 16.00 s), subcutaneous closure (174.0 ± 15.86 s vs. 198.0 ± 15.62 s), and the total surgical time (24.30 ± 1.44 min vs. 23.00 ± 1.30 min) did not differ between BG and CG, respectively (p > 0.05). Leukogram at 48 h post-surgery did not differ between groups (p = 0.20). No differences were observed in the subcutaneous and the abdominal wall thickness (cm) assessed by ultrasonography at 48 h in BG (0.31 ± 0.04, 0.80 ± 0.05) and CG (0.34 ± 0.03, 0.72 ± 0.06), respectively. Similarly, 15 days post-surgery the same structures did not differ between BG (0.26 ± 0.02, 0.74 ± 0.08) and CG (0.26 ± 0.03, 0.64 ± 0.05) (p > 0.05). In one bitch from each group, a mild seroma was observed on one side of the surgical wound 48 h after surgery (p = 1.00). The procedures in which barbed sutures were used had an average additional cost of R$ 200.00 ± 11.66 (p < 0.0001). Conclusions Barbed suture has proven to be efficient and safe for abdominal and subcutaneous closure. However, considering its current high cost in addition thatthe surgical time of bitches with pyometra undergone OH was not reduced, no advantages were observed with theuse of barbed sutures for this type of surgery.

Background: Pulmonary neoplasia is an abnormal proliferation of cells at the lung tissue, and may be classified as primary, secondary or metastatic, and multisystemic. However, primary neoplasia is rare in canids. Additionally, neoplasms may be classified according to their biological behavior as either malignant or benign. Malignant tumors are more prevalent. The diagnosis can be difficult due to nonspecific symptoms, taking into account that some affected patients do not have symptoms at all; thus, the real incidence of the disease might remain undiagnosed on certain occasions. Therefore, we aimed to report an unprecedented successful case of lung lobectomy surgery in dogs with the aid of infrared video thermometry, which showed real-time images during the surgical procedure. Case: A 10-year-old male dog, crossbreed with Cocker Spaniel, was attended with a history of lethargy, dullness, progressive weight loss, cough, and difficulty breathing. Due to the chronic severity of the clinical signs and the normal physiological clinical examination, complementary exams were requested, such as radiography of the cervical and thoracic regions, cranial and thoracoabdominal computed tomography (TCT). The results of the exams showed the presence of a nodule mass in the right caudal pulmonary lobe which caused a lateral deviation to the left hemithorax of the cardiac silhouette. The patient underwent a surgical procedure with an approach through the thoracic region, at the height of the seventh intercostal space, and a total lobectomy was performed. Throughout the surgical procedure, video thermometry in real-time through the MART station (Metabolic Activity in Real-Time FLIR SC325®) was used to determine the viability of adjacent tissues through temperature differences measured in degrees Celsius. The surgical fragment containing the direct caudal pulmonary lobe and the mass were sent to the pathology sector for anatomopathological evaluation. An abundant papillary epithelial proliferation was visualized through microscopy, presenting areas of necrosis and inflammatory polymorphonuclear infiltrate. The nodule was diagnosed as lung papillary adenocarcinoma. Discussion: Adenocarcinomas consist of papillary, acinar, solid, or mixed glandular structures. They may originate from the airways, bronchial glands, or bronchoalveolar region, and often show invasive growth presenting a rudimentary and irregular shape. The clinical signs are non-specific, including exercise intolerance, non-productive cough, chronic respiratory signs such as tachypnoea or dyspnea, reduced appetite, weight loss, lethargy, chest palpation pain, hemothorax, pneumothorax, and pleural effusion. The diagnosis is concluded through imaging diagnostic tests, of which chest radiography is the most important, followed by magnetic resonance and computed tomography. Also, histopathological examination is essential to determine a definitive diagnosis. The treatment of choice for adenocarcinoma nodules is a surgical excision of the tumor mass. However, the type of surgical approach is determined by different factors such as size, location, and involvement of adjacent structures. The ablation of pulmonary masses may be performed either via partial or total lobectomies. Nevertheless, partial lobectomy is more often performed on non-neoplastic masses or to obtain material for biopsy, whilst total lobectomy is recommended for a malignant neoplasm removal. The prognosis is always guarded, having a more favorable outcome in masses of the smaller diameter without the involvement of adjacent structures.(AU)

Purpose: In this study, hemostatic efficacy of Ankaferd Blood Stopper (ABS), a new generation hemostatic agent, was compared in the presence of heparin effect. Methods: Forty-eight Wistar albino rats were divided into two main groups as heparinized and nonheparinized, and these two main groupswere divided into six subgroups as control, Surgicel and ABS (n = 8). Grade 2 liver injury was performed on rats as standard. All groups were compared in terms of weight, laceration surface area, prothrombin time (PT), activated partial thromboplastin time (aPTT), international normalized ratio (INR), bleeding time, bleeding amount, hemoglobin (Hb) levels, macroscopic and microscopic reactions to the agent used. Results: Whereas there was no statistically significant difference between weight, laceration surface area, PT, INR and preoperative Hb values in the heparinized and nonheparinized groups, postoperative Hb, bleeding time, bleeding amount and aPTT values were statistically different (p 0.05). In the heparin-hemostat interaction, the ABS group had the lowest bleeding in the heparinized group in terms of the amount of bleeding compared to the control and Surgicel groups (F = 0.764; p = 0.047). In macroscopic and microscopic comparison, there was no difference between the groups in terms of cell necrosis andfresh bleeding (p > 0.05), it was found that the Surgicel group had statistical significantly higher reaction scores (p < 0.05) than the other groups in terms of other parameters. Conclusions: Ankaferd Blood Stopper can be safely and effectively used in surgical practice and in patients with additional diseases requiring heparinization, since it causes minimal reaction in the liver and decreases the amount of bleeding especially in the heparinized group.(AU)
