RESUMO

Many surgeons request use of 10% povidone-iodine (PI) for vaginal antisepsis; however, when PI is contraindicated, some surgeons request use of chlorhexidine gluconate (CHG) instead. The purpose of this randomized controlled trial was to determine any significant differences in self-reported symptoms associated with vaginal antisepsis with either 10% PI scrub or 4% CHG with 4% isopropyl alcohol. The control group comprised 62 participants who underwent vaginal antisepsis with the PI product, and the intervention group comprised 58 participants who underwent vaginal antisepsis with the CHG product. Participants completed surveys immediately before surgery, immediately after surgery, and 48 to 72 hours after surgery. No significant differences were found in the reported vaginal symptoms between the two groups for any survey. One participant in the intervention group reported symptoms consistent with an allergic reaction. Additional studies are needed on the use of CHG for vaginal antisepsis.

Transfusions of red blood cells (RBCs) are frequently administered for a variety of clinical conditions. The use of an infusion pump has become common practice in nursing. Lack of evidence regarding hemolysis associated with transfusing older RBCs using an infusion pump led one 361-bed acute care suburban medical center to maintain its nursing policy to infuse all blood products by gravity. A combined simulated and clinical study was conducted with 25 units of RBCs. No significant hemolysis was demonstrated as measured by hemolysis index and serum potassium levels. As a result of this study, the nursing policy at the medical center was changed.

BACKGROUND: This quasi-experimental research study used two similar nursing units to test the effects of teach back on Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) scores. METHOD: A pretest-posttest design tested 24 nurses' knowledge, attitudes, and beliefs about teach back. Education specialists provided a 1-hour teaching session on teach back to all nurses in the intervention unit. RESULTS: A significant improvement in knowledge scores in the pretest-posttest was found using paired t tests (p = .002). Qualitative analysis of nurses' comments demonstrated strong support for teach back in the post-test. The HCAHPS scores were not significantly improved in the intervention unit when compared with the control unit. CONCLUSION: More research needs to be conducted to determine the effectiveness of teach back on HCAHPS scores. J Contin Educ Nurs. 2017;48(1):47-52.

A multi-site descriptive retrospective study of physiologic, demographic, and psychosocial factors associated with 30-day readmissions for heart failure was conducted in six Magnet-designated hospitals. Results can be used to plan effective nursing interventions to target readmitted patients.

RESUMO

The mock trial, which is an educational tool used in many different professions, is a scripted form of debate that teaches the critical thinking process. Using the mock trial approach, two nurse educators at one medical center taught nursing staff and students the major elements of evidence-based practice. The verdict to be decided in the trial was whether family-witnessed resuscitation should be used in practice. This article outlines the steps taken to carry out this endeavor. Nursing faculty can use this mock trial approach to teach a variety of concepts in nursing education.

RESUMO

The incidence and prevalence of overweight is increasing to alarming rates in children and adolescents in the United States. Hispanic children are one of the highest risk groups for overweight. Many different factors are positively correlated with childhood overweight in Hispanics: lower socioeconomic status, lacking health insurance or being under-insured, poor diet, decreased physical activity, overweight status of parents, mother's perception of overweight, and degree of acculturation. Pediatric nurses are in a pivotal position to assist in curtailing the epidemic. Various evidence-based practices to prevent and treat pediatric overweight are discussed with recommendations to intervene, particularly with Hispanic youth.
