RESUMO

La longevidad del tratamiento restaurador indirecto en dientes con tratamiento endodóntico depende de varios factores. Siendo un factor muy importante, la cementación del retenedor intrarradicular. El objetivo de este artículo fue realizar una revisión de literatura sobre los aspectos relevantes para el éxito clínico en la cementación adhesiva de los pernos prefabricados. Se abordaron los siguientes temas: material de los pernos prefabricados, tratamientos de superficie, soluciones de irrigación utilizadas para la limpieza del conducto radicular y sistemas adhesivos. Se realizó una búsqueda bibliográfica de los últimos 5 años, en portugués e inglés, utilizando las bases de datos: Pub Med y BIREME. Del total de artículos obtenidos se seleccionaron los artículos con vinculación directa al tema, resultando en 40 publicaciones científicas. Se concluyó que la aplicación de silano y la silanización térmica aumenta la fuerza de unión entre el perno de fibra y el agente cementante. El hipoclorito de sodio al 2.5% no interfirió en la fuerza de unión de los pernos de fibra cuando se usa la técnica de ácido total junto con cemento resinoso de polimerización dual. Los adhesivos autograbantes se han mostrado prometedores, especialmente cuando se combinan con cementos autopolimerizables. (AU)


The longevity of indirect restorative treatment in teeth with endodontic treatment depends on several factors. A very important factor is the cementation of the intraradicular retainer.The aim of this study was the relevant aspects for clinical success in the adhesive cementation of fiber posts. The following topics were developed: fiber post material, surface treatments, irrigation solutions used for root canal cleaning, and adhesive systems. A bibliographic search of the last 5 years was carried out, in Portuguese and English, using the databases: PubMed and Bireme. From the total of articles obtained, articles with direct link to the topic were selected, resulting in 40 scientific publications. It was concluded that: the application of silane and thermal silanization increases the bond strength between the fiber bolt and the cementing agent. The 2.5% sodium hypochlorite did not interfere with the bond strength of the fiber bolts when the total acid technique is used in conjunction with dual-set resinous cement. Self-etching adhesives have shown promise, especially when combined with self-curing cements. (AU)

Introducción: La obturación del sistema de conductos presenta un papel clave en el éxito del tratamiento de endodoncia. En un intento de mejorar las propiedades de los cementos selladores, recientemente se han introducido en el mercado los cementos selladores a base de silicatos. Por ello, al llevar a cabo la obturación del sistema de conductos, es de utilidad conocer las propiedades físicas que los diferentes cementos selladores presentan. Objetivo: El objetivo del presente trabajo fue la revisión bibliográfi ca de las propiedades físicas de utilidad clínica que presentan los nuevos cementos a base de silicatos, y compararlas con las propiedades físicas de los cementos convencionales a base de resina epóxica. Material y método: Tras establecer la pregunta de investigación adaptada, se llevó a cabo una revisión de la literatura en dos bases de datos (Medline vía Pubmed y Wiley Library vía Biblioteca Chrocane) combinando términos MeSH (Medical Subject Headings) y términos libres. Además, se llevó a cabo una búsqueda electrónica manual. Las propiedades físicas de utilidad clínica seleccionadas fueron la decoloración, capacidad de sellado, radiopacidad, tiempo de fraguado y solubilidad. Resultados: Se obtuvieron 224 estudios potenciales. Finalmente, aplicando los criterios de inclusión y exclusión, se incluyeron 22 estudios en la revisión. Los distintos estudios compararon diferentes propiedades físicas de los cementos a base de silicatos, comparándolos con los cementos a base de resina. (AU)


Introduction: In the endodontic treatment success, the fi lling of the root canal system plays a key role. To improve properties of the sealers used in these treatments, new silicate-based sealers have recently been introduced into the market. Therefore, when performing the endodontic treatment, it is useful to know the physical properties of the different sealers. Objectives: The aim of the present study was to review the literature and compare the clinically useful physical properties of the new silicate-based cements with the physical properties of conventional epoxy resin-based cements. Methods: After establishing the adapted research question, a literature review was carried out in two databases (Medline via Pubmed and Wiley Library via Chrocane Library) combining MeSH (Medical Subject Headings) and free terms. In addition, a manual electronic search was performed. The clinically useful physical properties selected were discolouration, sealability, radiopacity, setting time and solubility. Results: A total of 224 potential studies were obtained and 20 were selected for full text reading. Additionally, 4 studies were selected through electronic handsearching. After exclusion of 2 studies, 22 studies were fi nally included in the review, which analyzed the following physical properties: 2 tooth discolouration; 4 sealing ability; 11 radiopacity; 9 setting time; and 12 solubility. 9 of the selected articles evaluated several properties. La biocompatibilidad de los cementos selladores es una característica que debe tenerse en cuenta, ya que podría influir en el resultado del tratamiento de conductos. El objetivo de este estudio fue analizar la citotoxicidad de los cementos selladores a base de silicatos BioRoot® RCS y Nano-ceramic Sealer®, y el sellador a base de resina epóxica AH Plus Jet®, así como su respuesta inflamatoria asociada en fibroblastos humanos. Se seleccionaron 36 dientes unirradiculares, se instrumentaron y obturaron con los selladores seleccionados. A continuación, se pusieron en contacto con el medio de cultivo para obtener los medios acondicionados a las 24, 48 y 72 horas. Los medios acondicionados obtenidos fueron cultivados con la línea celular de fibroblastos L-132. La viabilidad de las células se evaluó mediante el ensayo del bromuro de 3-(4,5-dimetiltiazol-2-il)-2,5 difenil tetrazolio (MTT). La respuesta inflamatoria se analizó mediante la medición de los niveles de IL-6 determinados a través del ensayo ELISA (ensayo por inmunoabsorción ligado a enzimas) y mediante los niveles de nitrito (ensayo de fluorescencia). Todos los selladores mostraron un cierto grado de citotoxicidad durante el tiempo de evaluación, mientras que sólo el BioRoot® RCS desencadenó una respuesta pro-inflamatoria. Es necesario seguir investigando para evaluar otros aspectos de la biocompatibilidad de los cementos selladores a base de silicato. (AU)


Biocompatibility of sealers is a feature to be taken into account as it might influence the outcome of the treatment. The aim of this study was to analyze the cytotoxicity of the silicate-based root canal sealers BioRoot™ RCS and Nanoceramic Sealer™and the epoxy-resin based sealer AH Plus Jet™, as well as their associated inflammatory response in human fibroblast. Thirty-six human teeth were selected, prepared, filled with the selected sealers and put in contact with culture medium to obtain extracts at 24, 48 and 72 hours. Then, the extracts were cultured with the L-132 fibroblast cell line. Cell viability was evaluated using the 3-(4,5-dimethylthiazol-2-yl)-2,5 diphenyl tetrazolium bromide (MTT) assay. Inflammatory response was analyzed by measuring IL-6 levels determined by ELISA and nitrite levels (fluorescence assay). All sealers showed a certain degree of cytotoxicity during the evaluation time, while only the BioRoot™ RCS triggered a pro-inflammatory response. Further research is needed to assess other aspects of biocompatibility of silicate-based root canal sealers. (AU)

Background: Periapical surgery focuses on the treatment of teeth with persistent periapical lesions when ortho-grade root canal treatment fails. Although MTA® is the gold standard material for retrograde filling, Biodentine®- a tricalcium silicate-based cement - has been proposed in order to resolve several of its limitations. A systematicreview has been carried out to compare the physicochemical properties of Biodentine® versus MTA® as root-endfilling material in periapical surgery.Material and Methods: An electronic search was conducted by two independent examiners during March 2020 inthe Cochrane, PubMed-MEDLINE and Scopus databases. In addition, a manual search was made in specializedjournals. Comparative human or in vitro studies that evaluated bond strength, the presence of marginal gap andsealing ability were included. No restriction on publication date was applied. Animal studies, clinical cases, casesseries and expert opinions were excluded.Results: After analyzing 147 initially selected studies, 13 publications were included. Regarding bond strength,the studies seemed to evidence better performance of Biodentine® in both acidic and blood contaminatedenvironments. In relation to the presence of marginal gap and sealing ability, the studies yielded contradictory results. According to some authors, the sealing ability of Biodentine® is greater than that of MTA® during the first 24hours, though both materials prove equal after one week. Other authors recorded no significant differences.Conclusions: Considering the limitations and heterogeneity of the studies included, there is not sufficient evidence toconfirm the clinical superiority of Biodentine® as a root-end filling material in periapical surgery.(AU)

OBJETIVO: Comparar, en un estudio experimental in vitro, el porcentaje de áreas no obturadas entre la técnica de compactación lateral en frío (TCL) y la técnica de compactación lateral con ultrasonido (TCLU). MATERIALES Y MÉTODOS: El estudio fue realizado en una muestra de 68 dientes humanos extraídos. Se asignaron de manera aleatoria en dos grupos de 34 dientes cada uno. Todos los dientes se cortaron a 16±1 mm de longitud desde el ápice hacia la corona, se instrumentaron con el sistema rotatorio Protaper Next (Dentsply) y se realizó protocolo de irrigación final con NaOCl al 2.5% y EDTA al 17% neutralizados con suero fisiológico entre ellos. El Grupo I se obturó con TCL y el Grupo II con TCLU. En ambos grupos se utilizó Sealapex como cemento. Los dientes fueron seccionados horizontalmente a 2, 4, 6 y 8 mm desde el ápice; cada corte se observó con un microscopio estereoscópico (56X) y se tomaron fotografías. Se calculó el porcentaje de área obturada con gutapercha, cemento y de áreas no obturadas, utilizando el programa ImageJ. El análisis de los resultados se llevó acabo mediante la suma de rangos de Wilcoxon. RESULTADOS: El grupo TCLU presentó un mayor porcentaje de gutapercha en el conducto radicular en comparación con el grupo TCL (94.4% y 87.4% respectivamente), menor porcentaje de áreas no obturadas (1.3% y 4.2%), y menor porcentaje de áreas ocupadas con cemento (3.8% y 7.2%). Las diferencias fueron estadísticamente significativas (P<0.05).CONCLUSIÓN: Los resultados de este estudio muestran que la modificación de la técnica de compactación lateral con ultrasonido descrita en esta investigación presenta resultados in vitro superiores a TCL


AIM: To compare, in an Experimental in vitro study, the percentage of non-filled areas between cold lateral compaction technique (CLT) and lateral compaction with ultrasonic technique (LUT). METHODS: The study was made on in a sample of 68 extracted human teeth. They were randomized into two groups of 34 teeth each. All teeth were cut 16±1 mm from the apex and instrumented with the Protaper Next rotary system (Dentsply) and a final irrigation protocol was performed with NaOCl 2.5% and EDTA 17% neutralized with normal saline solution between them. Group I was filled with CLT and Group II with LUT. In both groups Sealapex was used as a cement. The teeth were sectioned horizontally at 2, 4, 6 and 8 mm from the apex; slices were the viewed through a stereomicroscope (56X) and pictures were taken. The percentage of gutta-percha filled area, percentage of non-filled area and percentage of sealer filled area were calculated for each section using the ImageJ program. Statistical analysis was performed using the Wilcoxon Rank-Sum test. RESULTS: LUT had a higher percentage of gutta-percha in the root canal compared to CLT (94.4% and 87.4% respectively), lower percentage of non-filled area (1.3% and 4.2% respectively), and lower percentage of area occupied by sealer (3.8 % and 7.2% respectively). The differences were statistically significant(P <0.05).CONCLUSION: The results of this study showed that the modification of lateral compaction technique with ultrasonic described in this investigation presents in vitro results superior to CLT

OBJETIVO: Los materiales que se emplean para procedimientos dentales en los que el material contacta directamente con el tejido pulpar y tejidos periodontales han de cumplir una serie de propiedades, entre ellas la biocompatibilidad. A su vez, han de ser materiales que eviten el paso de fluidos y microorganismos con el fin de preservar las condiciones óptimas de los tejidos. El objetivo de esta revisión sistemática fue evaluar la porosidad de los diferentes cementos de agregado trióxido mineral (MTA). MÉTODO: Dos investigadores realizaron búsquedas avanzadas en: PubMed Central, PubMed, Web of Science, Scopus, Cochrane Li-brary, Science Direc y Science Database (Proquest) siguiendo los criterios PRISMA. Los criterios de inclusión contemplaban los es-tudios in vitro, publicados en inglés desde enero de 1993 hasta abril de 2019, que analizaran y midieran la porosidad del MTA. Se excluyeron los estudios in vivo, piloto, editoriales, cartas, revisiones sistemáticas, reseñas literarias, resúmenes de conferencias y trabajos de fin de grado. RESULTADOS: Un total de 853 artículos resultó de la búsqueda inicial, de los que solo 19 pasaron los criterios de inclusión, exclusión y el análisis del riesgo de sesgo. En ellos, se analizan distintas técnicas de condensación del MTA, radiopacificadores, sustratos, etc., en busca de materiales que presenten menor porosidad. CONCLUSIONES: La elevada heterogeneidad de los estudios sobre porosidad y el hecho de que éstos no estén estandarizados, pue-de condicionar la validez externa o generalización de los resultados entre los distintos investigadores para un mismo material y procedimiento. En los estudios que realizan una comparación entre varios materiales, concluyen que tanto MTA ProRoot® y Bio-dentineTM obtuvieron mejores resultados en cuanto a porosidad. OBJECTIVE: Materials used for dental procedures in which the material directly contacts the pulp and periodontal tissues must have a series of characteristics, such as biocompatibility. Equally, they must be materials that prevent the passage of fluids and microorganisms aiming at keeping the optimal conditions of the tissues intact. The objective of this systematic review was to evaluate the porosity of the different cements of mineral trioxide aggregate (MTA). METHOD: Two researchers conducted advanced searches in: PubMed Central, PubMed, Web of Science, Scopus, Cochrane Library, ScienceDirect and ScienceDatabase (Proquest) following the PRISMA criteria. Inclusion criteria took into account in vitro studies, published in English from January 1993 to April 2019, to analyse and measure the porosity of the MTA. In vivo and pilot studies were excluded, as well as editorials, letters, systematic reviews, literary reviews, conference abstracts and dissertations. RESULTS: A total of 853 articles resulted from the initial search, of which only 19 met the criteria for inclusion, exclusion and analysis of the risk of bias. In them, different condensation techniques of the MTA, radiopacifiers, substrates, etc., are analysed in quest of materials with less porosity. CONCLUSIONS: The high heterogeneity of the studies on porosity and the fact that they are not standardised, can condition the external validity or generalisation of the results among the different researchers for the same material and procedure. In the studies that make a comparison between several materials, they conclude that both MTA ProRoot® and BiodentineTM obtained better outcomes in terms of porosity as compared with other marketed brands. Objetivos. El propósito de este trabajo fue evaluar los efectos biológicos de MTA Repair HP y ProRoot MTA en células madre procedentes de ligamento periodontal (hPDLSCs) tras ser ex-puestos los cementos a ambiente ácido y neutro. Material y Métodos: Discos de cada material (n=30) fueron ex-puestos a un tampón fosfato-salino (pH 7.4) o a ácido butírico (pH 5.2) durante 7 días, posteriormente se realizaron pruebas biológicas in vitro usando hPDLSCs. A partir de eluatos de los diferentes materiales de obturación a retro, se analizaron pruebas de viabilidad celular y apoptosis. Para evaluar la adhesión celular, hPDLSCs se sembraron directamente sobre la superficie de los materiales y se observaron bajo microscopio electrónico de barrido. Para analizar estadísticamente los resultados se usaron los test ANOVA y Tukey test (p < 0.05). Resultados: Los cementos endodónticos expuestos a ambiente ácido mostraron un similar grado de adhesión celular, y sorprendentemente, MTA Repair HP a pH 5.2 exhibió una mayor viabilidad celular que ProRootMTA (p<0.05). A pH 7.4, ProRooT MTA obtuvo una tasa mayor de viabilidad celular que con MTA Repair HP. Conclusiones: Los materiales ProRoot MTA y MTA Repair HP presentaron adecuadas propiedades biológicas en los diferentes ambientes, en términos de viabilidad celular, apoptosis y adhesión


Objectives: The purpose of this work was to evaluate the biolo-gical effects of MTA Repair HP and ProRoot MTA on stem cells from periodontal ligament (hPDLSCs) after exposure to acidic and neutral environments. Material and Methods: Discs of each material (n=30) were ex-posed to phosphate buffered saline (pH = 7.4) or butyric acid (pH = 5.2) for 7 days, and biological testing was carried out in vitro on hPDLSCs. Cell viability and apoptosis assays were performed using eluates of each root-end filling material. To evaluate cell attachment to the different materials, hPDLSCs were directly seeded onto the material surfaces and analyzed by scanning electron microscopy. Statistical differences were assessed by ANOVA and Tukey test (p < 0.05).Results: Endodontic cements exposure to an acidic environment showed a similar degree of cell adherence, and, surprisingly, MTA Repair HP exhibited higher cell viability rates at pH 5.2 than Pro-Root MTA, whereas ProRoot MTA 7.4 showed higher cell viability rates than MTA Repair HP. Conclusions: Adequate biological properties of ProRoot MTA and MTA Repair HP in terms of cell viability, cell death and cell attach-ment were observed in both environments

Objetivo. Evaluar la interfase entre tres cementos biocerámicos y la pared del conducto radicular, comparándolos con un cemento de resina. Material y Métodos: Se seleccionaron 80 dientes unirradiculares, los cuales se dividieron en 4 grupos de acuerdo al cemento de obturación: 1 (MTA FillApex), 2 (EndoSequence BC Sealer), 3 (Bio Root) y 4 (AHPlus). Los dientes se decoronaron dejando una longitud estándar de 15 mm; se instrumentaron con el sistema Mtwo hasta la lima 35/.04. Se irrigó con hipoclorito al 5’25%, EDTA al 17% y se finalizó la irrigación con solución salina. La obturación se realizó con gutapercha 35/.04 y técnica de cono único. Las muestras fueron cortadas a 3, 5 y 8 mm del ápice y se observaron con un estereomicroscopio. Obtenidas las imágenes, se midió el perímetro del conducto en el que hubo adaptación del cemento (expresado como porcentaje respecto a la longitud total del perímetro del conducto) a tres niveles: apical, medio y coronal. Los resultados se analizaron mediante el test de Kruskal Wallis, con una significación de p < 0,05. Resultados: El grupo 1 presentó significativamente peor adaptación a las paredes dentinarias que el resto de cementos estudia-dos en todos los tercios (apical, 63,71%; medio, 69,53%; coronal, 50,82%). Aunque no se encontraron diferencias significativas, el tercio que mejor adaptación presentó fue el apical. Conclusiones: MTA Fillapex, globalmente, se comportó significativamente peor que los otros cementos evaluados. No existen diferencias significativas entre la adaptación a las paredes dentinarias del BioRoot y el Endosequence y el AHPlus


Objective: To evaluate the interface between three bioceramic-based sealers and the root canal wall, comparing them with a resin-based sealer. Material and Methods: 80 single-rooted teeth were selected and divided into 4 groups according to the sealer: 1 (MTA FIllapex), 2 (EndoSequence BC Sealer), 3 (Bio Root) y 4 (AHPlus). Crowns were sectioned at 15mm from the apex; all the roots were instrumented with Mtwo system until 35/.04 file. Canals were irrigated with 5’25% sodium hypochlorite, 17% EDTA and a final rinse with saline solution. The roots were obturated with 35/.04 gutta-percha and single-cone technique. The samples were sectioned at 3, 5 and 8 mm from the apex and were observed with a stereo microscope. The images taken were analyzed; the root canal perimeter where there was adaptation was measure (expressed as a percentage regarding to the root canal total perimeter) at three levels: apical, middle and coronal. The results were analyzed statistically by the Kruskal Wallis test with a p<0,05 significance level. Results: Group 1 presented significantly worst adaptation to the canal walls than the other studied sealers (apical, 63,71%; middle, 69,53%; coronal, 50,82%). The third with the worst adaptation was the apical, even though there were no statistical differences. Conclusions: MTA Fillapex, overall, behaved worse than the other sealers studied. There was no statistical difference between AH Plus, BioRoot and EndoSequence on the canal wall adaptation

El bisfenol A (BPA, por sus siglas en inglés) es un producto químico utilizado en la producción de policarbonatos y resinas epoxi, cuyo uso está muy extendido en la industria. El BPA puede pasar al organismo por medio de la dieta, ya que sus moléculas tienen migración probable de los recipientes fabricados con este producto a los alimentos contenidos en dichos recipientes. La presencia de BPA en los materiales de uso odontológico está muy extendida. Una gran cantidad de productos están elaborados con resinas epoxi (procedentes de la reacción entre la epiclorohidrina y el bisfenol A). El presente estudio se analizaron cuatro cementos de uso endodóntico basados en resina (Ah 26(R), Ah Plus(R), TopSeal(R) y Simpliseal(R)) a través de un procedimiento de extracción líquido-sólido y análisis en cromatógrafo de gases acoplado a espectrometría de masas. Los resultados obtenidos muestran valores alarmantemente superiores a los 5x10-6% en peso que permite la Unión Europea para la migración de BPA desde el envase al alimento, obteniéndose unos valores de 7% (Simpliseal(R)); 22% (AH 26(R)); 46% (TopSeal(R)) y 74% (AH Plus(R)). El presente estudio es pionero en el estudio de la concentración de BPA en materiales endodónticos


Bisphenol A (BPA) is a compound used in the production of polycarbonates and epoxy resins and is widely used in the industry. BPA can be passed into the body through the diet, since its molecules can easily migrate from the containers made with BPA to the food in those containers. The presence of BPA in dental materials is very common. A large number of products are made with epoxy resins (from the reaction between epichlorohydrin and bisphenol A). This study analyzed four resin-based endodontic cements (Ah 26(R), Ah Plus(R), TopSeal(R) and Simpliseal(R)) using a liquid-solid extraction procedure and gas chromatograph analysis coupled to mass spectrometry. The results obtained show alarmingly higher values than the 5x10-6% by weight that the European Union allows for the migration of BPA from packaging to food, obtaining values of 7% (Simpliseal(R)), 22% (AH 26(R)), 46% (TopSeal(R)) and 74% (AH Plus(R)). This study is a pioneer in the investigation of the concentration of BPA in endodontic materials

Objetivo: El objetivo de este estudio fue determinar, el pH, la conductividad y la solubilidad al someter el agregado trióxido mineral (MTA) y el cemento Pórtland (CP) a diferentes irrigantes utilizados en endodoncia. Material y Métodos: Se estudiaron un total de 210 muestras, 105 de cemento ProRoot MTA(R) blanco y 105 de CP blanco. Se seleccionaron los siguientes irrigantes: hipoclorito sódico al 2,5% y al 5%, ácido cítrico al 10% y al 20%, clorhexidina al 2%, EDTA al 17%, y suero fisiológico como control. Se expusieron 15 muestras de MTA y 15 de CP a cada tipo de irrigante. Se midieron los resultados a 1, 5, 15, 30 y 60 minutos. Resultados: La mayoría de cambios del pH y la conductividad fueron de pequeña magnitud, aunque estadísticamente significativos. Destacan la alcalinización del suero fisiológico y la acidificación del hipoclorito sódico. Con MTA disminuye la conductividad en hipoclorito sódico y aumenta la conductividad en EDTA. Con CP hubo una mayor pérdida de conductividad del hipoclorito sódico y del ácido cítrico. Todas las muestras disminuyeron el peso considerablemente después de la exposición a cualquiera de los irrigantes, y en general ésta fue mayor para el CP que el MTA. Conclusiones: El contacto del material de reparación con los irrigantes utilizados habitualmente en endodoncia altera poco el pH y la conductividad pero aumenta marcadamente su solubilidad. Estos cambios afectan menos al MTA que al CP


Objective. The aim of this study was to determine changes to pH, conductivity, and solubility when mineral trioxide aggregate (MTA) and Portland cement (PC) are exposed to different endodontic irrigants. Methods. The study included a total of 210 samples, 105 white ProRoot MTA® cement and 105 white PC cement. The following irrigants were tested: 2.5% and 5% sodium hypochlorite, 10% and 20% citric acid, 2% chlorhexidine, 17% ethylenediamine tetra-acetic acid (EDTA), and physiological serum as a control. Fifteen samples of each material were exposed to each irrigant. PH, conductivity, and solubility were measured at baseline and after 1, 5, 15, 30 and 60 minutes exposure to irrigants. Results. Most changes in pH and conductivity were of small magnitude, although statistically significant. For MTA and PC physical serum produced alkalinization, while sodium hypochlorite produced acidification. MTA lost conductivity when exposed to sodium hypochlorite but this increased with exposure to EDTA. PC underwent greater losses of conductivity when exposed to sodium hypochlorite and citric acid. All samples decreased in weight significantly after exposure to any of the irrigants, and the loss was generally greater for PC than MTA. Conclusions. When repair materials are exposed to the irrigants normally used in endodontics, pH levels and conductivity alter slightly, while solubility increases markedly. These effects were greater for PC than MTA

Objetivo: El propósito de este estudio fue determinar la rugosidad superficial, al someter al agregado de trióxido mineral (MTA) y al cemento Pórtland (CP) a diferentes irrigantes utilizados en endodoncia. Material y Métodos: Se estudiaron un total de 210 muestras, 105 de cemento ProRoot MTA® blanco y 105 de CP blanco. Se seleccionaron los siguientes irrigantes: hipoclorito sódico al 2,5% y al 5%, ácido cítrico al 10% y al 20%, clorhexidina al 2%, EDTA al 17%, y suero fisiológico como control. Se expusieron 15 muestras de cada cemento por cada tipo de irrigante durante 15 minutos. El estudio de la rugosidad superficial se hizo con el microscopio Leica DCM 3D que permite una perfilometría con tecnología dual confocal e interferométrica. Se evaluó la rugosidad media (Ra), el valor cuadrático medio (RMS) y la profundidad de rugosidad máxima (PV) al inicio y a los 15 minutos de la inmersión de las muestras. Resultados: El CP aumentó significativamente la rugosidad en los parámetros Ra y RMS tras la irrigación con hipoclorito al 2,5%, ácido cítrico al 10% y al 20%, y suero fisiológico. En términos de PV el CP solo aumentó la rugosidad tras la irrigación con ácido cítrico al 20% y suero fisiológico. El MTA no mostró cambios significativos respecto a la rugosidad Ra, RMS y PV con ninguno de los irrigantes. Conclusiones: La rugosidad superficial del CP puede modificarse por la exposición a alguno de los irrigantes utilizados en endodoncia, mientras que la rugosidad del MTA no se modifica significativamente


Objectives: This study set out to determine changes to surface roughness when mineral trioxide aggregate (MTA) and Portland cement (PC) are exposed to the different irrigants used in endodontics. Material and Methods: The study included a total of 210 samples, 105 white ProRoot MTA® cement and 105 white PC cement. The following irrigants were tested: 2.5% and 5% sodium hypochlorite, 10% and 20% citric acid, 2% chlorhexidine, 17% ethylenediamine tetra-acetic acid (EDTA), and physiological serum as a control. Fifteen samples of each material were exposed to each irrigant for 15 minutes. Surface roughness evaluation was performed using a Leica DCM 3D microscope for dual system (confocal and interferometric) profilometry. Mean roughness (Ra), root mean square (RMS) and maximum roughness depth (PV) were evaluated before and after 15 minutes sample immersion in each irrigant. Results: PC significantly increased roughness for Ra and RMS parameters after irrigation with 2.5% sodium hypochlorite, 10% and 20% citric acid, and physiological serum. PC underwent an increase in the PV parameter after immersion in 20% citric acid and physiological serum. MTA did not show significant changes in Ra, RMS or PV with any of the irrigants. Significance. The surface roughness of PC may be modified by exposure to some of the irrigants used in endodontics, while the roughness of MTA is not modified significantly

Introducción: un adhesivo para prótesis dentales debe mostrar, como principales propiedades, una fuerza adhesiva y una viscosidad perdurables en el tiempo. Objetivo: Determinar la capacidad de fijación de prótesis de un nuevo adhesivo protésico durante 8 días en condiciones normales de uso. Se evaluaron sus propiedades organolépticas y la percepción de eficacia por los participantes. Métodos: Estudio clínico exploratorio, longitudinal y observacional en 20 pacientes con prótesis dental completa removible. El producto se aplicó 1 vez/día, durante 8 días, en la arcada superior. Se establecieron 5 visitas del estudio, con determinaciones de la resistencia a la tracción en el momento basal (T0), a los 30 minutos (T30'), a las 12 horas (T12) y a las 24 horas (T24). Resultados: La fuerza media empleada para desplazar la prótesis dental fue de 188,13±113,52 cN; 418,50 ±93,60 cN; 470,50±76,03 cN y 428,0±140,73 cN para los puntos temporales T0, T30', T12 y T24, respectivamente; todas las diferencias fue-ron significativas vs. basal (p≤0,0001). El total de pacientes mostró una percepción satisfactoria sobre la eficacia y las características organolépticas del producto. No se registraron efectos adversos. Conclusiones: El adhesivo dental estudiado, aplicado durante 8 días en condiciones normales de uso, se asoció a un incremento significativo en la resistencia a la tracción durante 24 horas, aumento que fue máximo a las 12 horas. El adhesivo dental presentó muy buena aceptabilidad y tolerancia (AU)


Introduction: The ideal denture adhesive should show, as main properties, a durable adhesive strength and viscosity. Objective: To determine the denture retention capacity of a dental adhesive for 8 days under normal conditions of use. Its organoleptic properties and perception of efficacy were evaluated by the participants. Methods: Exploratory, longitudinal and observational clinical study in 20 patients with complete removable dental prostheses. The product was applied once a day for 8 days in the upper arch. Five study visits were established, with determinations of tensile strength at baseline (T0), at 30 minutes, at 12 and at 24 hours. Results: The mean force used to displace the dental prosthesis was 188.13±113.52 cN; 418.50±93.60 cN; 470.50±76.03 cN and 428.0±140.73 cN for the time points T0, T30', T12 and T24, respectively; all differences were significant vs. baseline (p≤0,0001). The total of patients showed a satisfactory impression on the efficiency and organoleptic characteristics of the product. No adverse effects were reported. Conclusions: The dental adhesive studied, applied during 8 days under normal conditions of use, was associated with a signigicant 24-hour increase in tensile strength, effect which was maximum at 12 hours. The cream presented very good acceptability and tolerance (AU)

Antecedentes: la literatura odontológica demuestra que los tratamientos endodónticos fallidos, comúnmente son debidos a la persistencia de los microorganismos. Una característica importante de los cementos obturadores de canales es su habilidad para penetrar en los túbulos dentinarios, logrando así un sellado impermeable entre las paredes dentinarias y el material de obturación central, reduciendo las filtraciones potenciales y aumentando la resistencia a la fractura. Objetivo: Evaluar mediante microscopía electrónica de barrido (MEB) la penetración de MTA Fillapex, cemento de Grossman (CG) y top seal a lo largo del conducto radicular. Materiales y métodos: Estudio experimental in vitro, en 36 dientes premolares humanos divididos aleatoriamente en 3 grupos, (n= 12), los cuales fueron preparados con técnica endodóntica Step Back. Cada grupo se dividió en 2 subgrupos utilizando como irrigante NaOCl al 2,5%, al igual que NaOCl al 2,5% + EDTA al 17%. Los canales fueron obturados con técnica de condensación lateral, seccionados longitudinalmente para observar mediante microscopia electrónica de barrido a nivel de los tercios cervical, medio y apical, la penetración del cemento al interior de los túbulos dentinarios. Se realizó análisis estadístico con SPSS v 22 IBM con p< 0,05 y las pruebas Kruskal-Wallis y Mann-Whitney. Resultados: Mediante la prueba Kruskal-Wallis, los cementos de Grossman y top seal al utilizar NaOCl + EDTA presentaron valores de p= 0,025 y p= 0,002 en los tercios medio y apical respectivamente. La prueba de Mann Whitney mostró un valor p= 0,031 en el tercio apical para el cemento MTA Fillapex al utilizar NaOCl. Los valores de la mediana fueron MTA Fillapex tercio medio= 2,0000, tercio apical= 1,0000; top seal tercios medio y apical= 4,0000; cemento de Grossman tercios medio y apical= 4,0000. Al ser igual el valor de la mediana entre los cementos top seal y Grossman se utilizaron los valores de la varianza con un resultado estadísticamente significativo para el cemento de Grossman= 0,276 en el tercio medio y top seal= 0,176 en el tercio apical. La prueba de Mann Whitney arrojó un valor p= 0,031 en el tercio apical para el cemento MTA Fillapex. Con un valor de mediana de NaOCl= 3,0000, NaOCl + EDTA= 1,0000. Conclusiones: Al comparar los cementos obturadores MTA Fillapex, top seal y cemento de Grossman utilizando como irrigante NaOCl + EDTA se observó mayor penetración del cemento de Grossman. A nivel del tercio apical se observó mayor penetración con el cemento top seal. El cemento obturador MTA Fillapex presentó mayor penetración a nivel del tercio apical al utilizar como irrigante NaOC (AU)


Background: the literature has shown that failed endodontic treatments are commonly due to the persistence of microorganisms. One important characteristic of root canal filling cements is their ability to penetrate the dentinal tubules, achieving an impermeable sealing between the dentinal walls and the central filling material, reducing potential filtrations and increasing the resistance to fracture. Objective: To evaluate through scanning electron microscopy (SEM) the penetration of MTA Fillapex, Grossman sealer and top seal all along the root canal. Methods: In vitro experimental study, in 36 human teeth randomly divided into 3 groups, (n= 12), which were prepared with the Step Back endodontic technique. Each group was divided into 2 subgroups using NaOCl 2.5%, and NaOCl 2.5% + EDTA 17% as irrigating solutions. The root canals were sealed using the lateral condensation technique, longitudinally sectioned to observe through scanning electron microscopy the cervical, medium and apical thirds, the penetration of the cement inside the dentinal tubules. The statistical analysis was performed with SPSS v22 IBM with p < 0.05 and the Kruskal-Wallis and Mann-Whitney tests. Results: through the Kruskal- Wallis test the Grossman sealer and top seal when using NaOCl + EDTA showed values of p= 0.025 and p= 0.002 in the medium and apical thirds respectively. The Mann-Whitney test showed a value of p= 0.031 in the apical third for the MTA Fillapex when using NaOCl. The median values were MTA Fillapex medium third= 2.000, apical third= 1.000; top seal medium and apical thirds= 4.000. Since the median values between the top seal and Grossman sealer were the same, the values from the variance were used, with a statistically significant result for the Grossman sealer= 0.276 in the medium third and top seal = 0.176 in the apical third. The Mann-Whitney test showed a value p= 0.031 in the apical third for the MTA Fillapex. With a median value of NaOCl= 3.000, NaOCl + EDTA= 1.000. Conclusions: When comparing the filling cements MTA Fillapex, top seal and Grossman sealer using NaOCl + EDTA as an irrigating solution, more penetration from the Grossman sealer was observed in medium third. At the apical third, more penetration from the top seal sealer was observed. The MTA Fillapex cement showed the greatest penetration at the apical third when using NaOCl as an irrigating solution (AU)

Introducción: El objetivo del estudio fue obtener información analítica sobre la pureza química y homogeneidad de los materiales de uso endodóntico MTA-Angelus® y Biodentine®. Material y métodos: Se analizaron ambos materiales mediante la técnica de microextracción en fase sólida (SPME), combinada con cromatografía de gases acoplada a espectrómetro de masas (GC-MS) para separar e identificar los compuestos orgánicos volátiles (COVs) contenidos. Por otro lado, utilizando microscopía de fuerza atómica (AFM) se estudió la topografía y rugosidad de las superficies de dientes tras la realización de obturaciones a retro in vitro con ambos cementos. Resultados: Se identificaron diversos COVs en ambos materiales mediante SPME GCMS. Entre los compuestos detectados, destacaron el alfa - metilestireno (presente en MTA-Angelus(R)) y el diclorometano (detectado tanto en MTA-Angelus(R) como en Biodentine(R)) que son potencialmente tóxicos y perjudiciales para la salud. Los resultados del estudio con AFM demostraron la existencia de diferencias en la superficie de los cementos. Las imágenes y rugosidad finales fueron más homogéneas en Biodentine®. Conclusiones: Los cementos endodónticos MTA-Angelus® y Biodentine® contienen ciertos compuestos perjudiciales para la salud. Se evidencia la necesidad de un control más exigente de los riesgos para la salud de estos. El estudio AFM mostró que Biodentine® presenta un superficie más homogénea (AU)


Introduction: The aim of this research was to obtain analytical information about the chemical purity and the homogeneity of the endodontic materials MTA Angelus(TM) and Biodentine(TM). Methods: On the one hand, solid phased microextraction-gas chromatography-mass spectrometry (SPME-GCMS) was carried out with MTAAngelus and Biodentine materials, isolating and identifying the contained volatile organic compounds (VOCs). On the other hand, atomic force microscopy (AFM) was used to study topography and roughness aspects on teeth after in vitro root end fillings with both materials. Results: First, several VOCs have been detected by SPME-GCMS in both materials. It is noteworthy alpha methylstirene (identified in MTA Angelus(TM)) or dichloromethane (detected both in MTA Angelus(TM) and Biodentine(TM)) due to its potential toxicity and harmful health effects. Secondly, AFM study showed differences among the surface materials. Final images and roughness results showed more homogeneous cement in the Biodentine product. Conclusions: Endodontic cements MTAAngelus and Biodentine contains some harmful health compounds. The need of a stricter health risks control of both materials became evident. AFM study demonstrated a more homogenous surface in Biodentine (AU)

RESUMO

La reabsorción cervical invasiva (RCI) es una entidad poco frecuente de carácter agresivo que causa la destrucción de los tejidos dentales si no se trata. El diagnóstico de la RCI es complejo y de acuerdo a la literatura presente, en estadíos muy avanzados el tratamiento no es predecible, lo que conlleva la extracción del diente. Actualmente, gracias al avance de nuevas tecnologías para el radiodiagnóstico como la tomografía computerizada de haz cónico (CBCT), el microscopio dental, así como la introducción de nuevos materiales como el Agregado Trióxido Mineral (MTA(R)) el enfoque de este tratamiento y el pronóstico en estos casos puede mejorar. Recientemente, se ha introducido el Biodentine(R), un cemento biocerámico, como material sustitutivo del MTA(R) con claras ventajas sobre este como un menor tiempo de fraguado, que no presenta discoloración y unas propiedades mecánicas similares a la dentina. A continuación se presenta un caso de reabsorción cervical invasiva tratada con Biodentine® con controles clínicos, radiográficos y mediante CBCT a 6 meses


Invasive cervical resorption (ICR) is an uncommon but aggresive entity that cause the destruction of dental tissues if left untreaten. The diagnosis of ICR is complex and according to the literature, the treatment in advanced cases is not predictable, which can ultimately lead to extraction of the tooth. The current development of innovative technologies such as the cone-beam computed tomography (CBCT) and the dental operating microscope, as well as new materials such as mineral trioxide aggregate (MTA(R)) can facilitate improvements in the treatment approach and the prognosis of ICR. Recently, BiodentineTM, a new bioceramic cement, has been introduced as a substitutive material for MTA(R) with several advantages related to setting time, the elimination of discoloration and similar mechanic properties as dentine. A case of ICR is presented in which the management of the lesion using Biodentine is reported along with the clinical, radiographic and 6-month CBCT follow-up

El síndrome incisivo molar o MIh se describe como una hipomineralización de origen sistémico que afecta de 1 a 4 de los primeros molares permanentes y que se asocia frecuentemente con alteraciones de los incisivos, siendo su aparición cada vez más frecuente. Los dientes afectados van a presentar un aumento de su contenido proteico y una disminución del contenido mineral. El tratamiento del MIh supone un reto para el odontólogo. Las características clínicas de este desorden, como la ruptura posteruptiva o la formación de cavidades atípicas, complican la obtención de un resultado duradero. Los últimos estudios apuntan a que los sistemas adhesivos de autograbado consiguen una fuerza de adhesión mayor que otros sistemas. El pretratamiento de la lesión puede producir un aumento de la fuerza adhesiva, aunque los resultados observados no permiten estandarizar este procedimiento. Las opciones más prometedoras son el hipoclorito sódico y las resinas infiltrantes. La mejor opción terapéutica para el MIh actualmente consiste en la realización de una reconstrucción con resina compuesta, utilizando un sistema adhesivo de autograbado, valorando la posibilidad de realizar un pretratamiento con hipoclorito sódico o resinas infiltrantes (AU)


The molar incisor syndrome (MIh) is described as a hypomineralization of systemic origin affecting 1 to 4 of the first permanent molars that is frequently associated with alterations of the incisors, and its appearance is increasing. Affected teeth will file a raise of its protein content and decreased mineral content. The treatment of MIh is a challenge for the dentist. The clinical features of this disorder, as posteruptive rupture or the formation of atypical cavities complicate obtaining a lasting result. Recent studies suggest that selfetching adhesive systems achieve a greater adhesion force than other systems. Pretreatment of injury may result in increased adhesive strength, although the observed results do not allow to standardize this procedure. The most promising options are sodium hypochlorite and infiltrating resins. The best therapeutic option for MIh is currently performing a reconstruction with composite resin using a self-etching adhesive system, seeing the possibility of pretreatment with sodium hypochlorite or infiltrating resins (AU)

BACKGROUND: Many researchers have tried to enhance materials functions in different aspects of science using nano-modification method, and in many cases the results have been encouraging. To evaluate the histopathological responses of the micro-/nano-size cement-type biomaterials derived from calcium silicate-based composition with addition of nano tricalcium aluminate (3CaO.Al2 O3 ) on bone healing response. MATERIAL AND METHODS: Ninety mature male rabbits were anesthetized and a bone defect was created in the right mandible. The rabbits were divided into three groups, which were in turn subdivided into five subgroups with six animals each based on the defect filled by: white mineral trioxide aggregate (WMTA), Nano-WMTA, WMTA without 3CaO.Al2 O3 , Nano-WMTA with 2% Nano-3CaO.Al2 O3 , and empty as control. Twenty, forty and sixty days postoperatively the animals were sacrificed and the right mandibles were removed for histopathological evaluations. Kruskal-Wallis test with post-hoc comparisons based on the LSMeans procedure was used for data analysis. RESULTS: All the experimental materials provoked a moderate to severe inflammatory reaction, which significantly differed from the control group (p< 0.05). Statistical analysis of bone formation and bone regeneration data showed significant differences between groups at 40- and 60- day intervals in all groups. Absence of 3CaO.Al2 O3 leads to more inflammation and foreign body reaction than other groups in all time intervals. CONCLUSIONS: Both powder nano-modification and addition of 2% Nano-3CaO.Al2 O3 to calcium silicate-based cement enhanced the favorable tissue response and osteogenesis properties of WMTA based materials

BACKGROUND: To determine the toxicity of aqueous dilutions of a universal self-priming dental adhesive (DA) and comparing these with those elicited by exposure to ionizing radiation (IR), Zoledronic acid (Z) treatment and the synergic effects of the combined treatment with IR+Z. MATERIAL AND METHODS: The genotoxic effect of DA was determined by the increase in the frequency of micronuclei in cytokinesis-blocked in cultured human lymphocytes before and after exposure to 2Gy of X-rays. The cytotoxic effect was studied by using the MTT cell viability test in normal prostate cell lines (PNT2) after exposure to different X-ray doses (0Gy-20Gy). The cell lines divided into different groups and treated with different test substances: DA in presence of O2 , DA in absence of O2 , Z-treated and control. RESULTS: An in vitro dose-dependent and time-dependent cytotoxic effect of DA, Z and IR on PNT2 cells (p > 0.001) was demonstrated. DA without-O2, following the recommendations of manufacturers, had a more pronounced effect of increasing cell death than DA with-O2 (p < 0.001). In the genotoxicity assay, DA at 25% of its original concentration significantly increased chromosome damage (p < 0.001). The samples studied were found to be toxic, and the samples photo-polymerized in absence of O2 showed a bigger cytotoxic effect comparable to the additive toxic effect showed by the combined treatment of IR+Z. CONCLUSIONS: Additional effort should be carried out to develop adhesives, which would reduce the release of hazardous substances; since toxic effects are similar to that reported by other agents whose clinical use is controlled by the health authorities

OBJECTIVES: To evaluate socket healing, incidence of acute alveolar ostieitis (AO) and associated pain following single molar tooth extraction in patients who receive intra-alveolar 0.2% chlorhexidine (CHX) gel, and those who rinsed with 0.12 % CHX rinse. Study DESIGN: A prospective randomized clinical trial was conducted on two parallel groups of patients. Group1(141 patients): Rinsed with 0.12 % CHX rinse from the second postoperative day, two times daily for a week.Group2 (160 patients): Who had direct intra-alveolar application of 0.2% CHX gel and day 3 post-operatively. The socket was evaluated 3 and 7 day postoperatively for the presence of AO by checking probing tenderness in the socket, empty socket, food debris, halitosis and pain assessment by VAS. RESULTS: Forty-eight AO cases were diagnosed out of 301 extractions (15.9%). In Group1, 25 cases were found(17.7%) while 23 cases were found in Group2 (14.4%). The difference was not statistically significant (p = 0.428).Presence of empty socket and food debris in Group1 were higher than in Group2 but the difference was not statistically significant (p= 0.390 & p = 0.415). Occurrence of halitosis in Group2 was more than Group1, but the difference was not significant (p= 0.440). Statistical significance was found between AO in extraction done by root separation (29%) and those routinely extracted (12.3 %) (p = 0.001).CONCLUSIONS: Postoperative evaluation of molar extraction sockets that received direct intra-alveolar application of0.2% CHX gel showed insignificant less occurrence of AO when compared with 0.12 % CHX rinse

La fractura dental coronaria es la injuria dental traumática más común de los dientes permanentes. El collage dentario es una alternativa restauradora que consiste en pegar fragmentos dentarios con un procedimiento semejante al que se utiliza para unir pedazos sueltos mediante pegamentos convencionales. El presente artículo describe el caso de un paciente con fracturas coronarias en las piezas 11 y 21 que fueron tratadas mediante un collage dentario. El paciente acudió a la consulta con los fragmentos fracturados procediéndose a realizar el tratamiento de conductos de dichas piezas y colocación de postes de fibras de vidrio. Posteriormente se procedió a la unión de los fragmentos previa realización de un colgajo mucoperióstico debido a la invasión del espacio biológico. A la evaluación clínica inmediata y a los tres años de seguimiento fue aceptable tanto funcional como estética


Dental crown fracture is the most common traumatic dental injury for permanent teeth. Tooth fragment reattachment is a restorative alternative which consists in adhering tooth fragments with a procedure suchlike the one used to join loose pieces by conventional adhesives. This article describes a clinical case of a patient with upper central incisors crowns fractured which were treated by tooth reattachment. Patient attended dental practice with the broken teeth and their fragments, and was immediately referred to receive root canal treatments and then the placement of fiberglass posts. Later, the fragments attachment was made after carrying a mucoperiosteal flap because of the biological space invasion. There were acceptable functional and esthetic results at the immediate clinical assessment and after three years of monitoring
